| 4 | Clinical characteristics of COVID-19 patients who underwent tracheostomy and its effect on outcome:A retrospective observational study显示文摘BACKGROUND The exponential rise in Coronavirus disease 2019(COVID-19)cases has resulted in an increased number of patients requiring prolonged ventilatory support and subsequent tracheostomy.With the limited availability of literature regarding the outcomes of COVID-19 patients with tracheostomy,we attempted to study the clinical characteristics and multiple parameters affecting the outcomes in these patients.AIM To determine all-cause mortality following tracheostomy and its association with various risk factors in COVID-19 patients.METHODS This retrospective study included 73 adult COVID-19 patients admitted to the ICU between 1 April,2020 and 30 September,2021 who underwent tracheostomy as a result of acute respiratory failure due to COVID-19.The data collected included demographics(age,sex),comorbidities,type of oxygen support at admission,severity of COVID-19,complications,and other parameters such as admission to tracheostomy,intubation to tracheostomy,ICU stay,hospital stay,and outcome.RESULTS This study included 73 adult patients with an average age of 52±16.67 years,of which 52%were men.The average time for admission to tracheostomy was 18.12±12.98 days while intubation to tracheostomy was 11.97±9 days.The mortality rate was 71.2%and 28.8%of patients were discharged alive.The mean duration of ICU and hospital stay was 25±11 days and 28.21±11.60 days,respectively.Greater age,severe COVID-19,mechanical ventilation,shock and acute kidney injury were associated with poor prognosis;however,early tracheostomy in intubated patients resulted in better outcomes.CONCLUSION Patients with severe COVID-19 requiring mechanical ventilation have a poor prognosis but patients with early tracheostomy may benefit with no added risk.We recommend that the timing of tracheostomy be decided on a case-by-case basis and a well-designed randomised controlled trial should be performed to elucidate the potential benefit of early tracheostomy in such patients. | Yudhyavir Singh Kapil Dev Soni Abhishek Singh Nikita Choudhary Fahina Perveen Richa Aggarwal Nishant Patel Shailendra Kumar Anjan Trikha | 2022 | World Journal of Virology2022,11,6: | 0 |
| 5 | Preoperative serum cortisone levels are associated with cognition in preschool‑aged children with tetralogy of Fallot after corrective surgery:new evidence from human populations and mice显示文摘Background Tetralogy of Fallot(TOF)is the most common cyanotic congenital heart disease.Children with TOF would be confronted with neurological impairment across their lifetime.Our study aimed to identify the risk factors for cerebral morphology changes and cognition in postoperative preschool-aged children with TOF.Methods We used mass spectrometry(MS)technology to assess the levels of serum metabolites,Wechsler preschool and primary scale of intelligence-Fourth edition(WPPSI-Ⅳ)index scores to evaluate neurodevelopmental levels and multimodal magnetic resonance imaging(MRI)to detect cortical morphological changes.Results Multiple linear regression showed that preoperative levels of serum cortisone were positively correlated with the gyrification index of the left inferior parietal gyrus in children with TOF and negatively related to their lower visual spaces index and nonverbal index.Meanwhile,preoperative SpO_(2) was negatively correlated with levels of serum cortisone after adjusting for all covariates.Furthermore,after intervening levels of cortisone in chronic hypoxic model mice,total brain volumes were reduced at both postnatal(P)11.5 and P30 days.Conclusions Our results suggest that preoperative serum cortisone levels could be used as a biomarker of neurodevelopmental impairment in children with TOF.Our study findings emphasized that preoperative levels of cortisone could influence cerebral development and cognition abilities in children with TOF. | Si-Yu Ma Yu-Ting Liu Yue-Shuang Cun Qiang Wang Ming-Cui Fu Ke-De Wu Xin-Yu Cai Shu-Ting Cheng·Nishant Patel Min Da Liang Hu Zhuoga Deqin Xue-Jun Kang Ming Yang Xu-Ming Mo | 2024 | World Journal of Pediatrics2024,20,2: | 0 |
| 6 | Modern advances in reducing anxiety and pain associated with cystoscopy:Systematic review显示文摘AIM:To investigate if music reduces anxiety and pain in the Veterans Affairs population undergoing flexible cystoscopy. METHODS:This study was reviewed and approved by the University of California,San Diego Human Research Protections Program Institutional Review Board. Patients were prospectively randomized to undergo flexible cystoscopy with or without music. Thirty-eight patients were randomized into either the No Music group(n = 24) or the Music group(n = 14). We used the state-trait anxiety inventory and the visual analog pain scale,respectively. Statistics were generated and compared using an independent t-test and chi-squared tests. P values < 0.05 were considered statistically significant. Outpatient cystoscopy is a safe and useful procedure employed frequently in Urology for diagnosis and evaluation of genitourinary pathologies. However,cystoscopy-related distress cannot be ignored. Three components of outpatient cystoscopy have been evaluated to improve the cystoscopic experience:local anesthetic control,cystoscopic equipment redesign and environmental modification. We reviewed the literature pertaining to these modifications. RESULTS:The mean age was 65.3 and 67.1 years for men in the No Music and Music groups,respectively.Although,the majority of patients in each group selfidentified as Caucasians(66%),African American,Hispanic and other ethnicities represented 13%,8% and 13% respectively. The majority of patients(68%) reported experiencing hematuria. Thirty-four percent had a history of bladder cancer,and eighteen percent had a history of prostate cancer. Ten patients(26%) admitted to taking antidepressants. Physiologic parameters that correlated to pain and anxiety(systolic blood pressure,diastolic blood pressure,and heart rate) were statistically similar in both groups prior to and after flexible cystoscopy.
