Chinese Stroke Center Alliance:a national effort to improve healthcare quality for acute stroke and transient ischaemic attack:rationale, design and preliminary findings显示文摘Background In June 2015,the Chinese Stroke Association(CSA)initiated the Chinese Stroke Center Alliance(CSCA)to establish the national hospital-based stroke care quality assessment and improvement platform. This article outlines its objectives, operational structure, patient population, quality improvement(QI)intervention tools, data elements, data collection methodology and current patient and hospital data. Methods The CSCA is a national,hospital-based,multicentre,voluntary,multifaceted intervention and continuous QI initiative. This multifaceted intervention includes stroke centre development, written care protocols, workshops and a monitoring/feedback system of evidencebased performance measures. The data coordinating centre of the CSCA resides at the China National Clinical Research Center for Neurological Diseases,Beijing Tiantan Hospital.results As of July 2017,1576 hospitals in China have contributed detailed clinical information to serve as a benchmark for the stroke care quality of 433264 patients with acute stroke/transient ischaemic attacks(TIA),including 352572(81.38%)acute ischaemic stroke,30362(7.01%)TIA,42080(9.71%)spontaneous intracranial haemorrhage,5505(1.27%)subarachnoid haemorrhage and 2745(0.63%)not specified stroke. Conclusion The CSCA programme is designed to establish a continuous national stroke registry and help healthcare providers develop stroke centres and treat patients in a consistent manner in accordance with accepted national guidelines and, ultimately, improve patient outcomes. It supports the CSA mission to reduce stroke burden in China.Yongjun Wang Zixiao Li Yilong Wang Xingquan Zhao Liping Liu Xin Yang Caiyun Wang Hongqiu Gu Fuying Zhang Chunjuan Wang Ying Xian David Z Wang Qiang Dong Anding Xu Jizong Zhao Chinese Stroke Center Alliance investigators 2018Stroke & Vascular Neurology2018,3,4:38
Stenting for symptomatic intracranial arterial stenosis in China:1-year outcome of a multicentre registry study显示文摘background and purpose A multicentre prospective registry study of individually tailored stenting for a patient with symptomatic intracranial atherosclerotic stenosis(ICAS)combined with poor collaterals in China showed that the short-term safety and efficacy of stenting was acceptable. However,it remained uncertain whether the low event rate could be of a long term.We reported the 1-year outcome of this registry study to evaluate the long-term efficacy of individually tailored stenting for patients with severe symptomatic ICAS combined with poor collaterals. Methods Patients with symptomatic ICAS caused by 70%-99% stenosis located at the intracranial internal carotid, middle cerebral, intracranial vertebral or basilar arteries combined with poor collaterals were enrolled.Balloon-mounted stent or balloon plus self-expanding stent were selected based on the ease of vascular access and lesion morphology determined by the operators. The primary outcome was the rate of 30-day stroke, transient ischaemic attack and death, and 12-month ischaemic stroke within the same vascular territory, haemorrhagic stroke and vascular death after stenting.results From September 2013 to January 2015,300 patients(ages 58.3±9.78 years)were recruited.Among them,159 patients were treated with balloon-mounted stent and 141 with balloon plus self-expanding stent.During the 1-year follow-up,25 patients had a primary end point event. The probability of primary outcome at 1 year was 8.1%(95% CI 5.3% to 11.7%).In 76 patients with digital subtraction angiography follow-up,27.6%(21/76)had re-stenosis≥50% and 18.4%(14/76)had re-stenosis≥70%.No baseline characteristic was associated with the primary outcome. Conclusion The event rate remains low over 1 year of individually tailored stenting for patients with severe symptomatic ICAS combined with poor collaterals.Further randomised trial of comparing individually tailored stenting with best medical therapy is needed.Ning Ma Yong Zhang Jie Shuai Changchun Jiang Qiyi Zhu Kangning Chen Li Liu Baomin Li Xiangqun Shi Lianbo Gao Yajie Liu Feng Wang Yongli Li Tieyan Liu Hongbo Zheng Dapeng Mo Feng Gao Yilong Wang Yongjun Wang Lei Feng Zhongrong Miao 2018Stroke & Vascular Neurology2018,3,3:29
Application of next-generation sequencing technology to precision medicine in cancer: joint consensus of the Tumor Biomarker Committee of the Chinese Society of Clinical Oncology显示文摘Next-generation sequencing(NGS) technology is capable of sequencing millions or billions of DNA molecules simultaneously. Therefore, it represents a promising tool for the analysis of molecular targets for the initial diagnosis of disease, monitoring of disease progression, and identifying the mechanism of drug resistance. 4Cerebral small vessel disease or intracranial large vessel atherosclerosis may carry different risk for future strokes显示文摘Background The effect of cerebral small vessel disease(CSVD)and intracranial arterial stenosis(ICAS)on stroke outcomes remains unclear. Methods Data of 1045 patients with minor stroke or transient ischaemic attack(TIA)were obtained from 45 sites of the Clopidogrel in High-Risk Patients with Acute Non-disabling Cerebrovascular Events(CHANCE)trial. We assessed the associations of burdens of CSVD and ICAS with new strokes and bleeding events using multivariate Cox regression models and those with modified Rankin Scale(mRS)scores using ordinal logistic regression models. Results Among the 1045 patients,CSVD was present in 830 cases(79.4%)and ICAS in 460(44.0%).Patients with>1 ICAS segment showed the highest risk of new strokes(HR 2.03,95%CI 1.15 to 3.56,p=0.01).No association between CSVD and the occurrence of new strokes was found. The presence of severe CSVD(common OR(cOR)2.01,95%CI 1.40 to 2.89,p<0.001)and>1 ICAS segment(cOR 2.15,95%CI 1.57 to 2.93,p<0.001)was associated with higher mRS scores.Severe CSVD(HR 10.70,95%CI 1.16 to 99.04,p=0.04),but not ICAS,was associated with a higher risk of bleeding events.Six-point modified CSVD score improved the predictive power for bleeding events and disability.Interpretation CSVD is associated with more disability and bleeding events, and ICAS is associated with an increased risk of stroke and disability in patients with minor stroke and TIA at 3 months.CSVD and ICAS may represent different vascular pathologies and play distinct roles in stroke outcomes.Huimin Chen Yuesong Pan Lixia Zong Jing Jing Xia Meng Yuyuan Xu Hongyi Yan Xingquan Zhao Liping Liu Hao Li S Claiborne Johnston Yongjun Wang Yilong Wang 2020Stroke & Vascular Neurology2020,5,2:13
Chinese Stroke Association guidelines for clinical management of cerebrovascular disorders:executive summary and 2019 update of the management of high-risk population显示文摘Aim Cerebrovascular disease is the leading cause of death and disability in China, causing a huge burden among patients and their families.Hence, stroke prevention is critical, especially in the high-risk population. Here,we present the evidence-based guideline suitable for the Chinese population. Methods Literature search of PubMed and Cochrane library(from January 1964 to June 2019)was done. After thorough discussion among the writing group members, recommendations were listed and summarised. This guideline was reviewed and discussed by the fellow writing committees of the Chinese Stroke Association’s Stroke. Results This evidence-based guideline was written in three parts:controlling the risk factors of stroke, utilisation of antiplatelet agents and assessing the risks of first-ever stroke. All recommendations were listed along with the recommending classes and levels of evidence. Conclusions This guideline provides recommendations for primary prevention of cerebrovascular disease among high-risk population in China.Controlling related risk factors, appropriately using antiplatelet agents, assessing the risk of developing first-ever stroke should help reduce the rate of cerebrovascular disease in China.Yilong Wang Shangrong Han Haiqiang Qin Huaguang Zheng Bin Jiang Yong Cao Yuan Gao Ling Guan Qian Jia Yong Jiang Yuming Jiao Shuya Li Yapeng Li Zixiao Li Wei Liu Xiaojuan Ru Dongling Sun Haixin Sun Penglian Wang Tingting Wang Lixia Zong Lei Guo Xuewei Xie Yuming Xu Yuyuan Xu Xiaomeng Yang Yingying Yang Mengyuan Zhou Wenzhi Wang Chinese Stroke Association Stroke Council Guideline Writing Committee 2020Stroke & Vascular Neurology2020,5,3:11
Association of elevated hs-CRP and multiple infarctions with outcomes of minor stroke or TIA: subgroup analysis of CHANCE randomised clinical trial显示文摘Background and purpose The relationship of high-sensitive C-reactive protein(hs-CRP)levels and infarction numbers with the prognosis of stroke is uncertain. This study evaluated the association of different hs-CRP levels and infarction numbers with the prognosis of acute minor ischaemic stroke or transient ischaemic attack(TIA).Methods A subset of 807 patients with both hs-CRP measurement and baseline MRI was included from the Clopidogrel in High-risk Patients with Acute Non-disabling Cerebrovascular Events trial. The primary efficacy outcome was the occurrence of an ischaemic stroke at the 1-year follow-up.Infarction numbers were classified as multiple acute infarctions(MAIs),single acute infarction and no acute infarction(NAI).The association between different hs-CRP levels with different infarction numbers and the risk of any outcome was analysed using multivariable Cox regression models. Results Among the 807 patients,84(10.4%)patients had a recurrent ischaemic stroke within 1 year. After adjustment for conventional confounding factors, patients with both elevated hs-CRP levels and MAIs were associated with approximately 4.7-fold of risk of ischaemic stroke within 1 year(16.7% vs 3.5%,HR 4.68,95% CI 1.54 to 14.23,p=0.007),compared with those with non-elevated hs-CRP levels and NAI.Similar results were observed for the composite events. Conclusions Combined elevated hs-CRP levels and MAIs may increase 1-year stroke risk stratification efficiency in patients with minor ischaemic stroke or TIA compared with using those markers alone, which indicated that the combination of inflammatory and imaging markers might improve the effectiveness of risk stratification concerning minor ischaemic stroke or TIA.Guangyao Wang Jing Jing Jiejie Li Yuesong Pan Hongyi Yan Xia Meng Xingquan Zhao Liping Liu Hao Li David Z Wang Yongjun Wang Yilong Wang 2021Stroke & Vascular Neurology2021,6,1:11
Concurrent intracranial and extracranial artery stenosis and the prognosis of transient ischaemic symptoms or imaging-negative ischaemic stroke显示文摘Background and purpose Transient ischaemic attack(TIA),transient symptoms with infarction(TSI)and diffusion-weighted imaging(DWI)-negative acute ischaemic stroke(AIS)share similar aetiologies but are considered to have a rather benign prognosis. We intended to investigate the association between intracranial atherosclerotic stenosis(ICAS),extracranial atherosclerotic stenosis(ECAS)and the prognosis of patients with TIA,TSI and DWI-negative AIS.Methods Clinical and imaging data of eligible participants were derived from the Chinese Intracranial Atherosclerosis study, according to symptom duration, acute infarction on DWI and discharge diagnosis.Based on the severity and location of arterial atherosclerosis,we categorised the study population into four groups:no or<50% ICAS and no ECAS;≥50% ICAS but no ECAS;no or<50%ICAS with ECAS;and concurrent≥50% ICAS and ECAS.Using multivariable Cox regression models,we analysed the relationship between the severity and distribution of large artery atherosclerosis and the prognosis of TIA,TSI and DWI-negative AIS.Results A total of 806 patients were included,67.3% of whom were male. The median age of the study participants was 63 years.Patients in the concurrent≥50% ICAS and ECAS subgroup had both a significantly higher 1-year recurrence rate(adjusted HR 3.4(95%CI 1.15 to 10.04),p=0.027)and a higher risk of composite vascular events(adjusted HR 3.82(95%CI 1.50 to 9.72),p=0.005).Conclusions Concurrent ICAS and ECAS is associated with a higher possibility of 1-year recurrent stroke or composite vascular events.Large artery evaluation is necessary to assess patients with transient ischaemic symptoms or DWI-negative AIS.Progress in shortening the time interval between symptom onset and large vessel evaluation is needed.Yue Suo Jing Jing Yuesong Pan Weiqi Chen Hongyu Zhou Hao Li Yuehua Pu Liping Liu Xingquan Zhao Yilong Wang Xia Meng Yongjun Wang 2021Stroke & Vascular Neurology2021,6,1:10
Endovascular therapy for Acute ischemic Stroke Trial (EAST): study protocol for a prospective, multicentre control trial in China显示文摘Background and purpose:5 recent trials have shown the benefit of endovascular treatment for acute ischaemic stroke(AIS)due to large vessel occlusion of the anterior circulation. This study aims to evaluate the safety and efficacy of Solitaire thrombectomy in patients with moderate-to-severe stroke in the Chinese population, which has a high prevalence of intracranial atherosclerosis. Methods and analysis:This multicentre prospective control study will involve 17 stroke centres in China, and plans to recruit 150 patients in the intervention group, and 150 patients in the medical group,in which patients meet enrolment criteria but refuse intervention.Patients with AIS due to large vessel occlusion indicated for treatment with Solitaire stent retriever within 12 hours of symptom onset, and who meet the inclusion and exclusion criteria, will be enrolled in this study. The primary efficacy endpoint is functional independence as defined by a modified Rankin Scale(mRS)score≤2 at 90 days or by functional improvement as defined by mRS,using shift analysis. The procedural efficacy endpoint is arterial recanalisation of the occluded target vessel measured by a modified Thrombolysis in Cerebral Infarction(mTICI)score equal or superior to 2b right following the use of the study device. The primary safety endpoint is symptomatic intracranial haemorrhage(sICH)within 24±3 hours postprocedure.Ethics and dissemination:The protocol was approved by the Ethics Committee at the coordinating centre and by the local Institutional Review Board of each participating centre.Trial registration number:NCT02350283.Zhongrong Miao Xiaochuan Huo Feng Gao Xiaoling Liao Chunjuan Wang Ya Peng Yibin Cao Shengli Chen Meng Zhang Changchun Jiang Xiaoxiang Peng Cunfeng Song Liping Wei Qiyi Zhu Zaiyu Guo Li Liu Hang Lin Hua Yang Wei Wu Hui Liang Anding Xu Kangning Chen Xingquan Zhao Yuesong Pan Hao Li Liping Liu Yilong Wang Yongjun Wang EAST investigators 2016Stroke & Vascular Neurology2016,1,2:8
DELLA and EDS1 Form a Feedback Regulatory Module to Fine-Tune Plant Growth-Defense Tradeoff in Arabidopsis显示文摘Plants maintain a dynamic balance between growth and defense, and optimize allocation of resources for survival under constant pathogen infections. However, the underlying molecular regulatory mechanisms, especially in response to biotrophic bacterial infection, remain elusive. Here,we demonstrate that DELLA proteins and EDS1,an essential resistance regulator, form a central module modulating plant growth-defense tradeoffs via direct interaction. When infected by Pst DC3000,EDS1 rapidly promotes salicylic acid(SA)biosynthesis and resistance-related gene expression to prime defense response, while pathogen infection stabilizes DELLA proteins RGA and RGL3 to restrict growth in a partially EDS1-dependent manner, which facilitates plants to develop resistance to pathogens. However, the increasingly accumulated DELLAs interact with EDS1 to suppress SA overproduction and excessive resistance response.Taken together, our findings reveal a DELLA-EDS1-mediated feedback regulatory loop by which plants maintain the subtle balance between growth and defense to avoid excessive growth or defense in response to constant biotrophic pathogen attack.Yuge Li Yuhua Yang Yilong Hu Hailun Liu Ming He Ziyin Yang Fanjiang Kong Xu Liu Xingliang Hou 2019Molecular Plant2019,12,11:8
Whole genome sequencing of 10K patients with acute ischaemic stroke or transient ischaemic attack: design, methods and baseline patient characteristics显示文摘Background and purpose Stroke is the second leading cause of death worldwide and the leading cause of mortality and long-term disability in China, but its underlying risk genes and pathways are far from being comprehensively understood. We here describe the design and methods of whole genome sequencing(WGS)for 10914 patients with acute ischaemic stroke or transient ischaemic attack from the Third China National Stroke Registry(CNSR-III).Methods Baseline clinical characteristics of the included patients in this study were reported.DNA was extracted from white blood cells of participants.Libraries are constructed using qualified DNA,and WGS is conducted on BGISEQ-500 platform. The average depth is intended to be greater than 30×for each subject.Afterwards,Sentieon software is applied to process the sequencing data under the Genome Analysis Toolkit best practice guidance to call genotypes of single nucleotide variants(SNVs)and insertion-deletions.For each included subject,21 fingerprint SNVs are genotyped by MassARRAY assays to verify that DNA sample and sequencing data originate from the same individual. The copy number variations and structural variations are also called for each patient. All of the genetic variants are annotated and predicted by bioinformatics software or by reviewing public databases. Results The average age of the included 10914 patients was 62.2±11.3 years, and 31.4%patients were women. Most of the baseline clinical characteristics of the 10914 and the excluded patients were balanced. Conclusions The WGS data together with abundant clinical and imaging data of CNSR-III could provide opportunity to elucidate the molecular mechanisms and discover novel therapeutic targets for stroke.Si Cheng Zhe Xu Yang Liu Jinxi Lin Yong Jiang Yilong Wang Xia Meng Anxin Wang Xinying Huang Zhimin Wang Guohua Chen Songdi Wu Zhengchang Jia Yongming Chen Xuerong Qiu Jun Wu Binbin Song Weizhong Ji Zhongping An Wenjun Xue Lili Zhao Yu Geng Hongyan Li Hao Li Yongjun Wang 2021Stroke & Vascular Neurology2021,6,2:8
Gastrointestinal bleeding during acute ischaemic stroke hospitalisation increases the risk of stroke recurrence显示文摘Objective Gastrointestinal(GI)bleeding in patients who had a stroke is strongly associated with a higher risk of death and loss of independence. However,it is unknown whether GI bleeding increases risk for recurrence of stroke. In this study,we assess the potential relationship between GI bleeding and stroke recurrence in patients within 12 months of an acute ischaemic stroke(AIS),using the China National Stroke Registry(CNSR).Methods This study included 22216 patients who had an ischaemic stroke included in the CNSR from 2007 to 2008.We analysed baseline patient characteristics,GI bleeding and outcomes of patients who had an AIS,specifically stroke recurrence at 3,6 and 12 months. We used multivariable logistic regression to evaluate a possible association between GI bleeding and stroke recurrence.results Of the 12415 patients included in our study,12.3%,15.5%and 17.7%had a stroke recurrence at 3,6 and 12 months, respectively.GI bleeding was an independent stroke recurrence risk factor in patients after ischaemic stroke at 3 months(adjusted OR 1.481,95%CI 1.118 to 1.962),6 months(adjusted OR 1.448,95%CI 1.106 to 1.896)and 12 months(adjusted OR 1.350;95%CI 1.034 to 1.763).Conclusion GI bleeding was associated with the increased risk of stroke recurrence after an AIS.Wanliang Du Xingquan Zhao Yilong Wang Yuesong Pan Gaifen Liu Anxin Wang Ruijun Ji Liping Liu Hongqiu Gu Kehui Dong Penglian Wang Yongjun Wang 2020Stroke & Vascular Neurology2020,5,2:7
Advances in environmental behaviors and effects of dissolved organic matter in aquatic ecosystems显示文摘Dissolved organic matter(DOM) is a group of compounds that have complex chemical structures and multiple interactions with their surrounding materials. More than one trillion tons of DOM are stocked in the world's aquatic ecosystems. DOM is a very important part of aquatic ecosystem productivity and plays a crucial role in global carbon cycling. DOM has rich environmental behaviors and effects such as influencing the bioavailability of contaminants, serving as an important inducer of reactive oxygen species(ROS), and protecting aquatic organisms from the harm of dangerous ultraviolet radiation. There have been many systematic studies on the composition, structure, and sources of DOM because such studies are much easier to conduct than studies on the environmental behaviors and effects of DOM. Due to many factors, the research systems of DOM's environmental behaviors and effects are still being developed and have become a hotspot of environmental science. This review paper focuses on some critical progress, problems, and trends of DOM's environmental behaviors and effects in aquatic ecosystems, including mutual exchange mechanisms between DOM and particulate organic matter(POM) with influencing factors, photochemical behaviors of DOM especially inducing ROS, binding interactions between DOM and anthropogenic organic contaminants(AOC), interactions between DOM and microorganisms, effects of DOM on pollutants' bioavailability, ecotoxicity, and ecological risks. Hopefully, this paper will contribute to a more systematic understanding of the DOM environmental behaviors and effects and to promoting further relevant studies.HE Wei BAI ZeLin LI YiLong KONG XiangZhen LIU WenXiu YANG Chen YANG Bin XU FuLiu 2016Science China Earth Sciences2016,59,4:6
Risk factors associated with 90-day recurrent stroke in patients on dual antiplatelet therapy for minor stroke or high-risk TIA:a subgroup analysis of the CHANCE trial显示文摘Objective Many patients receiving dual antiplatelet therapy still had recurrent strokes. We aimed to identify factors associated with recurrent stroke at 90 days in patients receiving dual antiplatelet therapy in Clopidogrel in High-risk patients with Acute Non-disabling Cerebrovascular Events trial. Methods Patients with transient ischaemic attack or minor stroke receiving clopidogrel and aspirin in the trial were analysed in the study. The primary outcome was recurrent stroke within 90 days after the index event.Cox proportional hazard model with backward selection was used to identify factors associated with stroke.results Among 2584 patients,212(8.2%)had a recurrent stroke,216(8.4%)had a composite of stroke, myocardial infarction,or vascular death and 204(7.9%)had ischaemic stroke within 90 days.Multivariate analysis identified the following factors associated with stroke:history of hypertension with poor blood pressure control(HR,1.92;95%CI 1.22 to 3.03),the high baseline National Institute of Health Stroke Scale(NIHSS)score of 2 and 3(2.12(1.07 to 4.21)and 4.11(2.05 to 8.22),respectively),time from onset to randomisation of<12 hours(1.47(1.12 to 1.94)),the lipid-lowering therapy(0.61(0.47 to 0.83)),the open-label aspirin dose at day 1 of≥300 mg(1.98(1.45 to 2.69)).Intracranial arterial stenosis(ICAS)was significantly associated with stroke in the sensitivity analysis(2.17(1.16 to 4.04)).conclusions The high baseline NIHSS score, hypertension with poor blood pressure control,ICAS,time from onset to randomisation of less than 12 hours and no lipid-lowering therapy were associated with stroke, suggesting that patients with identified predictors still remain to be at high risk of recurrent stroke although being under the dual antiplatelet therapy.Runqi Wangqin Xianwei Wang Yilong Wang Ying Xian Xingquan Zhao Liping Liu Hao Li Xia Meng Yongjun Wang 2017Stroke & Vascular Neurology2017,2,4:6
A novel STING agonist-adjuvanted pan-sarbecovirus vaccine elicits potent and durable neutralizing antibody and T cell responses in mice, rabbits and NHPs显示文摘The emergence of SARS-CoV-2 variants and potentially other highly pathogenic sarbecoviruses in the future highlights the need for pan-sarbecovirus vaccines. Here,we discovered a new STING agonist,CF501,and found that CF501-adjuvanted RBD-FC vaccine(CF501/RBD-FC)elicited significantly stronger neutralizing antibody(nAb)and T cell responses than Alum-and cGAMP-adjuvanted RBD-FC in mice.Vaccination of rabbits and rhesus macaques(nonhuman primates,NHPs)with CF501/RBD-FC elicited exceptionally potent nAb responses against SARS-CoV-2 and its nine variants and 41 S-mutants,SARS-CoV and bat SARSr-CoVs.CF501/RBD-FC-immunized hACE2-transgenic mice were almost completely protected against SARS-CoV-2 challenge, even 6 months after the initial immunization.NHPs immunized with a single dose of CF501/RBD-FC produced high titers of nAbs.The immunized macaques also exhibited durable humoral and cellular immune responses and showed remarkably reduced viral load in the upper and lower airways upon SARS-CoV-2 challenge even at 108 days post the final immunization.Thus,CF501/RBD-Fc can be further developed as a novel pan-sarbecovirus vaccine to combat current and future outbreaks of sarbecovirus diseases.Zezhong Liu Jie Zhou Wei Xu Wei Deng Yanqun Wang Meiyu Wang Qian Wang Ming Hsieh Jingming Dong Xinling Wang Weijin Huang Lixiao Xing Miaoling He Chunlin Tao Youhua Xie Yilong Zhang Youchun Wang Jincun Zhao Zhenghong Yuan Chuan Qin Shibo Jiang Lu Lu 2022Cell Research2022,32,3:5
The efficacy and safety of nimodipine in acute ischemic stroke patients with mild cognitive impairment: a double-blind, randomized,placebo-controlled trial显示文摘Nimodipine might be effective in subcortical vascular dementia(VaD). Its benefit in preventing further cognitive decline in patients with acute ischemic stroke(AIS) and vascular mild cognitive impairment(VaMCI) remains to be established. In this multicenter, double-blind trial, we randomly assigned 654 eligible patients to nimodipine 30 mg three times a day or placebo. The primary outcome was any cognitive decline defined by the changes on the Mini-Mental State Examination(DMMSE à3) or vascular AD assessment scale cognitive subscale(DADAS-cog ! 4) at 6 months. Secondary outcomes included any distribution shift of DADAS-cog, DMMSE or cognitive improvement defined by DADAS-cog à2, or DMMSE ! 0. The primary outcome in the nimodipine group and placebo group were similar for DMMSE à3(4.18% and 7.22%, respectively, P = 0.15) and DADAS-cog ! 4(8.36% and 8.93% respectively,P = 0.88). The distribution shift of DADAS-cog and DMMSE differed significantly between the two groups(P = 0.03 and P = 0.05 respectively). Cognitive improvement occurred in 55.4% in the nimodipine group and 43.6% in the placebo group measured by DADAS-cog à2(Odds Ratio, 1.54; 95% confidence interval[CI] 1.10–2.14, P < 0.01) or 84.0% and 74.6% respectively by DMMSE ! 0(Odds Ratio, 1.79; 95% CI 1.18–2.70, P < 0.01). Nimodipine was associated with better cognitive function in the memory domain. The adverse events rate was similar in two groups. This study is registered with ClinicalTrials.gov,NCT01220622. Nimodipine did not show benefit to prevent cognitive decline in AIS patients with VaMCI, but improved cognition moderately, especially measured in the memory domain.Huaguang Zheng Yilong Wang Anxin Wang Hao Li David Wang Xingquan Zhao Penglian Wang Haipeng Shen Lijun Zuo Yuesong Pan Zixiao Li Xia Meng Xianwei Wang Weixiong Shi Yi Ju Liping Liu Kehui Dong Chunxue Wang Rubo Sui Rong Xue Xiaoping Pan Xiaoyua Niu Benyan Luo Yi Sui Huali Wang Tao Feng Yongjun Wang 2019Science Bulletin2019,64,2:5
A pan-sarbecovirus vaccine induces highly potent and durable neutralizing antibody responses in non-human primates against SARS-CoV-2 Omicron variant显示文摘Dear Editor,Since the outbreak of Coronavirus Disease 2019(COVID-19)caused by severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infection,numerous SARS-CoV-2 variants of concern(VOCs),such as Alpha(B.1.1.7),Beta(B.1.351),Gamma(P.1),and Delta(B.1.617.2),have emerged.Recently, the newly identified SARS-CoV-2 VOC,Omicron(B.1.1.529),has rapidly spread in many countries1 and is expected to replace Delta as the dominant variant circulating in the world.Compared to other VOCs,Omicron contains at least 32 mutations in spike(S)protein, including 15 mutations in the receptor-binding domain(RBD),and these mutations are known to confer resistance to neutralizing antibodies(nAbs)and sera of convalescent patients and people who have received COVID-19 vaccines.2–5 Therefore, vaccines able to induce potent and durable neutralizing antibodies against Omicron and other VOCs are urgently needed.Zezhong Liu Jasper Fuk-Woo Chan Jie Zhou Meiyu Wang Qian Wang Guangxu Zhang Wei Xu Kenn Ka-Heng Chik Yilong Zhang Youchun Wang Kwok-Yung Yuen Lu Lu Shibo Jiang 2022Cell Research2022,32,5:4
Temporal and spatial cellular and molecular pathological alterations with single-cell resolution in the adult spinal cord after injury显示文摘Spinal cord injury(SCI)involves diverse injury responses in different cell types in a temporally and spatially specific manner. Here, using single-cell transcriptomic analyses combined with classic anatomical, behavioral, electrophysiological analyses,we report, with single-cell resolution, temporal molecular and cellular changes in crush-injured adult mouse spinal cord.Data revealed pathological changes of 12 different major cell types, three of which infiltrated into the spinal cord at distinct times post-injury.We discovered novel microglia and astrocyte subtypes in the uninjured spinal cord, and their dynamic conversions into additional stage-specific subtypes/states.Most dynamic changes occur at 3-days post-injury and by day-14 the second wave of microglial activation emerged, accompanied with changes in various cell types including neurons, indicative of the second round of attacks.By day-38,major cell types are still substantially deviated from uninjured states, demonstrating prolonged alterations. This study provides a comprehensive mapping of cellular/molecular pathological changes along the temporal axis after SCI,which may facilitate the development of novel therapeutic strategies, including those targeting microglia.Chen Li Zhourui Wu Liqiang Zhou Jingliang Shao Xiao Hu Wei Xu Yilong Ren Xingfei Zhu Weihong Ge Kunshan Zhang Jiping Liu Runzhi Huang Jing Yu Dandan Luo Xuejiao Yang Wenmin Zhu Rongrong Zhu Changhong Zheng Yi Eve Sun Liming Cheng 2022Signal Transduction and Targeted Therapy2022,7,4:4
Dampness in dwellings and its associations with asthma and allergies among children in Chongqing: A cross-sectional study显示文摘The city of Chongqing has high outdoor temperatures and humidity throughout the year and consequently a high risk of dampness and mold problems in dwellings. As part of the China,Children,Homes,Health(CCHH),associations between home characteristics and children’s asthma, allergies, and related symptoms were investigated in Chongqing,China.A cross-sectional questionnaire survey on home characteristics including dampness and symptoms of asthma and allergies in 5299 children was performed.Data for 4754 children(3–6 years)and their homes were analyzed. Results showed that 35.1%of homes were reported to have'damp bedding',14.3%'condensation on window panes(higher than 5 cm)',11.6%'mouldy odor',9.3%'water leakage',8.3%'damp stains'and 5.3%'visible mould'.Wheezing and rhinitis ever were reported for 27.0%and 51.1%children respectively, and rhinitis, wheezing, cough at night and eczema in the last 12 months were reported for 38.1%,20.5%,18.9%and 13.2%respectively.Doctor-diagnosed asthma was reported for 8.3%of children, and doctor-diagnosed rhinitis for 6.2%.Dampness in homes was generally strongly associated with asthma and allergies among Chongqing children. The strongest association was found between'visible mould'and doctor-diagnosed rhinitis, and the adjusted Odds Ratio(ORA)was 2.27(95%CI:1.48,3.49).'Damp bedding'was significantly associated with all asthma and allergic symptoms, but doctor-diagnosed asthma and rhinitis. The behavior of'putting bedding to sunshine'could effectively reduce the prevalence of asthma and allergic symptoms, and the risk of bedding affected with damp was reduced significantly when bedding was put to sunshine frequently.WANG Han LI BaiZhan YANG Qin YU Wei WANG Juan LIU YiLong OU YangJin SUNDELL Jan 2013Chinese Science Bulletin2013,58,34:4
Dual versus mono antiplatelet therapy for acute non-cardioembolic ischaemic stroke or transient ischaemic attack:a systematic review and meta-analysis显示文摘Objective Recent years have seen new evidence on the efficacy and safety of dual antiplatelet therapy for secondary stroke prevention. We updated a meta-analysis of randomised controlled trials evaluating dual antiplatelet versus monotherapy for patients with acute non-cardioembolic ischaemic stroke(IS)or transient ischaemic attack(TIA).Methods We searched PubMed and identified randomised controlled trials evaluating dual antiplatelet versus monotherapy for acute non-cardioembolic IS or TIA within 3 days of ictus up to May 2018.Risk ratio(RR)with 95%CI were calculated using random effects models.Clinical endpoints included stroke recurrence, composite vascular events and major bleeding.results 18 randomised controlled trials including 15515 patients were pooled in the meta-analysis.When compared with monotherapy among patients with acute IS or TIA,dual antiplatelet therapy reduced the risk of stroke recurrence(RR 0.69;95%CI 0.61 to 0.78;p<0.001)and composite vascular events(RR 0.72;95%CI 0.64 to 0.80;p<0.001).Dual therapy was associated with a significant increase in the risk of major bleeding(RR 1.77;95%CI 1.09 to 2.87;p=0.02)when all trial data were combined. However, when all previous trials before the completion of the POINT trial were analysed, dual antiplatelet versus monotherapy was not associated with a significant increase in the risk of major bleeding(RR 1.46;95%CI 0.77 to 2.75;p=0.25).Conclusions Among patients with acute noncardioembolic IS or TIA within 3 days of ictus, dual antiplatelet therapy was associated with a reduction in stroke recurrence, and composite vascular events, when compared with monotherapy. However,a significant increase in the risk of major bleeding was observed.Yingying Yang Mengyuan Zhou Xi Zhong Yongjun Wang Xingquan Zhao Liping Liu Yilong Wang 2018Stroke & Vascular Neurology2018,3,2:4
Effect of Chitin Treatment on Softening of Actinidia arguta显示文摘[Objectives] This study was conducted to discuss the effect of chitin treatment on softening of Actinidia arguta Huanyou No. 1 as the test material.[Methods]Huanyou No. 1 was sprayed with chitin, and the changes of some physiological and biochemical indexes in fruit softening were investigated. [Results]Chitin treatment significantly improved single fruit weight, increased soluble solid content, but decreased titratable acid content and significantly reduced the content of cellulose and hemicellulose; and the chitin treatment significantly inhibited the breakdown of pectin, cellulose and hemicellulose, and inhibited the activities of PG enzyme and β-Gal enzyme. [Conclusions] Chitin spray treatment could increase the yield of A. arguta, and improve its quality and storage property.Xiangsu LIU Jiao AN Ming Liu Lihua JIN Yilong PIAO 2018Agricultural Biotechnology2018,7,2:4
