1Management of oligometastatic non-small cell lung cancer patients:Current controversies and future directions显示文摘Oligometastatic non-small cell lung cancer(NSCLC)describes an intermediate stage of NSCLC between localized and widely-disseminated disease.This stage of NSCLC is characterized by a limited number of metastases and a more indolent tumor biology.Currently,the management of oligometastatic NSCLC involves radical treatment(radiotherapy or surgery)that targets the metastatic lesions and the primary tumor to achieve disease control.This approach offers the potential to achieve prolonged survival in patients who,in the past,would have only received palliative measures.The optimal therapeutic strategies for the different scenarios of oligometastatic disease(intracranial vs extracranial disease,synchronous vs metachronous)remain undefined.Given the lack of head-to-head studies comparing radiotherapy to surgery in these patients,the decision to apply surgery or radiotherapy(with or without systemic treatment)must be based on prognostic factors that allow us to classify patients.This classification will allow us to select the most appropriate therapeutic strategy on an individualized basis.In the future,the molecular or microRNA profiles will likely improve the treatment selection process.The objective of the present article is to review the most relevant scientific evidence on the management of patients with oligometastatic NSCLC,focusing on the role of radiotherapy and surgery.We also discuss areas of controversy and future directions.Felipe Counago Javier Luna Luis Leonardo Guerrero Blanca Vaquero María Cecilia Guillén-Sacoto Teresa González-Merino Begona Taboada Verónica Díaz Belén Rubio-Viqueira Ana Aurora Díaz-Gavela Francisco JoséMarcos Elia del Cerro 2019World Journal of Clinical Oncology2019,10,10:5
2High prevalence of hepatic steatosis and vascular thrombosis in COVID-19:A systematic review and meta-analysis of autopsy data显示文摘BACKGROUND Coronavirus disease 2019(COVID-19)disease can frequently affect the liver.Data on hepatic histopathological findings in COVID-19 is scarce.AIM To characterize hepatic pathological findings in patients with COVID-19.METHODS We conducted a systematic review with meta-analysis registered on PROSPERO(CRD42020192813),following PRISMA guidelines.Eligible trials were those including patients of any age and COVID-19 diagnosis based on a molecular test.Histopathological reports from deceased COVID-19 patients undergoing autopsy or liver biopsy were reviewed.Articles including less than ten patients were excluded.Proportions were pooled using random-effects models.Q statistic and I2 were used to assess heterogeneity and levels of evidence,respectively.RESULTS We identified 18 studies from 7 countries;all were case reports and case series from autopsies.All the patients were over 15 years old,and 67.2%were male.We performed a meta-analysis of 5 studies,including 116 patients.Pooled prevalence estimates of liver histopathological findings were hepatic steatosis 55.1%[95%confidence interval(CI):46.2-63.8],congestion of hepatic sinuses 34.7%(95%CI:7.9-68.4),vascular thrombosis 29.4%(95%CI:0.4-87.2),fibrosis 20.5%(95%CI:0.6-57.9),Kupffer cell hyperplasia 13.5%(95%CI:0.6-54.3),portal inflammation 13.2%(95%CI:0.1-48.8),and lobular inflammation 11.6%(95%CI:0.3-35.7).We also identified the presence of venous outflow obstruction,phlebosclerosis of the portal vein,herniated portal vein,periportal abnormal vessels,hemophagocytosis,and necrosis.CONCLUSION We found a high prevalence of hepatic steatosis and vascular thrombosis as major histological liver features.Other frequent findings included portal and lobular inflammation and Kupffer cell hyperplasia or proliferation.Further studies are needed to establish the mechanisms and implications of these findings.Luis Antonio Díaz Francisco Idalsoaga Macarena Cannistra Roberto Candia Daniel Cabrera Francisco Barrera Alejandro Soza Rondell Graham Arnoldo Riquelme Marco Arrese Michael D Leise Juan Pablo Arab 2020World Journal of Gastroenterology2020,26,48:3
3Mapping aboveground biomass by integrating geospatial and forest inventory data through a k-nearest neighbor strategy in North Central Mexico显示文摘As climate change negotiations progress,monitoring biomass and carbon stocks is becoming an important part of the current forest research.Therefore,national governments are interested in developing forest-monitoring strategies using geospatial technology.Among statistical methods for mapping biomass,there is a nonparametric approach called k-nearest neighbor(kNN).We compared four variations of distance metrics of the kNN for the spatially-explicit estimation of aboveground biomass in a portion of the Mexican north border of the intertropical zone.Satellite derived,climatic,and topographic predictor variables were combined with the Mexican National Forest Inventory(NFI)data to accomplish the purpose.Performance of distance metrics applied into the kNN algorithm was evaluated using a cross validation leave-one-out technique.The results indicate that the Most Similar Neighbor(MSN)approach maximizes the correlation between predictor and response variables(r=0.9).Our results are in agreement with those reported in the literature.These findings confirm the predictive potential of the MSN approach for mapping forest variables at pixel level under the policy of Reducing Emission from Deforestation and Forest Degradation(REDD+).Carlos A AGUIRRE-SALADO Eduardo J TREVIO-GARZA Oscar A AGUIRRE-CALDERóN Javier JIMNEZ-PREZ Marco A GONZLEZ-TAGLE José R VALDZ-LAZALDE Guillermo SNCHEZ-DíAZ Reija HAAPANEN Alejandro I AGUIRRE-SALADO Liliana MIRANDA-ARAGóN 2014Journal of Arid Land2014,6,1:3
4Effective use of FibroTest to generate decision trees in hepatitis C显示文摘AIM:To assess the usefulness of FibroTest to forecast scores by constructing decision trees in patients with chronic hepatitis C.METHODS:We used the C4.5 classification algorithm to construct decision trees with data from 261 patients with chronic hepatitis C without a liver biopsy.The FibroTest attributes of age,gender,bilirubin,apolipoprotein,haptoglobin,α2 macroglobulin,and γ-glutamyl transpeptidase were used as predictors,and the FibroTest score as the target.For testing,a 10-fold cross validation was used.RESULTS:The overall classification error was 14.9%(accuracy 85.1%).FibroTest's cases with true scores of F0 and F4 were classified with very high accuracy(18/20 for F0,9/9 for F0-1 and 92/96 for F4) and the largest confusion centered on F3.The algorithm produced a set of compound rules out of the ten classification trees and was used to classify the 261 patients.The rules for the classification of patients in F0 and F4 were effective in more than 75% of the cases in which they were tested.CONCLUSION:The recognition of clinical subgroups should help to enhance our ability to assess differences in fibrosis scores in clinical studies and improve our understanding of fibrosis progression.Dana Lau-Corona Luís Alberto Pineda Héctor Hugo Avilés Gabriela Gutiérrez-Reyes Blanca Eugenia Farfan-Labonne Rafael Núez-Nateras Alan Bonder Rosalinda Martínez-García Clara Corona-Lau Marco Antonio Olivera-Martínez Maria Concepción Gutiérrez-Ruiz Guillermo Robles-Díaz David Kershenobich 2009World Journal of Gastroenterology2009,15,21:2

7Positive interactions by cushion plants in high mountains:fact or artifact?显示文摘Positive interactions are defined as non-trophic interactions where at least one of the interacting species is benefited in terms of fitness and the other remains unaffected.Nevertheless,the bidirectional feedbacks between species may be positive,neutral or negative.Thus,if facilitated species induce negative effects on their‘nurses’,the assumed definition of positive interactions could be reconsidered.Methods We assessed if ecological interactions between cushions of Azorella madreporica and their facilitated species are positive.Specifically,we tested if cover of facilitated species has any costs for cushion plants from an ecophysiological perspective,and if these costs increase with the amount of cover of facilitated species.In addition,through pathway analysis and correlations,we assessed if cover and richness of facilitated species have a direct and/or indirect effect on the fitness of cushion plants.Important Findings We found that facilitated plant species induced a significant cost for their nurses(cushion plants),and this cost increases with cover of the facilitated species.Additionally,the facilitated species exert a strong direct negative effect on the cushion’s fitness and a moderate indirect negative cost evident through the nutrient status and physiological performance of cushion plants.We thus contribute evidence that positive interactions between high mountain cushion plants of central Chile and their‘facilitated’species may be an artifact more than a fact,especially when bidirectional effects are considered;contrasting with the majority of studies that document only one side of the interaction.Marco A.Molina-Montenegro Rómulo Oses Ian S.Acuña-Rodríguez Cristian Fardella Ernesto I.Badano Patricio Torres-Morales Jorge Gallardo-Cerda Cristian Torres-Díaz 2016Journal of Plant Ecology2016,9,2:1
