1Randomized controlled multicenter clinical trial for integrated treatment of community-acquired pneumonia based on Traditional Chinese Medicine syndrome differentiation显示文摘OBJECTIVE:To evaluate the efficacy and safety of treatment based on syndrome differentiation ofTraditional Chinese Medicine(TCM) for community-acquired pneumonia(CAP).METHODS:A total of 240 CAP patients were randomly divided into the following two groups:thecontrol group was treated by anti-infection plus conventional medicine treatment;and the trial group was treated by TCM plus the above-mentioned treatment given to the controls.The course of treatment was 14 days,and the patients were followed up for 7 days.RESULTS:Of the 240 patients,235 accomplished the whole process of treatment.The five patients who withdrew from the study were brought into an intent-to-treat analysis.The therapeutic effects of the trial group were superior to those of the control group(P<0.01).The trial group took less time to become clinically stable,with a higher score in the quality of life(P<0.01).There were no significant differences in mortality rate(P>0.05),white blood cell count(P>0.05),bacterial clearance rate(P>0.05),and adverse reactions between the two integrated groups.CONCLUSION:Treatment based on TCM syndrome differentiation for CAP has the advantages of resulting in less time to achieve a stable clinical condition,improvement of clinical symptoms and quality of life,and is comparatively safe.Jiansheng Li Xueqing Yu Suyun Li Haifeng Wang Yunping Bai Minghang Wang Zikai Sun Wei Zhang Zhaoshan Zhou Xianhua Jia Qingwei Zhou 2012Journal of Traditional Chinese Medicine2012,32,4:11
2Characteristics of fecal microbiota in non-alcoholic fatty liver disease patients显示文摘This study was designed to investigate the gut microbiota of patients with non-alcoholic fatty liver disease. The inclusive and exclusive criteria for NAFLD patients and healthy subjects were formulated, and detailed clinical data were collected. The genomic DNA of stool samples were extracted for 16 S r DNA sequencing, and the amplified V4-region was sequenced on the Illumina Miseq platform. Metastats analysis was performed to identify the differential taxa between the groups. Redundancy analysis was used to evaluate the association between gut microbial structure and clinical variables. Thirty NAFLD patients and37 healthy controls were involved. The 16 S r DNA sequencing showed that there was a dramatic variability of the fecal microbiota among all the individuals. Metastats analysis identified eight families and 12 genera with significant differences between the two groups. When some clinical parameters, such as waist-to-hip ratio(WHR) and homeostasis model assessment of insulin resistance(HOMA-IR), were enrolled in Redundancy analysis, the distribution of the two group of samples was obviously changed. The compositional shifts in fecal bacterial communities of NAFLD patients from the healthy controls were mainly at family or genus levels. 3A bionic artificial joint system and investigation of tribological performance显示文摘Joint replacement is one of the most common and successful operations in orthopaedic surgery. It consists of replacing painful, arthritic, worn or cancerous parts of the joint with artificial surfaces shaped in such a way as to allow joint movement. The polyethylene component has been implicated as the key factor affecting the lifetime of these prostheses. This paper introduces a bionic artificial joint system which aims to prolong the lifetime of such prostheses. Included are bionic system design, investigation of bionic therapeutic lubricants and analysis of the mechanical properties of hip prostheses. 5Japanese Encephalitis Virus NS1' Protein Antagonizes Interferon Beta Production显示文摘Japanese encephalitis virus(JEV) is a mosquito-borne virus and the major cause of viral encephalitis in Asia. NS1', a52-amino acid C-terminal extension of NS1, is generated with a-1 programmed ribosomal frameshift and is only present in members of the Japanese encephalitis serogroup of flaviviruses. Previous studies demonstrated that NS1' plays a vital role in virulence, but the mechanism is unclear. In this study, an NS1' defected(rG66A) virus was generated. We found that rG66A virus was less virulent than its parent virus(pSA14) in wild-type mice. However, similar mortality caused by the two viruses was observed in an IFNAR knockout mouse model. Moreover, we found that rG66A virus induced a greater type Ⅰ interferon(IFN) response than that by pSA14, and JEV NS1' significantly inhibited the production of IFN-b and IFN-stimulated genes. 7Dexamethasone does not affect endolymphatic hydrops(EH)in patients with Meniere's disease within 24 h,and intratympanic administration of gadolinium plus dexamethasone simplifies high-quality imaging of EH using a novel protocol of 7 min显示文摘Background:Intratympanic administration of gadolinium chelate allows for a better visualization of endolymphatic hydrops(EH)using MRI than intravenous injection and was recently further improved to facilitate high-quality imaging of EH in 7 min.The aim of the present study was to simplify the intratympanic administration protocol by mixing gadolinium chelate with therapeutic dexamethasone and to evaluate the effects of this mixture on the visualization of EH in MRI.Materials and methods:In an in vitro study,the potential impact of gadolinium-diethylenetriamine pentaacetic acid(Gd-DTPA)on the stability of dexamethasone was evaluated by analyzing dynamic changes in dexamethasone with high-performance liquid chromatography(HPLC)after mixing with GdDTPA.Ten patients with definite Meniere's disease(MD)were recruited to study the potential interference of dexamethasone on MRI visualization of EH,and 49 patients with MD were recruited to evaluate the effect of intratympanic injection of Gd-DTPA mixed with dexamethasone on MRI of EH using a 3T MR machine and a novel heavily T2-weighted 3-dimensional fluid-attenuated inversion recovery reconstructed using a magnitude plus zero-filled interpolation(hT2FLAIR-MZFI)sequence.Results:The retention times and peak area of dexamethasone in HPLC were not modified by the addition of Gd-DTPA.EH grading in the cochlea and vestibule was not influenced in any ear by intratympanic injection of dexamethasone.Excellent inner ear images were obtained from all patients,and EHs with various grades were displayed.There were significant correlations between diagnosis and cochlear EH(p<0.01,Spearman's Rho),between diagnosis and vestibular EH(p<0.01,Spearman's Rho),and between cochlear and vestibular EH(p<0.01,Spearman's Rho).The distribution of Gd-DTPA plus dexamethasone negatively correlated with the grade of vestibular EH.Injury of the endolymph-perilymph barrier was detected in one cochlea and three vestibules of 59 inner ears with MD.Conclusions:Intratympanic administration of Gd-DTPA plus dexamethasone yielded high-quality MRI images of EH in patients with MD using a novel 7-min protocol and simplified the clinical application.Intratympanic administration of Gd-DTPA plus dexamethasone might be used to test its therapeutic effect in future work.Jing Zou Zikai Zhao Hongbin Li Guoping Zhang Qing Zhang Jianping Lu Ilmari Pyykko 2022Journal of Otology2022,17,3:2
8An efficient migrating birds optimization algorithm with idle time reduction for Type-I multi-manned assembly line balancing problem显示文摘Multi-manned assembly line,which is broadly utilized to assemble high volume products such as automobiles and trucks,allows a group of workers to assemble different tasks simultaneously in a multi-manned workstation.This additional characteristic of parallel operators increases the complexity of the traditional NP-hard assembly line balancing problem.Hence,this paper formulates the Type-I multi-manned assembly line balancing problem to minimize the total number of workstations and operators,and develops an efficient migrating birds optimization algorithm embedded into an idle time reduction method.In this algorithm,a new decoding mechanism is proposed which reduces the sequence-dependent idle time by some task assignment rules;three effective neighborhoods are developed to make refinement of existing solutions in the bird improvement phases;and temperature acceptance and competitive mechanism are employed to avoid being trapped in the local optimum.Comparison experiments suggest that the new decoding and improvements are effective and the proposed algorithm outperforms the compared algorithms.ZHANG Zikai TANG Qiuhua LI Zixiang HAN Dayong 2021Journal of Systems Engineering and Electronics2021,32,2:2

11DRNet:Towards fast,accurate and practical dish recognition显示文摘Existing algorithms of dish recognition mainly focus on accuracy with predefined classes,thus limiting their application scope.In this paper,we propose a practical two-stage dish recognition framework(DRNet)that yields a tradeoff between speed and accuracy while adapting to the variation in class numbers.In the first stage,we build an arbitrary-oriented dish detector(AODD)to localize dish position,which can effectively alleviate the impact of background noise and pose variations.In the second stage,we propose a dish reidentifier(DReID)to recognize the registered dishes to handle uncertain categories.To further improve the accuracy of DRNet,we design an attribute recognition(AR)module to predict the attributes of dishes.The attributes are used as auxiliary information to enhance the discriminative ability of DRNet.Moreover,pruning and quantization are processed on our model to be deployed in embedded environments.Finally,to facilitate the study of dish recognition,a well-annotated dataset is established.Our AODD,DReID,AR,and DRNet run at about 14,25,16,and 5 fps on the hardware RKNN 3399 pro,respectively.CHENG SiYuan CHU BinFei ZHONG BiNeng ZHANG ZiKai LIU Xin TANG ZhenJun LI XianXian 2021Science China(Technological Sciences)2021,64,12:1

13Mitigation of power system forced oscillations based on unified power flow controller显示文摘Forced oscillations(FOs), or low-frequency oscillations(LFOs) caused by periodic, continuous, small power disturbances, threaten the security and stability of power systems.Flexible AC transmission system(FACTS)devices can effectively mitigate LFOs via stability control.We propose a novel method that mitigates FOs by shifting the resonant frequency.Based on the features of the linearized swing equation of a generator, a resonant frequency shift can be achieved by controlling the synchronous torque coefficient using a unified power flow controller(UPFC).Because of the resonance mechanism, the steady-state response of an FO can be effectively mitigated when the resonant frequency changes from the original one, which was close to the disturbance frequency.The principle is that a change in resonant frequency affects the resonance condition.Simulations are conducted in a single-machine infinite-bus(SMIB) system, and the simulation results verify that the method is straightforward to implement and can significantly mitigate FOs.The controller robustness when the resonant frequency is not accurately estimated is also analyzed in the simulations.Ping JIANG Zikai FAN Shuang FENG Xi WU Hui CAI Zhenjian XIE 2019Journal of Modern Power Systems and Clean Energy2019,7,1:1

19Location of the AICA influences the severity but not occurrence of ISSNHL:A reappraisal using high-resolution 3 T MRI显示文摘Objective:To investigate the potential influence of anatomical variation in the anterior inferior cerebellar artery(AICA)on the occurrence and severity of idiopathic sudden sensorineural hearing loss(ISSNHL).Methods:Ninety ISSNHL patients were enrolled.The anatomical location of the AICA was exhibited using high-resolution magnetic resonance imaging(MRI),and the various AICA types classified by previously reported Chavda and Gorrie methods were analyzed.The severity of hearing loss in the ipsilateral ear among different AICA types was compared.Results:Approximately 85.6%of subjects had unilateral ISSNHL(uISSNHL),and the others had bilateral ISSNHL(bISSNHL).In the uISSNHL group,the ratios of different AICA types were similar between the ipsilateral and contralateral ears.The ratios of the different AICA types in the bISSNHL group were similar to those in the uISSNHL group.In the uISSNHL group,pure tone audiometry(PTA)thresholds at 2 kHz,4 kHz and 8 kHz of patients with Chavda type II AICA were higher than those of patients with Chavda type I and type III,with a significant difference at 4 kHz between type I and type II.There was a tendency of the PTA threshold in patients with Chavda type II or Gorrie type C to gradually increase from low to high frequency zones.Conclusion:When the AICA enters the IAC(Chavda type II)or crosses between the 7th and 8th cranial nerves(Gorrie type C),the severity and frequency of hearing impairment in ISSNHL but not the occurrence of ISSNHL will be affected.Guoping Zhang Hongbin Li Zikai Zhao Mingxing Zhang Jing Zou 2023Journal of Otology2023,18,4:0
