32018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). 4Ganoderic acid hinders renal fibrosis via suppressing the TGF- β/Smad and MAPK signaling pathways显示文摘Renal fibrosis is considered as the pathway of almost all kinds of chronic kidney diseases(CKD)to the end stage of renal diseases(ESRD).Ganoderic acid(GA)is a group of lanostane triterpenes isolated from Ganoderma lucidum,which has shown a variety of pharmacological activities.In this study we investigated whether GA exerted antirenal fibrosis effect in a unilateral ureteral obstruction(UUO)mouse model.After UUO surgery,the mice were treated with GA(3.125,12.5,and 50 mg·kg^−1·d^−1,ip)for 7 or 14 days.Then the mice were sacrificed for collecting blood and kidneys.We showed that GA treatment dose-dependently attenuated UUO-induced tubular injury and renal fibrosis;GA(50 mg·kg^−1·d^−1)significantly ameliorated renal disfunction during fibrosis progression.We further revealed that GA treatment inhibited the extracellular matrix(ECM)deposition in the kidney by suppressing the expression of fibronectin,mainly through hindering the over activation of TGF-β/Smad signaling.On the other hand,GA treatment significantly decreased the expression of mesenchymal cell markers alpha-smooth muscle actin(α-SMA)and vimentin,and upregulated E-cadherin expression in the kidney,suggesting the suppression of tubular epithelial-mesenchymal transition(EMT)partially via inhibiting both TGF-β/Smad and MAPK(ERK,JNK,p38)signaling pathways.The inhibitory effects of GA on TGF-β/Smad and MAPK signaling pathways were confirmed in TGF-β1-stimulated HK-2 cell model.GA-A,a GA monomer,was identified as a potent inhibitor on renal fibrosis in vitro.These data demonstrate that GA or GA-A might be developed as a potential therapeutic agent in the treatment of renal fibrosis.Xiao-qiang Geng Ang Ma Jin-zhao He Liang Wang Ying-li Jia Guang-ying Shao Min Li Hong Zhou Shu-qian Lin Jian-hua Ran Bao-xue Yang 2020Acta Pharmacologica Sinica2020,41,5:43

6A single center investigation of bare-metal or drug-eluting stent restenosis from 1633 consecutive Chinese Han ethnic patients显示文摘Background Stents are widely used in China but the clinical impression is somehow that restenosis is less common because of the lower prevalence of coronary artery disease (CAD) and associated risk factors in Chinese populations. However, no large-sample published studies are available on angiographic stent restenosis including those of bare-metal stent (BMS) or drug-eluting stent (DES) in Chinese Han ethnic population. Mothods A total of 1633 consecutive patients with CAD who had undergone coronary stenting, quantitative coronary angiography (QCA) were retrospectively studied. At the time of stent implantation and at 7 months post-stenting 675 patients had a follow-up angiography. Statistical analysis was made with the chi-square test for categorical variables, unpaired t test for continuous variables, univariate or multivariate regression for baseline and angiographic characteristics and the Kaplan-Meier method for rate of target lesion revascularization (TLR). Results Stent restenosis was defined as ≥50% diameter stenosis in the dilated segment. A total of 675 patients with 1074 lesions were subjected to angiographic follow-up for 7 months on average. Of these lesions, 448 were implanted with BMS whereas 626 lesions with DES. At 7 months, bare-metal in-stent restenosis occured in 148 lesions (33.0%), and bare metal in-segment restenosis in 155 lesions (34.6%) in contrast to drug-eluting in-stent restenosis in 48 lesions (7.7%) and drug-eluting in-segment restenosis in 73 lesions (11.7%) (P<0.001 compared with BMS respectively). Late loss in both in-stent and in segment was higher in BMS than in DES groups [(1.00±0.69) vs (0.28±0.52); (0.78±0.71) vs (0.21±0.52), P<0.001 respectively]. Angulated lesion, lesion length, pre-procedural minimal luminal diameter (MLD), and BMS were independent predictors for TLR, (P<0.01 respectively), whereas current smoker, ostial lesion, and stent overlapping, post-procedure in-stent MLD, lesion length, and stent types were independent predictors for in-segment restenosis (P<0.01 respectively). Standard coronary risk factors such as hypertension, hyperlipidemia, diabetes, and history of CAD were not associated with a higher rate of restenosis caused by BMS or DES implantation in our Chinese Han ethnic population. 12Blocking FSH inhibits hepatic cholesterol biosynthesis and reduces serum cholesterol显示文摘Menopause is associated with dyslipidemia and an increased risk of cardio-cerebrovascular disease.The classic view assumes that the underlying mechanism of dyslipidemia is attributed to an insufficiency of estrogen.In addition to a decrease in estrogen, circulating follicle-stimulating hormone (FSH)levels become elevated at menopause.In this study,we find that blocking FSH reduces serum cholesterol via inhibiting hepatic cholesterol biosynthesis.First,epidemiological results show that the serum FSH levels are positively correlated with the serum total cholesterol levels,even after adjustment by considering the effects of serum estrogen,in addition,the prevalence of hypercholesterolemia is significantly higher in peri-menopausal women than that in premenopausal women.Furthermore,we generated a mouse model of FSH elevation by intraperitoneally injecting exogenous FSH into ovariectomized (OVX)mice,in which a normal level of estrogen (E2)was maintained by exogenous supplementation. Consistently,the results indicate that FSH,independent of estrogen,increases the serum cholesterol level in this mouse model. 18EXPERIMENTAL INVESTIGATION OF CHARACTERISTIC FREQUENCY IN UNSTEADY HYDRAULIC BEHAVIOUR OF A LARGE HYDRAULIC TURBINE显示文摘The features of unsteady flow such as pressure variation and fluctuation in a large hydraulic turbine usually lead to the instability of operation. This article reports the recent in site investigation concerning the characteristic frequencies in pressure fluctuation, shaft torsional oscillation and structural vibration of a prototype 700 MW Francis turbine unit. The investigation was carried out for a wide load range of 200 MW-700 MW in the condition of water head 57 m-90 m. An extensive analysis of both time-history and frequency data of these unsteady hydraulic behaviours was conducted. It was observed that the pressure fluctuation in a draft tube is stronger than that in upstream flow passage. The low frequency with about one third of rotation frequency is dominative for the pressure fluctuation in part load range. Also the unsteady features of vibration of head cover and torsional oscillation of shaft exhibited the similar features. Numerical analysis showed that the vibration and oscillation are caused by vortex rope in the draft tube. In addition, a strong vibration with special characteristic frequency was observed for the head cover in middle load range. The pressure fluctuation in the draft tube with the same frequency was also recorded. 19Insight-HXMT observations of the first binary neutron star merger GW170817显示文摘Finding the electromagnetic(EM) counterpart of binary compact star merger, especially the binary neutron star(BNS) merger,is critically important for gravitational wave(GW) astronomy, cosmology and fundamental physics. On Aug. 17, 2017,Advanced LIGO and Fermi/GBM independently triggered the first BNS merger, GW170817, and its high energy EM counterpart,GRB 170817 A, respectively, resulting in a global observation campaign covering gamma-ray, X-ray, UV, optical, IR, radio as well as neutrinos. The High Energy X-ray telescope(HE) onboard Insight-HXMT(Hard X-ray Modulation Telescope) is the unique high-energy gamma-ray telescope that monitored the entire GW localization area and especially the optical counterpart(SSS17 a/AT2017 gfo) with very large collection area(~1000 cm^2) and microsecond time resolution in 0.2-5 MeV. In addition,Insight-HXMT quickly implemented a Target of Opportunity(ToO) observation to scan the GW localization area for potential X-ray emission from the GW source. Although Insight-HXMT did not detect any significant high energy(0.2-5 MeV) radiation from GW170817, its observation helped to confirm the unexpected weak and soft nature of GRB 170817 A. Meanwhile,Insight-HXMT/HE provides one of the most stringent constraints(~10^(-7) to 10^(-6) erg/cm^2/s) for both GRB170817 A and any other possible precursor or extended emissions in 0.2-5 MeV, which help us to better understand the properties of EM radiation from this BNS merger.
