| 2 | Expenditure and financial burden for the diagnosis and treatment of colorectal cancer in China:a hospital.based,multicenter,cross-sectional survey显示文摘Background: The increasing prevalence of colorectal cancer(CRC) in China and the paucity of information about relevant expenditure highlight the necessity of better understanding the financial burden and effect of CRC diagnosis and treatment. We performed a survey to quantify the direct medical and non-medical expenditure as well as the resulting financial burden of CRC patients in China.Methods: We conducted a multicenter, cross-sectional survey in 37 tertiary hospitals in 13 provinces across China between 2012 and 2014. Each enrolled patient was interviewed using a structured questionnaire. All expenditure data were inflated to the 2014 Chinese Yuan(CNY; 1 CNY = 0.163 USD). We quantified the overall expenditure and financial burden and by subgroup(hospital type, age at diagnosis, sex, education, occupation, insurance type, household income, clinical stage, pathologic type, and therapeutic regimen). We then performed generalized linear modeling to determine the factors associated with overall expenditure.Results: A total of 2356 patients with a mean age of 57.4 years were included, 57.1 % of whom were men; 13.9% of patients had stage I cancer; and the average previous-year household income was 54,525 CNY.The overall average direct expenditure per patient was estimated to be 67,408 CNY, and the expenditures for stage Ⅰ, Ⅱ, Ⅲ, and Ⅳ disease were 56,099 CNY, 59,952 CNY, 67,292 CNY, and 82,729 CNY, respectively. Non-medical expenditure accounted for 8.3%of the overall expenditure. The 1-year out-of-pocket expenditure of a newly diagnosed patient was 32,649 CNY, which accounted for 59.9% of their previous-year household income and caused 75.0% of families to suffer an unmanageable financial burden. Univariate analysis showed that financial burden and overall expenditure differed in almost all subgroups(P < 0.05), except for sex. Multivariate analysis showed that patients who were treated in specialized hospitals and those who were diagnosed with adenocarcinoma or diagnosed at a later stage were likely to spend more,whereas those with a lower household income and those who underwent surgery spent less(all P < 0.05).Conclusions: For patients in China, direct expenditure for the diagnosis and treatment of CRC seemed catastrophic,and non-medical expenditure was non-ignorable. | 3 | Expression and correlation of CD44v6, vascular endothelial growth factor, matrix metalloproteinase-2, and matrix metalloproteinase-9 in Krukenberg tumor显示文摘AIM: To explore the expression and correlation of CD44v6,vascular endothelial growth factor (VEGF), matrix metalloproteinase (MMP)-2 and matrix metalloproteinase (MMP)-9 in Krukenberg and primary epithelial ovarian carcinoma.METHODS: The expressions of CD44v6, VEGF, MMP-2and MMP-9 were detected by immunohistochemical method in 20 cases of normal ovarian tissues, 38 cases of Krukenberg tumor and 45 cases of primary epithelial ovarian carcinoma.RESULTS: The expression of CD44v6 (primary epithelial ovarian carcinoma tissue vs normal ovarian tissue:x2= 4.516, P= 0.034; Krukenberg tumor tissue vsnormal ovarian tissue: x2 = 19.537, P= 0.001) and VEGF (primary epithelial ovarian carcinoma tissue vs normal ovarian tissue: P = 0.026; Krukenberg tumor tissue vs normal ovarian tissue: x2= 22.895, P = 0.001) was significantly higher in primary epithelial ovarian carcinoma tissue and Krukenberg tumor tissue than in normal ovarian tissue.The positive expression rate of MMP-2 and MMP-9 was 0%in the normal ovarian tissue. The positive expression rate of CD44v6 (x2 = 10.398, P = 0.001), VEGF (x2 = 13.149,P= 0.001), MMP-2 (x2= 33.668, P= 0.001) and MMP-9(x2= 38.839, P = 0.001) was remarkably higher in Krukenberg tumor than in primary epithelial ovarian carcinoma. The correlation of CD44v6, VEGF, MMP-2, and MMP-9 was observed in primary epithelial ovarian carcinoma and Krukenberg tumor.CONCLUSION: CD44v6, VEGF, MMP-2, and MMP-9 are involved in ovarian carcinoma, gastric cancer and Krukenberg tumor. | 7 | Study on Female Sexual Dysfunction in Type 2 Diabetic Chinese Women显示文摘Objective To investigate the female sexual dysfunction (FSD) in type 2 diabetes patients, by comparing the sexual function between type 2 diabetic women and non-diabetic women with Female Sexual Function Index (FSFI). Methods 115 type 2 diabetic women and 107 age-matched non-diabetes women were enrolled with similar backgrounds. Their sexual functions were evaluated with FSFI. Metabolic parameters such as body mass index, blood lipid profile, hemoglobin A1C, plasma glucose were also collected. Results Total score of FSFI of the type 2 diabetic women were significantly lower than that of the non-diabetic controls (18.27±8.96 vs. 23.02±5.78, P=0.000). Scores of the FSFI domains (desire, arousal, lubrication, orgasm, satisfaction, pain) of the type 2 diabetic group were also lower than those of the control group. According to the FSD criterion (FSFI<25) available in China, the percentage of FSD in the type 2 diabetic group was significantly higher than that of the control group (79.2% vs. 55.0%, P<0.001). These trends seemed more prominent in pre-menopause subgroups. The logistic regression analysis indicated that age and diabetes were independent risk factors of FSD. Body Mass Index (BMI) also had influence in the diabetes group. | 11 | Origin, Flow of Formation Water and Hydrocarbon Accumulation in the Zhenwu Area of the North Jiangsu Basin, China显示文摘In order to understand the origin and flow of formation water and to evaluate the hydrocarbon accumulation and preservation conditions, the properties of formation water chemistry and dynamics of the Zhenwu area in the southern Gaoyou Sag, North Jiangsu Basin, China, have been investigated. The results show that Xuzhuang oilfield is infiltrated discontinuously by meteoric water under gravity, which consequently leads to the desalination of formation water. Formation water in the Zhenwu and Caozhuang oilfields is less influenced by meteoric water infiltration, and the origin is interpreted to be connate water. Hydrocarbon migration, accumulation and preservation are closely related to the hydrodynamic field of formation water. Formation water concentrates gradually during the process of centrifugal flow released by mudstone compaction and the centripetal flow of meteoric water infiltration, leading to the high salinity of the central part. The geological conditions of the southern fault-terrace belt are poor for hydrocarbon accumulation and preservation as meteoric water infiltration, leaching and oxidation, while the central part, i.e., northern Zhenwu and Caozhuang oilfields is beneficial for an abundance of hydrocarbon accumulation. | 12 | Endothelial cell-specific anticoagulation reduces inflammation in a mouse model of acute lung injury显示文摘Tissue factor (TF)-dependent coagulation contributes to lung inflammation and the pathogenesis of acute lung injury (ALI). In this study, we explored the roles of targeted endothelial anticoagulation in ALI using two strains of transgenic mice expressing either a membrane-tethered human tissue factor pathway inhibitor (hTFPI) or hirudin fusion protein on CD31^+ cells, including vascular endothelial cells (ECs). ALI was induced by intratracheal injection of LPS, and after 24 h the expression of TF and protease-activated receptors (PARs) on EC in lungs were assessed, alongside the extent of inflammation and injury. The expression of TF and PARs on the EC in lungs was upregulated after ALI. In the two strains of transgenic mice, expression of either of hTFPI or hirudin by EC was associated with significant reduction of inflammation, as assessed by the extent of leukocyte infiltration or the levels of proinflammatory cytokines, and promoted survival after LPS-induced ALI. The beneficial outcomes were associated with inhibition of the expression of chemokine CCL2 in lung tissues. The protection observed in the CD31-TFPI-transgenic strain was abolished by injection of an anti-hTFPI antibody, but not by prior engraftment of the transgenic strains with WT bone marrow, confirming that the changes observed were a specific transgenic expression of anticoagulants by EC. These results demonstrate that the inflammation in ALI is TF and thrombin dependent, and that expression of anticoagulants by EC significantly inhibits the development of ALI via repression of leukocyte infiltration, most likely via inhibition of chemokine gradients. | 13 | Medical expenditure for esophageal cancer in China:a 10-year multicenter retrospective survey(2002-2011)显示文摘Background: Esophageal cancer is associated with substantial disease burden in China, and data on the economic burden are fundamental for setting priorities in cancer interventions. The medical expenditure for the diagnosis and treatment of esophageal cancer in China has not been fully quantified. This study aimed to examine the medical expenditure of Chinese patients with esophageal cancer and the associated trends.Methods: From 2012 to 2014, a hospital-based multicenter retrospective survey was conducted in 37 hospitals in 13 provinces/municipalities across China as a part of the Cancer Screening Program of Urban China. For each esophageal cancer patient diagnosed between 2002 and 2011, clinical information and expense data were extracted by using structured questionnaires. All expense data were reported in Chinese Yuan(CNY; 1 CNY = 0.155 USD) based on the2011 value and inflated using the year-specific health care consumer price index for China.Results: A total of 14,967 esophageal cancer patients were included in the analysis. It was estimated that the overall average expenditure per patient was 38,666 CNY, and an average annual increase of 6.27% was observed from 2002(25,111 CNY) to 2011(46,124 CNY). The average expenditures were 34,460 CNY for stage Ⅰ,39,302 CNY for stage Ⅱ,40,353 CNY for stage Ⅲ, and 37,432 CNY for stage IV diseases(P < 0.01). | 14 | Distribution and Geochemical Characteristics of Fluids in Ordovician Marine Carbonate Reservoirs of the Tahe Oilfield显示文摘The Ordovician carbonate reservoirs in the Tahe Oilfield are highly heterogeneous, which have undergone multiple superimposed transformations by tectonic activities and karst processes, leading to an extremely complex fluid distribution. The geochemical characteristics of geofluids also display great disparities. Results show that the vertical distribution of oil and gas are continuous, however the oil-water interfaces in different blocks of the Tahe Ordovician Oilfield are numerous. Meteoric water infiltration is regarded as the main reason for the high oil-water interface and high water content to the north of Tahe Oilfield, especially in well blocks S78–S73. The isotopic values of deuterium-oxygen in the groundwater and carbon-oxygen from calcite veins confirm that formation water in Ordovician reservoirs of the Tahe Oilfield was a mix of meteoric water and connate water, and the proportion of meteoric water gradually increases from south to north, while connate water decreases. The Tahe Ordovician reservoirs are characterised by multiple hydrocarbon charges, and the general migrating direction is from southeast to northwest and from east to west. High production could be obtained in the northern area of the Tahe Oilfield since the oil layers are thick and oil is highly saturated. The residual water within the reservoirs is low, and heavy oil is dominant in this area. Only a small amount of pore water has been replaced by oil in the southern Tahe Oilfield, leading to low oil saturation and a high content of residual water. Crude oil is herein mainly of medium-light type. | 15 | Human papillomavirus 16 physical status detection in preinvasive and invasive cervical carcinoma by multiplex real-time polymerase chain reaction显示文摘Objective: To explore an ideal approach for detecting the physical status of HPV-16 in clinic use and to investigate the integrated HPV-16 in CINs and cervical cancer. Methods: Multiplex real-time PCR method was established to quantify the copy numbers of E2 and E6 genes (E2/E6) for analysis of the physical status of HPV-16 DNA and this assay was compared to Southern blot analysis. HPV-16-containing paraffin-embedded tissues including 49 CINs and 51 cervical squamous cancers were detected using the method. Results: (1) The cutoff ratio of E2/E6 to distinguish pure episomal from mixed HPV-16, was 0.81 in the multiplex real-time PCR; (2) The agreement rate between multiplex real-time PCR and Southern blot was 81.5% (the Kappa statistic was 0.844, P<0.001); (3) HPV-16 DNA existed in an episomal form in 57.1% and mixed form in 42.9% of CIN I lesions; The concomitant form of HPV-16 (>70%) constituted the majority in CIN II and CIN III; HPV-16 DNA mostly integrated into the host chromosome (s) in squamous cervical cancers (68.6%); (4) The incidence of HPV-16 integration was increased with the degree of cervical lesions; (5) The frequency of pure integrated HPV-16 in stage II+III (88%) was signifi- cantly higher than that in stage I (33.3%). | 16 | Preparation and application of ZnO doped Pt-CeO2 nanofibers as electrocatalyst for methanol electro-oxidation显示文摘ZnO doped Pt/CeO_2 nanocomposites were prepared by electrospinning and reduction impregnation.Xray diffraction(XRD),transmission electron microscopy(TEM),energy disperse spectroscopy(EDS) and X-ray photoelectron spectroscopy(XPS) were employed to characterize the nanocomposites.It is observed that ZnO and CeO_2 form the hexagonal wurtzite phase and cubic fluorite phase in the nanocomposite,respectively,whilst Pt nanoparticles(NPs) with the number-averaged size of ca.3.1 nm are uniformly distributed on the surface of nanofibers.The mass fraction of Pt NPs in the nanocomposites is about 10 wt%.The doping of ZnO is effective to promote reactive oxygen species,surface reaction sites and the interaction between Pt and oxides.The catalytic performance of nanocomposites was evaluated by the methanol electro-oxidation.indexed with the catalytic activity,stability of catalyst.As a result,it is found that the nanocomposite exhibits much higher activity and stability for methanol oxidation than the undoped Pt/CeO_2 catalyst. | Yingping Zheng Xin Zhang Zhengying Zhang Ying Li Yueming Sun Yongbing Lou Xiaojun Li Yang Lu | 2018 | Journal of Rare Earths2018,36,9: | 5 |
| 17 | Pharmacokinetic and pharmacodynamic analysis of ferulic acidpuerarin-astragaloside in combination with neuroprotective in cerebral ischemia/reperfusion injury in rats显示文摘Objective:To investigate the effects of the active ingredients combined therapy on inflammatory factors interleukin 1 beta(IL-1β)and neuropeptide Y(NPY)based on pharmacodynamics in rats.Methods:The animal model was built by transient middle cerebral artery occlusion(MCAO).The method for evaluating the concentrations of the FA-Pr-AI components in rat plasma was established by using HPLC and the expression levels of IL-1βand NPY were determined by ELISA.A new mathematics method of the trend of percentage rate of change(PRC)was used to assess the correlation between pharmacokinetics(PK)and pharmacodynamics(PD).Results:FA-Pr-Al in combination reduced neurological deficits,decreased infarct volume and inhibited the expression levels of IL-1βand NPY(all P<0.05)compared with the model group.FA,Pr and Al all displayed two compartment open models in rats.Clockwise hysteresis loops were obtained by time-concentration-effect curves.IL-1βand NPY level changes in the plasma followed an opposite trend to the plasma concentration tendency after C_(max)was reached.Astragaloside's PRC value was significantly higher than those of FA and puerarin between 120 to 180 min.Conclusions:The pharmacokinetics of FA-PrAl in combination were closely related its pharmacodynamics in treating ischemia/reperfusion injury,and the components of FA-Pr-Al may have a synergistic pharmacological effect.Astragaloside may play a more pronounced role in regulating IL-1βand NPY levels compared with puerarin or FA. | Li-Jun Ge Shou-Yan Fan Jie-Hong Yang Yi Wei Zhen-Hong Zhu Yi-Jia Lou Ying Guo Hai-Tong Wan Yi-Qiang Xie | 2015 | Asian Pacific Journal of Tropical Medicine2015,8,4: | 5 |
