2Risk calculators for hepatocellular carcinoma in patients affected with chronic hepatitis B in Asia显示文摘Risk calculators are widely used in many clinical fields,and integrate several important risk factors through the conversion of a risk function into a single measure of risk.Several studies have been carried out to create risk calculators for the prediction of hepatocellular carcinoma(HCC)in patients with chronic hepatitis B(CHB).Most of them were hospital-based,with limited sample sizes and insufficient external validation.These study groups collaborated to establish the REACH-B risk score,which incorporated five clinical variables to predict HCC risk.This risk score was then validated ininternational clinical cohorts.Evidence suggests that quantitative serum HBsAg level provides additional predictability of HCC,especially in patients with low levels of hepatitis B virus DNA.This novel marker was incorporated into a risk calculator and was internally validated.This tool will hopefully be externally validated in the near future.Risk calculators can be used to support clinical practice,and to establish preventive measures;several'off-label'extension usages have also been implemented.Albeit beneficial,several precautions and discussions should be noted in using the risk calculators.The future development of risk calculators for CHB patients can be extended by applying them to additional CHB-related outcomes,and by incorporating emerging risk parameters.Hwai-I Yang Mei-Hsuan Lee Jessica Liu Chien-Jen Chen 2014World Journal of Gastroenterology2014,20,20:5
3Accuracy of colon tumor localization:Computed tomography scanning as a complement to colonoscopy显示文摘AIM:To determine the utility of computed tomography(CT) scanning in localizing colon tumors.METHODS:At a single tertiary care teaching hospital,a retrospective chart review was conducted on patients who underwent surgery for colon malignancies between January 2004 and May 2006.One hundred and four charts containing all of the following data were reviewed:preop erative colonoscopy report,preoperative CT report,surgical operative report,tumor pathology report.The colon was divided into five segments from the cecum to the sigmoid and the location of the lesions was categorized into one of these areas.The tumor location was considered 'erroneous' if its location determined during surgery differed from the location determined by colonoscopy or CT.RESULTS:Over all,tumor location was accurately determined via colonoscopy in 83/104 cases(79.8%) and erroneously in 21/104(20.2%) of cases.CT scan accurately localized colon tumors in 52/104(50.0%) of cases,incorrectly localized tumors in 18/104(17.3%) of cases,and did not detect known tumors in 34/104(32.7%) of cases.Of the 21 tumors erroneously located by colonoscopy,11(52.4%) were accurately localized by CT scan.The average tumor size for all patients in this study was 5.72(+/-3.11) cm.The average size of tumors properly located by colonoscopy and CT was 5.39(+/-3.34) cm and 6.79(+/-3.48) cm,respectively.The average size of the tumors not detected by CT was 3.98(+/-1.75) cm.CONCLUSION:CT scanning may be used in concert with colonoscopy to help localize colon tumors.Jessica Lee Anthony Voytovich William Pennoyer Kristy Thurston Robert A Kozol 2010World Journal of Gastrointestinal Surgery2010,2,1:3

6Robotic assisted laparoscopic simple suprapubic prostatectomy--The Smith Institute for Urology experience with an evolving technique显示文摘Objective:Despite a multitude of minimally invasive surgical options available for benign prostatic enlargement,open simple prostatectomy(OSP)remains the standard for large prostates(typically greater than 100 g).OSP,however,is associated with significant morbidity.Recently,a few reports touting robotic application to simple prostatectomy have been published.Herein,we reviewed our series of robotic assisted laparoscopic simple suprapubic prostatectomy(RALSSP)and detailed modifications in our technique as our experience increased.Methods:All RALSSP cases performed between January 2013 and January 2014 were reviewed for demographics,pre-operative features,and perioperative outcomes.All parameters were tabulated and mean values were calculated.Student’s t-test was utilized with p<0.05 deemed significant.Details regarding surgical technique were reviewed and highlighted.Results:Fifteen patients underwent RALSSP during this period.Mean age of these men was 68.7 years.Mean body mass index(BMI)was 28.5 kg/m^2.American Society of Anesthesiologists(ASA)score was on average 2.6.Average International Prostate Symptom Score(IPSS)was 16.2 with the majority of men experiencing some adverse clinical sequela of such benign prostatic hyperplasia(BPH).For those patients not in retention,preoperative post-void residual(PVR)was 428 mL.All patients underwent successful RALSSP without need for conversion or need for blood transfusion.Mean estimated blood loss(EBL)was 290 mL.Five patients underwent other concurrent procedures(e.g.,cystolithotomy).Mean length of hospital stay(LOS)was 2.4 days and only five patients required continuous bladder irrigation(CBI)postoperatively.Postoperative PVR improved to a mean of 33 mL and IPSS improved to 4.5(p<0.001).No major complications were identified.Adaptation of low transverse cystotomy,utilization of a robotic tenaculum in the#3 arm with its control by a surgeon on a second console,and the utilization of mucosal advancement have all subjectively aided in performance of RALSSP and perioperative outcomes.Conclusion:RALSSP allows for feasible performance of prostate adenoma enucleation with low risk of blood transfusion,short LOS,and significant improvement in IPSS and PVR;all while maintaining a minimally invasive approach.The use of a robotic tenaculum controlled by the secondary console and the mucosal advancement facilitate excellent outcomes and may play a role in minimizing hematuria and need for CBI.Sammy E.Elsamra Nikhil Gupta Haris Ahmed David Leavitt Jessica Kreshover Louis Kavoussi Lee Richstone 2014Asian Journal of Urology2014,1,1:2

20Circulating immune cell landscape in patients who had mild ischaemic stroke显示文摘Introduction Patients who had a mild ischaemic stroke who present with subtle or resolving symptoms sometimes go undiagnosed,are excluded from treatment and in some cases clinically worsen.Circulating immune cells are potential biomarkers that can assist with diagnosis in ischaemic stroke.Understanding the transcriptomic changes of each cell population caused by ischaemic stroke is critical because they work closely in a complicated relationship.In this study,we investigated peripheral blood mononuclear cells(PBMCs)transcriptomics of patients who had a stroke using a single-cell RNA sequencing to understand peripheral immune response after mild stroke based on the gene expression in an unbiased way.Methods Transcriptomes of PBMCsfrom 10 patients who had an acute ischaemic stroke within 24 hours after stroke onset were compared with 9 race-matched/age-matched/gender-matched controls.Individual PBMCs were prepared with ddSeqTM(Illumina-BioRad)and sequenced on the Illumina NovaSeq 6000 platform.Results Notable population changes were observed in patients who had a stroke,especially in NK cells and CD14+monocytes.The number of NK cells was increased,which was further confirmed by flow cytometry.Functional analysis implied that the activity of NK cells also is enhanced in patients who had a stroke.CD14+monocytes were clustered into two groups;dendritic cell-related CD14+monocytes and NK cell-related CD14+monocytes.We found CD14+monocyte subclusters were dramatically reduced in patients who had a stroke.Discussion This is the first study demonstrating the increased number of NK cells and new monocyte subclusters of mild ischaemic stroke based on the transcriptomic analysis.Our findings provide the dynamics of circulating immune response that could assist diagnosis and potential therapeutic development of mild ischaemic stroke.Young-Eun Cho Hyangkyu Lee Heekyong R.Bae Hyungsuk Kim Sijung Yun Rany Vorn Ann Cashion Mary Jo Rucker Mariam Afzal Lawrence Latour Jessica Gill 2022Stroke & Vascular Neurology2022,7,4:1
