| 1 | Prognostic factors of refractory NSCLC patients receiving anlotinib hydrochloride as the third-or further-line treatment显示文摘Objective:Anlotinib hydrochloride is a multitarget tyrosine kinase inhibitor that targets vascular endothelial growth factor receptor,fibroblast growth factor receptor,platelet-derived growth factor receptor,c-Kit,and c-MET;therefore,it exhibits both antitumor and anti-angiogenetic activities.A phase III trial has shown that anlotinib improved progression-free survival(PFS)and overall survival(OS)in patients with advanced non-small cell lung cancer(NSCLC),who presented with progressive disease or intolerance after standard chemotherapy.This study aimed to analyze the characteristics of patients receiving anlotinib treatment to determine the dominant populations who are fit for the treatment.Methods:Data were collected from March 2015 to January 2017 from a randomized,double-blind,placebo-controlled,multicenter,phase III trial of anlotinib(ALTER0303).A total of 437 patients were enrolled and randomly allocated(2:1)to the anlotinib and placebo groups.Kaplan–Meier analysis and log-rank test were performed to compare PFS and OS.Cox proportional hazards model was adopted for multivariate prognostic analysis.Results:Multivariate analysis indicated that high post-therapeutic peripheral blood granulocyte/lymphocyte ratio and elevated alkaline phosphatase levels were independent risk factors for PFS.Meanwhile,elevated thyroid-stimulating hormone,blood glucose,and triglyceride levels;hypertension;and hand–foot syndrome were independent protective factors of PFS.High posttherapeutic peripheral blood granulocyte/lymphocyte ratio,an Eastern Cooperative Oncology Group(ECOG)score≥2,and the sum of the maximal target lesion length at baseline were independent risk factors of OS,and hypertriglyceridemia was an independent protective factor of OS.Conclusions:This study preliminarily explored the possible factors that affected PFS and OS after anlotinib treatment in patients with advanced refractory NSCLC,and the baseline characteristics of the therapeutically dominant populations were then identified. | Jing Wang Yizhuo Zhao Qiming Wang Li Zhang Jianhua Shi Zhehai Wang Ying Cheng Jianxing He Yuankai Shi Hao Yu Yang Zhao Weiqiang Chen Yi Luo Xiuwen Wang Kejun Nan Faguang Jin Jian Dong Baolan Li Zhujun Liu Baohui Han Kai Li | 2018 | Cancer Biology & Medicine2018,15,4: | 46 |

| 5 | Nonintubated uniportal video-assisted thoracoscopic surgery for primary spontaneous pneumothorax显示文摘Objective:The objective of the current study was to evaluate the feasibility and safety of nonintubated uniportal video-assisted thoracoscopic surgery(VATS) for the management of primary spontaneous pneumothorax(PSP).Methods:From November 2011 to June 2013,32 consecutive patients with PSP were treated by nonintubated uniportal thoracoscopic bullectomy using epidural anaesthesia and sedation without endotracheal intubation.An incision 2 cm in length was made at the 6^(th) intercostal space in the median axillary line.The pleural space was entered by blunt dissection for placement of a soft incision protector.Instruments were then inserted through the incision protector to perform thoracoscopic bullectomy.Data were collected within a minimum follow-up period of 10 months.Results:The average time of surgery was 49.0 min(range,33-65 min).No complications were recorded.The postoperative feeding time was 6 h.The mean postoperative chest tube drainage and hospital stay were19.3 h and 41.6 h,respectively.The postoperative pain was mild for 30 patients(93.75%) and moderate for two patients(6.25%).No recurrences of pneumothorax were observed at follow-up.Conclusions:The initial results indicated that nonintubated uniportal video-assisted thoracoscopic operations are not only technically feasible,but may also be a safe and less invasive alternative for select patients in the management of PSP.This is the first report to include the use of a nonintubated uniportal technique in VATS for such a large number of PSP cases.Further work and development of instruments are needed to define the applications and advantages of this technique. | Shuben Li Fei Cui Jun Liu Xin Xu Wenlong Shao Weiqiang Yin Hanzhang Chen Jianxing He | 2015 | Chinese Journal of Cancer Research2015,27,2: | 15 |
| 6 | Advanced treatment of municipal wastewater by nanofiltration: Operational optimization and membrane fouling analysis显示文摘Municipal sewage from an oxidation ditch was treated for reuse by nanofiltration(NF) in this study. The NF performance was optimized, and its fouling characteristics after different operational durations(i.e., 48 and 169 hr) were analyzed to investigate the applicability of nanofiltration for water reuse. The optimum performance was achieved when transmembrane pressure = 12 bar, p H = 4 and flow rate = 8 L/min using a GE membrane. The permeate water quality could satisfy the requirements of water reclamation for different uses and local standards for water reuse in Beijing. Flux decline in the fouling experiments could be divided into a rapid flux decline and a quasi-steady state. The boundary flux theory was used to predict the evolution of permeate flux. The expected operational duration based on the 169-hr experiment was 392.6 hr which is 175% longer than that of the 48-hr one. High molecular weight(MW) protein-like substances were suggested to be the dominant foulants after an extended period based on the MW distribution and the fluorescence characteristics. The analyses of infrared spectra and extracellular polymeric substances revealed that the roles of both humic- and polysaccharide-like substances were diminished, while that of protein-like substances were strengthened in the contribution of membrane fouling with time prolonged. Inorganic salts were found to have marginally influence on membrane fouling. | 7 | Safety and feasibility of video-assisted thoracoscopic surgery for stage IIIA lung cancer显示文摘Objective: The current study was prospectively designed to explore the application of video-assisted thoracoscopic surgery(VATS) radical treatment for patients with stage IIIA lung cancer, with the primary endpoints being the safety and feasibility of this operation and the second endpoints being the survival and complications after the surgery.Methods: A total of 51 patients with radiologically or mediastinoscopically confirmed stage IIIA lung cancer underwent VATS radical treatment, during which the standard pulmonary lobectomy and mediastinal lymph node dissection were performed after pre-operative assessment. The operative time, intraoperative blood loss/complications, postoperative recovery, postoperative complications, and lymph node dissection were recorded and analyzed. This study was regarded as successful if the surgical success rate reached 90% or higher.Results: A total of 51 patients with non-small cell lung cancer(NSCLC) were enrolled in this study from March 2009 to February 2010. The median post-operative follow-up duration was 50.5 months. Of these 51 patients, 41(80.4%) had N2 lymph node metastases. All patients underwent the thoracoscopic surgeries, among whom 50(98%) received pulmonary lobectomy and mediastinal lymph node dissection completely under the thoracoscope, 6 had their incisions extended to about 6 cm due to larger tumor sizes, and 1 had his surgery performed using a 12 cm small incision for handling the adhesions between lymph nodes and blood vessels. No patient was converted to conventional open thoracotomy. No perioperative death was noted. One patient received a second surgery on the second post-operative day due to large drainage(>1,000 mL), and the postoperative recovery was satisfactory. | 8 | Effects of Temperature and Moisture on Soil Organic Matter Decomposition Along Elevation Gradients on the Changbai Mountains, Northeast China显示文摘Decomposition of soil organic matter(SOM) is of importance for CO_2 exchange between soil and atmosphere and soil temperature and moisture are considered as two important factors controlling SOM decomposition. In this study, soil samples were collected at 5 elevations ranging from 753 to 2 357 m on the Changbai Mountains in Northeast China, and incubated under different temperatures(5, 10, 15, 20, 25, and 30?C) and soil moisture levels(30%, 60%, and 90% of saturated soil moisture) to investigate the effects of both on SOM decomposition and its temperature sensitivity at different elevations. The results showed that incubation temperature(F = 1 425.10, P < 0.001), soil moisture(F = 1 327.65, P < 0.001), and elevation(F = 1 937.54, P < 0.001) all had significant influences on the decomposition rate of SOM. The significant effect of the interaction of incubation temperature and soil moisture on the SOM decomposition rate was observed at all the 5 sampling elevations(P < 0.001). A two-factor model that used temperature and moisture as variables fitted the SOM decomposition rate well(P < 0.001) and could explain 80%–93% of the variation of SOM decomposition rate at the 5 elevations. Temperature sensitivity of SOM decomposition, expressed as the change of SOM decomposition rate in response to a 10?C increase in temperature(Q_(10)), was significantly different among the different elevations(P < 0.01), but no apparent trend with elevation was discernible. In addition, soil moisture and incubation temperature both had great impacts on the Q_(10) value(P < 0.01), which increased significantly with increasing soil moisture or incubation temperature. Furthermore, the SOM decomposition rate was significantly related to soil total Gram-positive bacteria(R^2= 0.33, P < 0.01) and total Gram-negative bacteria(R^2= 0.58, P < 0.001). | 11 | Effect of internal iliac artery chemotherapy after transurethral resection of bladder tumor for muscle invasive bladder cancer显示文摘Objective: To evaluate the clinical effect of transurethral resection of bladder tumor(TUR-BT) combined with internal iliac artery chemotherapy and intravesical instillation therapy for muscle invasive bladder cancer(MIBC).Methods: From February 2007 to April 2014, 62 patients with MIBC were treated with TUR-BT combined with intravesical instillation therapy, with or without internal iliac artery chemotherapy, and the chemotherapy regimen is gemcitabine and cisplatin(GC). The bladder preservation and survival rate as well as cancer-specific survival(CSS) rate and overall survival(OS) rate of the two groups were compared.Results: Sixty-two patients were followed-up for 26-102 months with an average of 58.4±3.1 months. Recurrence-free survival(RFS) at 2-year for TUR + GC group and TUR group were 77.8% and 53.8%, respectively. Bladder preserved rate(BPR) at 3-year for TUR + GC group and TUR group were 94.4% and 80.8%. CSS rate at 2-year for TUR + GC group and TUR group were 94.4% and 84.6%. The diseasefree survival(DFS) at 1-year for TUR + GC group and TUR group were 83.3% and 61.5%, and 77.8% and 53.8% for the 2nd year. | 12 | Evolution of the Ordos Plateau and environmental effects显示文摘Based on the analysis of temporary-spatial distribution, geomorphic position, contact relationship with underlying strata and grain size of red clay, we studied the formation and environmental background of red clay. During late Miocene-Pliocene, the Ordos Block finished the transformation from the basin to the plateau, which had an obvious environmental effect on the topography, indicated by the formation of highland undergoing wind erosion and lowland receiving red clay deposits. The red clay materials were sourced from dusts carried by wind energy and covered on the initial topography. Unlike Quaternary loess dust covering the overall the Loess Plateau, red clay deposited on the highland would be transported to the lowlands by wind and fluvial process. As a result, there was no continuous 'Red Clay Plateau' in the Ordos region and red clay was only preserved in former lowlands. However, red clay was discontinuously distributed through the Loess Plateau and to some extent modified the initial topography. The differential uplift in interior plateau is indicated by the uplift of northern Baiyushan, central Ziwuling and southern Weibeibeishan. The Weibeibeishan Depression formed earlier and became the sedimentary center of red clay resulting in the thicker red clay deposits in Chaona, Lingtai and Xunyi. Since Quaternary the aridity in the northern plateau enhanced and accelerated loess accumulation caused the formation of the Loess Plateau. During the late Pleistocene the rapid uplift led to the enhancement of erosion. | 15 | Detrital Zircon U-Pb Geochronology from Greywackes in the Niujuanzi Ophiolitic Mélange, Beishan Area, NW China: Provenance and Tectonic Implications显示文摘The Niujuanzi ophiolitic mélange represents the remnant of oceanic crust between the Dunhuang massif and Mingshui-Hanshan massif. Greywacke from different tectonic slices in the Niujuanzi ophiolitic mélange were analyzed for detrital zircon U-Pb geochronology, trace elements and whole-rock trace elements to infer their provenance and the evolution of the Niujuanzi Ocean. Sample N-76 s containing Carboniferous spores has the youngest zircon age of 323 Ma, while sample N-85 s without fossils has the youngest zircon age of 449 Ma. The two samples were deposited no earlier than 323 and 449 Ma, respectively. The greywackes are depleted in large ion lithophile elements, and are relatively enriched in high field strength elements. The age spectra and trace element concentrations indicate that the sediments may have been deposited near the trench. The Hf, U, and Yb contents of zircons from sample N-76 s vary widely, whereas those from sample N-85 s have a narrow range. Sample N-76 s contains both continental and oceanic zircons, while sample N-85 s contains only continental zircons. The sediments were derived from the continental arc and accretionary wedge. The Paleozoic oceanic crust zircons have ages of 430–500 Ma, indicating the timing of the expansion of the HongliuheNiujuanzi-Xichangjing Ocean expansion. | 18 | Isorhapontigenin protects against doxorubicin-induced cardiotoxicity via increasing YAP1 expression显示文摘As an effective anticancer drug, the clinical limitation of doxorubicin(Dox) is the time-and dose-dependent cardiotoxicity. Yes-associated protein 1(YAP1) interacts with transcription factor TEA domain 1(TEAD1) and plays an important role in cell proliferation and survival. However, the role of YAP1 in Dox-induced cardiomyopathy has not been reported. In this study, the expression of YAP1 was reduced in clinical human failing hearts with dilated cardiomyopathy and Dox-induced in vivo and in vitro cardiotoxic model. Ectopic expression of Yap1 significantly blocked Dox-induced cardiomyocytes apoptosis in TEAD1 dependent manner. Isorhapontigenin(Isor) is a new derivative of stilbene and responsible for a wide range of biological processes. Here, we found that Isor effectively relieved Doxinduced cardiomyocytes apoptosis in a dose-dependent manner in vitro. Administration with Isor(30 mg/kg/day, intraperitoneally, 3 weeks) significantly protected against Dox-induced cardiotoxicity in mice. Interestingly, Isor increased Dox-caused repression in YAP1 and the expression of its target genes in vivo and in vitro. Knockout or inhibition of Yap1 blocked the protective effects of Isor on Dox-induced cardiotoxicity. The quantization process is formulated with the imperfection that random packet loss occurs at the logarithmic quantizer. A Luenberger observer is designed, based on the packet loss rate and the imperfect quantized measurement. A novel SMC law is synthesized by utilization of an integral sliding surface. The stochastic stability of the resulting closed-loop system is analyzed in terms of Lyapunov stability, and a set of solvable matrix inequalities are established for practical application requirements. | 20 | Development of a machine learning model to predict early recurrence for hepatocellular carcinoma after curative resection显示文摘Background:Early recurrence is common for hepatocellular carcinoma(HCC)after surgical resection,being the leading cause of death.Traditionally,the COX proportional hazard(CPH)models based on linearity assumption have been used to predict early recurrence,but predictive performance is limited.Machine learning models offer a novel methodology and have several advantages over CPH models.Hence,the purpose of this study was to compare random survival forests(RSF)model with CPH models in prediction of early recurrence for HCC patients after curative resection.Methods:A total of 4,758 patients undergoing curative resection from two medical centers were included.Fifteen features including age,gender,etiology,platelet count,albumin,total bilirubin,AFP,tumor size,tumor number,microvascular invasion,macrovascular invasion,Edmondson-Steiner grade,tumor capsular,satellite nodules and liver cirrhosis were used to construct the RSF model in training cohort.Discrimination,calibration,clinical usefulness and overall performance were assessed and compared with other models.Results:Five hundred survival trees were used to generate the RFS model.The five highest Variable Importance(VIMP)were tumor size,macrovascular invasion,microvascular invasion,tumor number and AFP.In training,internal and external validation cohort,the C-index of RSF model were 0.725[standard errors(SE)=0.005],0.762(SE=0.011)and 0.747(SE=0.016),respectively;the Gönen&Heller’s K of RSF model were 0.684(SE=0.005),0.711(SE=0.008)and 0.697(SE=0.014),respectively;the time-dependent AUC(2 years)of RSF model were 0.818(SE=0.008),0.823(SE=0.014)and 0.785(SE=0.025),respectively.The RSF model outperformed early recurrence after surgery for liver tumor(ERASL)model,Korean model,American Joint Committee on Cancer tumor-node-metastasis(AJCC TNM)stage,Barcelona Clinic Liver Cancer(BCLC)stage and Chinese stage.The RSF model is capable of stratifying patients into three different risk groups(low-risk,intermediate-risk,high-risk groups)in the training and two validation cohorts(all P<0.0001).A web-based prediction tool was built to facilitate clinical application(https://recurrenceprediction.shinyapps.io/surgery_predict/).Conclusions:The RSF model is a reliable tool to predict early recurrence for patients with HCC after curative resection because it exhibited superior performance compared with other models.This novel model will be helpful to guide postoperative follow-up and adjuvant therapy. | Jianxing Zeng Jinhua Zeng Kongying Lin Haitao Lin Qionglan Wu Pengfei Guo Weiping Zhou Jingfeng Liu | 2022 | Hepatobiliary Surgery and Nutrition2022,11,2: | 6 |
