1A life support-based comprehensive treatment regimen dramatically lowers the in-hospital mortality of patients with fulminant myocarditis: a multiple center study显示文摘Fulminant myocarditis(FM) has unacceptable high mortality. This study aimed to evaluate the therapeutic efficacy of a life support-based comprehensive treatment regimen(LSBCTR), a completely novel treatment regimen, for FM. A total of 169 FM patients recruited from January 2008 to December 2018 were divided into two groups: patients receiving LSBCTR(81 cases),which includes(i) mechanical life support(positive pressure respiration, intra-aortic balloon pump with or without extracorporeal membrane oxygenation),(ii) immunomodulation therapy using sufficient doses of glucocorticoids and immunoglobulins, and(iii) application of neuraminidase inhibitors, and those receiving conventional treatment(88 cases). The endpoints were in-hospital death and heart-transplantation. Of all the population, 44 patients(26.0%) died in hospitals. Inhospital mortality was 3.7%(3/81) for LSBCTR group and 46.6%(41/88) for traditional treatment(P<0.001). Early application of LSBCTR, mechanical life support, neuraminidase inhibitors, and immunomodulation therapy significantly contributed to reduction in in-hospital mortality. This study describes a novel treatment regimen for FM patients that dramatically reduces inhospital mortality. 2Chinese expert consensus statement on the diagnosis and treatment of fulminant myocarditis显示文摘Fulminant myocarditis is primarily caused by infection with any number of a variety of viruses. It arises quickly, progresses rapidly, and may lead to severe heart failure or circulatory failure presenting as rapid-onset hypotension and cardiogenic shock,with mortality rates as high as 50%–70%. Most importantly, there are no treatment options, guidelines or an expert consensus statement. Here, we provide the first expert consensus, the Chinese Society of Cardiology Expert Consensus Statement on the Diagnosis and Treatment of Fulminant Myocarditis, based on data from our recent clinical trial(NCT03268642). In this statement, we describe the clinical features and diagnostic criteria of fulminant myocarditis, and importantly, for the first time,we describe a new treatment regimen termed life support-based comprehensive treatment regimen. The core content of this treatment regimen includes(i) mechanical life support(applications of mechanical respirators and circulatory support systems,including intraaortic balloon pump and extracorporeal membrane oxygenation),(ii) immunological modulation by using sufficient doses of glucocorticoid, immunoglobulin and(iii) antiviral reagents using neuraminidase inhibitor. 3Impact of mechanical circulatory support and immunomodulation therapy on outcome of patients with fulminant myocarditis: Chinese registry of fulminant myocarditis显示文摘Dear Editor,Patients presenting with acute myocarditis and sudden hemodynamic instability (termed fulminant myocarditis [FM]) still have a high mortality and need for heart transplantation, up to 28% at 60 days.1,2,3 Recent scientific statements and expert opinion consensus suggests early use of temporary mechanical circulatory supports (t-MCS).3,4 Specifically, Chinese scientific statement proposed an extensive use of t-MCS combined with immunoregulatory therapy (IT),4 although formal trials are lacking. We present a multicenter, retrospective study to compare the outcome of patients who were treated with t-MCS and IT vs. patients who didn’t receive these treatments. We included patients with the diagnosis of FM based on the presence of viral prodromal signs/symptoms followed by acute onset of severe heart failure (HF) without other relevant differential diagnosis or pre-existing cardiac disorders. 4Temporal echocardiography findings in patients with fulminant myocarditis: beyond ejection fraction decline显示文摘The features of myocardial strains from speckle-tracking echocardiography (STE) have not been well defined in fulminant myocarditis (FM) patients.In this study,changes in the left ventricular ejection fraction (LVEF) and global and layer-specific myocardial strains over time were monitored.We aimed to determine the echocardiographic patterns of FM and ascertain their significance in FM treatment.Twenty patients who were clinically diagnosed with FM and received mechanical life support were prospectively enrolled.Conventional echocardiographic measurements were obtained,and serial strain echocardiography was performed from admission to hospital discharge until LVEF recovery (> 50%).Global/regional peak systolic longitudinal strains (GLS/RLS) and layer-specific longitudinal strains were quantified,and their changes with time were monitored in 14 FM patients.All patients had severely impaired cardiac function.Steep improvement in LVEF and GLS were observed within 6 days.Layer-specific strain analysis showed that reduction at admission or recovery at discharge in the endocardium and epicardium strains were equal.In conclusion,FM patients who received mechanical circulatory supports exhibited steep improvement in ventricular function within 6 days.The patchy and diffused distribution pattern of reduced RLS and equally and severely impaired strain in the endocardium and epicardium are valuable features in the diagnosis of FM.Houjuan Zuo Rui Li Fei Ma Jiangang Jiang Kun Miao Haojie Li Eike Nagel Marijana Tadic Hong Wang Dao Wen Wang 2020Frontiers of Medicine2020,14,3:10
5Decreased serum albumin level indicates poor prognosis of COVID-19 patients: hepatic injury analysis from 2,623 hospitalized cases显示文摘Coronavirus disease 2019(COVID-19) is a global pandemic which has caused numerous deaths worldwide. The present study investigated the roles of hypoproteinemia in the clinical outcome and liver dysfunction of COVID-19 patients. In this retrospective study, we extracted data from 2,623 clinically confirmed adult COVID-19 patients(≥18 years old) between January 29,2020 and March 6, 2020 in Tongji Hospital, Wuhan, China. The patients were divided into three groups—non-critically ill,critically ill, and death groups—in accordance with the Chinese Clinical Guideline for COVID-19. Serum albumin, low-density lipoproteins cholesterol(LDL-C), and high-density lipoproteins cholesterol(HDL-C) concentrations and inflammatory cytokines levels were measured and compared among these three groups. The median age of these 2,623 patients was 64 years old(interquartile range(IQR), 52–71). Among the patients enrolled in the study, 2,008(76.6%) were diagnosed as non-critically ill and 615(23.4%) were critically ill patients, including 383(14.6%) critically ill survivors and 232(8.8%) critically ill deaths in the hospital. Marked hypoalbuminemia occurred in 38.2%, 71.2%, and 82.4% patients in non-critically ill, critically ill, and death groups, respectively, on admission and 45.9%, 77.7%, and 95.6% of these three groups, respectively, during hospitalization. We also discovered that serum low-density lipoprotein(LDL) and HDL levels were significantly lower in critically ill and death groups compared to non-critically ill group. Meanwhile, the patients displayed dramatically elevated levels of serum inflammatory factors, while a markedly prolonged activated partial thromboplastin time(APTT) in critically ill patients reflected coagulopathy. 6Mapping and validation of a dominant salt tolerance gene in the cultivated soybean(Glycine max) variety Tiefeng 8显示文摘Salt is an abiotic stress factor that strongly affects soybean growth and production. A single dominant gene has been shown to confer salt tolerance in the soybean cultivar Tiefeng 8.The objective of the present study was to genetically map the salt-tolerance gene in an F2:3population and a recombinant inbred line(RIL) population derived from a cross between two cultivated soybeans, Tiefeng 8(tolerant) and 85-140(sensitive). The F2:3families and RILs were treated with 200 mmol L-1Na Cl to evaluate salt tolerance. The F2:3population showed 1(42 tolerant): 2(132 segregating): 1(65 sensitive) segregation, indicating a single dominant gene for salt tolerance in Tiefeng 8. A sequence-characterized amplified region(SCAR) marker from a previously identified random amplified polymorphic DNA(RAPD)marker and four insertion/deletion polymorphism(In Del) markers were developed within the mapping region. Using these markers along with SSR markers, the salt-tolerance gene was mapped within 209 kb flanked by SCAR marker QS08064 and SSR marker Barcsoyssr_3_1301 on chromosome 3. Three markers that cosegregated with the salt tolerance gene and SCAR marker QS08064 were used to genotype 35 tolerant and 23 sensitive soybean accessions. These markers showed selection efficiencies of 76.2% to94.2%. 7Nuclear miR-665 aggravates heart failure via suppressing phosphatase and tensin homolog transcription显示文摘Although numerous miRNAs have been discovered,their functions in the different subcellular organelles have remained obscure.In this study,we found that miR-665 was enriched in the nucleus of cardiomyocytes,and then investigated the underlying role of nuclear miR-665 in heart failure.RNA fluorescence in situ hybridization assays in human heart tissue sections and primary cardiomyocytes showed that miR-665 was localized in the nucleus of cardiomyocytes.Increased expression of nuclear miR-665 was observed not only in the cardiomyocytes isolated from the heart of mice treated in vivo by transverse aortic constriction(TAC),but also in phenylephrine(PE)-treated cultured cardiomyocytes in vitro.To further explore the role of miR-665 in heart failure,a type 9 recombinant adeno-associated virus(rAAV)system was employed to manipulate the expression of miR-665 in mice.Overexpression of miR-665 aggravated TAC-induced cardiac dysfunction,while down-expression of miR-665 showed opposite effects.Bioinformatic prediction and biological validation confirmed that the PTEN(phosphatase and tensin homolog)gene was one of the targets of miR-665 in the nucleus.Furthermore,restoring PTEN expression significantly eliminated the destructive effects of miR-665 over-expression in TAC-induced cardiac dysfunction.Our data showed that nuclear miR-665 aggravates heart failure via inhibiting PTEN expression,which provided a therapeutic approach for heart failure.Jiahui Fan Xudong Zhang Xiang Nie Huaping Li Shuai Yuan Beibei Dai Jiabing Zhan Zheng Wen Jiangang Jiang Chen Chen Daowen Wang 2020Science China(Life Sciences)2020,63,5:4
8Redefinition and Formation Age of the Tanjianshan Group in Xitieshan Region, Qinghai显示文摘The Tanjianshan Group, which was previously divided into a, b, c and d formations, has been controversial for a long time. It mainly distributes in the northern margin of Qaidam Basin and is an important early Paleozoic greenschist facies metamorphic volcanic sedimentary rock formation. Detailed field investigation and zircon LA-ICPMS U-Pb dating of the key strata suggest that the original lower part of a Formation(a-1) versus the original middle upper of d Formation(d-3 and d-4), the original upper part of a Formation(a-2) and b Formation versus the original lower part of d Formation(d-1 and d-2) of Tanjianshan Group are contemporaneous heterotopic facies volcanicclasolite deposit, respectively. The former formations formed during the middle-late Ordovician(463–458 Ma), while the latter ones formed in the late Ordovician(about 445 Ma). The original c formation of Tanjianshan Group, which formed after 430 Ma, is similar to the Maoniushan Formation of Kunlun Mountains and north Qaidam Basin. According to the rules of stratigraphic division and naming, new stratum formations of Tanjianshan Group are re-built and divided into Duancenggou(O1-2td), Zhongjiangou(O2-3tz) and Xitieshan(O3tx) formations. The original c Formation is separated from Tanjianshan Group and is renamed as the Wuminggou Formation(S3-D1w), which shows a discordant contact with underlying Tanjianshan Group and overlying Amunike Formation(D3a). 16Tectonic Uplift of the Yili Basin during the Last Stage of the Late Pleistocene: Evidence from ESR and OSL Dating of Sediments in the Huocheng Area, Xinjiang显示文摘The Quaternary sediments in the Yili Basin can serve as archives for studying the Cenozoic basin-mountain relationship. In this study, based on typical natural sections and boreholes, the surficial sediments of the Huocheng area were studied, and their sedimentary ages were obtained using the optically stimulated luminescence(OSL) and electron spin resonance(ESR) dating methods. These dates, combined with changes in the sedimentary facies, provided details of the neotectonic movement in the Yili Basin and adjacent areas. By dating sediments from five sections and three boreholes, we determined that the surficial sediments of the Huocheng area were mainly formed in the Late Pleistocene, with scattered instances of Holocene sediments. The surficial sediments mainly consisted of alluvial fan facies, fluvial facies, lacustrine facies, and desert facies. 19An investigation of the visual features of urban street vitality using a convolutional neural network显示文摘As a well-known urban landscape concept to describe urban space quality,urban street vitality is a subjective human perception of the urban environment but difficult to evaluate directly from the physical space.The study utilized a modern machine learning computer vision algorithm in the urban build environment to simulate the process,which starts with the visual perception of the urban street landscape and ends with the human reaction to street vitality.By analyzing the optimized trained model,we tried to identify urban street vitality’s visual features and evaluate their importance.A region around the Mochou Lake in Nanjing,China,was set as our study area.Seven investigators surveyed the area,recorded their evaluation score on each site’s vitality level with a corresponding picture taken on site.A total of 370 pictures and recorded score pairs from 231 valid survey sites were used to train a convolutional neural network.After optimization,a deep neural network model with 43 layers,including 11 convolutional ones,was created.Heat maps were then used to identify the features which lead to high vitality score outputs.The spatial distributions of different types of feature entities were also analyzed to help identify the spatial effects.The study found that visual features,including human,construction site,shop front,and roadside/walking pavement,are vital ones that correspond to the vitality of the urban street.The consistency of these critical features with traditional urban vitality features indicates the model had learned useful knowledge from the training process.Applying the trained model in urban planning practices can help to improve the city environment for better attraction of residents’activities and communications.Yi Qi Sonam Chodron Drolma Xiang Zhang Jing Liang Haibing Jiang Jiangang Xu Tianhua Ni 2020Geo-Spatial Information Science2020,23,4:1
