2Nonintubated uniportal video-assisted thoracoscopic surgery for primary spontaneous pneumothorax显示文摘Objective:The objective of the current study was to evaluate the feasibility and safety of nonintubated uniportal video-assisted thoracoscopic surgery(VATS) for the management of primary spontaneous pneumothorax(PSP).Methods:From November 2011 to June 2013,32 consecutive patients with PSP were treated by nonintubated uniportal thoracoscopic bullectomy using epidural anaesthesia and sedation without endotracheal intubation.An incision 2 cm in length was made at the 6^(th) intercostal space in the median axillary line.The pleural space was entered by blunt dissection for placement of a soft incision protector.Instruments were then inserted through the incision protector to perform thoracoscopic bullectomy.Data were collected within a minimum follow-up period of 10 months.Results:The average time of surgery was 49.0 min(range,33-65 min).No complications were recorded.The postoperative feeding time was 6 h.The mean postoperative chest tube drainage and hospital stay were19.3 h and 41.6 h,respectively.The postoperative pain was mild for 30 patients(93.75%) and moderate for two patients(6.25%).No recurrences of pneumothorax were observed at follow-up.Conclusions:The initial results indicated that nonintubated uniportal video-assisted thoracoscopic operations are not only technically feasible,but may also be a safe and less invasive alternative for select patients in the management of PSP.This is the first report to include the use of a nonintubated uniportal technique in VATS for such a large number of PSP cases.Further work and development of instruments are needed to define the applications and advantages of this technique.Shuben Li Fei Cui Jun Liu Xin Xu Wenlong Shao Weiqiang Yin Hanzhang Chen Jianxing He 2015Chinese Journal of Cancer Research2015,27,2:15
3Long-term survival outcomes of video-assisted thoracic surgery for patients with non-small cell lung cancer显示文摘Background: Video-assisted thoracic surgery(VATS) has been shown to be a safe alternative to conventional thoracotomy for patients with non-small cell lung cancer(NSCLC). However, popularization of this relatively novel technique has been slow, partly due to concerns about its long-term outcomes. The present study aimed to evaluate the long-term survival outcomes of patients with NSCLC after VATS, and to determine the significant prognostic factors on overall survival.Methods: Consecutive patients diagnosed with NSCLC referred to one institution for VATS were identified from a central database. Patients were treated by either complete-VATS or assisted-VATS, as described in previous studies. A number of baseline patient characteristics, clinicopathologic data and treatment-related factors were analyzed as potential prognostic factors on overall survival.Results: Between January 2000 and December 2007, 1,139 patients with NSCLC who underwent VATS and fulfilled a set of predetermined inclusion criteria were included for analysis. The median age of the entire group was 60 years, with 791 male patients(69%). The median 5-year overall survival for Stage I, II, III and IV disease according to the recently updated TNM classification system were 72.2%, 47.5%, 29.8% and 28.6%, respectively. 4Emergency medical services use and its association with acute ischaemic stroke evaluation and treatment in Singapore显示文摘Background Emergency medical services(EMS)is a critical link in the chain of stroke survival.We aimed to assess EMS use for stroke in Singapore,identify characteristics associated with EMS use and the association of EMS use with stroke evaluation and treatment.Methods The Singapore Stroke Registry combines nationwide EMS and public hospital data for stroke cases in Singapore.Multivariate regressions with the generalised estimating equations were performed to examine the association between EMS use and timely stroke evaluation and treatment.results Of 3555 acute ischaemic patients with symptom onset within 24 hours admitted to all five public hospitals between 2015 and 2016,68%arrived via EMS.Patients who used EMS were older,were less likely to be female,had higher stroke severity by National Institute of Health Stroke Scale and had a higher prevalence of atrial fibrillation or peripheral arterial disease.Patients transported by EMS were more likely to receive rapid evaluation(door-to imaging time≤25 min 34.3%vs 11.1%,OR=2.74(95%CI 1.40 to 5.38))and were more likely to receive intravenous tissue plasminogen activator(tPA,22.8%vs 4.6%,OR=4.61(95%CI 3.52 to 6.03)).Among patients treated with tPA,patients who arrived via EMS were more likely to receive timely treatment than self-transported patients(door-to needle time≤60 min 52.6%vs 29.4%,OR=2.58(95%CI 1.35 to 4.92)).Conclusions EMS use is associated with timely stroke evaluation and treatment in Singapore.Seamless EMS-Hospital stroke pathways and targeted public campaigns to advocate for appropriate EMS use have the potential to improve acute stroke care.Hanzhang Xu Ying Xian Fung Peng Woon Janet Prvu Bettger Daniel T Laskowitz Yih Yng Ng Marcus Eng Hock Ong David Bruce Matchar Deidre Anne De Silva 2020Stroke & Vascular Neurology2020,5,2:2

6Ubiquitin-specific protease 47 regulates intestinal inflammation through deubiquitination of TRAF6 in epithelial cells显示文摘Deubiquitinates(DUBs) alter the stabilities, localizations or activities of substrates by removing their ubiquitin conjugates,which are closely related to the development of inflammatory response. Here, we show that ubiquitin-specific protease 47(USP47) prevents inflammation development in inflammatory bowel disease(IBD). Compared with wild-type mice, Usp47 knockout mice are more susceptible to dextran sodium sulfate(DSS)-induced acute and chronic colitis with higher inflammatory cytokines expression and severe intestinal tissue damage. Chimeric mouse experiments suggest that non-hematopoietic cells mainly contribute to the phenotype. And, DSS-induced colitis of the Usp47 knockout mice depends on commensal bacteria.Mechanistically, down-regulation of USP47 aggravates the activation of NF-κB signaling pathway by increasing the K63-linked poly-ubiquitination of tumor necrosis factor receptor-associated factor 6(TRAF6) in intestinal epithelial cells. Furthermore, the expression of USP47, negatively correlated with the degree of inflammation, is lower at colonic inflammatory lesions than that non-inflammatory sites from the intestine from ulcerative colitis(UC) and Crohn's disease(CD) patients. 8Self-reported diabetes treatment among Chinese middle-aged and older adults with diabetes:Comparison of urban residents,migrants in urban settings,and rural residents显示文摘Purpose:To compare self-reported diabetes treatments among Chinese urban residents,rural migrants in urban settings,and rural residents.Methods:Data from 993 diabetics at least 45 years of age were collected from the China Health and Retirement Longitudinal Study conducted in 2011.Multiple logistic regressions were performed to examine the associations between individual characteristics and diabetes treatments.Results:In total,719/993(72.4%)of the respondents received treatment for diabetes;of which<8% used insulin therapy.Urban residents were more likely than rural residents to use insulin therapy[odds ratio(OR)=0.44,confidence interval(CI):0.20-0.99;p<0.05],and more likely to use traditional Chinese medicine than migrants(OR=0.30,CI:0.10-0.96;p<0.05).Overall,rural residents showed lower treatment rates than urban and migrant populations.Conclusion:Efforts to improve and enhance diabetes treatments,particularly among rural residents,are urgently needed in China.Hanzhang Xu Jianfeng Luo Bei Wu 2015International Journal of Nursing Sciences2015,2,1:0
9USP7 deubiquitinates and stabilizes NOTCH1 in T-cell acute lymphoblastic leukemia显示文摘T-cell acute lymphoblastic leukemia(T-ALL)is a highly aggressive leukemia that is primarily caused by aberrant activation of the NOTCH1 signaling pathway.Recent studies have revealed that posttranslational modifications,such as ubiquitination,regulate NOTCH1 stability,activity,and localization.However,the specific deubiquitinase that affects NOTCH1 protein stability remains unestablished.Here,we report that ubiquitin-specific protease 7(USP7)can stabilize NOTCH1.USP7 deubiquitinated NOTCH1 in vivo and in vitro,whereas knockdown of USP7 increased the ubiquitination of NOTCH1.USP7 interacted with NOTCH1 protein in T-ALL cells,and the MATH and UBL domains of USP7 were responsible for this interaction.Depletion of USP7 significantly suppressed the proliferation of T-ALL cells in vitro and in vivo,accompanied by downregulation of the NOTCH1 protein level.Similarly,pharmacologic inhibition of USP7 led to apoptosis of T-ALL cells.More importantly,we found that USP7 was significantly upregulated in human T-ALL cell lines and patient samples,and a USP7 inhibitor exhibited cell cytotoxicity toward primary T-ALL cells,indicating the clinical relevance of these findings.Overall,our results demonstrate that USP7 is a novel deubiquitinase that stabilizes NOTCH1.Therefore,USP7 may be a promising therapeutic target in the currently incurable T-ALL.Huizhuang Shan Xiangyun Li Xinhua Xiao Yuting Dai Jinyan Huang Junjun Song Meng Liu Li Yang Hu Lei Yin Tong Li Zhou Hanzhang Xu Yingli Wu 2018Signal Transduction and Targeted Therapy2018,3,1:0
10Parthenolide reveals an allosteric mode to inhibit the delSGylation activity of SARS-CoV-2 papain-like protease显示文摘The coronavirus papain-like protease(PLpro)of severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)is responsible for viral polypeptide cleavage and the deISGylation of interferon-stimulated gene 15(ISG15),which enable it to participate in virus replication and host innate immune pathways.Therefore,PLpro is considered an attractive antiviral drug target.Here,we show that parthenolide,a germacrane sesquiterpene lactone,has SARS-CoV-2 PLpro inhibitory activity.Parthenolide covalently binds to Cys-191 or Cys-194 of the PLpro protein,but not the Cys-111 at the PLpro catalytic site.Mutation of Cys-191 or Cys-194 reduces the activity of PLpro.Molecular docking studies show that parthenolide may also form hydrogen bonds with Lys-192,Thr-193,and Gln-231.Furthermore,parthenolide inhibits the deISGylation but not the deubiquitinating activity of PLpro in vitro.These results reveal that parthenolide inhibits PLpro activity by allosteric regulation.Zhihui Zou Huizhuang Shan Demeng Sun Li Xia Yulong Shi Jiahui Wan Aiwu Zhou Yunzhao Wu Hanzhang Xu Hu Lei Zhijian Xu Yingli Wu 2022Acta Biochimica et Biophysica Sinica2022,54,8:0
11Hsp90/C terminal Hsc70-interacting protein regulates the stability of Ikaros in acute myeloid leukemia cells显示文摘The stability of Ikaros family zinc finger protein 1(Ikaros),a critical hematopoietic transcription factor,can be regulated by cereblon(CRBN)ubiquitin ligase stimulated by immunomodulatory drugs in multiple myeloma.However,other stabilization mechanisms of Ikaros have yet to be elucidated.In this study,we show that the pharmacologic inhibition or knockdown of Hsp90 downregulates Ikaros in acute myeloid leukemia(AML)cells.Proteasome inhibitor MG132 but not autophagy inhibitor chloroquine could suppress the Hsp90 inhibitor STA-9090-induced reduction of Ikaros,which is accompanied with the increased ubiquitination of Ikaros.Moreover,Ikaros interacts with E3 ubiquitin-ligase C terminal Hsc70 binding protein(CHIP),which mediates the STA-9090-induced ubiquitination of Ikaros.In addition,the knockdown of Ikaros effectively inhibits the proliferation of leukemia cells,but this phenomenon could be rescued by Ikaros overexpression.Collectively,our findings indicate that the interplay between HSP90 and CHIP regulates the stability of Ikaros in AML cells,which provides a novel strategy for AML treatment through targeting the HSP90/Ikaros/CHIP axis.Meng Liu Jin Jin Yanjie Ji Huizhuang Shan Zhihui Zou Yang Cao Li Yang Ligen Liu Li Zhou Hu Lei Yunzhao Wu Hanzhang Xu Yingli Wu 2021Science China(Life Sciences)2021,64,9:0
12Experimental study on 3D internal penny-shaped crack propagation in brittle materials under uniaxial compression显示文摘Fractures are widely present in geomaterials of civil engineering and deep underground engineering.Given that geomaterials are usually brittle,the fractures can significantly affect the evaluation of underground engineering construction safety and the early warning of rock failure.However,the crack initiation and propagation in brittle materials under composite loading remain unknown so far.In this study,a three-dimensional internal laser-engraved cracking technique was applied to produce internal cracks without causing damage to the surfaces.The uniaxial compression tests were performed on a brittle material with internal cracks to investigate the propagation of these internal cracks at different dip angles under compression and shear.The test results show that the wing crack propagation mainly occurs in the specimen with an inclined internal crack,which is a mixed-ModeⅠ–Ⅱ–Ⅲfracture;in contrast,ModeⅠfracture is present in the specimen with a vertical internal crack.The fractography characteristics of ModeⅢfracture display a lance-like pattern.The fracture mechanism in the brittle material under compression is that the internal wing cracks propagate to the ends of the whole sample and cause the final failure.The initial deflection angle of the wing crack is determined by the participation ratio of stress intensity factors KII to KI at the tip of the internal crack.Jiyun Xu Hanzhang Li Haijun Wang Lei Tang 2023Deep Underground Science and Engineering2023,2,1:0
