| 1 | Oxaliplatin plus S-1 or capecitabine as neoadjuvant or adjuvant chemotherapy for locally advanced gastric cancer with D2lymphadenectomy: 5-year follow-up results of a phase II-III randomized trial显示文摘Objective: To compare the effect of neoadjuvant chemotherapy(NACT) with adjuvant chemotherapy(ACT)using oxaliplatin plus S-1(SOX) or capecitabine(CapeOX) on gastric cancer patients with D2 lymphadenectomy.Methods: This was a two-by-two factorial randomized phase II-III trial, and registered on ISRCTN registry(No. ISRCTN12206108). Locally advanced gastric cancer patients were randomized to neoadjuvant SOX,neoadjuvant CapeOX, adjuvant SOX, or adjuvant CapeOX arms. Primary analysis was performed on an intentionto-treat(ITT) basis using overall survival(OS) as primary endpoint.Results: This trial started in September 2011 and closed in December 2012 with 100 patients enrolled.Treatment completion rate was 56%, 52%, 38% and 30% in the four arms, respectively. NACT group had fewer dropouts due to unacceptable toxicity(P=0.042). Surgical complication rate did not differ by the four groups(P=0.986). No survival significant difference was found comparing NACT with ACT(P=0.664; 5-year-OS: 70% vs.74% respectively), nor between the SOX and CapeOX groups(P=0.252; 5-year-OS: 78% vs. 66% respectively).Subgroup analysis showed SOX significantly improved survival in patients with diffuse type(P=0.048).Conclusions: No significant survival difference was found between NACT and ACT. SOX and CapeOX had good safety and efficacy as neoadjuvant regimens. | 2 | Effects of dynamic ventilatory factors on ventilatorinduced lung injury in acute respiratory distress syndrome dogs显示文摘BACKGROUND:Mechanical ventilation is a double-edged sword to acute respiratory distress syndrome(ARDS) including lung injury,and systemic inflammatory response high tidal volumes are thought to increase mortality.The objective of this study is to evaluate the effects of dynamic ventilatory factors on ventilator induced lung injury in a dog model of ARDS induced by hydrochloric acid instillation under volume controlled ventilation and to investigate the relationship between the dynamic factors and ventilator-induced lung injuries(VILI) and to explore its potential mechanisms.METHODS:Thirty-six healthy dogs were randomly divided into a control group and an experimental group.Subjects in the experimental group were then further divided into four groups by different inspiratory stages of flow.Two mL of alveolar fluid was aspirated for detection of IL-8 and TNF-α.Lung tissue specimens were also extracted for total RNA,IL-8 by western blot and observed under an electronic microscope.RESULTS:IL-8 protein expression was significantly higher in group B than in groups A and D.Although the IL-8 protein expression was decreased in group C compared with group B,the difference was not statistically significant.The TNF-α ray degree of group B was significantly higher than that in the other groups(P<0.01),especially in group C(P>0.05).The alveolar volume of subjects in group B was significantly smaller,and cavity infiltration and cell autolysis were marked with a significant thicker alveolar septa,disorder of interval structures,and blurring of collagenous and elastic fiber structures.A large number of necrotic debris tissue was observed in group B.CONCLUSION:Mechanical ventilation with a large tidal volume,a high inspiratory flow and a high ventilation frequency can cause significant damage to lung tissue structure.It can significantly increase the expression of TNF-α and IL-8 as well as their mRNA expression.Furthermore,the results of our study showed that small tidal ventilation significantly reduces the release of proinflammatory media.This finding suggests that greater deterioration in lung injury during ARDS is associated with high inspiratory flow and high ventilation rate. | Rui-lan Wang Kan Xu Kang-long Yu Xue Tang Hui Xie | 2012 | World Journal of Emergency Medicine2012,3,4: | 8 |

An oesophageal cancer-specific ceRNA network was then constructed and investigated.RESULTS CeRNAs have the ability to reduce the targeting activityof miRNAs, leading to the de-repression of specific mRNAs with common miRNA response elements. | 17 | Loss of GRB2 associated binding protein 1 in arteriosclerosis obliterans promotes host autophagy显示文摘Background:Arteriosclerosis obliterans(ASO)is a major cause of adult limb loss worldwide.Autophagy of vascular endothelial cell(VEC)contributes to the ASO progression.However,the molecular mechanism that controls VEC autophagy remains unclear.In this study,we aimed to explore the role of the GRB2 associated binding protein 1(GAB1)in regulating VEC autophagy.Methods:In vivo and in vitro studies were applied to determine the loss of adapt protein GAB1 in association with ASO progression.Histological GAB1 expression was measured in sclerotic vascular intima and normal vascular intima.Gain-and loss-of-function of GAB1 were applied in VEC to determine the effect and potential downstream signaling of GAB1.Results:The autophagy repressor p62 was significantly downregulated in ASO intima as compared to that in healthy donor(0.80 vs.0.20,t=6.43,P<0.05).The expression level of GAB1 mRNA(1.00 vs.0.24,t=7.41,P<0.05)and protein(0.72 vs.0.21,t=5.97,P<0.05)was significantly decreased in ASO group as compared with the control group.Loss of GAB1 led to a remarkable decrease in LC3II(1.19 vs.0.68,t=5.99,P<0.05),whereas overexpression of GAB1 significantly led to a decrease in LC3II level(0.41 vs.0.93,t=7.12,P<0.05).Phosphorylation levels of JNK and p38 were significantly associated with gain-and loss-of-function of GAB1 protein.Conclusion:Loss of GAB1 promotes VEC autophagy which is associated with ASO.GAB1 and its downstream signaling might be potential therapeutic targets for ASO treatment. | Meng Ye Xiang-Jiang Guo Ke-Jia Kan Qi-Hong Ni Jia-Quan Chen Han Wang Xin Qian Guan-Hua Xue Hao-Yu Deng Lan Zhang | 2021 | Chinese Medical Journal2021,,1: | 1 |
