| 2 | Chinese expert consensus on cytoreductive surgery and hyperthermic intraperitoneal chemotherapy for peritoneal malignancies显示文摘Locoregional spread of abdominopelvic malignant tumors frequently results in peritoneal carcinomatosis(PC). The prognosis of PC patients treated by conventional systemic chemotherapy is poor, with a median survival of < 6 mo. However, over the past three decades, an integrated treatment strategy of cytoreductive surgery(CRS) + hyperthermic intraperitoneal chemotherapy(HIPEC) has been developed by the pioneering oncologists, with proved efficacy and safety in selected patients. Supported by several lines of clinical evidence from phases Ⅰ, Ⅱ and Ⅲ clinical trials, CRS + HIPEC has been regarded as the standard treatment for selected patients with PC in many established cancer centers worldwide. In China, an expert consensus on CRS + HIPEC has been reached by the leading surgical and medical oncologists, under the framework of the China Anti-Cancer Association. | 4 | Acute kidney injury in acute-on-chronic liver failure is different from in decompensated cirrhosis显示文摘AIM To evaluate the differences in acute kidney injury(AKI) between acute-on-chronic liver failure(ACLF) and decompensated cirrhosis(DC) patients. METHODS During the period from December 2015 to July 2017, 280 patients with hepatitis B virus(HBV)-related ACLF(HBV-ACLF) and 132 patients with HBV-related DC(HBV-DC) who were admitted to our center were recruited consecutively into an observational study. Urine specimens were collected from all subjects and the levels of five urinary tubular injury biomarkers were detected,including neutrophil gelatinase-associated lipocalin(NGAL), interleukin-18(IL-18), liver-type fatty acid binding protein(L-FABP), cystatin C(CysC), and kidney injury molecule-1(KIM-1). Simultaneously, the patient demographics, occurrence and progression of AKI, and response to terlipressin therapy were recorded. All patients were followed up for 3 mo or until death after enrollment. RESULTS AKI occurred in 71 and 28 of HBV-ACLF and HBV-DC patients, respectively(25.4% vs 21.2%, P = 0.358). Among all patients, the levels of four urinary biomarkers(NGAL, CysC, L-FABP, IL-18) were significantly elevated in patients with HBV-ACLF and AKI(ACLF-AKI), compared with that in patients with HBV-DC and AKI(DC-AKI) or those without AKI. There was a higher proportion of patients with AKI progression in ACLF-AKI patients than in DC-AKI patients(49.3% vs 17.9%, P = 0.013). Fortythree patients with ACLF-AKI and 19 patients with DC-AKI were treated with terlipressin. The response rate of ACLFAKI patients was significantly lower than that of patients with DC-AKI(32.6% vs 57.9%, P = 0.018). | 7 | Histological Subtypes of Lung Cancer in Chinese Males from 2000 to 2012显示文摘Objective To characterize the histological and epidemiological features of male lung cancer patients in China. Methods The demographic and histological information about male lung cancer patients identified from 2000-01-01 to 2012-12-31,was collected from the Cancer Hospital of the Chinese Academy of Medical Sciences. Relative frequencies(RF) were estimated for major histological subtypes and compared according to the years of diagnosis and birth. Results The RF of adenocarcinoma(ADC) increased from 21.96% to 43.36% and the RF of squamous cell carcinoma(SCC) decreased from 39.11% to 32.23% from 2000 to 2012 in the 15 427 male lung cancer patients included in this study(Z=17.909,P<0.0001; Z=-6.117,P<0.0001). The RF of ADC increased from 28.72% in 2000-2004,36.88% in 2005-2008 to 48.61% in 2009-2012 in patients born after 1960. The age-adjusted RF of ADC in 2007-2012 increased consistently in all the investigated areas. Conclusion The increased RF of ADC in male lung cancer patients highlights the need for further investigation of the etiologic factors of these tumors. We report the demonstration of an ASG based on 129Xe-Cs comagnetometer, which decreases the start-up time to 10 minutes and decreases the operation temperature by 40% as well. | 9 | Primary dolostone formation related to mantle-originated exhalative hydrothermal activities,Permian Yuejingou section,Santanghu area, Xinjiang,NW China显示文摘The Permian Lucaogou Formation is an important hydrocarbon source rock in the Junggar, Turpan, and Santanghu basins in Xinjiang, NW China. For the first time, dolostones associated with mantle-originated exhalative hydrothermal fluid flows are discovered in Yuejingou section in the Santanghu Basin area. They include dolomicrite, doloarenite, and a small amount of dolorudite, and are finely (0.05-0.15 cm thick) interlaminated with lime micrite and dolomicritic analcime laminites. Alkali feldspar and analcime grains are common in doloarenite and are interpreted as having been derived from analcime phonolites and peralkaline magmatic rocks. These magmatic fragments were brought up from subsurface by hydrothermal fluid flow and had experienced exhalative brecciation, transport, and deposition on the lake floor. The matrix consists dominantly of dolomite and ankerite smaller than 0.01 mm. The dolostones can be subdivided into four types on the basis of mineral composition and content. The detrital analcime and alkaline feldspar grains and tuff lithics are interpreted as intraclasts, which were deposited in an under-filled starved lake basin. The δ18OPDB values of dolostones are -5‰ to -21.1‰, and -11.9‰ on average; the 87Sr/86Sr ratios of dolostones are 0.70457 to 0.706194, and 0.705005 on average. These values, in combination with evidence of multi-episodes of peralkaline extrusion, suggest a mantle origin of the hydrothermal fluids, which may have promoted primary dolomite formation. The fluid from the upper mantle caused serpentinization of ultramafic rocks that intruded into the lower crust to obtain Mg2+ and Fe2+, and injected the ions into the lake water as the Mg and Fe sources for dolomite and ankerite. Hydro- thermal fluids associated with peralkaline magmatic rocks also provided Ca2+, Mg2+, Fe2+, and CO32-. Explosive breccias formed and dolostones were convoluted near the vent of hydrothermal fluid exhalation, whereas laminated dolostones formed farther away from the vent. The dolostones are primary dolomite deposition in an intracontinental rift basin and associated with mantle-originated hydrothermal fluids. | 11 | Multiple primary malignant tumors of upper gastrointestinal tract:A novel role of ^(18)F-FDG PET/CT显示文摘AIM: To evaluate the capacity of 18F-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT) for detecting multiple primary cancer of upper gastrointestinal (UGI) tract. METHODS: Fifteen patients (12 without cancer histories and 3 with histories of upper GI tract cancer) were investigated due to the suspicion of primary cancer of UGI tract on X-ray barium meal and CT scan. Subsequent whole body 18F-FDG PET/CT scan was carried out for initial staging or restaging. All the patients were fi nally confi rmed by endoscopic biopsy or surgery. The detection rate of multiple primary malignant cancers was calculated based on 18F-FDG PET/CT and endoscopic examinations.RESULTS: 18F-FDG PET/CT scan was positive in 32 suspicious lesions, 30/32 were true positive primary lesions, and 2/32 were false positive. In 15 suspicious lesions with negative 18F-FDG PET/CT scan, 12/15 were true negative and 3/15 were false negative. Among the 15 patients, 12 patients had 29 primary synchronous tumors confirmed by pathology, including 8 cases of esophageal cancers accompanied with gastric cancer and 4 of hypopharynx cancers with esophageal cancer. The other 3 patients had 4 new primary metachronous tumors, which were multiple primary esophageal cancers. PET/CT imaging detected local lymph node metastases in 11 patients. Both local lymph node metastases and distant metastases were detected in 4 patients. On a per-primary lesion basis, the sensitivity, specificity, accuracy, negative predictive value and positive predictive value of 18F-FDG PET/CT for detecting multiple primary cancer of UGI tract were 90.9%, 85.7%, 89.4%, 80% and 93.7%, respectively. | 15 | Interpretation Criteria for Standardized Western Blot for the Predominant Species of Borrelia Burgdorferi Sensu Lato in China显示文摘Objective Western blotting (WB;immunoblotting) is a widely used tool for the serodiagnosis of Lyme borreliosis (LB),but so far,no generally accepted criteria for its performance and interpretation have been established in China.The present study was designed to determine the criteria for standardized Western blot for the predominant species of Borrelia burgdorferi sensu lato in China,in which WB was produced with strain PD91 as the representative strain attributed to predominant genospecies Borrelia garinii of Borrelia burgdorferi sensu lato.Methods Approximately 13 bands between 14 and 100 kD were differentiated for strain PD91 by using Gel-Pro analysis software.In a study with 631 serum samples (taken from 127 patients with Lyme borreliosis and 504 controls),all observed bands were documented.To establish criteria for a positive WB result for strain PD91,receiver operating characteristic (ROC) curves were used.Results The following interpretation criteria were recommended:for IgG,at least one band of P83/100,P58,P39,P30,OspC,P17,P66,and OspA;for IgM,at least one band of P83/100,P58,OspA,P30,OspC,P17 or P41.In addition,syphilis,leptospirosis and other related diseases should be excluded when the positive band is P41 in IgM.For IgG criteria,the sensitivity is 73.2%,the specificity is 99.4% and Youden index is 0.726;for IgM criteria,the sensitivity is 50.6%,the specificity is 93.1% and Youden index is 0.437.Conclusion Standardization of WB assays is necessary for comparison of results from different laboratories.Moreover,the criteria of other genospecies of Borrelia burgdorferi sensu lato should be determined in the future to complete the criteria of WB for the diagnosis of the Lyme disease in China. | YI JIANG XUE-XIA HOU ZHEN GENG QIN HAO KANG-LIN WAN | 2010 | Biomedical and Environmental Sciences2010,23,5: | 13 |
| 16 | Seroepidemiological Investigation of Lyme Disease and Human Granulocytic Anaplasmosis among People Living in Forest Areas of Eight Provinces in China显示文摘Objective Lyme disease and Human granulocytic anaplasmosis are tick‐borne diseases caused by Borrelia burgdorferi and Anaplasma phagocytophilum respectively. We have investigated infection and co-infection of the two diseases in the population of forest areas of eight provinces in China by measuring seroprevalence of antibodies against B. burgdorferi and A. phagocytophilum. Methods Forest areas in 8 provinces were chosen for investigation using whole sampling and questionnaire survey methods. 3 669 serum samples from people in the forest areas were tested for the presence of antibodies by indirect immunofluorescent assay (IFA). Results Seroprevalence against B. burgdorferi was 3% to 15% and against A. phagocytophilum was 2% to 18% in the study sites in the 8 provinces in China. We also found co-infection of B. burgdorferi and A. phagocytophilum in 7 of the 8 provinces (the exception being the Miyun area in Beijing). The seroprevalence for both B. burgdorferi and A. phagocytophilum was significantly higher among people exposed to ticks than among people who were not exposed to ticks. Herein, we systemically studied the impact of fluorination and chlorination on the properties of resulting donors(PBDB-T-2 F and PBDB-T-2 Cl) and acceptors(IT-4 F and IT-4 Cl). The results suggest that all the OSCs based on different donor and acceptor combinations can deliver good PCEs around 13%–14%. Chlorination is more effective than fluorination in downshifting the molecular energy levels and broadening the absorption spectra. The influence of chlorination and fluorination on the crystallinity of the resulting materials is dependent on their introduction positions. | 19 | Mechanosignaling activation of TGFβmaintains intervertebral disc homeostasis显示文摘Intervertebral disc(IVD) degeneration is the leading cause of disability with no disease-modifying treatment.IVD degeneration is associated with instable mechanical loading in the spine, but little is known about how mechanical stress regulates nucleus notochordal(NC) cells to maintain IVD homeostasis. Here we report that mechanical stress can result in excessive integrin α_vβ_6-mediated activation of transforming growth factor beta(TGFβ), decreased NC cell vacuoles, and increased matrix proteoglycan production, and results in degenerative disc disease(DDD). Knockout of TGFβ type II receptor(TβRII) or integrin α_v in the NC cells inhibited functional activity of postnatal NC cells and also resulted in DDD under mechanical loading.Administration of RGD peptide, TGFβ, and α_vβ_6-neutralizing antibodies attenuated IVD degeneration. Thus,integrin-mediated activation of TGFβ plays a critical role in mechanical signaling transduction to regulate IVD cell function and homeostasis.
