| 1 | A rapid advice guideline for the diagnosis and treatment of 2019 novel coronavirus(2019-nCoV) infected pneumonia(standard version)显示文摘In December 2019, a new type viral pneumonia cases occurred in Wuhan, Hubei Province;and then named '2019 novel coronavirus(2019-nCoV)' by the World Health Organization(WHO) on 12 January 2020. For it is a never been experienced respiratory disease before and with infection ability widely and quickly, it attracted the world’s attention but without treatment and control manual. For the request from frontline clinicians and public health professionals of 2019-nCoV infected pneumonia management, an evidence-based guideline urgently needs to be developed. Therefore, we drafted this guideline according to the rapid advice guidelines methodology and general rules of WHO guideline development;we also added the first-hand management data of Zhongnan Hospital of Wuhan University. This guideline includes the guideline methodology, epidemiological characteristics, disease screening and population prevention, diagnosis, treatment and control(including traditional Chinese Medicine), nosocomial infection prevention and control, and disease nursing of the 2019-nCoV. Moreover, we also provide a whole process of a successful treatment case of the severe 2019-nCoV infected pneumonia and experience and lessons of hospital rescue for 2019-nCoV infections. | 3 | Genotypic Characterization of Methicillin-resistant Staphylococcus aureus Isolated from Pigs and Retail Foods in China显示文摘Objective To investigate the genotypic diversity of Methicillin-resistant Staphylococcus aureus(MRSA) isolated from pigs and retail foods from different geographical areas in China and further to study the routes and rates of transmission of this pathogen from animals to food. Methods Seventy-one MRSA isolates were obtained from pigs and retail foods and then characterized by multi-locus sequencing typing(MLST), spa typing, multiple-locus variable number of tandem repeat analysis(MLVA), pulsed-field gel electrophoresis(PFGE), and antimicrobial susceptibility testing. Results All isolated MRSA exhibited multi-drug resistance(MDR). Greater diversity was found in food-associated MRSA(7 STs, 8 spa types, and 10 MLVA patterns) compared to pig-associated MRSA(3 STs, 1 spa type, and 6 MLVA patterns). PFGE patterns were more diverse for pig-associated MRSA than those of food-associated isolates(40 vs. 11 pulse types). Among the pig-associated isolates, CC9-ST9-t899-MC2236 was the most prevalent clone(96.4%), and CC9-ST9-t437-MC621(20.0%) was the predominant clone among the food-associated isolates. The CC9-ST9 isolates showed significantly higher antimicrobial resistance than other clones. Interestingly, CC398-ST398-t034 clone was identified from both pig-and food-associated isolates. Of note, some community-and hospital-associated MRSA strains(t030, t172, t1244, and t4549) were also identified as food-associated isolates. Conclusion CC9-ST9-t899-MC2236-MDR was the most predominant clone in pigs, but significant genetic diversity was observed in food-associated MRSA. | 5 | Protective effects of extracts of Schisandra chinensis stems against acetaminophen-induced hepatotoxicity via regulation of MAPK and caspase-3 signaling pathways显示文摘The present study was designed to evaluate protective activity of an ethanol extract of the stems of Schisandra chinensis(SCE) and explore its possible molecular mechanisms on acetaminophen(APAP) induced hepatotoxicity in a mouse model. The results of HPLC analysis showed that the main components of SCE included schisandrol A, schisandrol B, deoxyschisandrin, schisan--drin B, and schisandrin C and their contents were 5.83, 7.11, 2.13, 4.86, 0.42 mg·g^(-1), respectively. SCE extract was given for 7 con--secutive days before a single hepatotoxic dose of APAP(250 mg·kg^(-1)) was injected to mice. Our results showed that SCE pretreatment ameliorated liver dysfunction and oxidative stress, which was evidenced by significant decreases in aspartate transaminase(AST), alanine aminotransferase(ALT), malondialdehyde(MDA) contents and elevations in reduced glutathione(GSH) and superoxide dismutase(SOD) levels. These findings were associated with the result that the SCE pretreatment significantly decreased expression levels of 4-hydroxynonenal(4-HNE) and 3-nitrotyrosine(3-NT). SCE also significantly decreased the expression levels of Bax, mitogen-activated protein kinase(MAPK), and cleaved caspase-3 by APAP exposure. Furthermore, supplementation with SCE suppressed the expression levels of inducible nitric oxide synthase(iNOS) and cyclooxygenase-2(COX-2), suggesting alleviation of inflammatory response. | 6 | Heavy Oil and Oil Sands:Global Distribution and Resource Assessment显示文摘Global recoverable resources of heavy oil and oil sands have been assessed by CNPC using a geology-based assessment method combined with the traditional volumetric method, spatial interpolation method, parametric-probability method etc. The most favourable areas for exploration have been selected in accordance with a comprehensive scoring system. The results show:(1) For geological resources, CNPC estimate 991.18 billion tonnes of heavy oil and 501.26 billion tonnes of oil sands globally, of which technically recoverable resources of heavy oil and oil sands comprise 126.74 billion tonnes and 64.13 billion tonnes respectively. | 11 | Validity of the Chinese Version of the Patient-Generated Subjective Global Assessment (PG-SGA) in Lung Cancer Patients显示文摘The prevalence of malnutrition is high among oncology patients in China. Although the Patient- Generated Subjective Global Assessment (PG-SGA) is widely recognized as an important nutritional assessment tool, it has not been validated for Chinese cancer patients. The purpose of this study was to conduct an analysis of nutrition screening and assessment data to validate the Chinese version of the PG-SGA in Chinese patients with lung cancer. This was an observational, cross-sectional study of 2,000 consecutive adult patients with lung cancer treated at several tertiary hospitals in China. Anthropometric and patient descriptive data were collected. The PG-SGA generated a score for the nutritional risk and a global rating for the nutritional status. The internal consistency, external consistency, and construct validity were evaluated, and a known-groups comparison and principal component analyses were conducted to evaluate the reliability and validity of the PG-SGA. The known-groups comparison demonstrated the ability of the PG-SGA to differentiate lung cancer patients of different nutritional and body mass index (BMI) groups. A confirmatory factor analysis supported the original four-factor structure of the PG-SGA. In terms of the internal and external consistency, the PG-SGA demonstrated intra-class correlation coefficients (ICC) up to 0.987. Alternative form validity was also demonstrated between Box 4 of the PG-SGA and the Karnofsky Performance Status score, with excellent external consistency (r = 0.972, P < 0.001). The construct validity was supported, and selected questionnaire dimensions were evident in the principal component analysis. Significant Spearman correlations (P < 0.001) were demonstrated. The Chinese version of the PG-SGA demonstrated significant reliability and sufficient exploratory properties to support its validity. It seems to be a valid tool that can be used to access the nutritional status of Chinese lung cancer patients. | 14 | Comparison of Histological, Microbiological, and Molecular Methods in Diagnosis of Patients with TBLN Having Different Anti-TB Treatment Background显示文摘Objective The influence of anti-tuberculosis(TB) treatment history on tuberculous lymphadenitis(TBLN) diagnosis is unclear. Therefore, this study aims to evaluate the diagnostic methods, including histology, microbiology, and molecular tests, used for TBLN. Methods In this study, suspected patients with TBLN and having different anti-TB treatment background were enrolled. All the samples were tested simultaneously by histology, Ziehl-Neelsen(ZN) staining, mycobacterial culture(culture), Xpert MTB/RIF(xpert), real-time PCR, and high-resolution melting curve PCR(HRM). Thereafter, the performance of these methods on samples with different anti-TB treatment background was assessed. Results In our study, 89 patients were prospectively included 82 patients with TBLN and 7 with other diseases. The overall sensitivities of Xpert, real-time PCR, histology, ZN staining, and culture were 86.6%, 69.5%, 58.5%, 43.9%, and 22.0%, respectively. The anti-TB treatment history revealed dramatic influences on the sensitivity of culture(P < 0.0001). In fact, the treatment that lasted over 3 months also influenced the sensitivity of Xpert(P < 0.05). However, the treatment history did not affect the performance of remaining tests(P > 0.05). While the Patient-Generated Subjective Global Assessment (PG-SGA) is a widely used nutritional assessment tool, the validation for Chinese patients with gastric cancer has not been performed yet. The aim of this research is to validate the Chinese version of the PGSGA in Chinese gastric cancer patients by conducting analyses of nutrition screening and assessment data. Methods Two thousand consecutive adult patients with gastric cancer were included in this observational, cross-sectional study in several tertiary hospitals in China. A survey was made among those patients, which included several questionnaires and some anthropometric parameters. We calculated the parameters to validate the use of PG-SGA categorizing the nutritional status of gastric cancer patients. Then we performed statistical analysis to evaluate the internal consistency, external consistency, and construct validity. Results This study verified the ability of the PG-SGA to differentiate gastric cancer patients into different nutritional groups. The confirmatory factor analysis verified the original four-factor structure of the PG-SGA. The internal and external consistency was measured by a concordance analysis which showed an intraclass correlation coefficients (ICC) of 0.987. The correlations of the Box 4 score with the Karnofsky performance status score demonstrated alternative form validity with good external consistency (Pearson, r = 0.643, P < 0.001). All three anthropometric dimensions of the PG-SGA were supported by principal component analysis. Spearman correlations (P < 0.001) were significant in the majority of tests. Exponential fertilization can induce nutrient reserves within juvenile trees, but its validity on poplar is contingent on interaction with a scientific irrigation regime due to limited water resources under global warming. In the present study, we raised 3200 Populus 9 euramericana cv. ‘74/76' cuttings under four irrigation regimes of 0(I0), 60 %(I60), 80 %(I80), and100 %(I100) of field capacity for soil moisture content with or without(control) the employment of nitrogen(N) addition delivered as exponential fertilization at the rate of 8 g N cutting^(-1)(E8) in an open-air nursery in Beijing, China. Both height and diameter increased with the increment of soil moisture ratio or in response to exponential fertilization(EF) without any interactive effects. In general, concentrations of N, phosphorus(P),and potassium(K) declined with time in stem but foliar N concentration did not change. Under the I100 regime, EF increased foliar N concentration relative to the control but failed to affect N concentration in stem in September, when both N concentration and N content were increased by EF under the I80 regime. Stem-K content and concentration by EF under the I80 regime also increased in September,therefore EF-treated cuttings had a higher ratio of K content in stem to that in whole plant(%ANAR). Vector diagnosis for nutritional status indicated that EF resulted in dilution of K concentration but induced a steady-state P uptake in leaves under all irrigation regimes. Therefore, EF of N addition could promote N uptake to leaves of P. 9 euramericana cv. ‘74/76' cuttings, but it had a null effect on N and P reserves in stem and impaired K reserves.In conclusion, the irrigation regime of 80 % field capacity of soil moisture content was suggested for the culture of juvenile P. 9 euramericana cv. ‘74/76' cuttings.
