1Multimodality treatment in hepatocellular carcinoma patients with tumor thrombi in portal vein显示文摘AIM To compare the therapeutic effect andsignificances of multimodality treatment forhepatocellular carcinoma (HCC) with tumorthrombi in portal vein (PVTT).METHODS HCC patients (n = 147) with tumortrombi in the main portal vein or the first branchof portal vein were divided into four groups bythe several therapeutic methods. There wereconservative treatment group in 18 out ofpatients (group A); and hepatic artery ligation(HAL) and/or hepatic artery infusion (HAl)group in 18 patients (group B), in whompostoberative chemoembolization was doneperiodically; group of removal of HCC with PVTTin 79 (group C) and group of transcatheterhepatic arterial chemoembolization (TACE) orHAl and/or portal vein infusion (PVI) afteroperation in 32 (group D).RESULTS The median survival period was 12months in our series and the 1-, 3-, and 5-yearsurvival rates were 44.3%, 24.5% and 15.2%,respectively. The median survival times were 2,5, 12 and 16 months in group A, B, C and D,respectively. The 1-, 3- and 5-year survival rateswere 5.6%, 0% and 0% in group A; 22.2%,5.6% and 0% in group B; 53.9%, 26.9% and16.6% in group C; 79.3%, 38.9% and 26.8% ingroup D, respectively. 2Analysis of in vivo patterns of caspase 3 gene expression in primary hepatocellular carcinoma and its relationship to p21^(WAF1) expression and hepatic apoptosis显示文摘AIM To detect the expression of caspase 3gene in primary human hepatocellular carcinoma(HCC)and investigate its relationship to p21WAF1gene expression and HCC apoptosis.METHODS In situ hybridization was employedto determine caspase 3 and p21WAF1expression inHCC.In situ end-labeling was used to detecthepatocytic apoptosis in HCC.RESULTS Twenty-one of 39(53.8%)cases ofHCC were found to express caspase 3transcripts,while 45.2% of HCC failed toexpress caspase 3.Non-cancerous adjacent livertissues showed more positive caspase 3(87.5%,7/8)as compared with HCC(P<0.05).The expression of caspase 3 is correlated withHCC differentiation,72.2%(13/18)ofmoderately to highly differentiated HCC showedcaspase 3 transcripts positive,while only 38.1%of poorly differentiated HCC harbored caspase 3transcripts(P<0.05).No relationship wasfound between caspase 3 expression and tumorsize or grade or metastasis,although 52.5%(5/8)of HCC with metastasis were caspase 3positive and a little higher than that with nometastasis(51.6%,P>0.05).Expression of caspase 3 alone did not affect the apoptosisindex(AI)of HCC.The AI was 7.12%o in caspase3-positive tumors(n=21),while in caspase 3-negative cases(n=18)6.59%0(P>0.05).Expression of caspase 3 clearly segregated withp21WAF1positive tumors as compared withp21WAF1-negative cases(16 of 23,69.6% versus5 of 16,31.3%)with statistical significance(P=0.017).In the cases with positive caspase 3and negative p21WAF1,the Al was found slightlyhigher,but with no statistical significance,thanthat with expression of p21WAF1and caspase 3(7.21‰ vs 6.98‰,P>0.05).CONCLUSION Loss of caspase 3 expressionmay contribute to HCC carcinogenesis,althoughthe expression of caspase 3 does not correlatewell with cell apoptosis in HCC.p21WAF1may bemerely one of the inhibitors which can reducecaspase 3 mediated cell apoptosis in HCCs.Bao Hua Sun Jun Zhang Bao JǜWang Xi Ping Zhao You Kun Wang Zhi Qun Yu Dong Liang Yang Lian Jie Hao Department of Clinical Immunology,Tongji Hospital,Tongji Medical University,Wuhan 430030,Hubei Province,China 2000World Journal of Gastroenterology2000,6,3:65

6DNA-based vaccination induces humoral and cellular immune responses against hepatitis B virus surface antigen in mice without activation of Cmyc显示文摘AIM To develop a safe and effective DNAvaccine for inducing humoral and cellularimmunological responses against hepatitis Bvirus surface antigen(HBsAg).METHODS BALB/c mice were inoculated withNV-HB/s,a recombinant plasmid that had beeninserted S gene of hepatitis B virus genome andcould express HBsAg in eukaryotes.HBsAgexpression was measured by ABC immunohis-tochemical assay,generation of anti-HBs byELISA and cytotoxic T lymphocyte(CTL),byMTT method,existence of vaccine DNA bySouthern blot hybridization and activation ofoncogene C-myc by in situ hybridization,RESULTS With NV-HB/s vaccination byintramuscular injection,anti-HBs was initiallypositive 2 weeks after inoculation while all micetested were HBsAg positive in the muscles.Thetiters and seroconversion rate of anti-HBs weresteadily increasing as time went on and weredose-dependent.All the mice inoculated with100 μg NV-HB/s were anti-HBs positive onemonth after inoculation,the titer was 1:1024 ormore.The humoral immune response was similarinduced by either intramuscular or intradermalinjection.CTL activities were much stronger(45.26%)in NV-HB/s DNA immunized mice as compared with those(only 6%)in plasma-derived HBsAg vaccine immunized mice.Twomonths after inoculation,all muscle sampleswere positive by Southern-blot hybridization forNV.HB/s DNA detection,but decreased to 25%and all were undetectable by in situhybridization after 6 months.No oncogene C-myc activation was found in the muscle ofinoculation site.CONCLUSION NV-HB/s could generatehumoral and cellular immunological responsesagainst HBsAg that had been safely expressed insitu by NV-HB/s vaccination.Lian San Zhao Shan Qin Tao You Zhou Hong Tang Li Liu Bing Jun Lei 2000World Journal of Gastroenterology2000,6,2:24

8Blood Lipid Profile and Prevalence of Dyslipidemia in Chinese Adults显示文摘Objective To investigate the plasma lipid levels in a national representative sample of subjects and to determine the prevalence of dyslipidemia in the Chinese population.Methods Plasma lipid profile was analyzed using the data obtained during the Chinese national nutrition and health survey(CNHS)in 2002 which involved 14 252 participants at the age of 18 years or older.Results The mean levels of total cholesterol(TC),triglyceride(TG)and high density lipoprotein cholesterol(HDL-C)in the participants were 3.81 mmol/L,1.10 mmol/L,and 1.30 mmol/L,respectively.In the groups of participants at the age of 18-44 years,45-59 years,and over 60 years the mean TC level was 3.70 mmol/L,4.09 mmol/L and 4.21 mmol/L,respectively,and the mean TG level was 07 mmol/L,1.21 mmol/L,1.20 mmol/L,1.29 mmol/L,1.33 mmol/L,and 1.33 mmol/L,respectively.The prevalence of dyslipidemia in Chinese adults was 18.6% and 22.2% in males and 15.9% in females.Dyslipidemia prevalence was higher in urban districts than in rural areas(21.0% vs.17.7%).The prevalence of hypercholesterolemia,hypertriglyceridemia,and low HDL cholesterol was 2.9%,11.9%,and 7.4% respectively among the participants.Conclusion Dyslipidemia has become one of the important health risk factors in the Chinese population.There is no significantly difference in the prevalence of dyslipidemia between the groups of participants at the age of 45-59 years and over 60 years.This study provides important lipid profile data for policy making and guideline development for the prevention of dyslipidemia in the Chinese population.WEN-HUA ZHAO JIAN ZHANG YI ZHAI YUE YOU QING-QING MAN CHUN-RONG WANG HONG LI YING LI XIAO-GUANG YANG 2007Biomedical and Environmental Sciences2007,20,4:22

13Excavation and characterization of refuse in closed landfill显示文摘Refuse in landfills becomes stabilized as organic matter in refuse degrades and soluble inorganic substances dissolve during their long term stabilization process. In this paper, this process is also referred to as mineralization process and the resultant stabilized refuse referred to as aged refuse. Aged refuse contains a wide spectrum and huge quantity of microorganisms with strong decomposition capability for refractory organic matter present in some wastewater such as leachate. In this study, aged refuse excavated from 2 to 10 years old closed landfill compartments in Shanghai Refuse Landfill is characterized in terms of particulate distribution by screening, total nitrogen, total phosphorus, biodegradable matter. 17Potential role of strike-slip faults in opening up the South China Sea显示文摘Radiometric dates of key rock units indicate that a remnant Late Mesozoic ocean of the Huatung Basin is still preserved today east of the South China Sea(SCS).We integrate regional geology with a Cretaceous oceanic basement in the vicinity of the Huatung Basin to reconstruct the Huatung Plate east of the Eurasian continent.Results of geophysical investigations,four expeditions of deep-sea drilling and a renaissance of regional geology allow us to propose a hypothesis that the mechanism responsible for the SCS opening was raised from strike-slip fault on the east.The hypothesis suggests that the SCS opening could highly relate to the strike-slip faults inherited from Late Mesozoic structures onshore–offshore the SE Cathaysia Block to develop rhombic-shaped extensional basins en echelon on the thinned Eurasian continental crust in the Early Cenozoic.It was followed by sinistral strike-slip movements along the boundary between the Eurasian Plate and the Huatung Plate driven by oblique subduction of the Huatung Plate to the northwest coupled with slab-pull force by southward subduction of the Proto-SCS to open up the triangle-shaped oceanic East Sub-basin in the Early Oligocene(33/34 Ma).The spreading ridge then propagated southwestward in the step-over segment between the Zhongnan-Lile and the Red River strike-slip fault systems to open the triangle-shaped oceanic Southwest Sub-basin by 23 Ma.The plate boundary fault was subsequently converted into the Manila Trench when the Eocene Sierra Madre arc of the Huatung Plate had moved from the south to its present latitude by the Middle Miocene.Chi-Yue Huang Pinxian Wang Mengming Yu Chen-Feng You Char-Shine Liu Xixi Zhao Lei Shao Guangfa Zhong Graciano P.Yumul Jr 2019National Science Review2019,6,5:16

20Relationship between β-amyloid protein 1-42, thyroid hormone levels and the risk of cognitive impairment after ischemic stroke显示文摘BACKGROUND Post-stroke cognitive impairment(PSCI)is not only a common consequence of stroke but also an important factor for adverse prognosis of patients.Biochemical indicators such as blood lipids and blood pressure are affected by many factors,and the ability of evaluating the progress of patients with PSCI is insufficient.Therefore,it is necessary to find sensitive markers for predicting the progress of patients and avoiding PSCI.Recent studies have shown thatβ-amyloid protein 1-42(Aβ1-42)and thyroid hormone levels are closely related to PSCI,which may be the influencing factors of PSCI,but there are few related studies.AIM To investigate the relationship between serum levels of Aβand thyroid hormones in acute stage and PSCI and its predicted value.METHODS A total of 195 patients with acute cerebral infarction confirmed from June 2016 to January 2018 were enrolled in this study.Baseline data and serological indicators were recorded to assess cognitive function of patients.All patients were followed up for 1 year.Their cognitive functions were evaluated within 1 wk,3 mo,6 mo and 1 yr after stroke.At the end of follow-up,the patients were divided into PSCI and non-PSCI according to Montreal cognitive assessment score,and the relationship between biochemical indexes and the progression of PSCI was explored.RESULTS Compared with patients with non-PSCI,the levels of Aβ1-42,triiodothyronine(T3)and free thyroxin were lower in the patients with PSCI.Repeated measures analysis of variance showed that the overall content of Aβ1-42 and T3 in PSCI was also lower than that of the non-PSCI patients.Further analysis revealed that Aβ1-42(r=0.348),T3(r=0.273)and free thyroxin(r=0.214)were positively correlated with disease progression(P<0.05),suggesting that these indicators have the potential to predict disease progression and outcome.Cox regression analysis showed that Aβ1-42 and T3 were important factors of PSCI.Then stratified analysis showed that the lower the Aβ1-42 and T3,the higher risk of PSCI in patients who were aged over 70,female and illiterate.CONCLUSION Aβ1-42 and T3 have the ability to predict the progression of PSCI,which is expected to be applied clinically to reduce the incidence of PSCI and improve the quality of life of patients.Lei Mao Xiao-Han Chen Jian-Hua Zhuang Peng Li Yi-Xin Xu Yu-Chen Zhao Yue-Jin Ma Bin He You Yin 2020World Journal of Clinical Cases2020,8,1:13
