| 1 | Two-stage vs single-stage management for concomitant gallstones and common bile duct stones显示文摘AIM:To evaluate the safety and effectiveness of twostage vs single-stage management for concomitant gallstones and common bile duct stones.METHODS:Four databases,including PubMed,Embase,the Cochrane Central Register of Controlled Trials and the Science Citation Index up to September 2011,were searched to identify all randomized controlled trials(RCTs).Data were extracted from the studies by two independent reviewers.The primary outcomes were stone clearance from the common bile duct,postoperative morbidity and mortality.The secondary outcomes were conversion to other procedures,number of procedures per patient,length of hospital stay,total operative time,hospitalization charges,patient acceptance and quality of life scores.RESULTS:Seven eligible RCTs [five trials(n = 621) comparing preoperative endoscopic retrograde cholangiopancreatography(ERCP)/endoscopic sphincterotomy(EST) + laparoscopic cholecystectomy(LC) with LC + laparoscopic common bile duct exploration(LCBDE);two trials(n = 166) comparing postoperative ERCP/EST + LC with LC + LCBDE],composed of 787 patients in total,were included in the final analysis.The metaanalysis detected no statistically significant difference between the two groups in stone clearance from the common bile duct [risk ratios(RR) =-0.10,95% confidence intervals(CI):-0.24 to 0.04,P = 0.17],postoperative morbidity(RR = 0.79,95% CI:0.58 to 1.10,P = 0.16),mortality(RR = 2.19,95% CI:0.33 to 14.67,P = 0.42),conversion to other procedures(RR = 1.21,95% CI:0.54 to 2.70,P = 0.39),length of hospital stay(MD = 0.99,95% CI:-1.59 to 3.57,P = 0.45),total operative time(MD = 12.14,95% CI:-1.83 to 26.10,P = 0.09).Two-stage(LC + ERCP/EST) management clearly required more procedures per patient than single-stage(LC + LCBDE) management.CONCLUSION:Single-stage management is equivalent to two-stage management but requires fewer procedures.However,patient's condition,operator's expertise and local resources should be taken into account in making treatment decisions. | Jiong Lu Yao Cheng Xian-Ze Xiong Yi-Xin Lin Si-Jia Wu Nan-Sheng Cheng | 2012 | World Journal of Gastroenterology2012,18,24: | 107 |

| 5 | Does Helicobacter pylori infection play a role in iron deficiency anemia? | 6 | Direct Yaw Moment Control for Distributed Drive Electric Vehicle Handling Performance Improvement显示文摘For a distributed drive electric vehicle(DDEV) driven by four in-wheel motors, advanced vehicle dynamic control methods can be realized easily because motors can be controlled independently, quickly and precisely. And direct yaw-moment control(DYC) has been widely studied and applied to vehicle stability control. Good vehicle handling performance: quick yaw rate transient response, small overshoot, high steady yaw rate gain, etc, is required by drivers under normal conditions, which is less concerned, however. Based on the hierarchical control methodology, a novel control system using direct yaw moment control for improving handling performance of a distributed drive electric vehicle especially under normal driving conditions has been proposed. The upper-loop control system consists of two parts: a state feedback controller, which aims to realize the ideal transient response of yaw rate, with a vehicle sideslip angle observer; and a steering wheel angle feedforward controller designed to achieve a desired yaw rate steady gain. Under the restriction of the effect of poles and zeros in the closed-loop transfer function on the system response and the capacity of in-wheel motors, the integrated time and absolute error(ITAE) function is utilized as the cost function in the optimal control to calculate the ideal eigen frequency and damper coefficient of the system and obtain optimal feedback matrix and feedforward matrix. Simulations and experiments with a DDEV under multiple maneuvers are carried out and show the effectiveness of the proposed method: yaw rate rising time is reduced, steady yaw rate gain is increased, vehicle steering characteristic is close to neutral steer and drivers burdens are also reduced. The control system improves vehicle handling performance under normal conditions in both transient and steady response. | 7 | Zircon SHRIMP U-Pb geochronology of the Neoproterozoic Chengjiang Formation in central Yunnan Province(SW China) and its geological significance显示文摘The Chengjiang Formation is the earliest continental clastic deposit after the Jinning Orogeny in central Yunnan Province,and therefore its accurate depositional age is significant for understanding the formation and evolution of the Neoproterozoic rift basins in southern China.However,hampered by accuracy of the dating technique,the existing age data for the Chengjiang Formation are not very reliable.A large number of magmatic zircons were obtained from the tuff interbed in the lower part of the Chengjiang Formation in Jinyang area and the bottom part of the Chengjiang Formation in Dongchuan area,central Yunnan Province,and high-precision SHRIMP U-Pb dating was carried out on these zircons.The results show that the weighted mean 206 Pb/238 U ages are 797.8±8.2 and 803.1±8.7 Ma respectively.In combination with related geological evidence,it has been demonstrated that the bottom boundary age of the Chengjiang Formation should be 800±5 Ma,whereas the top boundary age could be ca 725 Ma.By synthetically analyzing the latest age data for the relevant strata,it has been confirmed that the Chengjiang Formation should be correlated with the Suxiong Formation and the Kaijianqiao Formation in western Sichuan Province,the Liantuo Formation in the middle and lower Yangtze,the Hongchicun Formation and the Shangshu Formation in northern Zhejiang Province,the Puling Formation in southern Anhui Province,the Luokedong Formation and the Majianqiao Formation in northwestern Jiangxi Province,the Wuqiangxi Formation in northwestern Hunan Province,the Fanzhao Formation and the Qingshuijiang Formation in southeastern Guizhou Province,and the Sanmenjie Formation and the Gongdong Formation in northern Guangxi Province.Sedimentary cycle analysis shows that the sedimentary filling sequence of the Neoproterozoic rift basins in southern China can be divided into four cycles.Among them,Cycle II began at ca.800 Ma,accompanied by intensive tectonic-thermal events.The zircon U-Pb ages from the bottom of the Chengjiang Formation reported in this paper indicate that the Neoproterozoic Kangdian rift subbasin probably started to develop at ca.800 Ma and therefore missed Cycle I of the Neoproterozoic sedimentary filling sequence in southern China. | JIANG XinSheng WANG Jian CUI XiaoZhuang ZHUO JieWen XIONG GuoQing LU JunZe LIU JianHui | 2012 | Science China Earth Sciences2012,55,11: | 25 |

| 11 | Correlation between gene expression of CCK-A receptor and emptying dysfunction of the gallbladder in patients with gallstones and diabetes mellitus显示文摘BACKGROUND: The incidence of gallstone is higher in patients with diabetes mellitus than in general population because of hypomotility and lowered emptying function. The aim of this study was to investigate the gene expression of cholecystokinin-A ( CCK-A) receptor in patients with gallstones and diabetes mellitus and its correlation with the hypomotility of the gallbladder. METHODS: Smooth muscle was taken from the gallbladder after cholecystectomy and total RNA was extracted from it by Trizol reagent to compose cDNA. CCK-A receptor was enlarged by reverse transcription-polymerase chain reaction (RT-PCR) and identified by electrophoresis. Based on the analysis of images, the relative expression of the CCK-A re- ceptor was obtained after comparison of the CCK-A recep- tor and β-action. The emptying function of the gallbladder was measured with B ultrasound before operation. RESULTS: Gallbladder ejetion fraction ( GBEF2 ) (%) (24.9±12.7) decreased more significantly in patients with gallstones and diabetes mellitus than in those with simple gallstone (61.5±8.5) (P <0.01). In the two groups, a frac- tion of 540bp of CCK-A receptor could be testified respec- tively, but the relative expression was different. Compared to the expression of β-action, it was 0. 400 ±0.068 in pa- tients with gallstones and diabetes mellitus, but 0. 622 ± 0.070 in those with gallstone only (F =7.169, P <0.01). The result was consistent with the decreased contractility. CONCLUSIONS: The gene expression of CCK-A receptor on the smooth muscle of the gallbladder may decrease more significantly in patients with gallstone and diabetes mellitus than in those with gallstone only; this result is con- sistent with the decreased motih’ty. Recently, noncoding RNAs such as micro RNAs(mi RNAs) and circular RNAs(circ RNAs) have been reported to contribute to the pathogenesis of AD. We found an age-dependent elevation of mi R-138 in APP/PS1(presenilin-1) mice. Mi R-138 inhibited the expression of ADAM10 [a disintegrin and metalloproteinase domain-containing protein 10], promoted amyloid beta(Ab) production, and induced synaptic and learning/memory deficits in APP/PS1 mice, while its suppression alleviated the AD-like phenotype in these mice. Overexpression of sirtuin 1(Sirt1), a target of mi R-138, ameliorated the mi R-138-induced inhibition of ADAM10 and elevation of Ab in vitro. The circ RNA HDAC9(circ HDAC9) was predicted to contain a mi R-138 binding site in several databases. Its expression was inversely correlated with mi R-138 in both Ab-oligomertreated N2 a cells and APP/PS1 mice, and it co-localized with mi R-138 in the cytoplasm of N2 a cells. Circ HDAC9 acted as a mi R-138 sponge, decreasing mi R-138 expression, and reversing the Sirt1 suppression and excessive Ab production induced by mi R-138 in vitro. | 13 | Severity of acute gastrointestinal injury grade is a good predictor of mortality in critically ill patients with acute pancreatitis显示文摘BACKGROUND Gastrointestinal(GI)dysfunction is a common and important complication of acute pancreatitis(AP),especially in patients with severe AP.Despite this,there is no consensus means of obtaining a precise assessment of GI function.AIM To determine the association between acute gastrointestinal injury(AGI)grade and clinical outcomes in critically ill patients with AP.METHODS Patients with AP admitted to our pancreatic intensive care unit from May 2017 to May 2019 were enrolled.GI function was assessed according to the AGI grade proposed by the European Society of Intensive Care Medicine in 2012,which is mainly based on GI symptoms,intra-abdominal pressure,and feeding intolerance in the first week of admission to the intensive care unit.Multivariate logistic regression analysis was performed to assess the association between AGI grade and clinical outcomes in critically ill patients with AP.RESULTS Among the 286 patients included,the distribution of patients with various AGI grades was 34.62%with grade I,22.03%with grade II,32.52%with grade III,and 10.84%with grade IV.The distribution of mortality was 0%among those with grade I,6.35%among those with grade II,30.11%among those with grade III,and 61.29%among those with grade IV,and AGI grade was positively correlated with mortality(χ2=31.511,P<0.0001).Multivariate logistic regression analysis showed that age,serum calcium level,AGI grade,persistent renal failure,and persistent circulatory failure were independently associated with mortality.Compared with the Acute Physiology and Chronic Health Evaluation II score(area under the curve:0.739 vs 0.854;P<0.05)and Ranson score(area under the curve:0.72 vs 0.854;P<0.01),the AGI grade was more useful for predicting mortality.CONCLUSION AGI grade is useful for identifying the severity of GI dysfunction and can be used as a predictor of mortality in critically ill patients with AP. | Ling Ding Hong-Yan Chen Jin-Yun Wang Hui-Fang Xiong Wen-Hua He Liang Xia Nong-Hua Lu Yin Zhu | 2020 | World Journal of Gastroenterology2020,26,5: | 20 |

| 19 | Microwave Exposure Impairs Synaptic Plasticity in the Rat Hippocampus and PC12 Cells through Over-activation of the NMDA Receptor Signaling Pathway显示文摘Objective The aim of this study is to investigate whether microwave exposure would affect the N-methyl-D-aspartate receptor(NMDAR) signaling pathway to establish whether this plays a role in synaptic plasticity impairment. Methods 48 male Wistar rats were exposed to 30 m W/cm2 microwave for 10 min every other day for three times. Hippocampal structure was observed through H&E staining and transmission electron microscope. PC12 cells were exposed to 30 m W/cm2 microwave for 5 min and the synapse morphology was visualized with scanning electron microscope and atomic force microscope. The release of amino acid neurotransmitters and calcium influx were detected. The expressions of several key NMDAR signaling molecules were evaluated. Results Microwave exposure caused injury in rat hippocampal structure and PC12 cells, especially the structure and quantity of synapses. The ratio of glutamic acid and gamma-aminobutyric acid neurotransmitters was increased and the intracellular calcium level was elevated in PC12 cells. A significant change in NMDAR subunits(NR1, NR2 A, and NR2B) and related signaling molecules(Ca2+/calmodulin-dependent kinase II gamma and phosphorylated c AMP-response element binding protein) were examined. Conclusion 30 m W/cm2 microwave exposure resulted in alterations of synaptic structure, amino acid neurotransmitter release and calcium influx. | 20 | Clinical Utility of the Ratio Between Circulating Fibrinogen and Fibrin (ogen) Degradation Products for Evaluating Coronary Artery Disease in Type 2 Diabetic Patients显示文摘Background: We investigated whether and to what extent the ratio between circulating fibrinogen (Fg) and its degradation products (FDP) reflects the severity of coronary artery disease (CAD) in type 2 diabetic patients. Methods: Plasma levels of Fg and FDP were determined, and Fg/FDP ratio was calculated in 344 consecutive patients with type 2 diabetes and chest pain on exertion undergoing coronary angiography. The severity of CAD was evaluated by the number of significant CAD (〉50% luminal diameter narrowing) and Gensini score. Results: Plasma Fg was higher, but Fg/FDP ratio was lower in patients with significant CAD (n = 255) compared with those without (n = 89), due to a disproportionate increase in FDP. Fg and FDP correlated positively, while Fg/FDP ratio negatively, with the number of diseased coronary arteries and the tertile of Gensini score (all P values for trend 〈 0.01). After adjusting for age, sex, risk factors for CAD, lipid profiles, glycosylated hemoglobin A1 c, creatinine, leukocyte count, and high-sensitivity C-reactive protein, Fg/FDP ratio remained an independent determinant for multivessel coronary disease (MVD) (odds ratio [OR], 0.869; 95% confidence interval [C/], 0.788~).958, P = 0.005) and high tertile of Gensini score (OR, 0.797, 95% CI, 0.682-0.930, P = 0.004). The area under the curve of Fg/FDP ratio was larger than that of Fg for predicting the presence of MVD (0.647 vs. 0.563, P = 0.048) and Gensini score 〉 30 (0.656 vs. 0.538, P = 0.026).
