Stepwise sedation for elderly patients with mild/moderate COPD during upper gastrointestinal endoscopy显示文摘AIM: To investigate stepwise sedation for elderly patients with mild/moderate chronic obstructive pulmonary disease (COPD) during upper gastrointestinal (GI) endoscopy. METHODS: Eighty-six elderly patients with mild/moderate COPD and 82 elderly patients without COPD scheduled for upper GI endoscopy were randomly assigned to receive one of the following two sedation methods: stepwise sedation involving three-stage administration of propofol combined with midazolam [COPD with stepwise sedation (group Cs), and nonCOPD with stepwise sedation (group Ns)] or continuous sedation involving continuous administration of propofol combined with midazolam [COPD with continuous sedation (group Cc), and non-COPD with continuous sedation (group Nc)]. Saturation of peripheral oxygen (SpO 2 ), blood pressure, and pulse rate were monitored, and patient discomfort, adverse events, drugs dosage, and recovery time were recorded. RESULTS: All endoscopies were completed successfully. The occurrences of hypoxemia in groups Cs, Cc, Ns, and Nc were 4 (9.3%), 12 (27.9%), 3 (7.3%), and 5 (12.2%), respectively. The occurrence of hypoxemia in group Cs was significantly lower than that in group Cc (P < 0.05). The average decreases in value of SpO 2 , systolic blood pressure, and diastolic blood pressure in group Cs were significantly lower than those in group Cc. Additionally, propofol dosage and overall rate of adverse events in group Cs were lower than those in group Cc. Finally, the recovery time in group Cs was significantly shorter than that in group Cc, and that in group Ns was significantly shorter than that in group Nc (P < 0.001). CONCLUSION: The stepwise sedation method is effective and safer than the continuous sedation method for elderly patients with mild/moderate COPD during upper GI endoscopy. | Can-Xia Xu Xiong Chen Yan Jia Ding-Hua Xiao Hui-Fang Zou Qin Guo Fen Wang Xiao-Yan Wang Shou-Rong Shen Ling-Ling Tong Ke Cao Xiao-Ming Liu | 2013 | World Journal of Gastroenterology2013,19,29: | 5
Bear bile powder attenuates senecionine-induced hepatic sinusoidal obstruction syndrome in mice显示文摘Hepatic sinusoidal obstruction syndrome(HSOS)via exposure to pyrrolizidine alkaloids(PAs)is with high mortality and there is no effective treatment in clinics.Bear bile powder(BBP)is a famous traditional animal drug for curing a variety of hepatobiliary diseases such as cholestasis, inflammation, and fibrosis. Here,we aim to evaluate the protective effect of BBP against HSOS induced by senecionine,a highly hepatotoxic PA compound. Our results showed that BBP treatment protected mice from senecionine-induced HSOS dose-dependently,which was evident by improved liver histology including reduced infiltration of inflammatory cells and collagen positive cells, alleviated intrahepatic hemorrhage and hepatic sinusoidal endothelial cells,as well as decreased conventional serum liver function indicators. In addition,BBP treatment lowered matrix metalloproteinase 9 and pyrrole-protein adducts, two well-known markers positively associated with the severity of PA-induced HSOS.Further investigation showed that BBP treatment prevents the development of liver fibrosis by decreasing transforming growth factor beta and downstream fibrotic molecules.BBP treatment also alleviated senecionine-induced liver inflammation and lowered the pro-inflammatory cytokines,in which taurours-odeoxycholic acid played an important role. What’s more,BBP treatment also decreased the accumulation of hydrophobic bile acids, such as cholic acid, taurocholic acid, glycocholic acid,as well.We concluded that BBP attenuates senecionine-induced HSOS in mice by repairing the bile acids homeostasis, preventing liver fibrosis, and alleviating liver inflammation. Our present study helps to pave the way to therapeutic approaches of the treatment of PA-induced liver injury in clinics. | JIANG Kai-Yuan ZHANG Yi YE Xuan-Ling XIONG Fen CHEN Yan JIA Xia-Li ZHANG Yi-Xin YANG Li XIONG Ai-Zhen WANG Zheng-Tao | 2022 | Chinese Journal of Natural Medicines2022,20,4: | 2
Chronic infusion of ELABELA alleviates vascular remodeling in spontaneously hypertensive rats via anti-inflammatory,anti-oxidative and anti-proliferative effects显示文摘Inflammatory activation and oxidative stress promote the proliferation of vascular smooth muscle cells(VSMCs),which accounts for pathological vascular remodeling in hypertension.ELABELA(ELA)is the second endogenous ligand for angiotensin receptor-like 1(APJ)receptor that has been discovered thus far.In this study,we investigated whether ELA regulated VSMC proliferation and vascular remodeling in spontaneously hypertensive rats(SHRs).We showed that compared to that in Wistar-Kyoto rats(WKYs),ELA expression was markedly decreased in the VSMCs of SHRs.Exogenous ELA-21 significantly inhibited inflammatory cytokines and NADPH oxidase 1 expression, reactive oxygen species production and VSMC proliferation and increased the nuclear translocation of nuclear factor erythroid 2-related factor(Nrf2)in VSMCs.Osmotic minipump infusion of exogenous ELA-21 in SHRs for 4 weeks significantly decreased diastolic blood pressure, alleviated vascular remodeling and ameliorated vascular inflammation and oxidative stress in SHRs.In VSMCs of WKY,angiotensin II(Ang II)-induced inflammatory activation, oxidative stress and VSMC proliferation were attenuated by pretreatment with exogenous ELA-21 but were exacerbated by ELA knockdown.Moreover,ELA-21 inhibited the expression of matrix metalloproteinase 2 and 9 in both SHR-VSMCs and Ang II-treated WKY-VSMCs.We further revealed that exogenous ELA-21-induced inhibition of proliferation and PI3K/Akt signaling were amplified by the PI3K/Akt inhibitor LY294002,while the APJ receptor antagonist F13A abolished ELA-21-induced PI3K/Akt inhibition and Nrf2 activation in VSMCs.In conclusion,we demonstrate that ELA-21 alleviates vascular remodeling through anti-inflammatory,anti-oxidative and anti-proliferative effects in SHRs,indicating that ELA-21 may be a therapeutic agent for treating hypertension. | Chao Ye Zhi Geng Ling-li Zhang Fen Zheng Ye-bo Zhou Guo-qing Zhu Xiao-qing Xiong | 2022 | Acta Pharmacologica Sinica2022,43,10: | 1

Bio-inspired untethered fully soft robots in liquid actuated by induced energy gradients显示文摘Soft robotics with new designs, fabrication technologies and control strategies inspired by nature have been totally changing our view on robotics. To fully exploit their potential in practical applications, untethered designs are preferred in implementation. However, hindered by the limited thermal/mechanical performance of soft materials,it has been always challenging for researchers to implement untethered solutions, which generally involve rigid forms of high energy-density power sources or high energy-density processes.A number of insects in nature, such as rove beetles, can gain a burst of kinetic energy from the induced surface-energy gradient on water to return to their familiar habitats, which is generally known as Marangoni propulsion.Inspired by such a behavior,we report the agile untethered mobility of a fully soft robot in liquid based on induced energy gradients and also develop corresponding fabrication and maneuvering strategies. The robot can reach a speed of 5.5 body lengths per second, which is 7-fold more than the best reported,0.69(body length per second),in the previous work on untethered soft robots in liquid by far.Further controlling the robots,we demonstrate a soft-robot swarm that can approach a target simultaneously to assure a hit with high accuracy.Without employing any high energy-density power sources or processes, our robot exhibits many attractive merits, such as quietness,no mechanical wear,no thermal fatigue, invisibility and ease of robot fabrication, which may potentially impact many fields in the future. | Liang Xiong Lyu Fen Li Kang Wu Pan Deng Seung Hee Jeong Zhigang Wu Han Ding | 2019 | National Science Review2019,6,5: | 1
Ferroelectric engineering of two-dimensional group-Ⅳ monochalcogenides:The effects of alloying and strain显示文摘In this work,we studied the stability and ferroelectricity of twodimensional(2D)group-IV monochalcogenides MX(M=Ge,Sn,or Pb;X=S,Se,or Te).Two competing crystal structures of these 2D compounds have been considered, including the rippled crystal structure with a space group of Pmn21,which can be obtained from exfoliation, and the P3m1 hexagonal crystal structure containing the X-M-M-X building blocks. We find that the total energies of the rippled phases of PbX,SnS and SnSe are lower than those of the corresponding hexagonal phases;particularly, the rippled phases of SnS and SnSe exhibit spontaneous polarizations and ferroelectricity. On the other hand, the hexagonal phases of GeX and SnTe are energetically more stable than the corresponding rippled phases;because the hexagonal phases are centrosymmetric, these 2D compounds are non-ferroelectric.To engineer the ferroelectricity of 2D group-Ⅳmonochalcogenides,we have investigated the effects of alloying and equibiaxial strain.Based on density functional theory calculations,we find that the rippled phases of GeX and SnTe can be stabilized via Pb alloying to achieve ferroelectricity. In addition,it is also found that equibiaxial tensile strain gives rise to ferroelectricity in the rippled phases of 2D PbX compounds. | Fen Xiong Xilin Zhang Zhen Lin Yue Chen | 2018 | Journal of Materiomics2018,4,2: | 1
Fouling process and anti-fouling mechanisms of dynamic membrane assisted by photocatalytic oxidation under sub-critical fluxes显示文摘Membrane fouling is often considered as a hindrance for the application of microfiltration/ultrafiltration(MF/UF) for drinking water production. A novel process of photocatalytic membrane reactor/dynamic membrane(PMR/DM), operating in a continuous mode under sub-critical flux, was proposed for the mitigation of membrane fouling caused by humic acids(HAs) in water. The mechanism of membrane fouling alleviation with synergistic photocatalytic oxidation and dynamic layer isolating effect was comprehensively investigated from the characterization of foulant evolution responsible for the reversible and irreversible fouling. The results showed that the PMR/DM utilized photocatalytic oxidation to enhance the porosity and hydrophilicity of the fouling layer by converting the high molecular weight(MW) and hydrophobic HA molecules with carboxylic functional groups and aromatic structures into low-MW hydrophilic or transphilic fractions, including tryptophan-like or fulvic-like substances. The fouling layer formed in the PMR/DM by combination of photocatalytic oxidation and DM running at a sub-critical flux of 100 L·h^-1·m^-2, was more hydrophilic and more porous, resulting in the lowest trans-membrane pressure(TMP) growth rates, as compared to the processes of ceramic membrane(CM), DM and PMR/CM.Meanwhile, the dynamic layer prevented the foulants, particularly the high-MW hydrophobic fractions, from contacting the primary membrane, which enabled the membrane permeability to be restored easily. | Tao Yang Fen Liu Houfeng Xiong Qiyong Yang Fushan Chen Changchao Zhan | 2019 | Chinese Journal of Chemical Engineering2019,27,8: | 1
