1Prognostic factors of refractory NSCLC patients receiving anlotinib hydrochloride as the third-or further-line treatment显示文摘Objective:Anlotinib hydrochloride is a multitarget tyrosine kinase inhibitor that targets vascular endothelial growth factor receptor,fibroblast growth factor receptor,platelet-derived growth factor receptor,c-Kit,and c-MET;therefore,it exhibits both antitumor and anti-angiogenetic activities.A phase III trial has shown that anlotinib improved progression-free survival(PFS)and overall survival(OS)in patients with advanced non-small cell lung cancer(NSCLC),who presented with progressive disease or intolerance after standard chemotherapy.This study aimed to analyze the characteristics of patients receiving anlotinib treatment to determine the dominant populations who are fit for the treatment.Methods:Data were collected from March 2015 to January 2017 from a randomized,double-blind,placebo-controlled,multicenter,phase III trial of anlotinib(ALTER0303).A total of 437 patients were enrolled and randomly allocated(2:1)to the anlotinib and placebo groups.Kaplan–Meier analysis and log-rank test were performed to compare PFS and OS.Cox proportional hazards model was adopted for multivariate prognostic analysis.Results:Multivariate analysis indicated that high post-therapeutic peripheral blood granulocyte/lymphocyte ratio and elevated alkaline phosphatase levels were independent risk factors for PFS.Meanwhile,elevated thyroid-stimulating hormone,blood glucose,and triglyceride levels;hypertension;and hand–foot syndrome were independent protective factors of PFS.High posttherapeutic peripheral blood granulocyte/lymphocyte ratio,an Eastern Cooperative Oncology Group(ECOG)score≥2,and the sum of the maximal target lesion length at baseline were independent risk factors of OS,and hypertriglyceridemia was an independent protective factor of OS.Conclusions:This study preliminarily explored the possible factors that affected PFS and OS after anlotinib treatment in patients with advanced refractory NSCLC,and the baseline characteristics of the therapeutically dominant populations were then identified.Jing Wang Yizhuo Zhao Qiming Wang Li Zhang Jianhua Shi Zhehai Wang Ying Cheng Jianxing He Yuankai Shi Hao Yu Yang Zhao Weiqiang Chen Yi Luo Xiuwen Wang Kejun Nan Faguang Jin Jian Dong Baolan Li Zhujun Liu Baohui Han Kai Li 2018Cancer Biology & Medicine2018,15,4:46

4Left posterior fascicular block: a new endpoint of ablation for verapamil-sensitive idiopathic ventricular tachycardia显示文摘Background Verapamil-sensitive, idiopathic left ventricular tachycardia (ILVT) with right bundle branch block configuration and left-axis deviation is known to be due to re-entry mechanism but the exact nature of reentrant circuit in ILVT is not fully elucidated. Radiofrequency (RF) ablation was applied during ventricular tachycardia (VT) and termination of the VT or abolishing the inducibility of the tachycardia was used as an endpoint for successful RF. In this study, the left posterior fascicular block in surface electrocardiogram (ECG) was used as a new endpoint of ablation to cure ILVT. Methods Electrophysiological studies and radiofrequency ablation were performed in 39 consecutive patients [30 men, 9 women; age ranging from 10 to 64 years, mean (29±16) years] with verapamil-sensitive ILVT and structurally normal hearts. VT could be terminated by the intravenous administration of verapamil in all patients. The target site was the midseptum of LV where the earliest Purkinje potentials were recorded during VT. RF current was applied to the target site with or without late diastolic potential (LDP) during sinus rhythm in 37 patients and during VT in 2 patients to meet the ablation endpoint: the left posterior fascicular block in the surface ECG. Results Thirty-seven patients with ILVT had been treated by RF ablation during sinus rhythm and two had been treated during VT. All of them met the endpoint of the left posterior fascicular block. Thirty-eight cases were symptom-free without medications during the follow-up period (range from 3 to 95 months, median 17 months). One patient developed a clinical recurrence and the left posterior fascicular block in surface ECG disappeared. The patient received another treatment. The endpoint was met and the procedure was successful. Conclusions The left posterior fascicular block in surface ECG used as an endpoint of RF ablation to treat ILVT is effective. It is important especially in those patients whose VT can not be induced or the inducible condition is unstable. 6Spatial distribution and focal mechanism solutions of the Wenchuan earthquake series:Results and implications显示文摘We relocate the spatial distribution of the devastating 12 May 2008 Wenchuan earthquake and its aftershocks.The relocation database is obtained from 89 stations deployed by the China Earthquake Administration,including 54 525 seismograms from 1 376 local earthquakes over MS3.5 between 12 May 2008 and 3 August 2008.The cross-correlation technique used in this paper has greatly improved the relocation precision by giving much more accurate P-wave differential travel-time measurements than those obtained from routinely picked phase onsets.At the same time,we pick P-wave polarity observations of the Wenchuan earthquake series (hereafter referred to as WES) from 1 023 stations in China and 59 IRIS (Incorporated Research Institutions of Seismology) stations.Then,employing a newly developed program CHNYTX,we obtain 83 well-determined focal mechanism solutions (hereafter referred to as FMSs).Based on spatial distribution and FMSs of the WES,we draw following conclusions:(1) The region near the main shock exhibits a buried low-angle northwest-dipping seismic zone with the main shock at its upper end and two conjugated seismic zones dipping southeast with roughly equal dip-angle;(2) The compressional directions of all kinds of FMSs of the WES are subhorizontal,which reflects the dominant stress in this area is compressional;(3) The principal compressional direction of the regional stress around Wenchuan is roughly perpendicular to the strike of Beichuan-Yingxiu fault,while around Qingchuan it is roughly parallel to the strike of Qingchuan fault.In intermediate part of the Longmenshan area,the principal compressional direction of the stress should be in-between;(4) The possibly existed molten materials in the lower crust of Songpan-Garze terrain have small contribution to the local stress state in Longmenshan area.The listric geometries of the Longmenshan faults most probably resulted from subhorizontal compression along NW-SE direction in history.Chen Cai Chunquan Yu Kai Tao Xingping Hu Yuan Tian Hao Zhang Xiaofeng Cui Jieyuan Ning 2011Earthquake Science2011,24,1:13

9How we can improve patients’comfort after Milligan- Morgan open haemorrhoidectomy显示文摘AIM:To demonstrate the value of Diosmin(flavonidic fraction)in the management of post-haemorhoidectomic symptoms.METHODS:Eighty-six consecutive patients with grades ⅢandⅣacute mixed hemorrhoids admitted to the Anorectal Surgical Department of First Affiliated Hospital,Xinjiang Medical University from April 2009 to April 2010,were enrolled in this study.An observerblinded,randomized trial was conducted to compare post-haemorhoidectomic symptoms with use of Diosmin flavonidic fraction vs placebo.Eighty-six patients were randomly allocated to receive Diosmin flavonidic fraction 500 mg for 1 wk(n=43)or placebo(n=43).The Milligan-Morgan open haemorrhoidectomy was performed by a standardized diathermy excision method.Pain,bleeding,heaviness,pruritus,wound edema and mucosal discharge were observed after surgery.The postoperative symptoms and hospitalization time were recorded.RESULTS:The mean age of the Diosmin group and controls was 53.2 and 51.3 years,respectively.In Diosmin group,haemorrhoid piles were of the third degree in 33 patients and the fourth degree in 10;and in the control group,29 were of the third degree and 14 were of the fourth degree.There was no statistically significance in age,gender distribution,degree and number of excised haemorrhoid piles,and the mean duration of haemorrhoidal disease between the two groups.There was a statistically significant improvement in pain,heaviness,bleeding,pruritus from baseline to the 8th week after operation(P<0.05).Patients taking Diosmin had a shorter hospitalization stay after surgery(P< 0.05).There was also a significant improvement on the proctoscopic appearance(P<0.001).However,there was no statistical difference between the two groups in terms of wound mucosal discharge.Two patients experienced minor bleeding at the 8th week in Diosmin group,and underwent surgery.CONCLUSION:Diosmin is effective in alleviating postoperational symptoms of haemorrhoids.Therefore,it should be considered for the initial treatment after haemorrhoid surgery.However,further prospective randomized trials are needed to confirm the findings of this study.Ma-Mu-Ti-Jiang A ba-bai-ke-re Hong-Guo Huang Wen-Ni Re Kai Fan Hui Chu Er-Ha-Ti Ai Mai-Mai-Ti-Tu-Er-Xun KE Li-Mu Yi-Rui Wang Hao Wen 2011World Journal of Gastroenterology2011,17,11:13

11Microstructure evolution of ZA72 magnesium alloy during partial remelting显示文摘The application of some semi-solid forming magnesium alloys is restricted due to their weak mechanical properties. To improve the mechanical properties, it is necessary to research the regularity and theory of semi-solid microstructure evolution of the alloy. In this study, microstructure evolution of ZA72 alloy during the partial remelting, and the effect of holding temperature and holding time on the semi-solid microstructure evolution of ZA72 magnesium alloy were investigated by means of OM, SEM and EDS analysis. The results indicate that the microstructure with small and spheroidal semi-solid particles which are available for thixo-forming can be obtained using proper heating parameters. After being isothermally treated at between 580 and 610 ℃ for 30 min, the equivalent size and shape factor of primary solid phase of ZA72 alloy decrease gradually, while the liquid volume fraction increases. When isothermally treated at 600 ℃ and held for different times from 15 to 60 min, with the increase of holding time, the equivalent size of primary particles decreases at first and then increases gradually; while the shape factor decreases gradually. The best heat treatment parameters in this experiment are to hold at 610 ℃ for 30 min. Compared with as-cast ZA72 alloy, the sizes of the eutectic phase and second α-Mg phase obtained in semi-solid state are smaller due to the higher solidification rate and the higher under-cooling degree than as-cast state. 12Laparoscopic vs open extended right hemicolectomy for colon cancer显示文摘AIM:To evaluate the feasibility,safety,and oncologic outcomes of laparoscopic extended right hemicolectomy(LERH)for colon cancer.METHODS:Since its establishment in 2009,the Southern Chinese Laparoscopic Colorectal Surgical Study(SCLCSS)group has been dedicated to promoting patients’quality of life through minimally invasive surgery.The multicenter database was launched by combining existing datasets from members of the SCLCSS group.The study enrolled 220 consecutive patients who were recorded in the multicenter retrospective database and underwent either LERH(n=119)or open extended right hemicolectomy(OERH)(n=101)for colon cancer.Clinical characteristics,surgical outcomes,and oncologic outcomes were compared between the two groups.RESULTS:There were no significant differences in terms of age,gender,body mass index(BMI),history of previous abdominal surgery,tumor location,and tumor stage between the two groups.The blood loss was lower in the LERH group than in the OERH group[100(100-200)mL vs 150(100-200)mL,P<0.0001].The LERH group was associated with earlier first flatus(2.7±1.0 d vs 3.2±0.9 d,P<0.0001)and resumption of liquid diet(3.6±1.0 d vs 4.2±1.0 d,P<0.0001)compared to the OERH group.The postoperative hospital stay was significantly shorter in the LERH group(11.4±4.7 d vs 12.8±5.6 d,P=0.009)than in the OERH group.The complication rate was 11.8%and17.6%in the LERH and OERH groups,respectively(P=0.215).Both 3-year overall survival[LERH(92.0%)vs OERH(84.4%),P=0.209]and 3-year disease-free survival[LERH(84.6%)vs OERH(76.6%),P=0.191]were comparable between the two groups.CONCLUSION:LERH with D3 lymphadenectomy for colon cancer is a technically feasible and safe procedure,yielding comparable short-term oncologic outcomes to those of open surgery.Li-Ying Zhao Pan Chi Wei-Xing Ding Shun-Rong Huang Si-Fen Zhang Kai Pan Yan-Feng Hu Hao Liu Guo-Xin Li 2014World Journal of Gastroenterology2014,20,24:9

14Prioritizing natural-selection signals from the deep-sequencing genomic data suggests multi-variant adaptation in Tibetan highlanders显示文摘Human genetic adaptation to high altitudes(>2500 m)has been extensively studied over the last few years,but few functional adaptive genetic variants have been identified,largely owing to the lack of deep-genome sequencing data available to previous studies.Here,w e build a list of putative adaptive variants,including 63 missense,7 loss-of-function;1,298 evolutionarily conserved variants and 509 expression quantitative traits loci.Notably,the top signal of selection is located in TMEM 247,a transmembrane protein-coding gene.The Tibetan version of TMEM 247 harbors one high-frequency(76.3%)missense variant,rsl 16983452(c.248C>T;p.Ala83Val);with the T allele derived from archaic ancestry and carried by>94%of Tibetans but absent or in low frequencies(<3%)in non-Tibetan populations.The rsl 16983452-T is strongly and positively correlated with altitude and significantly associated with reduced hemoglobin concentration(p=5.78×10^-5),red blood cell count(p=5.72×10^-7)and hematocrit(p=2.57×10^-6).In particular,TMEM247-rs 116983452 shows greater effect size and better predicts the phenotypic outcome than any E P A S1 variants in association with adaptive traits in Tibetans.Modeling the interaction between TM£M247-rsl 16983452 and EPAS1 variants indicates weak but statistically significant epistatic effects.Our results support that multiple variants may jointly deliver the fitness of the Tibetans on the plateau,where a complex model is needed to elucidate the adaptive evolution mechanism.Lian Deng Chao Zhang Kai Yuan Yang Gao Yuwen Pan Xueling Ge Yaoxi He Yuan Yuan Yan Lu Xiaoxi Zhang Hao Chen Haiyi Lou Xiaoji Wang Dongsheng Lu Jiaojiao Liu Lei Tian Qidi Feng Asifullah Khan Yajun Yang Zi-Bing Jin Jian Yang Fan Lu Jia Qu Longli Kang Bing Su Shuhua Xu 2019National Science Review2019,6,6:9
15Growth differentiation factor-15 combined with N-terminal prohormone of brain natriuretic peptide increase 1-year prognosis prediction value for patients with acute heart failure: a prospective cohort study显示文摘Background:Clinical assessment and treatment guidance for heart failure depends on a variety of biomarkers.The objective of this study was to investigate the prognostic predictive value of growth differentiation factor-15(GDF-15)and N-terminal prohormone of brain natriuretic peptide(NT-proBNP)in assessing hospitalized patients with acute heart failure(AHF).Methods:In total,260 patients who were admitted for AHF in the First Affiliated Hospital of Nanjing Medical University were enrolled from April 2012 to May 2016.Medical history and blood samples were collected within 24 h after the admission.The primary endpoint was the all-cause mortality within 1 year.The patients were divided into survival group and death group based on the endpoint.With established mortality risk factors and serum GDF-15 level,receiver-operator characteristic(ROC)analyses were performed.Cox regression analyses were used to further analyze the combination values of NT-proBNP and GDF-15.Results:Baseline GDF-15 and NT-proBNP were significantly higher amongst deceased than those in survivors(P<0.001).In ROC analyses,area under curve(AUC)for GDF-15 to predict 1-year mortality was 0.707(95%confidence interval[CI]:0.648–0.762,P<0.001),and for NT-proBNP was 0.682(95%CI:0.622–0.738,P<0.001).No statistically significant difference was found between the two markers(P=0.650).Based on the optimal cut-offs(GDF-15:4526.0 ng/L;NT-proBNP:1978.0 ng/L),the combination of GDF-15 and NT-proBNP increased AUC for 1-year mortality prediction(AUC=0.743,95%CI:0.685–0.795,P<0.001).Conclusions:GDF-15,as a prognostic marker in patients with AHF,is not inferior to NT-proBNP.Combining the two markers could provide an early recognition of high-risk patients and improve the prediction values of AHF long-term prognosis.Clinical trial registration:ChiCTR-ONC-12001944,http://www.chictr.org.cn.Ji Hao Iokfai Cheang Li Zhang Kai Wang Hui-Min Wang Qian-Yun Wu Yan-Li Zhou Fang Zhou Dong-Jie Xu Hai-Feng Zhang Wen-Ming Yao Xin-Li Li 2019Chinese Medical Journal2019,,19:8
16Destruction mechanism of gas explosion to ventilation facilities and automatic recovery technology显示文摘In order to overcome the heavy casualties caused by gas explosion, we verified the propagation law of shock wave in pipeline and the overpressure distribution of gas explosion by similar experiments according to the analyses of reasons for casualty and ventilation system model destroyed by gas explosion in the mining face. We summarized the gas composition after the explosion and its danger, analyzed the effects of the gas explosion shock wave to ventilation system and facilities and the laws of toxic gas spread and diffusion in the ventilation network after the explosion. We presented a technical proposal to control the smoke and recover the ventilation system after a gas explosion and developed a reserve air door and control system that were embed in the lane, and could close automatically in conditions of no pressure and electricity. The results showed that the reserve air door normally opened and could close automatically controlling the smoke flow and resuming the ventilation system when the gas explosion shock wave destroyed the original shutting air door which resulted in the air short circuit.Wang Kai 2012International Journal of Mining Science and Technology2012,22,3:8

19Additively manufactured biodegradable porous magnesium implants for elimination of implant-related infections:An in vitro and in vivo study显示文摘Magnesium(Mg)alloys that have both antibacterial and osteogenic properties are suitable candidates for orthopedic implants.However,the fabrication of ideal Mg implants suitable for bone repair remains challenging because it requires implants with interconnected pore structures and personalized geometric shapes.In this study,we fabricated a porous 3D-printed Mg-Nd-Zn-Zr(denoted as JDBM)implant with suitable mechanical properties using selective laser melting technology.The 3D-printed JDBM implant exhibited cytocompatibility in MC3T3-E1 and RAW267.4 cells and excellent osteoinductivity in vitro.Furthermore,the implant demonstrated excellent antibacterial ratios of 90.0% and 92.1% for methicillin-resistant S.aureus(MRSA)and Escherichia coli,respectively.The 3D-printed JDBM implant prevented MRSA-induced implant-related infection in a rabbit model and showed good in vivo biocompatibility based on the results of histological evaluation,blood tests,and Mg2+deposition detection.In addition,enhanced inflammatory response and TNF-α secretion were observed at the bone-implant interface of the 3D-printed JDBM implants during the early implantation stage.The high Mg^(2+)environment produced by the degradation of 3D-printed JDBM implants could promote M1 phenotype of macrophages(Tnf,iNOS,Ccl3,Ccl4,Ccl5,Cxcl10,and Cxcl2),and enhance the phagocytic ability of macrophages.The enhanced immunoregulatory effect generated by relatively fast Mg^(2+)release and implant degradation during the early implantation stage is a potential antibacterial mechanism of Mg-based implant.Our findings indicate that 3D-printed porous JDBM implants,having both antibacterial property and osteoinductivity,hold potential for future orthopedic applications.Kai Xie Nanqing Wang Yu Guo Shuang Zhao Jia Tan Lei Wang Guoyuan Li Junxiang Wu Yangzi Yang Wenyu Xu Juan Chen Wenbo Jiang Penghuai Fu Yongqiang Hao 2022Bioactive Materials2022,7,2:8
20Association of Overlapped and Un-overlapped Comorbidities with COVID-19 Severity and Treatment Outcomes: A Retrospective Cohort Study from Nine Provinces in China显示文摘Objective Several COVID-19 patients have overlapping comorbidities. The independent role of each component contributing to the risk of COVID-19 is unknown, and how some non-cardiometabolic comorbidities affect the risk of COVID-19 remains unclear.Methods A retrospective follow-up design was adopted. A total of 1,160 laboratory-confirmed patients were enrolled from nine provinces in China. Data on comorbidities were obtained from the patients’ medical records. Multivariable logistic regression models were used to estimate the odds ratio(OR) and 95% confidence interval(95% CI) of the associations between comorbidities(cardiometabolic or non-cardiometabolic diseases), clinical severity, and treatment outcomes of COVID-19.Results Overall, 158(13.6%) patients were diagnosed with severe illness and 32(2.7%) had unfavorable outcomes. Hypertension(2.87, 1.30–6.32), type 2 diabetes(T2 DM)(3.57, 2.32–5.49),cardiovascular disease(CVD)(3.78, 1.81–7.89), fatty liver disease(7.53, 1.96–28.96), hyperlipidemia(2.15, 1.26–3.67), other lung diseases(6.00, 3.01–11.96), and electrolyte imbalance(10.40, 3.00–26.10)were independently linked to increased odds of being severely ill. T2 DM(6.07, 2.89–12.75), CVD(8.47,6.03–11.89), and electrolyte imbalance(19.44, 11.47–32.96) were also strong predictors of unfavorable outcomes. Women with comorbidities were more likely to have severe disease on admission(5.46,3.25–9.19), while men with comorbidities were more likely to have unfavorable treatment outcomes(6.58, 1.46–29.64) within two weeks.Conclusion Besides hypertension, diabetes, and CVD, fatty liver disease, hyperlipidemia, other lung diseases, and electrolyte imbalance were independent risk factors for COVID-19 severity and poor treatment outcome.
