1Esophagojejunostomy after laparoscopic total gastrectomy by Or VilTM or hemi-double stapling technique显示文摘AIM: To investigate the feasibility, advantages and disadvantages of two types of anvil insertion techniques for esophagojejunostomy after laparoscopic total gastrectomy.METHODS: This was an open-label prospective cohort study. Laparoscopy-assisted radical total gastrectomy with D2 lymph node dissection was performed in 84 patients with primary non-metastatic gastric cancer confirmed by pre-operative histological examination. Overweight patients were excluded, as well as patients with peritoneal dissemination and invasion of adjacent organs. After total gastrectomy, all patients were randomized into two groups. Patients in Group Ⅰ underwent esophagojejunostomy using a transorally-inserted anvil(Or VilTM), while patients in Group Ⅱ underwent esophagojejunostomy using the hemi-double stapling technique(HDST). Both types of esophagojejunostomy were performed under laparoscopy. Patients' baseline characteristics, preoperative characteristics, perioperative characteristics, short-term postoperative outcomes and operation cost were comparedbetween the two groups. The primary endpoint was evaluation of the surgical outcome(operating time, time of digestive tract reconstruction and time of anvil insertion) and the medical cost of each operation(operation cost and total cost of hospitalization). The secondary endpoints were time to solid diet, post-surgical hospitalization time, time to defecation, time to ambulation and intra-operative blood loss. In addition, complications were assessed and compared. RESULTS: Laparoscopic total gastrectomy and esophagojejunostomy were successfully performed in all 84 patients, without conversion to laparotomy. There were no significant differences in the operative time and time for total gastrectomy between the two groups(287.8 ± 38.4 min vs 271.8 ± 46.1 min, P = 0.09, and 147.7 ± 31.6 min vs 159.8 ± 33.8 min, P = 0.09, respectively). The time for digestive tract reconstruction and for anvil insertion were significantly decreased in Group Ⅱ compared with Group I(47.8 ± 12.1 min vs 55.4 ± 15.7 min, P = 0.01, and 12.6 ± 4.7 min vs 18.7 ± 7.5 min, P = 0.001, respectively). Intraoperative blood loss(96.4 ± 32.7 m L vs 88.2 ± 36.9 m L, P = 0.28), time to defecation(3.5 ± 0.9 d vs 3.2 ± 1.1 d, P = 0.12), time to ambulation(3.9 ± 0.7 d vs 3.6 ± 1.1 d, P = 0.12), time to solid diet(7.6 ± 1.4 d vs 8.0 ± 2.7 d, P = 0.31) and total hospitalization(10.6 ± 2.6 d vs 10.8 ± 3.5 d, P = 0.80) were similar between the two groups. In addition, the total costs of hospitalization were similar between the two groups(73848.7 ± 11781.0 RMB vs 70870.3 ± 14003.5 RMB, P = 0.296), but operation cost was significantly higher in Group I compared with Group Ⅱ(32401.9 ± 1981.6 RMB vs 26961.9 ± 2293.8 RMB, P < 0.001).CONCLUSION: Anvil insertion was faster and easier using the HDST technique compared with Or VilTM, and was more cost-effective. 2Analytically Derived Wind Wave Growth Relations显示文摘By the use of the 3/2 power law presented by Toba combined with the significant wave energy balance equation forwind wave, wind wave growth at a limited fetch is analytically investigated. The new wind wave growth relations(WWGRs) are analytically derived with sheltering coefficient and wind drag coefficient as parameters. The geometricalaverage of observational values of sheltering coefficient and the arithmetic average of observational values of wind drag co-efficient are applied to determine the new WWGRs. 3Prognostic values of the integrated model incorporating the volume of metastatic regional cervical lymph node and pretreatment serum Epstein-Barr virus DNA copy number in predicting distant metastasis in patients with N1 nasopharyngeal carcinoma显示文摘Background: According to the 7 th edition of the American Joint Committee on Cancer(AJCC) staging system, over50% of patients with nasopharyngeal carcinoma(NPC) have N1 disease at initial diagnosis. However, patients with N1 NPC are relatively under-researched, and the metastasis risk of this group is not well-stratified. This study aimed to evaluate the prognostic values of gross tumor volume of metastatic regional lymph node(GTVnd) and pretreatment serum copy number of Epstein-Barr virus(EBV) DNA in predicting distant metastasis of patients with N1 NPC, and to develop an integrated prognostic model that incorporates GTVnd and EBV DNA copy number for this group of patients.Methods: The medical records of 787 newly diagnosed patients with nonmetastatic, histologically proven N1 NPC who were treated at Sun Yat-sen University Cancer Center between November 2009 and February 2012 were analyzed. Computed tomography-derived GTVnd was measured using the summation-of-area technique. Blood samples were collected before treatment to quantify plasma EBV DNA. The receiver operating characteristic(ROC) curve analysis was used to evaluate the cut-off point for GTVnd, and the area under the ROC curve was used to assess the predicted validity of GTVnd. The survival rates were assessed by Kaplan-Meier analysis, and the survival curves were compared using a log-rank test. Multivariate analysis was conducted using the Cox proportional hazard regression model.Results: The 5-year distant metastasis-free survival(DMFS) rates for patients with GTVnd > 18.9 vs.≤ 18.9 mL were82.2% vs. 93.2%(P < 0.001), and for patients with EBV DNA copy number > 4000 vs. < 4000 copies/mL were 83.5% vs.93.9%(P < 0.001). After adjusting for GTVnd, EBV DNA copy number, and T category in the Cox regression model, both GTVnd > 18.9 mL and EBV DNA copy number > 4000 copies/mL were significantly associated with poor prognosis(both P < 0.05). According to combination of GTVnd and EBV DNA copy number, all patients were divided into low-,moderate-, and high-risk groups, with the 5-year DMFS rates of 96.1,87.4, and 73.8%, respectively(P < 0.001). Multivariate analysis confirmed the prognostic value of this model for distant metastatic risk stratification(hazard ratio [HR],4.17; 95% confidence interval [CI] 2.34-7.59; P < 0.001).Conclusions: GTVnd and serum EBV DNA copy number are independent prognostic factors for predicting distant metastasis in NPC patients with N1 disease. 5Mitochondrial variants may influence the phenotypic manifestation of Leber's hereditary optic neuropathy-associated ND4 G11778A mutation显示文摘We report here the characterization of a five-generation Han Chinese family with Leber's hereditary optic neuropathy(LHON).Strikingly,this Chinese family displayed high penetrance and expressivity of visual loss.The average age-of-onset of vision loss was 18 years in this family.Nineteen(11 males/8 females) of 29 matrilineal relatives in this family developed visual loss with a wide range of severity,ranging from blindness to normal vision.Sequence analysis of mitochondrial genome in this pedigree revealed the presence of the ND4 G11778A mutation and 44 other variants belonging to Asian haplogroup M7b.The G11778A mutation is present at homoplasmy in matrilineal relatives of this Chinese family.Of other variants,the CO1 G6480A,ND5 T12811C and Cytb A15395G located at highly conserved residues of corresponding polypeptides.In fact,these variants were implicated to be involved in other clinical abnormalities.Here,these variants may act in synergy with the primary LHON-associated G11778A mutation.Thus,the mitochondrial dysfunction caused by the primary ND4 G11778A mutation may be worsened by these mitochondrial variants.The results imply that the G6480A,T12811C and A15395G variants might have a potential modifier role in increasing the penetrance and expressivity of the primary LHON-associated G11778A mutation in this Chinese family.Wanshi Cai Qun Fu Xiangtian Zhou Jia Qu Yi Tong Min-Xin Guan 2008Journal of Genetics and Genomics2008,35,11:4

7Progress and prospects of horizontal well fracturing technology for shale oil and gas reservoirs显示文摘By systematically summarizing horizontal well fracturing technology abroad for shale oil and gas reservoirs since the “13th Five-Year Plan”, this article elaborates new horizontal well fracturing features in 3D development of stacked shale reservoirs, small well spacing and dense well pattern, horizontal well re-fracturing, fracturing parameters optimization and cost control. In light of requirements on horizontal well fracturing technology in China, we have summarized the technological progress in simulation of multi-fracture propagation, horizontal well frac-design, electric-drive fracturing equipment, soluble tools and low-cost downhole materials and factory-like operation. 8An Outbreak of SARS in a Diabetes Room of a General Hospital without Infected Medical Staff显示文摘Objective To investigate the epidemiologic features of an outbreak of SARS that occurred in a single diabetes room of a general hospital in Beijing in late March 2003.Methods Field investigation was carried out in the ward,the nursing log and the hospitalization medical record of correlative patients were consulted.SARS-CoV in serum specimen from SARS patient was detected by PCR.Results The room where SARS outbreak occurred was on the 13th floor of the 16-story main ward building.There were 6 beds in the room,living with 6 female patients(aged 45-67)who were all hospitalized due to type2 diabetes.On March 24,2003,Patient 1 began to have a fever and cough,chest X-ray showed pneumonia.Five and six days later,Patient 2 and Patient 3 began to have a fever,respectively.Finally,all of these 3 patients died.Their beds were all at the same side of the room,and the other 3 patients at the opposite side were not infected.Serum SARS CoV-RNA of the Patient 3 was positive by nest-PCR.The daughter-in-law of Patient 1who accompanied Patient 1 by the bedside several days,mainly near the window,upwind of Patient 1,was not infected.Medical staff,family members and visitors of the 6 patients were not infected.Conclusions This outbreak was not transmitted by aerosol.The distance droplets travels could be up to 3.43 meters.Droplet spread has direction,and the droplets direction of propagation is closely related with the wind direction and speed.Those at the downwind position of SARS patients were susceptible to be infected.Medical staff wore face masks and good natural ventilation of this ward building may be important reasons for the prevention of infection.Chun-ling Liu Jun Cheng He Gao Bo Zhang Qun Yuan Ao Tong Yi-an Liang Hong Tang Xiao-hong Guan 2013国际感染病学(电子版)2013,2,4:3
9Influence of Polluted SY River on Child Growth and Sex Hormones显示文摘Objective To investigate the influence of the polluted SY River on children's growth and sex hormones,and provide scientific data for assessment of the polluted status of the SY River.Methods The study areas were selected randomly from the SY River Basin.Lead (Pb),mercury (Hg),arsenic (As),phthalates (DEP,DBP,DMP,DEHP),and bisphenol A (BPA) were measured both in the river water and in the drinking water.School children were selected by cluster sampling (n=154).Physical development indexes (height,weight,bust-circumference,and skinfold thickness) and sex hormones [testosterone (T) and estradiol (E2)] were measured for all the children.Results The contents of Pb and Hg exceeded Class V standards of surface water quality in each section of the river and other indicators exceeded Class III.Compared to the control area,the concentrations of Pb,Hg,As,BPA,DEP,and DBP in the drinking water were significantly higher than in the polluted area (P<0.05).Children from the control area had significantly lower E2 and T than children from the polluted area (P<0.05).Among anthropometric results,only skinfold thickness had statistically significant difference between the two groups (P<0.05),while the other indexes showed no significant differences between the two groups (P>0.05).Conclusion The drinking water has been polluted by the SY River and affected serum sex hormone levels of children living in the polluted area.TANG Chun Yu LI An Qi GUAN Yong Bo LI Yan CHENG Xue Min LI Ping LI Shi Qun LUO Yi Xin HUANG Qi CHEN Hong Yang CUI Liu Xin 2012Biomedical and Environmental Sciences2012,25,3:2
10Effect of Stocking Density on Water Quality and(Growth, Body Composition and Plasma Cortisol Content) Performance of Pen-Reared Rainbow Trout(Oncorhynchus mykiss)显示文摘The goal of the study was to examine the effect of stocking density on the water quality of culture area, as well as the growth, body composition and cortisol content of rainbow trout(Oncorhynchus mykiss). Pen-reared trout were stocked in densities of 40, 60, 80 fish individuals m^(-3)(4.6, 6.6, 8.6 kg m^(-3), SD1, SD2 and SD3 groups, respectively) for 300 days. Compared to the water from SD1 and SD2, that from SD3 exhibited significantly higher NH_4^+-N content and COD(chemical-oxygen-demand), and a significant reduction of dissolved oxygen in day 180(40.6 kg m^(-3)). Stocking density was significantly associated with body weight, standard length, VSI(viscerosomatic index), CF(condition factor) and FC(food coefficient) in group SD3, particularly in day 240 and day 300(45 or 49.3 kg m^(-3)). Increased crude fat and decreased crude protein were displayed in high density group when the density reached to 36 kg m^(-3). As a cumulative effect of density-related stress, VSI, CF, FC, moisture, and crude protein content varied over time in each density group(SD1, SD2, and SD3). In summary, trout exhibited a better growth performance in low density(26.3 kg m^(-3)) than those reared in high densities(36 and 45 kg m^(-3)).
