1Chinese consensus guidelines for diagnosis and management of gastrointestinal stromal tumor显示文摘In order to further promote the standardization of diagnosis and treatment of gastrointestinal stromal tumor(GIST) in China, the members of Chinese Society of Clinical Oncology(CSCO) Expert Committee on GIST thoroughly discussed the key contents of the consensus guidelines, and voted on the controversial issue. In final, the Chinese consensus guidelines for the diagnosis and management of GIST(2017 edition) was formed on the basis of 2013 edition consensus guidelines, which is hereby announced. 2Effects of AT1 receptor antagonist,Iosartan,on rat hepatic fibrosis induced by CCl_4显示文摘AIM To investigate effect of losartan,an AT1receptor antagonist,on hepatic fibrosis induced byCCl;and to determine whether or not AT1receptors are expressed on hepatic stellate cells,METHODS AND RESULTS Fifty male Sprague-Dawley rats,weighing(180±20)g,wererandomized into five groups(control group,modelgroup,and three losartan treated groups),inwhich all rats were given the subcutaneousinjection of 40% CCl4(every 3 days for 6 weeks)except for rats of control group.Rats of losartan-treated groups were treated with losartan(20 mg/kg,10 mg/kg,5 mg/kg,daily gavage),After 6weeks liver tissue and serum samples of all ratswere examined.Serum hyaluronic acid(HA),procollagen typeⅢ(PCⅢ)were detected byradioimmunoassays,van Giesion collagen stainingwas used to evaluate the extracellular matrix of ratswith liver fibrosis.The expression of AT1receptors,transforming growth factor-beta(TGF-β),and alpha-smooth muscle actin(a-SMA)inliver tissue were determined byimmunohistochemical techniques.Compared withmodel group,serum ALT and AST of losartan-treated groups were significantly reduced(t=4.20,P<0.01 and t=4.57,P<0.01).Serum HAand PCⅢalso had significant differences(t=3.53,P<0.01 and t=2.20,P<0.05).Thedegree of fibrosis was improved by losartan and correlated with the expressions of AT1 receptors,TGF-β,and α-SMA in liver tissue.CONCLUSION AT1 receptor antagonist,losartan,could limit the progression of the hepatic fibrosisinduced by CCl4.The mechanism may be related tothe decrease in the expression of AT1 receptorsand TGF-β,ameliorating the injury of hepatocytes;activation of local renin-angiotensin system mightrelate to hepatic fibrosis;and during progressionof fibrosis,activated hepatic stellate cells mightexpress AT1 receptors.Hong Shan Wei Ding Guo Li Han Ming Lu Yu Tao Zhan Zhi Rong Wang Xin Huang Jing Zhang Ji Lin Cheng Qin Fang Xu Department of Gastroenterology,Xinhua Hospital,Shanghai Second Medical University,Shanghai 200092,China 2000World Journal of Gastroenterology2000,6,4:42
3Prognostic value of systemic immune.inflammation index in patients with gastric cancer显示文摘Background:Inflammation-based indexes have been used to predict survival and recurrence in cancer patients.Systemic immune-inflammation index(Sll) was reported to be associated with prognosis in some malignant tumors.In the present study,we aimed to explore the association between Sll and the prognosis of patients with gastric cancer.Methods:We retrospectively analyzed data from 444 gastric cancer patients who underwent gastrectomy at the First Affiliated Hospital of Sun Yat-sen University between January 1994 and December 2005.Preoperative Sll was calculated.The Chi square test or Fisher's exact test was used to determine the relationship between preoperative Sll and clinicopathologic characteristics.Overall survival(OS) rates were estimated using the Kaplan-Meier method,and the effect of Sll on OS was analyzed using the Cox proportional hazards model.Receiver operating characteristic(ROC)curves were used to compare the predictive ability of Sll,NLR,and PLR.Results:Sll equal to or higher than 660 was significantly associated with old age,large tumor size,unfavorable Borrmann classification,advanced tumor invasion,lymph node metastasis,distant metastasis,advanced TNM stage,and high carcino-embryonic antigen level,high neutrophil-lymphocyte ratio,and high platelet-lymphocyte ratio(all P<0.05).High Sll was significantly associated with unfavorable prognosis(P<0.001) and Sll was an independent predictor for OS(P=0.015).Subgroups analysis further showed significant associations between high Sll and short OS in stage Ⅰ,Ⅱ,Ⅲ subgroups(all P<0.05).Sll was superior to NLR and PLR for predicting OS in patients with gastric cancer.Conclusion:Preoperative Sll level is an independent prognostic factor for OS in patients with gastric cancer.Kang Wang Feiyu Diao Zhijun Ye Xinhua Zhang Ertao Zhai Hui Ren Tong Li Hui Wu Yulong He Shirong Cai Jianhui Chen 2017Chinese Journal of Cancer2017,36,9:30
4The regulatory role of AT 1 receptor on activated HSCs in hepat,c fibrogenesis,effects of RAS inhibitors on hepatic fibrosis induced by CCl_4显示文摘AIM To assess the effect of ACE inhibitor andAng Ⅱ type Ⅰ(AT1)receptor antagonist inpreventing hepatic fibrosis caused by CCl4administration in rats;to investigate whether ornot there are expression of AT 1 receptors onhepatic stellate cells;and to observe the effectof Ang Ⅱ on proliferation and ECM synthesis ofcultured HSCs.METHODS Studies were conducted in maleSprague-Dawley rats.Except for thehepatofibrotic model group and the controlgroup,in three treated groups,either enalapril(5 mg/kg),or Iosartan(10 mg/kg),or enalapril+Iosartan were given to the fibrotic rats bydaily gavage,and saline vehicle was given tomodel and normal control rats.After 6 weeks,liver fibrosis was assessed directly by hepaticmorphometric analysis,which has beenconsidered the gold standard for thequantification of fibrosis.The expressions of AT1 receptors and(α-mooth muscle actin,α-SMA)in liver tissue or isolated hepatic stellate cells(HSCs)were detected by immunohistochemicaltechniques.The effect of Ang Ⅱ on HSCproliferation was determined by MTT method.Effect of Ang Ⅱ on collagen synthesis of HSCswas determined by 3H-proline incorporation.RESULTS Contrasted to the fibrosis in rats ofthe model group,groups of rats treated with either enalapril or Iosartan,or a combination oftwo drugs showed a limited expansion of theinterstitium(4.23±3.70 vs 11.22±4.79,P<0.05),but no difference was observedamong three treated groups(5.38±3.43,4.96±2.96,4.23±2.70,P>0.05).Expression of AT 1receptors was found in fibrotic interstitium offibrotic rats,whereas in normal control rats theywere limited to vasculature only to a very slightdegree.AT 1 receptors were also expressed onactivated HSCs in the culture.At concentrationsfrom 10-9to 10-5mol/L,Ang Ⅱ stimulated HSCproliferation in culture in a dose-dependentmanner.Increasing Ang Ⅱ concentrationsproduced corresponding increases in 3H-prolineincorporation.Differences among groups were significant.CONCLUSION Angiotensin-converting enzyme inhibitors and AT I blocker may slow the progression of hepatic fibrosis; activated HSCs express AT 1 receptors, and Ang Ⅱ can stimulate the proliferation and collagen synthesis of HSCs in a dose-dependent manner; and activation of RAS may be related to hepatic fibrogenesis induced by CCI4.Hong Shan Wei Han Ming Lu Ding Guo Li Yu Tao Zhan Zhi Rong Wang Xin Huang Ji Lin Cheng Qin Fang Xu Department of Gastroenterology,Xinhua Hospital,Shanghai Second Medical University,Shanghai 200092,China 2000World Journal of Gastroenterology2000,6,6:27
5A review of ecological restoration techniques in fluvial rivers显示文摘Rivers play an important role in people's living and agricultural production,however,intense human activities have broken the original ecological balance,and affected structures and functions of the river ecosystem.To restore the damaged river ecosystem back to a healthy status,effective ecological restoration measures need to be implemented.The main problems that the damaged rivers face are either the locally altered hydrological processes affected by construction of hydraulic facilities,or the deterioration of water quality resulted from pollution emissions,or both.In this study,ecological restoration techniques of the rivers affected by engineering control or pollution are reviewed respectively.In addition,three kinds of methods,i.e.physical,chemical and biological-ecological methods are introduced in details for the rivers affected by water pollution.At present,the development of river restoration techniques shows the following trends:1) the scale of ecological restoration is becoming larger;2) ecological restoration measures are required to meet multiple objectives;and 3) the management of water environment is changing from water quality management to aquatic ecosystem management.Baozhu Pan Jianping Yuan Xinhua Zhang Zhaoyin Wang Jiao Chen Jinyou Lu Wenjun Yang Zhiwei Li Na Zhao Mengzhen Xu 2016International Journal of Sediment Research2016,31,2:18
6Influence of hepatic arterial blockage on blood perfusion and VEGF,MMP-1 expression of implanted lver cancer in rats显示文摘AIM:To investigate the influence of hepatic arterial blodckage on blood perfusion of transplanted cancer in rat liver and the expression of vascular endothelial growth factor (VEGF)and matrix metalloproteinase-1(MMP-1),and to explore the mechanisms involved in transarterial embolization(TAE)-induced metastasis of liver cancer preliminarily.METHODS:Walker 256 carcinosarcoma was transplanted in to rat liver to establish the liver cancer model.Hepatic arterial ligation(HAL) was used to block the hepatic arterial blood supply and simulate TAE.Blood perfusion of tumor in control laparotomy control,and HAL group was analyzed by Hoechst 33342 labeling assay,the serum VEGF level was assayed by ELISA,the expression of VEGF and MMP-1 mRNA was detected by in situ hybridization.RESULTS:Two days after HAL.the number of Hoechst 33 342 labeled cells which represent the blood perfusion of tumor directly and hypoxia of tumor indirectliy in HAL group decreased significantly compared with that in control group (329±29VS 384±19,P<0.01),The serum VEGF level in the HAL group increased significantly as against that of the control group (93ng.L^-1±44ng.L^-1VS55ng.L^-1±19ng.L^-1,P<0.05),The expression of VEGFand MMP-1 mRNA in the tumor tissue of the HAL group increased significantly compared with that of the control and the laparotomy control groups(P<0.05),The blood perfusion data of the tumor represented by the number of Hoechst 33342 labeled cells,showed a good linear inverse correlation with the serum VEGF level(r=-0.606,P<0.05),and the expression of VEGF mRNA in the tumor tissue(r=-0.338,P<0.01).CONCLUSION:Blockage of hepatic arterial blood supply results in decreased blood perfusion and increased expression of metastasis-associated genes VEGF and MMP-1 of transplanted liver cancer in rats .Decreased blood Perfusion and hypoxia may be the major cause of upregulated expression of VEGF.Wei-Jian Guo Jie Li Wan-Long Ling Wen-Hua Gu Jun-Yan Zhuang,Department of Oncology,Xinhua Hospital of Shanghai Second Medical University,Shanghai 200092,China Yong-Rui Bai,Department of Radiotherapy,Xinhua Hospital of Shanghai Second Medical University,Shanghai 200092,China Wen-Zhu Zhang Yu-Fan Cheng,Department of Pathology,Xinhua Hospital of Shanghai Second Medical University,Shanghai 200092,China 2002World Journal of Gastroenterology2002,8,3:13
7Study on formation of non-metallic inclusions with lower melting temperatures in extra low oxygen special steels显示文摘Investigations were made both in laboratorial and industrial scales on formation of non-metallic inclusions with relatively lower melting temperatures to improve the fatigue property of the special steels which contained extra low oxygen.It was found in laboratory studies that steel/slag reaction time largely affected non-metallic inclusions in steel.With the slag/steel reaction time increasing from 30 to 90 min,inclusions of MgO-Al2O3 spinel were gradually changed into CaO-MgO-Al2O3 system inclusions which were surrounded by lower melting temperature softer CaO-Al2O3 surface layers.By using high basicity and as much as 41 mass% Al2O3 refining slag,the ratio of the lower melting temperature CaO-MgO-Al2O3 system inclusions was remarkably increased to above 80%.In the industrial experiments,it was found that the inclusions changed in the order of 'Al2O3 →MgO-Al2O3 system→CaO-MgO-Al2O3 system' in the secondary refining,and the change from MgO-Al2O3 system to CaO-MgO-Al2O3 system took place from the outside to the inside.The diffusion of CaO and MgO inside the layer of CaO-MgO-Al2O3 was considered as the controlling step of the inclusion transfer.Through LF and RH refining,most inclusions could be transferred to lower melting temperature CaO-Al2O3 and CaO-MgO-Al2O3 system inclusions.WANG XinHua JIANG Min CHEN Bing LI HaiBo 2012Science China(Technological Sciences)2012,55,7:12
8Secure Sensitive Data Sharing on a Big Data Platform显示文摘Users store vast amounts of sensitive data on a big data platform. Sharing sensitive data will help enterprises reduce the cost of providing users with personalized services and provide value-added data services.However, secure data sharing is problematic. This paper proposes a framework for secure sensitive data sharing on a big data platform, including secure data delivery, storage, usage, and destruction on a semi-trusted big data sharing platform. We present a proxy re-encryption algorithm based on heterogeneous ciphertext transformation and a user process protection method based on a virtual machine monitor, which provides support for the realization of system functions. The framework protects the security of users' sensitive data effectively and shares these data safely. 9An Estimation of Internal Soliton Forces on a Pile in the Ocean显示文摘Internal soliton forces on oil-platform piles in the ocean are estimated with the Morison Formula. Different from sur- face wave forces, which change only in magnitude along a pile, internal soliton forces can be distributed over the entire pile in the water and they change not only in magnitude but also in direction with depth. Our calculations show that the maximum total force caused by a soliton with its associated current of 2.1 m s-1 is nearly equal to the maximum total force exerted by a surface wave with a wavelength of 300 m and a wave-height of 18 m. The total internal soliton force is large enough to affect the operations of marine oil platforms and other facilities. 10A de novo Genome of a Chinese Radish Cultivar显示文摘Here, we report a high-quality draft genome of a Chinese radish(Raphanus sativus) cultivar. This draft contains 387.73 Mb of assembled scaffolds, 83.93% of the scaffolds were anchored onto nine pseudochromosomes and 95.09% of 43 240 protein-coding genes were functionally annotated. 184.75 Mb(47.65%) of repeat sequences was identified in the assembled genome. By comparative analyses of the radish genome against 10 other plant genomes, 2 275 genes in 780 gene families were found unique to R. sativus. 11Direction of arrival estimation and signal recovery based on single snapshot compressed sensing in frequency domain显示文摘Direction of arrival(DOA) estimation and signal recovery is the base of the underwater target localization,tracking and recognition.Based on the compressed sensing theory,a method for DOA estimation and source signal recovery is proposed using the single snapshot processing of the received array signal in frequency domain.The received array signal are transformed to frequency domain,and the single snapshot data in frequency domain are regarded as the measured data of the compressed sensing.According to the frequency,searching orientation and array manifold,the overcomplete array manifold is constructed as the sensing matrix of the compressed sensing.Both the target signal and power of the searching orientation are estimated by the basis pursuit method to complete DOA estimation and signal recovery.Simulation results show that the proposed method has a number of advantages over the minimum variance distortionless response(MVDR) method,including improved robustness to noise,fewer requirement in number of sensors and snapshots.And the correlation coefficient of the signal reaches up to 0.89.Experiment results in real environments verify that the proposed method performs more effectively in the detection of weak targets than the MVDR method and can be applied to real sonar system.KANG Chunyu LI Qianyan ZHANG Xinhua LI Jun 2016Chinese Journal of Acoustics2016,35,2:10
12SCM-198 protects endometrial stromal cells from oxidative damage through Bax/Bcl-2 and ERK signaling pathways显示文摘In creasi ng amounts of evidence dem on strated that accumulative reactive oxyge n species (ROS) and apoptosis of human endometrial stromal cells (ESCs) are closely associated with endometrial dysfunction induced by oxidative stress, which plays an important role in the pathological process of multiple gyn ecological and reproduction-related diseases. SCM-198, an alkaloid active component of Leonurus japonicas Houtt, has been reported to have anti-oxidative activity. However, the specific mechanisms of SCM-198 in the prevention of endometrial damage remain unknown. In the present study, we assessed the effect of SCM-198 on hydrogen peroxide (H2O2Hnduced oxidative injury in ESCs. ESCs were pretreated with SCM-198 for 4 h and then challenged with H2O2.Morphology ch a nges, apoptosis rate, and intracellular ROS producti on were measured to assess the level of oxidative injury. Flow cytometry and western blot analysis were performed to detect the expression levels of Bax, Bcl-2, active-caspase-3, and mitogen-activated protein kinases pathways. Classic inflammation cytokines were measured by real-time polymerase chain reactions. Our results showed that SCM-198 attenuated apoptosis and ROS generation of ESCs induced by H2O2. H2O2 induced the apparent apoptotic characteristics, including fragmentation of DNA, upregulation of Bax/Bcl2, activation of caspase-3, and secretion of inflammation cytokines, which were all ameliorated by SCM-198. Furthermore, H2O2-induced apoptosis-related ERK1/2 pathway activation was restrained by SCM-198 pretreatme nt. 15Success rate and complications of endoscopic extraction of common bile duct stones over 2 cm in diameter显示文摘BACKGROUND:Clinically,common bile duct (CBD) stones >2 cm are difficult to remove by endoscopic retrograde cholangiopancreatography (ERCP).To evaluate this observation,the rates of successful clearance of CBD stones and complications were compared between ERCP extraction of CBD stones of >2 cm and <2 cm in diameter.METHODS:All patients who had undergone endoscopic extraction of CBD stones at the Endoscopy Center of Shanghai First People's Hospital from May 2004 to May 2008 were reviewed.Patients with CBD stones of >2 cm in diameter were enrolled in the >2 cm group.Two matched controls with CBD stones of <2 cm in diameter were selected for each enrolled patient (<2 cm group).Patient characteristics,success rates,and complications during and after ERCP were compared.RESULTS:Seventy-two patients constituted the >2 cm group and 144 patients were in the <2 cm group.No significant differences were found in the patient characteristics,except for stone size and CBD diameter.Both the overall success rate and the success rate in the first ERCP session were lower in the >2 cm group (77.8% and 58.3%,respectively) than in the <2 cm group (91.7% and 83.3%,P<0.01).During ERCP,the incidence of hypoxemia (30.6%) and hemorrhaging papillae (18.1%) in the >2 cm group was higher than in the <2 cm group (13.2% and 6.3%,P<0.05).After ERCP,the rates of delayed papillae hemorrhage (13.9%),hyperamylasemia (23.6%),acute pancreatitis (8.3%) and biliary infection (18.1%) were higher in the >2 cm group than in the <2 cm group (3.5%,11.1%,2.1%,and 2.8%,respectively,P<0.05).CONCLUSION:The success rate of endoscopic extraction of CBD stones of >2 cm in diameter was lower but the complication rate was higher than that of stones of <2 cm in diameter.Xin-Jian Wan,Zheng-Jie Xu,Feng Zhu and Lei Li Department of Gastroenterology,Shanghai First People’s Hospital,Shanghai Jiaotong University,Shanghai 200080,China Department of Gastroenterology,Xinhua Hospital,Shanghai Jiaotong University Medical School,Shanghai 200092,China 2011Hepatobiliary & Pancreatic Diseases International2011,10,4:9
16Comparison of sentinel lymph node detection performances using blue dye in conjunction with indocyanine green or radioisotope in breast cancer patients: a prospective singlecenter randomized study显示文摘Objective:This randomized study aimed to compare the clinical efficacy between the novel dual tracer composed of indocyanine green(ICG)and blue dye(BD)and the conventional dual tracer composed of radioisotope and BD for sentinel lymph node(SLN)mapping in patients with breast cancer.Methods:This study enrolled 471 clinically lymph node-negative patients with primary breast cancer.All patients underwent mastectomy,and those undergoing sentinel lymph node biopsy(SLNB)were randomized to receive blue dye plus radioisotope(RB group)or BD plus ICG(IB group).The detection performances on SLN identification rate,positive SLN counts,detection sensitivity,and false-negative rate were compared between the two groups.Results:In the IB group,97%(194/200)of the patients who underwent the ICG and BD dual tracer injection showed fluorescentpositive lymphatic vessels within 2–5 min.The identification rate of SLNs was comparable between the IB group(99.0%,198/200)and the RB group(99.6%,270/271)(P=0.79).No significant differences were observed in the identification rate of metastatic SLNs(22.5%vs.22.9%,P>0.05,RB group vs.IB group,the same below),positive SLN counts(3.72±2.28 vs.3.91±2.13,P>0.05),positive metastatic SLN counts(0.38±0.84 vs.0.34±0.78,P>0.05),SLNB detection sensitivity(94.4%vs.92.5%,P>0.05),or false-negative rate(5.6%vs.7.5%,P>0.05)between the two groups.Conclusions:ICG can be used as a promising alternative tracer for radioisotope in SLN mapping,and when it is combined with BD in lymphangiography,it offers comparable detection sensitivity compared to the conventional lymphatic mapping strategies that are widely used in clinical practice.Long Yuan Xiaowei Qi Yi Zhang Xinhua Yang Fan Zhang Linjun Fan Li Chen Kongyong Zhang Ling Zhong Yanling Li Sijie Gan Wenying Fu Jun Jiang 2018Cancer Biology & Medicine2018,15,4:9
17BRAF^(V600E) vs. TIRADS in predicting papillary thyroid cancers in Bethesda system Ⅰ, Ⅲ, and Ⅴ nodules显示文摘Objective: Bethesda System for Reporting Thyroid Cytopathology(BSRTC) categories Ⅰ, Ⅲ, and Ⅴaccount for a significant proportion of fine needle aspiration cytology(FNAC) diagnoses. This study aimed to compare the diagnostic efficacy of BRAF^(V600E) mutation and the Thyroid Imaging Reporting and Data System(TIRADS) classification in differentiating papillary thyroid cancers(PTCs) from benign lesions among BSRTC I, III, and V nodules.Methods: A total of 472 patients with 479 nodules were enrolled in this prospective study. Ultrasound, BRAF^(V600E) mutation testing, and FNAC were performed in each nodule, followed by surgery or regular ultrasound examination.Results: In the BSRTC I category, BRAF^(V600E) showed similar sensitivity, higher specificity, and lower accuracy when compared with TIRADS. In the BSRTC III/V category, the sensitivity, specificity, and accuracy of BRAF^(V600E) were similar to those of TIRADS. In comparison to BRAF^(V600E) alone, the combination of the two methods significantly improved sensitivity(BSRTC Ⅰ:93.6% vs. 67.7%, P < 0.01; BSRTC Ⅲ: 93.8% vs. 75.0%, P < 0.01; BSRTC V: 96.0% vs. 85.3%, P < 0.001). When compared with TIRADS alone, the combination improved sensitivity in BSRTC Ⅰ nodules(93.6% vs. 74.2%, P < 0.05), increased sensitivity and decreased accuracy in BSRTC III nodules(93.8% vs. 75.0%, P < 0.01, 91.0% vs. 93.6%, P < 0.01), and improved both sensitivity and accuracy in BSRTC V nodules(96.0% vs. 82.0%, P < 0.001; 94.2% vs. 81.3%, P < 0.001).Conclusions: BRAF^(V600E) exhibited higher specificity and lower accuracy compared with TIRADS in BSRTC Ⅰ nodules, while the two methods showed similar diagnostic value in BSRTC Ⅲ/Ⅴ nodules. 18Cystathionine-γ-lyase ameliorates the histone demethylase JMJD3-mediated autoimmune response in rheumatoid arthritis显示文摘Cystathionine-γ-lyase(CSE),an enzyme associated with hydrogen sulfide(H2S)production,is an important endogenous regulator of inflammation.Jumonji domain-containing protein 3(JMJD3)is implicated in the immune response and inflammation.Here,we investigated the potential contribution of JMJD3 to endogenous CSE-mediated inflammation in rheumatoid arthritis(RA).Upregulated CSE and JMJD3 were identified in synovial fibroblasts(SFs)from RA patients as well as in the joints of arthritic mice.Knocking down CSE augmented inflammation in IL-1β-induced SFs by increasing JMJD3 expression.In addition,CSE−/−mice with collagen-induced arthritis(CIA)developed severe joint inflammation and bone erosion.Conversely,overexpressing CSE inhibited JMJD3 expression by the transcription factor Sp-1 and was accompanied by reduced inflammation in IL-1β-treated SFs.Furthermore,JMJD3 silencing or the administration of the JMJD3 inhibitor GSK-J4 significantly decreased the inflammatory response in IL-1β-treated SFs,mainly by controlling the methylation status of H3K27me3 at the promoter of its target genes.GSK-J4 markedly attenuated the severity of arthritis in CIA mice.In conclusion,suppressing JMJD3 expression by the transcription factor Sp-1 is likely responsible for the ability of CSE to negatively modulate the inflammatory response and reduce the progression of RA.Weijun Wu Ming Qin Wanwan Jia Zheng Huang Zhongzheng Li Di Yang Mengwei Huang Chenxi Xiao Fen Long Jianchun Mao Philip K.Moore Xinhua Liu Yi Zhun Zhu 2019Cellular & Molecular Immunology2019,16,8:9
19Prognostic value and nomograms of proximal margin distance in gastric cancer with radical distal gastrectomy显示文摘Objective:The proximal margin(PM)distance for distal gastrectomy(DG)of gastric cancer(GC)remains controversial.This study investigated the prognostic value of PM distance for survival outcomes,and aimed to combine clinicopathologic variables associated with survival outcomes after DG with different PM distance for GC into a prediction nomogram.Methods:Patients who underwent radical DG from June 2004 to June 2014 at Department of General Surgery,Nanfang Hospital,Southern Medical University were included.The first endpoints of the prognostic value of PM distance(assessed in 0.5 cm increments)for disease-free survival(DFS)and overall survival(OS)were assessed.Multivariate analysis by Cox proportional hazards regression was performed using the training set,and the nomogram was constructed,patients were chronologically assigned to the training set for dates from June 1,2004 to January 30,2012(n=493)and to the validation set from February 1,2012 to June 30,2014(n=211).Results:Among 704 patients with p TNM stage I,p TNM stage II,T1-2,T3-4,N0,differentiated type,tumor size≤5.0 cm,a PM of(2.1-5.0)cm vs.PM≤2.0 cm showed a statistically significant difference in DFS and OS,while a PM>5.0 cm was not associated with any further improvement in DFS and OS vs.a PM of 2.1-5.0 cm.In patients with p TNM stage III,N1,N2-3,undifferentiated type,tumor size>5.0 cm,the PM distance was not significantly correlated with DFS and OS between patients with a PM of(2.1-5.0)cm and a PM≤2 cm,or between patients with a PM>5.0 cm and a PM of(2.1-5.0)cm,so there were no significant differences across the three PM groups.In the training set,the C-indexes of DFS and OS,were 0.721 and 0.735,respectively,and in the validation set,the C-indexes of DFS and OS,were 0.752 and 0.751,respectively.Conclusions:It is necessary to obtain not less than 2.0 cm of PM distance in early-stage disease,while PM distance was not associated with long-term survival in later and more aggressive stages of disease because more advanced GC is a systemic disease.Different types of patients should be considered for removal of an individualized PM distance intra-operatively.We developed a universally applicable prediction model for accurately determining the 1-year,3-year and 5-year DFS and OS of GC patients according to their preoperative clinicopathologic characteristics and PM distance.Jun Luo Yuming Jiang Xinhua Chen Yuehong Chen Jhang Lopsang Gurung Tingyu Mou Liying Zhao Guoqing Lyu Tuanjie Li Guoxin Li Jiang Yu 2020Chinese Journal of Cancer Research2020,32,2:8
20Effects of High-temperature Char Layer and Pyrolysis Gas on NOx Reduction in a Typical Decoupling Combustion Coal-fired Stove显示文摘The suppression of nitrogen oxides(NO_x) is the key to reducing pollutant emission of a domestic coal-fired stove due to the limitation of technology condition and economic cost. The decoupling combustion(DC) technology invented by Institute of Process Engineering(IPE), Chinese Academy of Sciences(CAS) is characterized by that a traditional stove is separated into a pyrolysis and a combustion chamber as well as a bottom passage between them. In this study, the combustion of briquette from bituminous coal in different operation modes in a typical decoupling stove is tested and simulated to validate the advantage of DC technology over so-called reverse combustion. The smokeless and high-efficiency combustion of bituminous briquette with low emissions of NO_x and CO can be implemented by utilizing low NO_x combustion under low temperature and reduction atmosphere in the pyrolysis chamber as well as after-combustion of char and pyrolysis gas under high temperature and oxidation atmosphere in the combustion chamber.
