4Stenting for symptomatic intracranial arterial stenosis in China:1-year outcome of a multicentre registry study显示文摘background and purpose A multicentre prospective registry study of individually tailored stenting for a patient with symptomatic intracranial atherosclerotic stenosis(ICAS)combined with poor collaterals in China showed that the short-term safety and efficacy of stenting was acceptable.However,it remained uncertain whether the low event rate could be of a long term.We reported the 1-year outcome of this registry study to evaluate the long-term efficacy of individually tailored stenting for patients with severe symptomatic ICAS combined with poor collaterals.Methods Patients with symptomatic ICAS caused by 70%-99% stenosis located at the intracranial internal carotid,middle cerebral,intracranial vertebral or basilar arteries combined with poor collaterals were enrolled.Balloon-mounted stent or balloon plus self-expanding stent were selected based on the ease of vascular access and lesion morphology determined by the operators.The primary outcome was the rate of 30-day stroke,transient ischaemic attack and death,and 12-month ischaemic stroke within the same vascular territory,haemorrhagic stroke and vascular death after stenting.results From September 2013 to January 2015,300 patients(ages 58.3±9.78 years)were recruited.Among them,159 patients were treated with balloon-mounted stent and 141 with balloon plus self-expanding stent.During the 1-year follow-up,25 patients had a primary end point event.The probability of primary outcome at 1 year was 8.1%(95% CI 5.3% to 11.7%).In 76 patients with digital subtraction angiography follow-up,27.6%(21/76)had re-stenosis≥50% and 18.4%(14/76)had re-stenosis≥70%.No baseline characteristic was associated with the primary outcome.Conclusion The event rate remains low over 1 year of individually tailored stenting for patients with severe symptomatic ICAS combined with poor collaterals.Further randomised trial of comparing individually tailored stenting with best medical therapy is needed.Ning Ma Yong Zhang Jie Shuai Changchun Jiang Qiyi Zhu Kangning Chen Li Liu Baomin Li Xiangqun Shi Lianbo Gao Yajie Liu Feng Wang Yongli Li Tieyan Liu Hongbo Zheng Dapeng Mo Feng Gao Yilong Wang Yongjun Wang Lei Feng Zhongrong Miao 2018Stroke & Vascular Neurology2018,3,3:29

8Complement C3 activation regulates the production of tRNA-derived fragments Gly-tRFs and promotes alcohol-induced liver injury and steatosis显示文摘Complement is known to play a role in alcoholic fatty liver disease (AFLD), but the underlying mechanisms are poorly understood, thereby constraining the development of a rational approach for therapeutic intervention in the complement system. C3 deficiency has been shown to impart protective effects against ethanol-induced hepatic steatosis and inflammation. Here we demonstrate a protection effect in wild-type mice by treatment with CR2-Crry, a specific inhibitor of C3 activation. The expression of glycine transfer (t) RNA-derived fragments (Gly-tRFs) is upregulated in ethanol-fed mice and inhibition of Gly-tRFs in vivo decreases chronic ethanol feeding-induced hepatosteatosis without affecting inflammation. The expression of Gly-tRF was downregulated in C3-deficient or CR2-Crry-treated mice, but not in C5-deficient mice;Gly-tRF expression was restored by the C3 activation products C3a or Asp (C3a-des-Arg) via the regulation of CYP2E1. Transcriptome profiling of hepatic tissues showed that Gly-tRF inhibitors upregulate the expression of sirtuin1 (Sirt1) and subsequently affect downstream lipogenesis and β-oxidation pathways. Mechanistically, Gly-tRF interacts with AGO3 to downregulate Sirt1 expression via sequence complementarity in the 3′ UTR. Notably, the expression levels of C3d, CYP2E1 and Gly-tRF are upregulated, whereas Sirt1 is decreased in AFLD patients compared to healthy controls. Collectively, our findings suggest that C3 activation products contribute to hepatosteatosis by regulating the expression of Gly-tRF. 9Hepatic non-parenchymal cells and extracellular matrix participate in oval cell-mediated liver regeneration显示文摘AIM: To elucidate the interaction between nonparenchymal cells, extracellular matrix and oval cells during the restituting process of liver injury induced by partial hepatectomy (PH). METHODS: We examined the localization of oval cells, non-parenchymal cells, and the extracellular matrix components using immunohistochemical and double immunofluorescent analysis during the proliferation and differentiation of oval cells in N-2acetylaminofluorene (2-AAF)/PH rat model. RESULTS: By day 2 after PH, small oval cells began to proliferate around the portal area. Most of stellate cells and laminin were present along the hepatic sinusoids in the periportal area. Kupffer cells and fibronectin markedly increased in the whole hepatic lobule. From day 4 to 9, oval cells spread further into hepatic parenchyma, closely associated with stellate cells, fibronectin and laminin. Kupffer cells admixed with oval cells by day 6 and then decreased in the periportal zone. From day 12 to 15, most of hepatic stellate cells (HSCs), laminin and fibronectin located around the small hepatocyte nodus, and minority of them appeared in the nodus. Kupffer cells were mainly limited in the pericentral sinusoids. 11Spatial distribution, seasonal variation and regionalization of PM_(2.5) concentrations in China显示文摘In order to provide scientific support to policy makers in the regulation of PM2.5 pollution in China, it is important to accurately assess the current status, spatiotemporal characteristics and regionalization data for this air pollutant. An analysis of the pollution status of PM2.5 was conducted using daily averaged mass concentration data recorded in 74 cities in 2013 and 161 cities in 2014. The rotated empirical orthogonal function(REOF) method was applied to analyze this data. Results showed that the average annual PM2.5 concentration in urban areas of China is 62.2±21.5 ?g/m3, and that the distribution is spatially heterogeneous. The North China Plain, middle and lower Yangtze River Plain, Sichuan Basin and Guanzhong Plain had relatively high annual PM2.5 concentrations compared with the southeast coastal region, the Tibetan Plateau and the Yungui Plateau. PM2.5 mass concentrations tended to be higher in winter than in summer, however, the data for many cities showed a small peak in concentrations from May to July. An analysis of the spatial correlation of PM2.5 indicated a significant influence of topographic conditions. A lower correlation was observed where terrain features varied greatly. Based on the results of the REOF analysis and topographic characteristics, ten regions were identified in mid-eastern China, which could be considered as basic pollution prevention divisions for PM2.5; these include the North China Plain region, Pearl River Delta region, Jianghuai Plain region, middle Yangtze River Plain region, Northeast Plain region, Jiangnan coastal region, Sichuan Basin region, Qiantao Plain region, Guanzhong-Central Plain region and Yungui Plateau region. Seasonal variations in the regionalization data were observed, especially for the North China Plain and Pearl River Delta regions. Among the ten regions identified in this study, the North China Plain, Guanzhong-Central Plain, middle Yangtze River Plain and Jianghuai Plain had relatively high PM2.5 mass concentrations in comparison with the others. Therefore, these regions should be considered as the key regions to target in developing PM2.5 pollution prevention strategies. 12Deformation mechanism and stability control of roadway along a fault subjected to mining显示文摘It is difficult to maintain the roadway around a fault because of the fractured surroundings,complex stress environment,and large and intense deformation in the mining process.Based on a tailgate of panel S2205 in Tunliu colliery,in Shanxi province,China,we investigated the evolution of stress and displacement of rocks surrounding the roadway during the drivage and mining period using theoretical analysis,numerical simulation and field trial methods.We analyzed the deformation and failure mechanisms of the tailgate near a fault.The deformation of surrounding rock caused by unloading in the drivage period is large and asymmetric,the roadway convergence increases with activation of the fault and secondary fracture develops in the mining period.Therefore,we proposed a specific control technique of the roadway along a fault as follows:(1) High-strength yielding bolt not only supports the shallow rock to loadbearing structures,but also releases primary deformation energy by use of a pressure release device in order to achieve high resistance to the pressure retained;(2) Grouting of near-fault ribside after initial stabilization of the rock deformation is used to reinforce the broken rock,and to improve the integral load-bearing capacity of the roadway.The research results were successfully applied to a field trial.Yan Shuai Bai Jianbiao Li Wenfeng Chen Jigang Li Lei 2012International Journal of Mining Science and Technology2012,22,4:11
13Artificial intelligence system of faster region-based convolutional neural network surpassing senior radiologists in evaluation of metastatic lymph nodes of rectal cancer显示文摘Background: An artificial intelligence system of Faster Region-based Convolutional Neural Network (Faster R-CNN) is newly developed for the diagnosis of metastatic lymph node (LN) in rectal cancer patients. The primary objective of this study was to comprehensively verify its accuracy in clinical use. Methods: Four hundred fourteen patients with rectal cancer discharged between January 2013 and March 2015 were collected from 6 clinical centers, and the magnetic resonance imaging data for pelvic metastatic LNs of each patient was identified by Faster R-CNN. Faster R-CNN based diagnoses were compared with radiologist based diagnoses and pathologist based diagnoses for methodological verification, using correlation analyses and consistency check. For clinical verification, the patients were retrospectively followed up by telephone for 36 months, with post-operative recurrence of rectal cancer as a clinical outcome;recurrence-free survivals of the patients were compared among different diagnostic groups, by methods of Kaplan-Meier and Cox hazards regression model. Results: Significant correlations were observed between any 2 factors among the numbers of metastatic LNs separately diagnosed by radiologists, Faster R-CNN and pathologists, as evidenced by rradiologist-Faster R-CNN of 0.912, rPathologist-radiologist of 0.134, and rPathologist-Faster R-CNN of 0.448 respectively. The value of kappa coefficient in N staging between Faster R-CNN and pathologists was 0.573, and this value between radiologists and pathologists was 0.473. The 3 groups of Faster R-CNN, radiologists and pathologists showed no significant differences in the recurrence-free survival time for stage N0 and N1 patients, but significant differences were found for stage N2 patients. 14Deep neural network-assisted computed tomography diagnosis of metastatic lymph nodes from gastric cancer显示文摘Background:Artificial intelligence-assisted image recognition technology is currently able to detect the target area of an image and fetch information to make classifications according to target features.This study aimed to use deep neural netAVorks for computed tomography(CT)diagnosis of perigastric metastatic lymph nodes(PGMLNs)to simulate the recognition of lymph nodes by radiologists,and to acquire more accurate identification results.Methods:A total of 1371 images of suspected lymph node metastasis from enhanced abdominal CT scans were identified and labeled by radiologists and were used with 18,780 original images for faster region-based convolutional neural networks(FR-CNN)deep learning.The identification results of 6000 random CT images from 100 gastric cancer patients by the FR-CNN were compared with results obtained from radiologists in terms of their identification accuracy.Similarly,1004 CT images with metastatic lymph nodes that had been post-operatively confirmed by pathological examination and 11,340 original images were used in the identification and learning processes described above.The same 6000 gastric cancer CT images were used for the verification,according to which the diagnosis results were analyzed.Results:In the initial group,precision-recall curves were generated based on the precision rates,the recall rates of nodule classes of the training set and the validation set;the mean average precision(mAP)value was 0.5019.To verify the results of the initial learning group,the receiver operating characteristic curves was generated,and the corresponding area under the curve(AUC)value was calculated as 0.8995.After the second phase of precise learning,all the indicators were improved,and the mAP and AUC values were 0.7801 and 0.9541,respectively.Conclusion:Through deep learning,FR-CNN achieved high judgment effectiveness and recognition accuracy for CT diagnosis of PGMLNs.Yuan Gao Zheng-Dong Zhang Shuo Li Yu-Ting Guo Qing-Yao Wu Shu-Hao Liu Shu-Jian Yang Lei Ding Bao-Chun Zhao Shuai Li Yun Lu 2019Chinese Medical Journal2019,,23:11

16Bio-inspired special wetting surfaces via self-assembly显示文摘Self-assembly is the fundamental principle, which can occur spontaneously in nature. Through billions of years of evolution, nature has learned what is optimal. The optimized biological solution provides some inspiration for scientists and engineers. In the past decade, under the multi-disciplinary collaboration, bio-inspired special wetting surfaces have attracted much attention for both fundamental research and practical applications. In this review, we focus on recent research progress in bio-inspired special wetting surfaces via self-assembly, such as low adhesive superhydrophobic surfaces, high adhesive superhydrophobic surfaces, superamphiphobic surfaces, and stimuli-responsive surfaces. 17Numerical simulation of land subsidence and verification of its character for an iron mine using sublevel caving显示文摘Combined with a digital bored photography system and in-situ statistics concerning the joints and fissures of both ore-body and surrounding rock,a 2D discrete model was constructed using UDEC.The stress field and displacement field changes of different sublevel stoping systems were also studied.Changes in the overlying rock strata settlement pattern has been analyzed and validated by in-situ monitoring data.The results show that:in the caving process,there exists an obvious delay and jump for the overlying rock strata displacement over time,and a stable arch can be formed in the process of caving,which leads to hidden goafs.Disturbed by the mining activity,a stress increase occurred in both the hanging wall and the foot wall,demonstrating a hump-shaped distribution pattern.From the comparison between simulation results and in-situ monitoring results,land subsidence shows a slow-development,suddenfailure,slow-development cycle pattern,which leads eventually to a stable state.This pattern validates the existence of balanced arch and hidden goafs.Cao Shuai Song Weidong Deng Dan Lei Yuankun Lan Jianqiang 2016International Journal of Mining Science and Technology2016,26,2:9
18Cerebral mechanism of puncturing at He-Mu point combination for functional dyspepsia: study protocol for a randomized controlled parallel trial显示文摘Acupuncture is widely used to treat functional dyspepsia with satisfactory outcomes. Combination of the He and Mu acupoints is commonly used and has a synergistic effect on functional dyspepsia; however, its underlying mechanisms remain unclear. Therefore, a randomized controlled parallel clinical trial is currently underway at Chengdu University of Traditional Chinese Medicine, China. This trial is designed to explore the efficacy of and central responses to the He-Mu point combination in patients with functional dyspepsia using functional magnetic resonance imaging. A total of 105 patients with functional dyspepsia will be allocated into 3 groups: the low-He point group(puncturing at Zusanli(ST36)), Mu point group(puncturing at Zhongwan(CV12)), and He-Mu point combination group(puncturing at ST36 and CV12). Every participant will receive 20 sessions of manual acupuncture for 4 weeks. The needles will be inserted perpendicularly to a depth of 1 to 2 cun. The angle of rotation and twisting will range from 90 to 180 degrees, while lifting and thrusting will range from 0.3 to 0.5 cm. The various manipulations will be performed 60 to 90 times per minute. The needles will remain in place for 30 minutes, during which manipulation will be applied every 10 minutes. Magnetic resonance imaging will be performed before and after 20 sessions of acupuncture. The primary outcome is symptom improvement according to the Chinese version of the Nepean Dyspepsia Index. Secondary outcomes include the Leeds dyspepsia questionnaire, Self-Rating Anxiety Scale, Self-Rating Depression Scale, Beck Anxiety Inventory, Beck Depression Inventory, and visual analogue scale scores before and after 10 and 20 sessions of acupuncture. Needle sensation and adverse events will be used to assess the therapeutic effects. This study will promote more widespread awareness of the benefits of acupoint combination in the clinical setting and provide a further explanation of the neuromechanism by which acupuncture at the He-Mu point combination for functional dyspepsia. 19Identification and fine mapping of two blast resistance genes in rice cultivar 93-11显示文摘Rice blast, caused by Magnaporthe oryzae, is a major disease of rice almost worldwide. The Chinese indica cultivar 93-11 is resistant to numerous isolates of the blast fungus in China, and can be used as broad-spectrum resistance resource, particularly in japonica rice breeding programs. In this study, we identified and mapped two blast resistance genes, Pi60(t) and Pi61(t), in cv. 93-11 using F2 and F3 populations derived from a cross between the susceptible cv. Lijiangxintuanheigu(LTH) and resistant cv. 93-11 and inoculated with M. oryzae isolates from different geographic origins. Pi60(t) was delimited to a 274 kb region on the short arm of chromosome 11, flanked by InDel markers K1-4 and E12 and cosegregated with InDel markers B1 and Y10. Pi61(t) was mapped to a 200 kb region on the short arm(near the centromere) of chromosome 12, flanked by InDel markers M2 and S29 and cosegregating with InDel marker M9. In the 274 kb region of Pi60(t), 93-11 contains six NBS-LRR genes including the two Pia/ PiCO39 alleles(BGIOSGA034263 and BGIOSGA035032) which are quite close to the two Pia/ PiCO39 alleles(SasRGA4 and SasRGA5) in Sasanishiki and CO39, with only nine amino acids differing in the protein sequences of BGIOSGA035032 and SasRGA5. In the 200 kb region of Pi61(t), 93-11 contains four NBS-LRR genes, all of which show high identities in protein sequence with their corresponding NBS-LRR alleles in susceptible cv. Nipponbare. Comparison of the response spectra and physical positions between the target genes and other R genes in the same chromosome regions indicated that Pi60(t) could be Pia/PiCO39 or its allele, whereas Pi61(t) appears to be different from Pita, Pita-2, Pi19(t), Pi39(t) and Pi42(t) in the same R gene cluster. 20Sedimentation processes and sedimentary characteristics of tidal bores along the north bank of the Qiantang Estuary显示文摘A tidal bore is a unique Earth surface process, characterized by its highly destructive energy, predictable periodicities and magni-tudes, and the production of characteristic sedimentary features. Tidal bores and associated rapid flood flows are highly turbulent flows of the upper-flow regime with a velocity over several meters per second. Reynolds (Re) and Froude (Fr) numbers, respectively, are larger than 104 and 1.0, making them significantly different from regular tidal flows but analogous to turbidity currents. Until now, understanding of tidal-bore depositional processes and products has been limited because of the difficulty and hazards involved with gauging tidal bores directly. The Qiantang bore is known as the largest breaking bore in the world. Field surveys were carried out in May 2010, along the north bank of the Qiantang Estuary to observe the occurrence of peak bores, including regular observations of current, water level and turbidity at the main channel. Several short cores were sampled on the intertidal flats to study the characteristic sedimentary features of tidal bores. Hydrodynamic and sedimentological studies show that the processes of sediment resuspension, transport and deposition are controlled primarily by the tidal bores, and the subsequent abruptly accelerated and decelerated flood flows, which only account for one tenth of each semidiurnal tidal cycle in the estuary. Tidal-bore deposits are generally poorly sorted because of rapid sedimentation after highly mixed suspension by intense turbulence. This behavior is characteristic of the absence of tractive-current depositional components in a C-M diagram. It also goes along with well-developed massive bedding, graded bedding, basal erosion structures, convolute bedding and dewatering structures. Together, these sedimentary features can constitute fingerprinting of turbidites, widely distributed in the deep-water environment. However, a tidal bore is triggered by intensely deformed tidal waves propagating into a shallow-water environment, which returns to regular tidal flows rapidly after the passage of the bore head. The tidal-bore deposits are usually bounded by the intertidal-flat deposits with typical tidal beddings at the top and on both flanks. The difference between tidal-bore deposits (TBD) and tidal sandy/muddy deposits (TSD/TMD) is evident not only in sedimentary structures, but also in the grain-size composition. They can be clearly distinguished in grain-size bivariate plots, typically the plot of mean grain size vs. standard deviation (or sorting). Some trend variations generally exist in mean grain size with TBD>TSD>TMD, sorting with TMD>TBD>TSD (larger value indicating poorer sorting), and both skewness and kurtosis with TSD>TBD>TMD.
