| 2 | Perioperative blood management strategies for patients undergoing total knee replacement:Where do we stand now?显示文摘Total knee replacement(TKR) is one of the most common surgeries over the last decade. Patients undergoing TKR are at high risk for postoperative anemia and furthermore for allogeneic blood transfusions(ABT). Complications associated with ABT including chills, rigor, fever, dyspnea, light-headedness should be early recognized in order to lead to a better prognosis. Therefore, perioperative blood management program should be adopted with main aim to reduce the risk of blood transfusion while maximizing hemoglobin simultaneously. Many blood conservation strategies have been attempted including preoperative autologous blood donation, acute normovolemic haemodilution, autologous blood transfusion, intraoperative cell saver, drain clamping, pneumatic tourniquet application, and the use of tranexamic acid. For practical and clinical reasons we will try to classify these strategies in three main stages/pillars: Pre-operative optimization, intra-operative and post-operative protocols. | 3 | Hepatitis B virus reactivation in hepatitis B virus surface antigen negative patients receiving immunosuppression: A hidden threat显示文摘AIM: To present the characteristics and the course of a series of anti- hepatitis B virus core antibody (HBc) antibody positive patients, who experienced hepatitis B virus (HBV) reactivation after immunosuppression. METHODS: We retrospectively evaluated in our tertiary centers the medical records of hepatitis B virus surface antigen (HBsAg) negative patients who suffered from HBV reactivation after chemotherapy or immunosuppression during a 3-year period (2009-2011). Accordingly, the clinical, laboratory and virological characteristics of 10 anti-HBc (+) anti-HBs (-)/HBsAg (-) and 4 anti-HBc (+)/antiHBs (+)/HBsAg (-) patients, who developed HBV reactivation after the initiation of chemotherapy or immunosuppressive treatment were analyzed. Quantitative determination of HBV DNA during reactivation was performed in all cases by a quantitative real time polymerase chain reaction kit (COBAS Taqman HBV Test; cut-off of detection: 6 IU/mL). RESULTS: Twelve out of 14 patients were males; median age 74.5 years. In 71.4% of them the primary diagnosis was hematologic malignancy; 78.6% had received rituximab (R) as part of the immunosuppressive regimen. The median time from last chemotherapy schedule till HBV reactivation for 10 out of 11 patients who received R was 3 (range 2-17) mo. Three patients (21.4%) deteriorated, manifesting ascites and hepatic encephalopathy and 2 (14.3%) of them died due to liver failure. | 4 | Modeling Passengers Boarding in Aircraft Using Cellular Automata显示文摘Aircraft are profitable to their owners as long as they are in the air transporting passengers to their destinations;therefore it is vital to minimize as much as possible their preparation time on the ground.In this paper we simulate different boarding strategies with the help of a model based on cellular automata parallel computational tool,attempting to find the most efficient way to deliver each passenger to her/his assigned seat.Two seat arrangements are used,a small one based on Airbus A320/ Boeing 737 and a larger one based on Airbus A380/ Boeing777-300.A wide variety of parameters,including time delay for luggage storing,the frequency by which the passengers enter the plane,different walking speeds of passengers depending on sex,age and height,and the possibility of walking past their seat,are simulated in order to achieve realistic results,as well as monitor their effects on boarding time.The simulation results indicate that the boarding time can be significantly reduced by the simple grouping and prioritizing of passengers.In accordance with previous papers and the examined strategies,the outside-in and reverse pyramid boarding methods outperform all the others for both the small and large airplane seat layout.In the latter,the examined strategies are introduced for first time in an analogous way to the initial small seat arrangement of Airbus A320/ Boeing737 aircraft family.Moreover,since in real world scenarios,the compliance of all the passengers to the suggested group division and boarding strategy cannot be guaranteed,further simulations were conducted.It is clear that as the number of passengers disregarding the priority of the boarding groups increases,the time needed for the boarding to complete tends towards that of the random boarding strategy,thus minimizing the possible advantages gained by the proposed boarding strategies. | Themistoklis Giitsidis Georgios Ch.Sirakoulis | 2016 | IEEE/CAA Journal of Automatica Sinica2016，3，4： | 4 |

| 11 | Adolescent Lisfranc injury treated with TightRope^(TM):A case report and review of literature显示文摘BACKGROUND Lisfranc injuries are rare and can be easily missed. This injury is extremely rare in children, with limited published data. Different treatment options have been described; one of the options in adults is the 'mini' TightRope^(TM) Syndesmosis Device that provides non-rigid fixation with impressive results. However, there is no reference regarding the use of this device in children.CASE SUMMARY We describe the case of an 11-year-old girl who sustained a Lisfranc injury of her right foot that was initially missed in the Accident and Emergency department of her local hospital. This case was a ligamentous/periosteal sleeve avulsion type of Lisfranc injury and a percutaneous technique using the 'mini' TightRope^(TM)syndesmosis device was used. Clinical and radiological results were excellent at final follow up.CONCLUSION The 'mini' TightRope^(TM) syndesmosis device is a promising method of fixation for children with certain Lisfranc injuries. | 16 | Pegylated IFN-α2b added to ongoing lamivudine therapy in patients with lamivudine-resistant chronic hepatitis B显示文摘AIM: To investigate the role of pegylated-interferon (IFN)α-2b in the management of patients with lamivudine-resistant chronic hepatitis B. METHODS: Twenty consecutive anti-HBe positive patients were treated with pegylated IFNα-2b (100μg sc once weekly) for 12 mo. There was no interruption in lamivudine therapy. Hematology, liver biochemistry, serum HBV DNA levels were detected by PCR, and vital signs were also assessed. Liver histology was assessed in some patients at entry and at wk 52 for comparison. RESULTS: Nine patients (45%) had a partial viro-logical end-treatment response; seven patients (35%) showed complete virological end-treatment response. Eight patients (40%) showed biochemical end-treatment response. There was a trend for higher virological response rates in patients who had previously responded to IFN and relapsed compared to IFN non-responders (four out of seven patients vs none out of six patients, respectively; P=0.1). Patients without virological end-treatment response showed significant worsening of fibrosis [median score 2 (range, 1 to 3) vs median score 3 (range, 1 to 4)], in the first and second biopsy respectively (P=0.014), whereas necroinflammatory activity was not significantly affected. Patients with complete or partial virological end-treatment response did not show any significant changes in histological findings, possibly due to the small number of patients with paired biopsies (n = 5). Nevertheless, after 12 mo of follow-up, only one patient (5%) showed sustained virological response and only 2 patients (10%) showed sustained biochemical response. Two patients (10%) discontinued pegylated IFN both after 6 mo of treatment due to flu-like symptoms.
