2Prevalence of minimal hepatic encephalopathy and quality of life evaluations in hospitalized cirrhotic patients in China显示文摘AIM:To investigate the prevalence of minimal hepatic encephalopathy(MHE)and to assess corresponding health-related quality of life(HRQoL)in hospitalized cirrhotic patients in China.METHODS:This multi-center cross-sectional study included 16 teaching hospitals,which were members of 'Hepatobiliary Cooperation Group,Society of Gastroenterology,Chinese Medical Association',from different areas of China carried out between June and October in 2011.All the eligible hospitalized cirrhotic patients(n = 538)were required to complete triplicate number connection tests combined with one digit symbol test for diagnosing MHE.Patients' clinical examination data were complemented by a modified questionnaire assessing HRQoL.Written informed consent was obtained from each patient.RESULTS:Male was predominant(68.6%)in 519 patients who met the criteria of the study,with a mean age of 49.17 ± 11.02 years.The most common cause of liver cirrhosis was chronic hepatitis B(55.9%).The prevalence of MHE was 39.9% and varied by ChildPugh-Classification score(CPC-A:24.8%,CPC-B:39.4% and CPC-C:56.1%,P < 0.01).MHE(P < 0.01)and higher CPC scores(P < 0.01)were associated with a high HRQoL scores(reflecting poorer quality of life).The prevalence of MHE was proportionate to CPC(P = 0.01)and high quality of life scores(P = 0.01).CONCLUSION:Hospitalized cirrhotic patients have a high prevalence of MHE that is proportionate to the degree of liver function and HRQoL impairment.Ji-Yao Wang Ning-Ping Zhang Bao-Rong Chi Yu-Qing Mi Li-Na Meng Ying-Di Liu Jiang-Bin Wang Hai-Xing Jiang Jin-Hui Yang Yun Xu Xiao Li Jian-Ming Xu Guo Zhang Xin-Min Zhou Yu-Zheng Zhuge De-An Tian Jin Ye Yu-Lan Liu 2013World Journal of Gastroenterology2013,19,30:64

5Microstructure and Properties of Hot Working Die Steel H13MOD显示文摘Compared with H13 steel,the influences of different heat treatment process on the microstructure and properties of the new type of hot working die steel H13MOD were studied.The results show that the complete austenitizing temperature of H13MOD is around 1 030℃ and the quenching hardness achieves the maximum value at this temperature.While for H13,the complete austenitizing temperature is above 1 100℃ and the quenching hardness rise constantly with the quenching temperature increasing.In quenching process,the undissolved MC carbides can prevent the coarsening of grain in both steels.With the rise of quenching temperature,when MC carbides dissolve completely,the grain grows quickly.The hardness and strength of H13MOD at higher tempering temperature(above570℃)are nearly the same as those of H13,but its toughness is higher than that of H13.Mo2C carbide is the main strengthening phase in H13MOD,which is attributed to the higher content of Mo.The quantity of VC eutectic carbides is reduced because of lower content of V in H13MOD,which plays an important role in enhancing the impact toughness of H13MOD.Under a certain strength condition,H13MOD steel can be used in the environment that higher toughness is required and the service life of die casting mold can be improved.ZHOU Jian MA Dang-shen CHI Hong-xiao CHEN Zai-zhi LI Xiang-yang 2013Journal of Iron and Steel Research(International)2013,20,9:25
6Repurposing vitamin D for treatment of human malignancies via targeting tumor microenvironment显示文摘Tumor cells along with a small proportion of cancer stem cells exist in a stromal microenvironment consisting of vasculature, cancer-associated fibroblasts, immune cells and extracellular components.Recent epidemiological and clinical studies strongly support that vitamin D supplementation is associated with reduced cancer risk and favorable prognosis. Experimental results suggest that vitamin D not only suppresses cancer cells, but also regulates tumor microenvironment to facilitate tumor repression. In this review, we have outlined the current knowledge on epidemiological studies and clinical trials of vitamin D. Notably, wesummarized and discussed the anticancer action of vitamin D in cancer cells, cancer stem cells and stroma cells in tumor microenvironment, providing a better understanding of the role of vitamin D in cancer. 7Toward Cloud Computing QoS Architecture:Analysis of Cloud Systems and Cloud Services显示文摘Cloud can be defined as a new computing paradigm that provides scalable, on-demand, and virtualized resources for users. In this style of computing, users can access a shared pool of computing resources which are provisioned with minimal management efforts of users. Yet there are some obstacles and concerns about the use of clouds. Guaranteeing quality of service(Qo S) by service providers can be regarded as one of the main concerns for companies tending to use it. Service provisioning in clouds is based on service level agreements representing a contract negotiated between users and providers. According to this contract, if a provider cannot satisfy its agreed application requirements, it should pay penalties as compensation. 9Epigastric pain syndrome: What can traditional Chinese medicine do? A randomized controlled trial of Biling Weitong Granules显示文摘BACKGROUND Recent research suggests that although prokinetic agents,acid suppressors,and radical treatment for Helicobacter pylori infection may be effective in patients with functional dyspepsia(FD),a large proportion of patients still fail to respond to these treatments or may suffer from severe adverse reactions.Many traditional Chinese medicinal herbs can regulate the status of the entire body and have special advantages in the treatment of functional diseases.The present study was designed to verify the efficacy of Biling Weitong Granules(BLWTG),a traditional Chinese medicinal herbal compound formula,in alleviating epigastric pain syndrome(EPS)in FD patients,in an attempt to provide an effective prescription for the clinical treatment of this disease.AIM To evaluate the clinical efficacy and safety of BLWTG in treating EPS in patients with FD.METHODS In this multicenter,stratified,randomized,double-blind,placebo-controlled,parallel group clinical trial,eligible patients were randomized into the BLWTG and placebo groups who were treated for 6 wk.Efficacy indicators including the severity and frequency of EPS and the time to pain resolution and safety indicators including adverse events were observed and compared.RESULTS The baseline demographic data and clinical characteristics,such as epigastric pain symptoms,pain intensity,and frequency of attacks,were matched between the two groups before randomization.After 6 wk of treatment and after the center effect was eliminated,the epigastric pain was significantly improved in 28.33%and 85.59%of the patients in the placebo and BLWTG groups,respectively(P<0.05).At 6 wk,the resolution rate of epigastric pain was 15%and 69.49%in the placebo and BLWTG groups,respectively(P<0.05).The differences of total FD clinical score between these two groups were significant(P<0.05)at 2,4,and 6 wk(P<0.05).The scores of each item and the total score in the Functional Digestive Disorders Quality of Life Questionnaire showed significant differences between the two groups at 6 wk after both the center and interaction effects were eliminated(P<0.05).There was no significant difference in the incidence of adverse events between the two groups,and no serious adverse event was noted during the observation.CONCLUSION Compared with placebo,BLWTG markedly improved EPS in FD patients without causing serious adverse reactions.Yan-Dong Wen Fang Lu Ying-Pan Zhao Ping Wang Qian Yang Jun-Xiang Li Hui-Zhen Li Li-Li Chi Zheng-Hua Zhou Yan-Ping Tang Jin-Kang Xu Yang Zhao Xu-Dong Tang 2020World Journal of Gastroenterology2020,26,28:16
10Swelling of clay minerals in ammonium leaching of weathered crust elution-deposited rare earth ores显示文摘The effect of hydrated radius, cation valence, pH and solution concentration on the zeta potential of clay minerals was investigated, and the relation between zeta potential of clay minerals and swelling was discussed in different leaching agents as well. The results show that the zeta potential of clay minerals decreases with the hydrated ionic radius increasing. It could be seen that the zeta potential of the clay minerals in AICl_3 solution is positive,whereas that in NH_4C1, KCl and MgCl_2 solution is negative. And the zeta potential of clay minerals increases with the cation valence increasing. Moreover, the zeta potential of clay minerals decreases with the solution pH increasing,whereas that increases with the solution concentration increasing in different ammonium solutions. In addition,the swelling of clay minerals decreases while the zeta potential of clay minerals increasing in different ammonium solutions. 11Effects and mechanisms of electroacupuncture at PC6 on frequency of transient lower esophageal sphincter relaxation in cats显示文摘AIM: To investigate the effects of electroacupuncture (EA) at neiguan (PC6) on gastric distention-induced transient lower esophageal sphincter relaxations (TLESRs) and discuss the mechanisms of this treatment. METHODS: ProtocolⅠ: Twelve healthy cats underwent gastric distention for 60 min on the first day. Electrical acupoint stimulation was applied at the neiguan or a sham point on the hip in randomized order before gastric distention, on the third day and fifth day. Those cats that underwent EA at neiguan on the fifth day were named 'Neiguan Group' and the cats that underwent EA at a sham acupoint on the fifth day were named 'Sham Group' (control group). During the experiment the frequency of TLESRs and lower esophageal sphincter (LES) pressure were observed by a perfused sleeve assembly. Plasma levels of gastrin (GAS) and motilin (MTL) were determined by radioimmunoassay. Nitrite/nitrate concentration in plasma and tissues were measured by Griess reagent. The nuclei in the brain stem were observed by immunohistochemistry method of c-Fos and NADPH-d dyeing. Protocol Ⅱ: Thirty six healthy cats were divided into 6 groups randomly. We gave saline (2 mL iv. control group), phaclofen (5 mg/kg iv. GABA-B antagonist), cholecystokinin octapeptide (CCK-8) (1 μg/kg per hour iv.), L-Arginine (200 mg/kg iv.), naloxone (2.5 μmol/kg iv.) and tacrine (5.6 mg/kg ip. cholinesterase inhibitor) respectively before EA at Neiguan and gastric distention. And the frequencies of TLESRs in experimental groups were compared with the control group.RESULTS: ProtocolⅠ: Not only the frequency of gastric distention-induced TLESR in 60 min but also the rate of common cavity during TLESRs were significantlydecreased by EA at neiguan compared to that of sham acupoint stimulation. C-Fos immunoreactivity and NOS reactivity in the solitarius (NTS) and dorsal motor nucleus of the vagus (DMV) were significantly decreased by EA at neiguan compared to that of the sham group. However, the positive nuclei of C-Fos and NOS in reticular formation of the medulla (RFM) were increased by EA at neiguan. Protocol Ⅱ: The inhibited effect of EA at neiguan on TLESR’s frequency was completely restored by pretreatment with CCK (23.5/h vs 4.5/h, P < 0.05), L-arginine (17.5/h vs 4.5/h, P < 0.05) and naloxone(12/h vs 4.5/h, P < 0.05). On the contrary, phaclofen (6/h vs 4.5/h, P > 0.05) and tacrine (9.5/h vs 4.5/h, P > 0.05) did not influence it. CONCLUSION: Electric acupoint stimulation at Neiguan significantly inhibits the frequency of TLESR and the rate of common cavity during TLESR in cats. This effect appears to act on the brain stem, and may be mediated through nitric oxide (NO), CCK-A receptor and mu-opioid receptors. 13Toxicity of the brominated flame retardant tris-(2,3-dibromopropyl) isocyanurate in zebrafish (Danio rerio)显示文摘Tris-(2,3-dibromopropyl) isocyanurate (TBC) is a heterocyclic brominated flame retardant that was recently detected in the environment in China.TBC is semi-volatile and can accumulate in the lipid of some species,but little is known about its effect on aquatic organisms.We exposed adult zebrafish to 0,0.25,1 and 4mg/L TBC for 28 d and measured the effect on survival,growth,histopathology,hormone levels,enzyme activity,and gene expression.TBC exposure had no effect on survival or growth.We observed significant damage to the liver and gill,including hepatocellular swelling and fatty degeneration in the liver as well as proliferation and edema of epithelial cells in the gills.In addition,exposure to 4mg/L TBC induced proliferation of goblet cells in the intestine of both sexes,acellular areas in the testis,and thinly scattered vitellogenic granules in vitellogenic oocytes.TBC exposure had no effect on the levels of thyroid hormones,testosterone,estradiol,liver superoxide dismutase activity,malondialdehyde content,and brain cholinesterase activity.By contrast,hepatic vitellogenin and cytochrome P4501A gene expression was significantly down-regulated in both male and female zebrafish in response to TBC exposure.Our results suggest that exposure to TBC causes a variety of potential reproductive and endocrine toxic effects.ZHANG Xu LI Juan CHEN MinJie WU Le ZHANG Chi ZHANG Jie ZHOU QunFang LIANG Yong 2011Chinese Science Bulletin2011,56,15:13

16A new variational inequality formulation for unconfined seepage flow through fracture networks显示文摘Darcy's law only applying to the flow domain is extended to the entire fracture network domain including the dry domain.The partial differential equation(PDE) formulation for unconfined seepage flow problems for discrete fracture network is established,in which a boundary condition of Signorini's type is prescribed over the potential seepage surfaces.In order to reduce the difficulty in selecting trial functions,a new variational inequality formulation is presented and mathematically proved to be equivalent to the PDE formulation.The numerical procedure based on the VI formulation is proposed and the corresponding algorithm has been developed.Since a continuous penalized Heaviside function is introduced to replace a jump function in finite element analysis,oscillation of numerical integration for facture elements cut by the free surface is eliminated and stability of numerical solution is assured.The numerical results from two typical examples demonstrate,on the one hand the effectiveness and robustness of the proposed method,and on the other hand the capability of predicting main seepage pathways in fractured rocks and flow rates out of the drainage system,which is very important for performance assessments and design optimization of complex drainage system.JIANG QingHui YE ZuYang YAO Chi ZHOU ChuangBing 2012Science China(Technological Sciences)2012,55,11:11
17Negative reflection and negative surface wave conversion from obliquely incident electromagnetic waves显示文摘Complete control of spatially propagating waves(PWs)and surface waves(SWs)is an ultimate goal that scientists and engineers seek for,in which negative reflection of PW and negative surface wave are two exotic phenomena.Here,we experimentally demonstrate an anisotropic digital coding metasurface capable of controlling both PWs and SWs with a single coding pattern.On the basis of the digital description of coding metasurfaces,a simple coding method is proposed to allow dual functionalities(either PW or SW manipulations)under two orthogonal polarizations at arbitrarily oblique incidences,thus improving the adaptability of digital coding metasurfaces in more practical circumstances.With elaborately designed ellipse-shaped coding particles,we experimentally demonstrate various functions under oblique incidences,including the negative reflection of PW,negative SW,anomalous reflection and their arbitrary combinations,all having good agreements with theoretical and numerical predictions.We believe that the proposed method may enable the digital coding metasurfaces to have broad applications in radar detections,wireless communications and imaging.Shuo Liu Tie Jun Cui Ahsan Noor Zui Tao Hao Chi Zhang Guo Dong Bai Yan Yang Xiao Yang Zhou 2018Light(Science & Applications)2018,7,1:10
18Chinese Herbal Medicine Combined with Entecavir for HBeAg Positive Chronic Hepatitis B:Study Protocol for a Multi-Center,Double-Blind Randomized-Controlled Trial显示文摘Background: The domestic prevalence of chronic hepatitis B(CHB) in China is 7.18% in 2006, imposing great societal healthcare burdens. Nucleot(s)ide analogues(NUCs) anti-hepatitis B virus(HBV) therapies are widely applied despite the relatively low rate of seroconversion and high risk of drug-resistant mutation. More effective treatments for CHB deserve further explorations. Combined therapy of NUCs plus Chinese herbal medicine(CHM) is widely accepted in China, which is recognized as a prospective alternative approach. The study was primarily designed to confirm the hypothesis that Tiaogan-Yipi Granule(调肝益脾颗粒, TGYP) or Tiaogan-Jianpi-Jiedu Granule(调肝健脾解毒颗粒, TGJPJD) plus entecavir tablet(ETV) was superior over ETV monotherapy in enhancing HBeA g loss rate. Methods: The study was a nationwide, large-scale, multi-center, double-blind, randomized, placebo-control ed trial with a designed duration of 108 weeks. A total of 16 hospitals and 596 eligible Chinese HBeA g positive CHB patients were enrolled from November 2012 to September 2013 and randomly allocated into 2 groups in 1:1 ratio via central randomization system: experimental group(EG) and control group(CG). Subjects in EG received CM formulae(TGYP or TGJPJD, 50 g per dose, twice daily) plus ETV tablet(or ETV placebo) 0.5 mg per day in the first 24 weeks(stage 1), and CHM granule plus ETV tablet(0.5 mg per day) from week 25 to 108(stage 2). Subjects in CG received CHM Granule placebo plus ETV tablet(0.5 mg per day) for 108 weeks throughout the trial. The assessments of primary outcomes(HBV serum markers and HBV-DNA) were conducted by a third-party College of American Pathologists(CAP) qualified laboratory. Adverse effects were observed in the hospitals of recruitment. Discussion: The study was designed to compare the curative effect of CM plus ETV and ETV monotherapy in respect of HBe Ag loss, which is recognized by the European Association for the Study of the Liver as 'a valuable endpoint'. We believe this trial could provide a reliable status for patients' 'journey' towards durable responses after treatment discontinuation.
