Targeted therapy of gastrointestinal stromal tumours显示文摘Gastrointestinal stromal tumours(GISTs) are mesen-chymal neoplasms originating in the gastrointestinal tract, usually in the stomach or the small intestine, and rarely elsewhere in the abdomen. The malignant potential of GISTs is variable ranging from small lesions with a benign behaviour to fatal sarcomas. The majo-rity of the tumours stain positively for the CD-117(KIT) and discovered on GIST-1(DOG-1 or anoctamin 1) expression, and they are characterized by the presence of a driver kinase-activating mutation in either KIT or platelet-derived growth factor receptor α. Although surgery is the primary modality of treatment, almost half of the patients have disease recurrence following surgery, which highlights the need for an effective adjuvant therapy. Traditionally, GISTs are considered chemotherapy and radiotherapy resistant. With the advent of targeted therapy(tyrosine kinase inhibitors), there has been a paradigm shift in the management of GISTs in the last decade. We present a comprehensive review of targeted therapy in the management of GISTs.Ashish Jakhetiya Pankaj Kumar Garg Gaurav Prakash Jyoti Sharma Rambha Pandey Durgatosh Pandey 2016World Journal of Gastrointestinal Surgery2016,8,5:14
Preoperative therapy in locally advanced esophageal cancer显示文摘Esophageal cancer is an aggressive malignancy associated with dismal treatment outcomes. Presence of two distinct histopathological types distinguishes it from other gastrointestinal tract malignancies. Surgery is the cornerstone of treatment in locally advanced esophageal cancer(T2 or greater or node positive); however, a high rate of disease recurrence(systemic and loco-regional) and poor survival justifies a continued search for optimal therapy. Various combinations of multimodality treatment(preoperative/perioperative, or postoperative; radiotherapy, chemotherapy, or chemoradiotherapy) are being explored to lower disease recurrence and improve survival. Preoperative therapy followed by surgery is presently considered the standard of care in resectable locally advanced esophageal cancer as postoperative treatment may not be feasible for all the patients due to the morbidity of esophagectomy and prolonged recovery time limiting the tolerance of patient. There are wide variations in the preoperative therapy practiced across the centres depending upon the institutional practices, availability of facilities and personal experiences. There is paucity of literature to standardize the preoperative therapy. Broadly, chemoradiotherapy is the preferred neo-adjuvant modality in western countries whereas chemotherapy alone is considered optimal in the far East. The present review highlights the significant studies to assist in opting for the best evidence based preoperative therapy(radiotherapy, chemotherapy or chemoradiotherapy) for locally advanced esophageal cancer.Pankaj Kumar Garg Jyoti Sharma Ashish Jakhetiya Aakanksha Goel Manish Kumar Gaur 2016World Journal of Gastroenterology2016,22,39:13
Unusual disposition of lateral circumflex femoral artery: Anatomical description and clinical implications显示文摘The anatomical knowledge of arterial variations of lower limb is of utmost significance for the present day surgeons and interventional radiologists for minimizing complications during vascular reconstructive procedures, catheterization procedures and surgical intervention for embolism. Lateral Circumflex Femoral Artery(LCFA) isan important branch of Profunda Femoris artery and precise knowledge of its variations can be of great relevance during surgical and radiological procedures in femoral region. The present study reports a unique case of anomalous route taken by LCFA posterior to femoral nerve associated with a prominent muscular branch from Femoral artery mimicking the course of LCFA. Documentation of such variations is highly significant. It may serve as guideline for surgeons in reducing the incidence of postoperative complications where LCFA is used as a long vascular pedicle in anterolateral perforator thigh flap and in breast reconstruction after mastectomy. Ignorance of such variations can lead to fatal intraoperative haemorrhage and incapacitating sensory and motor deficit due to injury to femoral nerve branches which are closely related to these vessels.Shivi Goel Jyoti Arora Vandana Mehta Mona Sharma Rajesh Kumar Suri Gayatri Rath 2015World Journal of Clinical Cases2015,3,1:11
Recent advances in chronic granulomatous disease显示文摘Chronic granulomatous disease(CGD)is an inherited defect of phagocyte function due to defective NADPH oxidase.Patients with CGD are not able to effectively clear the infections because of the defect in the phagocyte production of oxygen free radicals and are prone to recurrent bacterial and fungal infections.Inflammatory complications are also noted in CGD such as colitis,non-infective granulomas causing gastrointestinal or urinary tract obstruction, hemophagocytic lymphohistiocytosis, and arthritis.Studies on toll-like receptor pathways and neutrophil extracellular traps in CGD have shed light on the role of NADPH oxidase in the innate immunity and pathogenesis of infections in CGD.Some reports also indicate a reduction of memory B cells and defective production of functional antibodies in CGD.Though the exact mechanisms for non-infective inflammatory complications in CGD are not yet clear, studies on efferocytosis and defective autophagy with inflammasome activation have made a substantial contribution to our understanding of the pathogenesis of inflammation in CGD.We also discuss the clinical and molecular features of p40phox defects and a newer genetic defect,EROS.Clinical phenotypes of X-linked carriers of CYBB are also discussed.Gummadi Anjani Pandiarajan Vignesh Vibhu Joshi Jitendra Kumar Shandilya Dharmagat Bhattarai Jyoti Sharma Amit Rawat 2020Genes & Diseases2020,7,1:6
Effect of stacking sequence on physical, mechanical and tribological properties of glass-carbon hybrid composites显示文摘A study was done to determine the effect of best stacking sequence(position and orientation)on the mechanical properties, friction and wear response of hybrid composites. The main purpose of the study was to determine the best available stacking sequence for which the physical, mechanical and three body abrasive wear rate is optimum.Test for mechanical properties(tensile strength, flexural strength, inter laminar shear strength, impact strength and hardness)was done on glass-carbon hybrid composites. The composites were then subjected to test for three body abrasive wear on a designed experimental setup.Five different factors(sliding velocity, fabric weight percentage, normal load, sliding distance and abrasive particle size)varied in steps to evaluate the friction and three body abrasive wear response of the composites. The design of experiment using Taguchi’s orthogonal array and analysis of variance(ANOVA)was applied to find out minimum specific wear rate. The results revealed the significance in arrangement of two different layers of fabric(glass and carbon)and the difference in properties obtained by them.Using Taguchi experimental analysis it has been observed that three body abrasive wear rate is minimum at 72 cm/s sliding velocity,30 wt.%fabric content,80 N normal load,90 m sliding distance and 125μm abrasive particle size. The scanning electron microscope(SEM)studies revealed the dynamics of three‐body abrasive wear and underlying micro‐mechanisms that result in the analysis of three body abrasive wear.Gaurav AGARWAL Amar PATNAIK Rajesh kumar SHARMA Jyoti AGARWAL 2014Friction2014,2,4:4
Genetics on early onset inflammatory bowel disease:An update显示文摘Inflammatory bowel disease(IBD)is more common in adults than in children.Onset of IBD before 17 years of age is referred as pediatric onset IBD and is further categorized as very early onset IBD(VEO-IBD)for children who are diagnosed before 6 years of age, infantile IBD who had the disease before 2 years of age and neonatal onset IBD for children less than 28 days of life.Children presenting with early onset disease may have a monogenic basis.Knowledge and awareness of the clinical manifestations facilitates early evaluation and diagnosis.Next generation sequencing is helpful in making the genetic diagnosis.Treatment of childhood IBD is difficult;targeted therapies and hematopoietic stem cell transplantation form the mainstay. In this review we aim to summarize the genetic defects associated with IBD phenotype. We describe genetic location and functions of various genetic defect associated with VEO-IBD with their key clinical manifestations. We also provide clinical clues to suspect these conditions and approaches to the diagnosis of these disorders and suitable treatment options.Johnson Nameirakpam Rashmi Rikhi Sanjay Singh Rawat Jyoti Sharma Deepti Suri 2020Genes & Diseases2020,7,1:3

Enhanced dielectric and optical properties of nanoscale barium hexaferrites for optoelectronics and high frequency application显示文摘M-type barium hexaferrites with chemical composition Ba_(1-x)Dy_xFe_(12-y)Cr_yO_(19)(x = 0.0, 0.1, 0.2, and y =0.0, 0.4, 0.5) were synthesized via sol–gel auto-combustion method. The samples were pre-sintered at 400℃ for 3 h and sintered at 950℃ for 5 h. The changes in the structural, dielectric, and optical properties were studied after the substitution of Dy^(3+) and Cr^(3+) ions. X-ray diffraction(XRD) analysis confirms the formation of single phase hexaferrites with the absence of secondary phase. FTIR analysis gives an idea of the formation of hexaferrites with the appearance of two peaks at 438 cm^(-1) and 589 cm^(-1). The field emission scanning electron micrographs(FESEM) show a combination of crystallites with large shapes close to hexagonal platelet-like shape and others with rice or rod-like shapes, whereas EDX and elemental analysis confirm the stoichiometry of prepared samples. The calculated band gap from UV-vis NIR spectroscopy spectra was found to decreases with increase in Dy^(3+)–Cr^(3+)substitution. The dielectric properties were explained on the basis of Maxwell–Wagner model. Enhancement of dielectric constant at higher frequencies was observed in all the samples. Low dielectric loss is also observed in all the samples and Cole–Cole plot shows that grain boundary resistance(Rgb) contribute most to the dielectric properties. The prepared samples exhibit properties that could be useful for optoelectronics and high frequency application.J Mohammed A B Suleiman Tchouank Tekou Carol T H Y Hafeez Jyoti Sharma Pradip K Maji Sachin Godara Kumar A K Srivastava 2018Chinese Physics B2018,27,12:1

Tailoring of structural, optical and electrical properties of anatase TiO_(2)via doping of cobalt and nitrogen ions显示文摘Pure(pristine)anatase and mono–doped and co–doped derivatives of TiO_(2)having nitrogen(N)and cobalt(Co)as dopants with respective fixed doping concentrations of 0.7 mol.%and 1.0 mol.%were synthesized using auto–combustion sol–gel technique. The doping effects at corresponding non–metal and transition metal sites of TiO_(2)on the basis of the structural, optical and electrical properties have been investigated.X–ray diffraction(XRD)measurement confirms the formation of pure anatase phase of TiO_(2)for all samples having I41/amd space group of tetragonal structure which has been also verified by the Raman spectroscopy measurement.Various crystallographic parameters have been calculated by performing Rietveld refinement of XRD data including average crystallite size that has been observed in the range of10–15 nm.Pure anatase phase indicates the incorporation of Co^(2+)into TiO_(2)lattice which assists the substitution of N in place of oxygen in co–doped TiO_(2).The band gap tuning towards the visible region from3.2 to 2.1 e V has been achieved with mono–doping and co–doping of the N and Co in TiO_(2)lattice.This can be described in terms of the formation of localized levels of N–2p and Co–3d states in mono–doping and an isolated intermediate band formation in co–doping case.Electrical properties have been investigated in details and explained as the synergetic effects of structural and inherent ionic characters of various dopants. The observed band gaps of all doped samples lie within the visible region which makes them pertinent as the solar energy harnessing materials for photocatalytic and photovoltaic applications.Anchal Sharma Puneet Negi Ruhit Jyoti Konwar Hemaunt Kumar Yogita Verma Shailja Prakash Chandra Sati Bhargav Rajyaguru Himanshu Dadhich N.A.Shah P.S.Solanki 2022Journal of Materials Science & Technology2022,,16:1
