| 1 | Clinical features and the traditional Chinese medicine therapeutic characteristics of 293 COVID-19 inpatient cases显示文摘Coronavirus disease 2019 (COVID-19) is now pandemic worldwide and has heavily overloaded hospitals in Wuhan City, China during the time between late January and February. We reported the clinical features and therapeutic characteristics of moderate COVID-19 cases in Wuhan that were treated via the integration of traditional Chinese medicine (TCM) and Western medicine. We collected electronic medical record (EMR) data, which included the full clinical profiles of patients, from a designated TCM hospital in Wuhan. The structured data of symptoms and drugs from admission notes were obtained through an information extraction process. Other key clinical entities were also confirmed and normalized to obtain information on the diagnosis, clinical treatments, laboratory tests, and outcomes of the patients. A total of 293 COVID-19 inpatient cases, including 207 moderate and 86 (29.3%) severe cases, were included in our research. Among these cases, 238 were discharged, 31 were transferred, and 24 (all severe cases) died in the hospital. Our COVID-19 cases involved elderly patients with advanced ages (57 years on average) and high comorbidity rates (61%). Our results reconfirmed several well-recognized risk factors, such as age, gender (male), and comorbidities, as well as provided novel laboratory indications (e.g., cholesterol) and TCM-specific phenotype markers (e.g., dull tongue) that were relevant to COVID-19 infections and prognosis. In addition to antiviral/antibiotics and standard supportive therapies, TCM herbal prescriptions incorporating 290 distinct herbs were used in 273 (93%) cases. The cases that received TCM treatment had lower death rates than those that did not receive TCM treatment (17/273= 6.2% vs. 7/20= 35%, P = 0.0004 for all cases;17/77= 22% vs. 7/9= 77.7%, P = 0.002 for severe cases). The TCM herbal prescriptions used for the treatment of COVID-19 infections mainly consisted of Pericarpium Citri Reticulatae, Radix Scutellariae, Rhizoma Pinellia, and their combinations, which reflected the practical TCM principles (e.g., clearing heat and dampening phlegm). Lastly, 59% of the patients received treatment, including antiviral, antibiotics, and Chinese patent medicine, before admission. This situation might have some effects on symptoms, such as fever and dry cough. By using EMR data, we described the clinical features and therapeutic characteristics of 293 COVID-19 cases treated via the integration of TCM herbal prescriptions and Western medicine. Clinical manifestations and treatments before admission and in the hospital were investigated. | 2 | The RNA-binding protein Musashi2 governs osteoblast-adipocyte lineage commitment by suppressing PPARγsignaling显示文摘Osteoporosis caused by aging is characterized by reduced bone mass and accumulated adipocytes in the bone marrow cavity. How the balance between osteoblastogenesis and adipogenesis from bone marrow mesenchymal stem cells(BMSCs) is lost upon aging is still unclear. Here, we found that the RNA-binding protein Musashi2(Msi2) regulates BMSC lineage commitment. Msi2 is commonly enriched in stem cells and tumor cells. We found that its expression was downregulated during adipogenic differentiation and upregulated during osteogenic differentiation of BMSCs. Msi2 knockout mice exhibited decreased bone mass with substantial accumulation of marrow adipocytes, similar to aging-induced osteoporosis. Depletion of Msi2 in BMSCs led to increased adipocyte commitment. Transcriptional profiling analysis revealed that Msi2 deficiency led to increased PPARγ signaling.RNA-interacting protein immunoprecipitation assays demonstrated that Msi2 could inhibit the translation of the key adipogenic factor Cebpα, thereby inhibiting PPAR signaling. Furthermore, the expression of Msi2 decreased significantly during the aging process of mice, indicating that decreased Msi2 function during aging contributes to abnormal accumulation of adipocytes in bone marrow and osteoporosis. | 7 | Clinical Epidemiology,Illness Profiles,and the Implication of COVID-19 Before and After the Nationwide Omicron Outbreak During the Winter of 2022显示文摘China.In this study,we aimed to investigate the clinical characteristics and outcomes of patients with coronavirus disease 2019(COVID-19)in the Beijing region.Methods:In this retrospective study,we enrolled inpatients admitted for COVID-19 in the Fifth Medical Center of Chinese PLA General Hospital in Beijing between November 10,2022,and January 30,2023.Demographic and clinical features and treatment outcomeswere comprehensively analyzed.We used logistic regression and linear regression analyses to explore the risk factors associated with disease severity and time of nucleic acid conversion,respectively.Results:A total of 1010 hospitalized patients with COVID-19 were enrolled.The median age was 43.0 years(interquartile range,28.0–63.0),and patients aged<60 years and≥60 years comprised 71.2%and 28.8%of total included patients,respectively.The clinical classification of mild(74.6%,753/1010),moderate(21.0%,212/1010),severe(2.7%,27/1010),and unidentified(1.8%,18/1010)was separately recorded;1005 patients were discharged,and 5 patients died in the hospital.The outbreak of the emerging epidemic witnessed an evident increase in the proportion of moderate(42.9%vs.16.4%)and severe(10.3%vs.1.1%)cases after December 7,2022.Patients with a moderate/severe classification had higher levels of procalcitonin,IL-6,serum ferritin,C-reactive protein,lactic dehydrogenase,serum urea nitrogen,and D-dimer and lower counts of CD4+T,CD8+T,and B cells(all P<0.001).Multivariable regression analysis revealed that increased odds of disease severity were associated with the following factors:age≥60 years,IL-6>7 pg/mL,lactic dehydrogenase level>245 U/L,cough,and fever at admission.Age≥80 years and chronic lung diseasewere independent risk factors in the nonmild group in elderly patients.In addition,the duration for nucleic acid to turn negativewas approximately 5.0 d(interquartile range,3.0–7.0).Prolonged time of nucleic acid conversion was associated with age≥60 years,serum urea nitrogen level>8.2 mmol/L,neutrophil count>7×10^(9)/L,and the presence of a chronic lung disease or carcinoma.Finally,unvaccinated patients accounted for 37.3%of enrolled patients;children and the elder people accounted for approximately half of that.The univariable analysis found that booster doses reduced disease severity and shortened the time of nucleic acid conversion in elderly patients.Conclusions:The outbreak ofOmicron rapidly increased the number of patientswith COVID-19 in Beijing.In elderly patients,booster doses may reduce disease severity and shorten the time of nucleic acid conversion.Healthcare systems should be optimized before an emerging epidemic outbreak. | Yuming Guo Zhe Xu Wen-Xin Wang Cheng Zhen Jinhua Hu Jinsong Mu Chengcheng Ji Xin Yuan Ruonan Xu Lei Huang Lei Shi Fanping Meng Junliang Fu Shuangnan Zhou Siyu Wang Fengyi Li Bo Tu Dawei Zhang Huihuang Huang Yufeng Mao Wen Xu Chao Zhang Xiuying Mu Jun Zhao Bo Jin Haibin Su Yinying Lu Yongqian Cheng Dong Ji Shaoli You Jinghui Dong Changchun Liu Mengmeng Zhang Yuanyuan Li Tianjun Jiang Yonggang Li Fu-Sheng Wang | 2023 | Infectious Diseases & Immunity2023,3,4: | 0 |
