5Association of Overlapped and Un-overlapped Comorbidities with COVID-19 Severity and Treatment Outcomes: A Retrospective Cohort Study from Nine Provinces in China显示文摘Objective Several COVID-19 patients have overlapping comorbidities. The independent role of each component contributing to the risk of COVID-19 is unknown, and how some non-cardiometabolic comorbidities affect the risk of COVID-19 remains unclear.Methods A retrospective follow-up design was adopted. A total of 1,160 laboratory-confirmed patients were enrolled from nine provinces in China. Data on comorbidities were obtained from the patients’ medical records. Multivariable logistic regression models were used to estimate the odds ratio(OR) and 95% confidence interval(95% CI) of the associations between comorbidities(cardiometabolic or non-cardiometabolic diseases), clinical severity, and treatment outcomes of COVID-19.Results Overall, 158(13.6%) patients were diagnosed with severe illness and 32(2.7%) had unfavorable outcomes. Hypertension(2.87, 1.30–6.32), type 2 diabetes(T2 DM)(3.57, 2.32–5.49),cardiovascular disease(CVD)(3.78, 1.81–7.89), fatty liver disease(7.53, 1.96–28.96), hyperlipidemia(2.15, 1.26–3.67), other lung diseases(6.00, 3.01–11.96), and electrolyte imbalance(10.40, 3.00–26.10)were independently linked to increased odds of being severely ill. T2 DM(6.07, 2.89–12.75), CVD(8.47,6.03–11.89), and electrolyte imbalance(19.44, 11.47–32.96) were also strong predictors of unfavorable outcomes. Women with comorbidities were more likely to have severe disease on admission(5.46,3.25–9.19), while men with comorbidities were more likely to have unfavorable treatment outcomes(6.58, 1.46–29.64) within two weeks.Conclusion Besides hypertension, diabetes, and CVD, fatty liver disease, hyperlipidemia, other lung diseases, and electrolyte imbalance were independent risk factors for COVID-19 severity and poor treatment outcome. 6Application of Avaatech X-ray fluorescence core-scanning in Sr/Ca analysis of speleothems显示文摘Application of X-ray fluorescence core-scanning(XRF-CS) on both marine and lake sediments has achieved remarkable results. However, its application has not been widely extended to the research on speleothems. In this study, we measure the Sr abundance and the Sr/Ca ratios of three stalagmites(two aragonite stalagmites, one calcite stalagmite) using the state-of-the-art fourth-generation Avaatech high-resolution XRF core scanner. Through comparisons among different scan paths and among different scan resolutions, as well as comparisons with inductively coupled plasma optical emission spectrometer(ICP-OES), Itrax XRF, and Artax XRF results, we confirm that the Avaatech XRF core scanner could precisely, quickly, and nondestructively analyze the high-resolution Sr abundance of speleothems. The value of βkP RPkeeps nonnegative independent of the line search. Under mild conditions, the global convergence of modified PRP method with the strong Wolfe-Powell line search is established. 8Validity of the Chinese Version of the Patient-Generated Subjective Global Assessment (PG-SGA) in Lung Cancer Patients显示文摘The prevalence of malnutrition is high among oncology patients in China. Although the Patient- Generated Subjective Global Assessment (PG-SGA) is widely recognized as an important nutritional assessment tool, it has not been validated for Chinese cancer patients. The purpose of this study was to conduct an analysis of nutrition screening and assessment data to validate the Chinese version of the PG-SGA in Chinese patients with lung cancer. This was an observational, cross-sectional study of 2,000 consecutive adult patients with lung cancer treated at several tertiary hospitals in China. Anthropometric and patient descriptive data were collected. The PG-SGA generated a score for the nutritional risk and a global rating for the nutritional status. The internal consistency, external consistency, and construct validity were evaluated, and a known-groups comparison and principal component analyses were conducted to evaluate the reliability and validity of the PG-SGA. The known-groups comparison demonstrated the ability of the PG-SGA to differentiate lung cancer patients of different nutritional and body mass index (BMI) groups. A confirmatory factor analysis supported the original four-factor structure of the PG-SGA. In terms of the internal and external consistency, the PG-SGA demonstrated intra-class correlation coefficients (ICC) up to 0.987. Alternative form validity was also demonstrated between Box 4 of the PG-SGA and the Karnofsky Performance Status score, with excellent external consistency (r = 0.972, P < 0.001). The construct validity was supported, and selected questionnaire dimensions were evident in the principal component analysis. Significant Spearman correlations (P < 0.001) were demonstrated. The Chinese version of the PG-SGA demonstrated significant reliability and sufficient exploratory properties to support its validity. It seems to be a valid tool that can be used to access the nutritional status of Chinese lung cancer patients. 9Transcription factors AS1 and AS2 interact with LHP1 to repress KNOX genes in Arabidopsis显示文摘Polycomb group proteins are important repressors of numerous genes in higher eukaryotes. However, the mechanism by which Polycomb group proteins are recruited to specific genes is poorly understood. In Arabidopsis, LIKE HETEROCHROMATIN PROTEIN 1(LHP1), also known as TERMINAL FLOWER 2, was originally proposed as a subunit of polycomb repressive complex 1(PRC1) that could bind the tri-methylated lysine 27 of histone H3(H3K27me3) established by the PRC2. In this work, we show that LHP1 mainly functions with PRC2 to establish H3K27me3, but not with PRC1 to catalyze monoubiquitination at lysine 119 of histone H2 A. Our results show that complexes of the transcription factors ASYMMETRIC LEAVES 1(AS1) and AS2 could help to establish the H3K27me3 modification at the chromatin regions of Class-I KNOTTED1-like homeobox(KNOX) genes BREVIPEDICELLUS andKNAT2 via direct interactions with LHP1. 10Comparison of Histological, Microbiological, and Molecular Methods in Diagnosis of Patients with TBLN Having Different Anti-TB Treatment Background显示文摘Objective The influence of anti-tuberculosis(TB) treatment history on tuberculous lymphadenitis(TBLN) diagnosis is unclear. Therefore, this study aims to evaluate the diagnostic methods, including histology, microbiology, and molecular tests, used for TBLN. Methods In this study, suspected patients with TBLN and having different anti-TB treatment background were enrolled. All the samples were tested simultaneously by histology, Ziehl-Neelsen(ZN) staining, mycobacterial culture(culture), Xpert MTB/RIF(xpert), real-time PCR, and high-resolution melting curve PCR(HRM). Thereafter, the performance of these methods on samples with different anti-TB treatment background was assessed. Results In our study, 89 patients were prospectively included 82 patients with TBLN and 7 with other diseases. The overall sensitivities of Xpert, real-time PCR, histology, ZN staining, and culture were 86.6%, 69.5%, 58.5%, 43.9%, and 22.0%, respectively. The anti-TB treatment history revealed dramatic influences on the sensitivity of culture(P < 0.0001). In fact, the treatment that lasted over 3 months also influenced the sensitivity of Xpert(P < 0.05). However, the treatment history did not affect the performance of remaining tests(P > 0.05). For rifampicin drug susceptibility test(DST), the anti-TB treatment showed only significant influence on the success rate of culture DST(P = 0.001), but not on those of Xpert and HRM tests(P > 0.05). 12Validity of the Chinese Version of the Patient-Generated Subjective Global Assessment (PG-SGA) in Gastric Cancer Patients显示文摘Purpose Malnutrition is severe among gastric cancer patients in China. While the Patient-Generated Subjective Global Assessment (PG-SGA) is a widely used nutritional assessment tool, the validation for Chinese patients with gastric cancer has not been performed yet. The aim of this research is to validate the Chinese version of the PGSGA in Chinese gastric cancer patients by conducting analyses of nutrition screening and assessment data. Methods Two thousand consecutive adult patients with gastric cancer were included in this observational, cross-sectional study in several tertiary hospitals in China. A survey was made among those patients, which included several questionnaires and some anthropometric parameters. We calculated the parameters to validate the use of PG-SGA categorizing the nutritional status of gastric cancer patients. Then we performed statistical analysis to evaluate the internal consistency, external consistency, and construct validity. Results This study verified the ability of the PG-SGA to differentiate gastric cancer patients into different nutritional groups. The confirmatory factor analysis verified the original four-factor structure of the PG-SGA. The internal and external consistency was measured by a concordance analysis which showed an intraclass correlation coefficients (ICC) of 0.987. The correlations of the Box 4 score with the Karnofsky performance status score demonstrated alternative form validity with good external consistency (Pearson, r = 0.643, P < 0.001). All three anthropometric dimensions of the PG-SGA were supported by principal component analysis. Spearman correlations (P < 0.001) were significant in the majority of tests. 13A new Multilocus Sequence Analysis Scheme for Mycobacterium tuberculosis显示文摘Objective Tuberculosis remains one of the most serious infectious diseases in the world. In this study, a scheme of Mycobacterium tuberculosis (M. tuberculosis) multilocus sequence analysis (MLSA) was established for the phylogenetic and epidemiology analysis. Methods To establish the scheme of M. tuberculosis MLSA, the genome of H37Rv, CCDC5079 and CCDC5180 were compared, and some variable genes were chosen to be the MLSA typing scheme. 44 M. tuberculosis clinical isolates were typed by MLSA, IS6110-RFLP, and soligotyping, to evaluate the MLSA methods. Results After comparison of the genome, seven high discrimination gene loci (recX, rpsL, rmlC, rpmG1, mprA, gcvH, ideR) were chosen to be the MLSA typing scheme finally. 11 variable SNP sites of those seven genes were found among the 44 M. tuberculosis isolate strains and 11 sequence types (STs) were identified. Based on the Hunter-Gaston Index (HGI), MLSA typing was not as good for discrimination at the strain level as IS6110-RFLP, but the HGI was much better than that of spoligotyping. In addition, the MEGA analysis result of MLSA data was similar to spoligotyping/PGG lineage, showing a strong phylogenetic signal in the modern strains of M. tuberculosis. The MLSA data analysis by eBURST revealed that 4 sequence types (ST) came into a main cluster, showing the major clonal complexes in those 44 strains. 14Distinct immune signatures discriminate between asymptomatic and presymptomatic SARS-CoV-2^(pos)subjects显示文摘Increasing numbers of SARS-CoV-2-positive(SARS-CoV-2^(pos))subjects are detected at silent SARS-CoV-2 infection stage(SSIS).Yet,SSIS represents a poorly examined time-window wherein unknown immunity patterns may contribute to the fate determination towards persistently asymptomatic or overt disease.Here,we retrieved blood samples from 19 asymptomatic and 12 presymptomatic SARS-COV-2^(pos)subjects,47 age/gender-matched patients with mild or moderate COVID-19 and 27 normal subjects,and interrogated them with combined assays of 44-plex CyTOF,RNA-seq and Olink.Notably,both asymptomatic and presymptomatic subjects exhibited numerous readily detectable immunological alterations,while certain parameters including more severely decreased frequencies of CD107a^(low)classical monocytes,intermediate monocytes,non-classical monocytes and CD62L^(hi)CD8^(+)Tnaive cells,reduced plasma STC1 level but an increased frequency of CD4^(+)NKT cells combined to distinguish the latter.Intercorrelation analyses revealed a particular presymptomatic immunotype mainly manifesting as monocytic overactivation and differentiation blockage,a likely lymphocyte exhaustion and immunosuppression,yielding mechanistic insights into SSIS fate determination,which could potentially improve 5ARS-CoV-2 management.Shanhe Yu Caixia Di Shijun Chen Mingquan Guo Jiayang Yan Zhaoqin Zhu Li Liu Ruixue Feng Yinyin Xie Ruihong Zhang Juan Chen Mengxi Wang Dong Wei Hai Fang Tong Yin Jinyan Huang Saijuan Chen Hongzhou Lu Jiang Zhu Jieming Qu 2021Cell Research2021,31,11:4
15Association between Low-density Lipoprotein Receptor-related Protein 5 Polymorphisms and Type 2 Diabetes Mellitus in Han Chinese:a Case-control Study显示文摘Objective To investigate the association between low-density lipoprotein receptor-related protein 5(LRP5) variants(rs12363572 and rs4930588) and type 2 diabetes mellitus(T2DM) in Han Chinese. Methods A total of 1842 T2 DM cases(507 newly diagnosed cases and 1335 previously diagnosed cases) and 7777 controls were included in this case-control study. PCR-RFLP was conducted to detect the genotype of the two single nucleotide polymorphisms(SNPs). Odds ratios(ORs) and 95% confidence intervals(95% CIs) were calculated to describe the strength of the association by logistic regression. Results In the study subjects, neither rs12363572 nor rs4930588 was significantly associated with T2 DM, even after adjusting for relevant covariates. When stratified by body mass index(BMI), the two SNPs were also not associated with T2 DM. Among the 3 common haplotypes, only haplotype TT was associated with reduced risk of T2DM(OR 0.820, 95% CI 0.732-0.919). In addition, rs12363572 was associated with BMI(P<0.001) and rs4930588 was associated with triglyceride levels(P=0.043) in 507 newly diagnosed T2 DM cases but not in healthy controls. 17Characterization of Avian Influenza A(H7N9) Virus Prevalence in Humans and Poultry in Huai′an,China:Molecular Epidemiology,Phylogenetic,and Dynamics Analyses显示文摘Objective To trace the source of human H7N9 cases in Huai'an and elucidate the genetic characterization of Huai'an strains associated with both humans and birds in live poultry market.Methods An enhanced surveillance was implemented when the first human H7N9 case was confirmed in Huai'an.Clinical specimens,cloacal swabs,and fecal samples were collected and screened by real-time reverse transcription-polymerase chain reaction(RT-PCR) for H7N9 virus.The positive samples were subjected to further RT-PCR and genome sequencing.The phylodynamic patterns of H7N9 virus within and separated from Huai'an and evolutionary dynamics of the virus were analyzed.Results Six patients with H7N9 infection were previously exposed to live poultry market and presented symptoms such as fever(>38.0 °C) and headaches.Results of this study support the hypothesis that live poultry markets were the source of human H7N9 exposure.Phylogenetic analysis revealed that all novel H7N9 viruses,including Huai'an strains,could be classified into two distinct clades,A and B.Additionally,the diversified H7N9 virus circulated in live poultry markets in Huai'an.Interestingly,the common ancestors of the Huai'an H7N9 virus existed in January 2012.The mean nucleotide substitution rates for each gene segment of the H7N9 virus were(3.09-7.26)×10-3 substitutions/site per year(95% HPD:1.72×10-3 to 1.16×10-2).Conclusion Overall,the source of exposure of human H7N9 cases in Huai'an was live poultry market,and our study highlights the presence of divergent genetic lineage of H7N9 virus in both humans and poultry specimens in Huai'an.YANG Peng Fei YAN Qing Li LIU Chun Cheng XING Ya Dong ZHANG Min Hui GAO Qiang YU Hao YAO Hai Bo HE Nan Jiang 2016Biomedical and Environmental Sciences2016,29,10:3
