2Long-term efficacy of perioperative chemoradiotherapy on esophageal squamous cell carcinoma显示文摘AIM: To investigate the role of perioperative chemoradiotherapy (CRT) in the treatment of locally advanced thoracic esophageal squamous cell carcinoma (ESCC). METHODS: Using preoperative computed tomography (CT)-based staging criteria, 238 patients with ESCC (stage ⅡⅢ ) were enrolled in this prospective study between January 1997 and June 2004. With informed consent, patients were randomized into 3 groups: preoperative CRT (80 cases), postoperative CRT (78 cases) and surgery alone (S) (80 cases). The 1-, 3-, 5and 10-year survival were followed up. Progressionfree survival (PFS) was chosen as the primary endpoint by treatment arm measured from study entry until documented progression of disease or death from any cause. The secondary endpoint was overall survival (OS) determined as the time (in months) between the date of therapy and the date of death. Other objectives were surgical and adjuvant therapy complications.RESULTS: With median follow-up of 45 mo for all the enrolled patients, significant differences in the 1-, 3-, 5-, 10-year OS (91.3%, 63.5%, 43.5%, 24.5% vs 91%, 62.8%, 42.3%, 24.4% vs 87.5%, 51.3%, 33.8%, 12.5%, P = 0.0176) and PFS (89.3%, 61.3%, 37.5%, 18.1% vs 89.1%, 61.1%, 37.2%, 17.8% vs 84.5%, 49.3%, 25.9%, 6.2%, P = 0.0151) were detected among the 3 arms. There were no significant differences in OS and PFS between the preoperative CRT and postoperative CRT arm (P > 0.05). For the patients who had radical resection, significant differences in median PFS (48 mo vs 61 mo vs 39.5 mo, P = 0.0331) and median OS (56.5 mo vs 72 mo vs 41.5 mo, P = 0.0153) were detected among the 3 arms, but there were no significant differences in OS and PFS between the preoperative CRT and postoperative CRT arm (P > 0.05). The local recurrence rates in the preoperative CRT, postoperative CRT group and S group were 11.3%, 14.1% and 35%, respectively (P < 0.05). No significant differences were detected among the 3 groups when comparing complications but tended to be in favor of the postoperative CRT and S groups (P > 0.05). Toxicities of CRT in the preoperative or postoperative CRT arms were mostly moderate, and could be quickly alleviated by adequate therapy. 4Multi-objective optimization of a mixed-flow pump impeller using modified NSGA-II algorithm显示文摘In order to maintain a uniform distribution of pareto-front solutions, a modified NSGA-II algorithm coupled with a dynamic crowding distance(DCD) method is proposed for the multi-objective optimization of a mixed-flow pump impeller. With the pump meridional section fixed, ten variables along the shroud and hub are selected to control the blade load by using a three-dimensional inverse design method. Hydraulic efficiency, along with impeller head, is applied as an optimization objective; and a radial basis neural network(RBNN) is adopted to approximate the objective function with 82 training samples. Local sensitivity analysis shows that decision variables have different impacts on the optimization objectives. Instead of randomly selecting one solution to implement, a technique for ordering preferences by similarity to ideal solution(TOPSIS) is introduced to select the best compromise solution(BCS) from pareto-front sets. The proposed method is applied to optimize the baseline model, i.e. a mixed- flow waterjet pump whose specific speed is 508 min?1?m3s?1?m. The performance of the waterjet pump was experimentally tested. Compared with the baseline model, the optimized impeller has a better hydraulic efficiency of 92% as well as a higher impeller head at the design operation point. Furthermore, the off-design performance is improved with a wider highefficiency operation range. After optimization, velocity gradients on the suction surface are smoother and flow separations are eliminated at the blade inlet part. 5Effect of neoadjuvant chemoradiotherapy on prognosis and surgery for esophageal carcinoma显示文摘AIM:To investigate the role of neoadjuvant chemoradiotherapy in prognosis and surgery for esophageal carcinoma by a meta-analysis.METHODS:PubMed and manual searches were done to identify all published randomized controlled trials(RCTs) that compared neoadjuvant chemoradiotherapy plus surgery(CRTS) with surgery alone(S) for esophageal cancer.According to the test of heterogeneity,a fi xed-effect model or a random effect model was used and the odds ratio(OR) was the principal measure of effects.RESULTS:Fourteen RCTs that included 1737 patients were selected with quality assessment ranging from A to C(Cochrane Reviewers' Handbook 4.2.2).OR(95% CI,P value),expressed as CRTS vs S(values>1 favor CRTS arm),was 1.19(0.94-1.48,P=0.28) for 1-year survival,1.33(1.07-1.65,P=0.69) for 2-year survival,1.76(1.42-2.19,P=0.11) for 3-year survival,1.41(1.06-1.87,P=0.11) for 4-year survival,1.64(1.28-2.12,P=0.40) for 5-year survival,0.82(0.39-1.73,P<0.0001) for rate of resection,1.53(1.33-2.84,P=0.007) for rate of complete resection,1.78(1.14-2.78,P=0.79) for operative mortality,1.12(0.89-2.48,P=0.503) for all treatment mortality,1.33(0.94-1.88,P=0.04) for the rate of adverse treatment,1.38(1.23-1.63,P=0.0002) for local-regional cancer recurrence,1.28(0.85-1.58,P=0.60) for distant cancer recurrence,and 1.27(0.86-1.65,P=0.19) for all cancer recurrence.A complete pathological response to chemoradiotherapy occurred in 10%-45.5% of patients.The 5-year survival benefi t was most pronounced when chemotherapy and radiotherapy were given concurrently(OR:1.45,95% CI:1.26-1.79,P=0.015) instead of sequentially(OR:0.85,95% CI:0.64-1.35,P=0.26).CONCLUSION:Compared with surgery alone,neoadjuvant chemoradiotherapy can improve the long-term survival and reduce local-regional cancer recurrence.Concurrent administration of neoadjuvant chemoradiotherapy was superior to sequential chemoradiotherapy.Jin Lv Xiu-Feng Cao Bin Zhu Lv Ji Lei Tao Dong-Dong Wang 2009World Journal of Gastroenterology2009,15,39:27
6Flow Ripple of Axial Piston Pump with Computational Fluid Dynamic Simulation Using Compressible Hydraulic Oil显示文摘The flow ripple, which is the source of noise in an axial piston pump, is widely studied today with the computational fluid dynamic(CFD) technology development. In the traditional CFD modeling, the fluid compressibility, which strongly influences the accuracy of the flow ripple simulation results, is often neglected. So a compressible sub-model was added with user defined function(UDF) in the CFD model to predict the flow ripple. At the same time, a test rig of flow ripple was built to study the validity of simulation. The flow ripple of pump was tested with different working parameters, including the rotation speed and the working pressure. The comparisons with experimental results show that the validity of the CFD model with compressible hydraulic oil is acceptable in analyzing the flow ripple characteristics. In this paper, the improved CFD model increases the accuracy of flow ripple rate to about one-magnitude order. Therefore, the compressible model of hydraulic oil is necessary in the flow ripple investigation of CFD simulation. The compressibility of hydraulic oil has significant effect on flow ripple, and the compression ripple takes about 88% of the total flow ripple of pump. Leakage ripple has the lowest proportion of about 4%, and geometrical ripple leakage ripple takes the remnant 8%. Besides, the influence of working parameters was investigated through the CFD simulations and experimental measurements. Comparison results show that the amplitude of flow ripple grows with the increasing of rotation speed and working pressure, and the flow ripple rate is independent of the rotation speed. However, flow ripple rate of piston pump grows with the increasing of working pressure, because the leakage ripple will increase with the pressure growing. 7Over-expression of Metastasis-associated in Colon Cancer-1 (MACC1) Associates with Better Prognosis of Gastric Cancer Patients显示文摘Objective: The aim of this study was to detect metastasis-associated in colon cancer-1 (MACC1) expression in Chinese gastric cancer and analyze the relationship between MACC1 expression and postoperative survival. Methods: The expression of MACC1 and c-MET protein in a sample of 128 gastric cancer tissues was detected by immunohistochemistry. A retrospective cohort study on the prognosis was carried out and data were collected from medical records. Results: The positive rate of MACC1 protein expression in gastric cancer was 47.66%, higher than that in adjacent noncancerous mucosa (P<0.001). MACC1 protein expression was not related to the clinicopathological variables involved. Kaplan-Meier analysis revealed that the survival of MACC1 positive group tended to be better than that of MACC1 negative group, particularly in patients with stage III carcinoma (P=0.032). Cox regression analysis revealed that MACC1 protein over-expression in gastric cancer tended to be a protective factor with hazard ratio of 0.621 (P=0.057). Immunohistochemical analysis showed that the positive rate of c-MET protein expression was much higher in cases with positive MACC1 expression in gastric cancer (P=0.002), but P53 expression was not associated with MACC1 expression. 9Efficacy and safety of metformin and sitagliptin based triple antihyperglycemic therapy(STRATEGY):a multicenter,randomized,controlled,non-inferiority clinical trial显示文摘Despite the current guideline's recommendation of a timely stepwise intensification therapy,the 'clinical inertia',termed as the delayed treatment intensification,commonly exists in the real world,which may be partly due to the relatively little substantial evidence and no clear consensus regarding the efficacy and safety of triple oral agents in patients inadequately controlled with dual therapy.In this clinical trial performed in 237 centers in China,5,535 type 2 diabetic patients inadequately controlled by previous therapies were treated with a stable metformin/sitagliptin dual therapy for 20 weeks.The patients who did not reach the glycated hemoglobin A1c(HbA1c) goal were then further randomized into glimepiride,gliclazide,repaglinide,or acarbose group for an additional 24-week triple therapy.A mean HbAlc reduction of 0.85%was observed when sitagliptin was added to the patients inadequately controlled with metformin in 16 weeks.Further HbAlc reductions in the 24-week triple therapy stage were 0.65%in glimepiride group,0.70%in gliclazide group,0.61%in repaglinide group,and 0.45%in acarbose group.The non-inferiority criterion for primary hypotheses was met for gliclazide and repaglinide,but not for acarbose,compared with glimepiride,when added to metformin/sitagliptin dual therapy.The incidences of adverse events(AEs) were 29.2%in the dual therapy stage and30.3%in the triple therapy stage.Metformin/sitagliptin as baseline therapy,with the addition of a third oral antihyperglycemic agent,including glimepiride,gliclazide,repaglinide,or acarbose,was effective,safe and well-tolerated for achieving an HbAlc<7.0%goal in type 2 diabetic patients inadequately controlled with previous therapies.The timely augmentation of up to three oral antihyperglycemic agents is valid and of important clinical benefit to prevent patients from exposure to unnecessarily prolonged hyperglycemia.Wen Xu Yiming Mu Jiajun Zhao Dalong Zhu Qiuhe Ji Zhiguang Zhou Bin Yao Anhua Mao Samuel S.Engel Bin Zhao Yan Bi Longyi Zeng Xingwu Ran Juming Lu Linong Ji Wenying Yang Weiping Jia Jianping Weng 2017Science China(Life Sciences)2017,60,3:19

11Recombinant Newcastle disease virus expressing African swine fever virus protein 72 is safe and immunogenic in mice显示文摘African swine fever(ASF) is a lethal hemorrhagic disease that affects wild and domestic swine. The etiological agent of ASF is African swine fever virus(ASFV). Since the first case was described in Kenya in 1921, the disease has spread to many other countries. No commercial vaccines are available to prevent ASF. In this study, we generated a recombinant Newcastle disease virus(r NDV) expressing ASFV protein 72(p72) by reverse genetics and evaluated its humoral and cellular immunogenicity in a mouse model. The recombinant virus, r NDV/p72, replicated well in embryonated chicken eggs and was safe to use in chicks and mice. The p72 gene in r NDV/p72 was stably maintained through ten passages. Mice immunized with r NDV/p72 developed high titers of ASFV p72 specific Ig G antibody, and had higher levels of Ig G1 than IgG2 a. Immunization also elicited T-cell proliferation and secretion of IFN-γ and IL-4. 12LGR5 is a promising biomarker for patients with stage Ⅰ and Ⅱ gastric cancer显示文摘Objective: To investigate Leucine-rich repeat-containing G protein-coupled receptor 5 (LGR5) expressions in gastric cancer and to evaluate its clinical significance. Methods: LGR5 expression was assessed by immunohistochemistry in 257 gastric cancer patients after surgery. The relationships between LGR5 expression and clinicopathological features and patients prognosis were statistically analyzed. Results: The expression of LGR5 was significantly higher in gastric cancers as a cancer stem cell marker than in adjacent normal tissues (P<0.001), and more frequently in patients with intestinal type, well-moderate differentiation and stage Ⅰ and Ⅱ (P<0.05). Although we found gastric cancer patients with LGR5 positive expression had a poorer prognosis, it didn't meet statistical significance (P>0.05). LGR5 negative expression was significantly related to the favorable overall survival in stage Ⅰ and Ⅱ gastric cancer patients (P<0.05). Furthermore, patients with high LGR5 expression tended to be more likely to get progression and have poorer progress-free survival (P<0.05). Multivariate Cox regression analysis revealed that LGR5 expression was an independent factor of overall survival for the patients with stage Ⅰ and Ⅱ gastric cancer (P<0.05). 14NUMERICAL INVESTIGATION OF THE VENTILATED CAVITATING FLOW AROUND AN UNDER-WATER VEHICLE BASED ON A THREE-COMPONENT CAVITATION MODEL显示文摘Based on the Reynolds-Averaged Navier-Stokes equations and mass transfer model,an approach,where a three-component cavitation model is proposed,is presented to simulate ventilated cavitating flow as well as natural cavitation.In the proposed cavitation model,the initial content of nucleus in the local flow field is updated instantaneously,and is coupled with the Rayleigh-Plesset equation to capture the cavity development.The proposed model is applied to simulate the cavitating flow around an under-water vehicle in different cavitation conditions.The results indicate that for the natural and ventilated cavitation simulation,the predicted cavitation characteristics including the cavity length,cavity diameter and cavity shape agree satisfactorily with the analytic and experimental results,for the ventilated cavitation,the proposed methods reproduce the special behavior that the axial line of the cavity bends and rises at the tail part.The study concludes that the ventilated flow rate of the non-condensate gas influences the development of natural cavitation as well as ventilated cavitation,and the vapor cavity is suppressed remarkably by the gas cavity with the increase of the gas ventilation.JI Bin LUO Xian-wu PENG Xiao-xing ZHANG Yao WU Yu-lin XU Hong-yuan 2010Journal of Hydrodynamics2010,22,6:16
15Microwave Exposure Impairs Synaptic Plasticity in the Rat Hippocampus and PC12 Cells through Over-activation of the NMDA Receptor Signaling Pathway显示文摘Objective The aim of this study is to investigate whether microwave exposure would affect the N-methyl-D-aspartate receptor(NMDAR) signaling pathway to establish whether this plays a role in synaptic plasticity impairment. Methods 48 male Wistar rats were exposed to 30 m W/cm2 microwave for 10 min every other day for three times. Hippocampal structure was observed through H&E staining and transmission electron microscope. PC12 cells were exposed to 30 m W/cm2 microwave for 5 min and the synapse morphology was visualized with scanning electron microscope and atomic force microscope. The release of amino acid neurotransmitters and calcium influx were detected. The expressions of several key NMDAR signaling molecules were evaluated. Results Microwave exposure caused injury in rat hippocampal structure and PC12 cells, especially the structure and quantity of synapses. The ratio of glutamic acid and gamma-aminobutyric acid neurotransmitters was increased and the intracellular calcium level was elevated in PC12 cells. A significant change in NMDAR subunits(NR1, NR2 A, and NR2B) and related signaling molecules(Ca2+/calmodulin-dependent kinase II gamma and phosphorylated c AMP-response element binding protein) were examined. Conclusion 30 m W/cm2 microwave exposure resulted in alterations of synaptic structure, amino acid neurotransmitter release and calcium influx. 16Association between Bmi1 and clinicopathological status of esophageal squamous cell carcinoma显示文摘AIM:To investigate the clinicopathological roles of Bmi1 in esophageal squamous cell carcinoma(ESCC) .METHODS:Quantitative real-time polymerase chain reaction and immunohistochemical staining for Bmi1 were performed in cancerous and adjacent non-cancerous paraffi n-embedded esophageal specimens.RESULTS:The Bmi1 expression level was unaffected by gender and age.The level of Bmi1 mRNA in ESCC was signif icantly higher than that in the adjacent non-cancerous tissues(2.181 ± 2.158 vs 0.931 ± 0.894,P = 0.0152) ,and its over-expression was aggressively associated with lymph node metastasis(3.580 ± 2.487 vs 1.703 ± 0.758,P = 0.0003) ,poorer cell differentiation(P = 0.0000) and advanced pathological stage(3.827 ± 2.673 vs 1.590 ± 0.735,P = 0.0001) .The patients were divided into high-expression and low-expression groups based on the median expression level of Bmi1 mRNA,and a shorter overall survival time in the former group was observed.Immunohistochemistry for Bmi1 oncoprotein showed diffusely positive,focally positive and negative expression in 44,16 and 10 of 70 ESCC cases,respectively,compared with three,two and five of 10 adjacent non-cancerous cases(P=0.027) .The positive rate of the oncoprotein in samples of histological grade was higher than that of grade(P=0.031) ,but its expression had no relation to the lymph node metastasis and pathological staging.In 70 ESCC samples,Bmi1 showed high intense expression in the cytoplasm and less or even no expression in the nucleus.CONCLUSION:Bmi1 was over-expressed in ESCC.Increased Bmi1 mRNA expression was significantly associated with ESCC progression,and the oncoprotein was largely distributed in the cytoplasm of tumor cells.Xiao-Ting He Xiu-Feng Cao Lv Ji Bin Zhu Jin Lv Dong-Dong Wang Pei-Hua Lu Heng-Guan Cui 2009World Journal of Gastroenterology2009,15,19:15
17Molecular Epidemiological Characteristics of Streptococcus pyogenes Strains Involved in an Outbreak of Scarlet Fever in China,2011显示文摘Objective To investigate molecular characterization of streptococcus pyogenes isolates involved in an outbreak of scarlet fever in China in 2011.Methods Seventy-four Streptococcal pyogenes involved in an outbreak of scarlet fever were isolated from pediatric patients in the areas with high incidence in China from May to August of 2011.Emm genotyping,pulsed-field gel electrophoresis (PFGE),superantigen (SAg) genes and antimicrobial susceptibility profiling were analyzed for these isolates.Results A total of 4 different emm types were identified.Emm12 was the most prevalent type which contained four predominating PFGE patterns corresponding to four different virulence and superantigen profiles.Emm12 (79.7%) and emml (14.9%) accounted for approximately 94%of all the isolates.The speA gene was all negative in emm12 isolates and positive in emml isolates.All strains were resistant to erythromycin,and 89.4%of them were resistant to erythromycin,tracycline,and clindamycin simultaneously.Conclusion Several highly diversified clones with a high macrolide resistance rate comprise a predominant proportion of circulating strains,though no new emm type was found in this outbreak.The data provide a baseline for further surveillance of scarlet fever,which may contribute to the explanation of the outbreak and development of a GAS vaccine in China.YOU Yuan Hai SONG Yan Yan YAN Xiao Mei WANG Hai Bin ZHANG Meng Han TAO Xiao Xia Li Lei Lei ZHANG Yu Xin JIANG Xi Hong ZHANG Bing Hua ZHOU Hao XIAO Di JIN Lian Mei FENG Zi Jian LUO Feng Ji ZHANG Jian Zhong 2013Biomedical and Environmental Sciences2013,26,11:15

19Identification of PRDX6 as a regulator of ferroptosis显示文摘Ferroptosis is a newly characterized iron-dependent form of nonapoptotic regulated cell death triggered by lipid reactive oxygen species(LOOH).The dysregulation of ferroptosis is highly related to cancer,and the induction of ferroptosis is also proposed as a potential strategy for cancer therapy.Although several key regulators have been identified that are involved in ferroptosis,the molecular mechanism underlying this process remains largely unknown.Here,we report that Peroxiredoxin-6(PRDX6)is a bona fide negative regulator of ferroptotic cell death.The knockdown of intracellular PRDX6 significantly enhances LOOH and ferroptotic cell death triggered by ferroptosis inducers(Erastin and RSL-3),which is correlated with the transcriptional activation of heme oxygenase-1.Moreover,overexpression of heme oxygenase-1 enhances both Erastin-and RSL-3-triggered LOOH,suggesting that heme oxygenase-1 mediates PRDX6 silencing-enhanced ferroptosis.More importantly,the application of a specific PRDX6 phospholipase A2(iPLA2)inhibitor,MJ-33,synergistically enhances the ferroptosis induced by Erastin,suggesting that PRDX6 removes LOOH through its iPLA2 activity.Thus,our findings reveal an essential role of PRDX6 in protecting cells against ferroptosis and provide a potential target to improve the antitumor activity of ferroptosis-based chemotherapy.Bin Lu Xiao-bing Chen Yu-cai Hong Hong Zhu Qiao-jun He Bo Yang Mei-dan Ying Ji Cao 2019Acta Pharmacologica Sinica2019,40,10:13
20From intrinsic dielectric loss to geometry patterns: Dual-principles strategy for ultrabroad band microwave absorption显示文摘As electromagnetic absorbers with wide absorption bandwidth are highly pursued in the cutting-edge electronic and telecommunication industries, the traditional dielectric or magnetic bulky absorbers remain concerns of extending the effective absorption bandwidth. In this work, a dual-principle strategy has been proposed to make a better understanding of the impact of utilizing conductive absorption fillers coupled with implementing artificial structures design on the absorption performance. In the comparison based on the microscopic studies, the carbon nanotubes (CNTs)-based absorbers are confined to narrow operating bandwidth and relatively fixed response frequency range, which can not fulfill the ever-growing demands in the application. With subsequent macroscopic structure design based on the CNTs-based dielectric fillers, the artificial patterns show much more broadened absorption bandwidth, covering the majority of C-band, the whole X-band, and Ku-band, due to the tailored electromagnetic parameters and more reflections and scatterings.
