| 1 | 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). | 2 | Prevalence and evolution of drug resistance HIV-1 variants in Henan, China显示文摘To understand the prevalence and evolution of drug resistant HIV strains in Henan China after the implementation of free antiretroviral therapy for AIDS patients. 45 drug nave AIDS patients, 118 AIDS patients who received three months antiretroviral therapy and 124 AIDS patients who received six months antiretroviral treatment were recruited in the southern part of Henan province. Information on general condition, antiretroviral medicines, adherence and clinical syndromes were collected by face to face interview. Meanwhile, 14ml EDTA anticoagulant blood was drawn. CD4/CD8 T cell count, viral load and genotypic drug resistance were tested. The rates of clinical improvement were 55.1% and 50.8% respectively three months and six months after antiretroviral therapy. The mean CD4 cell count after antiretroviral therapy was significantly higher than in drug nave patients. The prevalence rate of drug resistant HIV strains were 13. 9%, 45.4% and 62.7% in drug nave patients, three month treatment patients and six month treatment patients, respectively. The number of resistance mutation codons and the frequency of mutations increased significantly with continued antiretroviral therapy. The mutation sites were primarily at the 103, 106 and 215 codons in the three-month treatment group and they increased to 15 codon mutations in the six-month treatment group. From this result, the evolution of drug resistant strains was inferred to begin with the high level NNRTI resistant strain, and then develop low level resistant strains to NRTIs. The HIV strains with high level resistance to NVP and low level resistance to AZT and DDI were highly prevalent because of the AZT+DDI+NVP combination therapy. These HIV strains were also cross resistant to DLV, EFV, DDC and D4T. Poor adherence to therapy was believed to be the main reason for the emergence and prevalence of drug resistant HIV strains. The prevalence of drug resistant HIV strains was increased with the continu- ation of antiretroviral therapy in the southern part of Henan province. | 3 | Surveillance-based evidence:elimination of schistosomiasis as a public health problem in the Peoples’Republic of China显示文摘Background:A steady progress on schistosomiasis control in the Peoples’Republic of China(P.R.China)was achieved and broadened into the twelve-year medium and long term national plan(MLNP)which marled the implementation of an integrated control strategy across all endemic areas in P.R.China in 2004.To understand the endemic trends of schistosomiasis to assess the effectiveness of an integrated strategy,we conducted an analysis of schistosomiasis surveillance data spanned from 2005 to 2015.Methods:The schistosomiasis sentinel surveillance data from sentinel sites were collected and analyzed from 2005 to 2015.In these sentinel sites,residents aged 6 years or above were screened annually by indirect hemagglutination assay(IHA),while only antibody positives were followed by stool examination either Kato-katz method(KK)and/or hatching technique(HT).Domestic animals raised in sentinel sites were examined by HT for confirming the infection of schistosomes.Snail investigation was conducted each year through systematic sampling method combined with environmental sampling method.The snails collected from field were tested by microscopic dissection method.The infection rates of schistosomes in residents,domestic animals and snails,as well as the indicators reflecting the snails’distribution were calculated and analyzed.ANOVA analysis was used to examine the changes of the number of eggs per gram feces in population and Chi-square test was used to examine any change in proportions among groups.Results:A total of 148902 residents from sentinel sites attended this study and 631676 blood samples were examined by IHA test during the 11 covered years.The annual average antibody positive rates presented a significant decrease trends,from 17.48%(95%CI:17.20–17.75%)in 2005 to 5.93%(95%CI:5.71–6.15%)(χ2=8890.47,P<0.001)in 2015.During 2005–2015,the average infection rate of schistosomes in residents declined from 2.07%(95%CI:1.96–2.17%)to 0.13%(95%CI:0.09–0.16%),accompanied by significant decrease of infection intensity in population.In 2015,the stool positives were only found in farmers,fishermen and boatmen with infection rate of 0.16%(95%CI:0.11–0.20%),0.17%(95%CI:0–0.50%)respectively.The infection rate of schistosomes in domestic animals dropped from 9.42%(538/5711,95%CI:8.66–10.18%)to 0.08%(2/2360,95%CI:0–0.20%)from 2005 to 2015.Infections were found in eight species of domestic animals at the beginning of surveillance while only two cattle were infected in 2015.Totally 98 ha of new snail habitats were found,while 94.90%(93/98)distributed in lake and marshland regions.The percentage of frames with snails decreased from 16.96%(56884/335391,95%CI:16.83–17.09%)in 2005 to 4.28%(18121/423755,95%CI:4.22–4.34%)in 2014,with a slightly increase in 2015.Meanwhile,the infection rate of schistosomes in snails was decreased from 0.26%(663/256531,95%CI:0.24–0.28%)to zero during 2005–2015.Conclusions:The infection rate of schistosomes declined significantly,providing evidence that the goal of the MLNP was achieved.Elimination of schistosomiasis as a public health problem defined as WHO was also reached in P.R.China nationwide.Surveillance-response system should be improved and strengthened to realize the final goal of schistosomiasis elimination. | Jing Xu Shi-Zhu Li Li-Juan Zhang Robert Bergquist Hui Dang Qiang Wang Shan Lv Tian-Ping Wang Dan-Dan Lin Jian-Bing Liu Guang-Hui Ren Kun Yang Yang Liu Yi Dong Shi-Qing Zhang Xiao-Nong Zhou | 2020 | Infectious Diseases of Poverty2020,9,3: | 20 |
| 4 | Knowledge of, attitudes towards, and practice relating to schistosomiasis in two subtypes of a mountainous region of the People’s Republic of China显示文摘Background:Schistosomiasis japonica is still endemic in the People’s Republic of China(P.R.China)in five provinces of lake and marshland regions and in two provinces of mountainous regions.Studies elucidated that individual and community perception,attitudes towards schistosomiasis,and hygiene behaviors were crucial factors for preventing schistosomiasis.This study sought to assess the knowledge of,attitudes towards,and practices(KAP)relating to schistosomiasis in two subtypes of a mountainous region in Eryuan County,Yunnan Province,P.R.China.The study’s aim is to make suggestions for establishing more specific and effective control measures for disease transmission and interruption in two subtypes of a mountainous region with low-level infection rates.Methods:A cross-sectional study of 3,000 inhabitants was carried out in the Yongle(plateau basin)and Xinzhuang(plateau canyon)communities of Eryuan County,Yunnan Province in November and December 2011.Stratified cluster random sampling was undertaken using a uniform set of quantitative questionnaires administered by trained assistants.This was further supported with qualitative data from in-depth interviews(IDIs)conducted with ten farmers and ten students.All participants were examined for schistosomiasis using both a serological test(indirect hemagglutination assay[IHA])and a stool examination(Kato-Katz).Results:The total schistosomiasis knowledge rate in Yongle(83.4%)was significantly lower than that in Xinzhuang(95.5%).In both communities,among the respondents aged 15 years or below,more than one third didn’t know the name,endemic areas,and animal reservoirs of schistosomiasis.The majority of respondents in Eryuan acquired their schistosomiasis knowledge from doctors,followed by handouts and hearing from others.The infection rate was once the highest in Yongle,but is now the highest in Xinzhuang,where there are more risk factors for schistosomiasis,such as frequently grazing cattle,digging vegetables or cutting grass in the field,as well as raising cattle by free grazing.Conclusion:In short,Eryuan County’s overall knowledge rate of schistosomiasis was found to be high.Due to various dominating risk factors,different control strategies should be designed keeping in mind the two different subtypes of endemic areas for schistosomiasis in mountainous regions,namely plateau basins and plateau canyons. | Lu Liu Guo-Jing Yang Hong-Ru Zhu Kun Yang Lin Ai | 2014 | Infectious Diseases of Poverty2014,3,1: | 14 |

| 6 | Effects of professional rehabilitation training on the recovery of neurological function in young stroke patients显示文摘Young stroke patients have a strong desire to return to the society,but few studies have been conducted on their rehabilitation training items,intensity,and prognosis.We analyzed clinical data of young and middle-aged/older stroke patients hospitalized in the Department of Neurological Rehabilitation,China Rehabilitation Research Center,Capital Medical University,China from February 2014 to May 2015.Results demonstrated that hemorrhagic stroke(59.6%) was the primary stroke type found in the young group,while ischemic stroke(60.0%)was the main type detected in the middle-aged/older group.Compared with older stroke patients,education level and incidence of hyperhomocysteinemia were higher in younger stroke patients,whereas,incidences of hypertension,diabetes,and heart disease were lower.The average length of hospital stay was longer in the young group than in the middle-aged/older group.The main risk factors observed in the young stroke patients were hypertension,drinking,smoking,hyperlipidemia,hyperhomocysteinemia,diabetes,previous history of stroke,and heart disease.The most accepted rehabilitation program consisted of physiotherapy,occupational therapy,speech therapy,acupuncture and moxibustion.Average rehabilitation training time was 2.5 hours/day.Barthel Index and modified Rankin Scale scores were increased at discharge.Six months after discharge,the degree of occupational and economic satisfaction declined,and there were no changes in family life satisfaction.The degrees of other life satisfaction(such as friendship) improved.The degree of disability and functional status improved significantly in young stroke patients after professional rehabilitation,but the number of patients who returned to society within 6 months after stroke was still small. | Chao-jin-zi Li Xiao-xia Du Kun Yang Lu-ping Song Peng-kun Li Qiang Wang Rong Sun Xiao-ling Lin Hong-yu Lu Tong Zhang | 2016 | Neural Regeneration Research2016,11,11: | 11 |
| 7 | Role of Contrast Enhanced Ultrasound in Radiofrequency Ablation of Metastatic Liver Carcinoma显示文摘Objective:To investigate the application of contrast enhanced ultrasound(CEUS) in planning and guiding for radiofrequency ablation(RFA) for metastatic liver carcinoma(MLC).Methods:One hundred and thirty-five patients with clinically and pathologically diagnosed MLC(from gastrointestinal tumors) were included in the present study,and 104 of them had received CEUS prior to RFA to assess the number,size,shape,infiltration,location and enhancing features of the lesions.Among the 104 patients,21(20.1%) were excluded from RFA treatment due to too many lesions or large infiltrative range based on CEUS.The remaining 83 patients with 147 lesions underwent RFA(group A).During the same period,other 31 patients with 102 lesions serving as control group were treated based on findings of conventional ultrasound without contrast(group B).The patients underwent follow-up enhanced CT at the 1st month,and then every 3-6 months after RFA.The tumor was considered as early necrosis if no contrast enhancement was detected in the treated area on the CT scan at the 1st month.Results:In group A,72 of 147 MLC lesions(48.9%) showed increased sizes on CEUS.Among them,48 lesions(66.6%) appeared enlarged in arterial phase,and 24(33.3%) showed enlarged hypoechoic area in parenchymal phase.CEUS showed total 61 additional lesions in 35 patients(42.1%)(ranged from 8 to 15 mm) compared with conventional ultrasound(US),and 42(68.8%) of them were visualized in parenchymal phase only.There were total 208 lesions in group A underwent RFA with CEUS planning,and the tumor necrosis rate was 94.2%(196/208).In this group,local recurrence was found in 16 lesions(7.7%) during 3-42 months' following up,and new metastases were seen in 30 cases(36.1%).For group B,the tumor necrosis rate was 86.3%(88/102),local recurrence in 17 lesions(16.7%),and new metastases in 13 cases(41.9%).Tumor early necrosis and recurrence rates were significantly different between the two groups(P=0.018,P=0.016,respectively).Conclusion: CEUS played an important role in RFA for liver metastases by candidate selecting and therapy planning, which helped to improve the outcome of the treatment. | Jin-yu Wu Min-hua Chen Wei Yang Shu-zhi Lin Wei Wu Shan-shan Yin HuiZhang Kun Yan | 2012 | Chinese Journal of Cancer Research2012,24,1: | 11 |
| 8 | 在胃的癌症的帮助 laparoscopy 对开的激进的 gastrectomy 的短期的功效显示文摘 AIM:To investigate the short-term benefits of laparoscopic radical gastrectomy(LARG)and open radical gastrectomy(ORG)in patients with gastric cancer.METHODS:A total of 400 patients with gastric cancer aged≤65 years who were treated at General Hospital of Lanzhou Military Region were enrolled.Among these,200 patients underwent LARG between October2008 and August 2012(LARG group);and 200 patients underwent ORG between March 2000 and September2008(ORG group).The short-term therapeutic benefits between the two groups were analyzed.RESULTS:The LARG procedure offered significantly better benefits to the patients compared to the ORG procedure,including less intraoperative blood loss(103.1±19.5 mL vs 163.0±32.9 mL,P<0.0001),shorter postoperative hospital stay(6.8±1.2 d vs 9.5±1.6 d,P<0.0001),less frequent occurrence of postoperative complications(6.5%vs 13.5%,P=0.02),shorter time to mobilization(1.0±0.3 vs 3.3±0.4 d,P<0.0001),shorter time to bowel opening(3.3±0.7 d vs 4.5±0.7 d,P<0.0001),and shorter time to normal diet(3.0±0.4 vs d 3.8±0.5 d,P<0.0001).However,LARG required a longer time to complete than the ORG procedure(192.3±20.9 min vs 180.0±26.9 min,P<0.0001).CONCLUSION:Compared to ORG,LARG is safer,more effective,and less invasive for treating gastric cancer,with better short-term efficacy. | Hong-Tao Li Xiao-Peng Han Lin Su Wan-Kun Zhu Wei Xu Kun Li Qing-Chuan Zhao Hua Yang Hong-Bin Liu | 2014 | World Journal of Gastrointestinal Surgery2014,6,4: | 10 |
| 9 | Clinical study of ultrasound and microbubbles for enhancing chemotherapeutic sensitivity of malignant tumors in digestive system显示文摘Objective: To explore the safety of ultrasound and microbubbles for enhancing the chemotherapeutic sensitivity of malignant tumors in the digestive system in a clinical trial, as well as its efficacy.Methods: From October 2014 to June 2016, twelve patients volunteered to participate in this study. Eleven patients had hepatic metastases from tumors of the digestive system, and one patient had pancreatic carcinoma.According to the mechanical index(MI) in the ultrasound field, patients were classified into four groups with MIs of 0.4, 0.6, 0.8 and 1.0. Within half an hour after chemotherapy, patients underwent ultrasound scanning with ultrasound microbubbles(SonoVue) to enhance the efficacy of chemotherapy. All adverse reactions were recorded and were classified in 4 grades according to the Common Terminology Criteria for Adverse Events version 4.03(CTCAE V4.03). Tumor responses were evaluated by the Response Evaluation Criteria in Solid Tumors version1.1 criteria. All the patients were followed up until progression.Results: All the adverse reactions recorded were level 1 or level 2. No local pain occurred in any of the patients.Among all the adverse reactions, fever might be related to the treatment with ultrasound combined with microbubbles. Six patients had stable disease(SD), and one patient had a partial response(PR) after the first cycle of treatment. At the end of follow-up, tumor progression was restricted to the original sites, and no new lesions had appeared.Conclusions: Our preliminary data showed the potential role of a combined treatment with ultrasound and microbubbles in enhancing the chemotherapeutic sensitivity of malignant tumors of the digestive system. | 11 | Circulating tumor cells:biology and clinical significance显示文摘Circulating tumor cells(CTCs)are tumor cells that have sloughed off the primary tumor and extravasate into and circulate in the blood.Understanding of the metastatic cascade of CTCs has tremendous potential for the identification of targets against cancer metastasis.Detecting these very rare CTCs among the massive blood cells is challenging.However,emerging technologies for CTCs detection have profoundly contributed to deepening investigation into the biology of CTCs and have facilitated their clinical application.Current technologies for the detection of CTCs are summarized herein,together with their advantages and disadvantages.The detection of CTCs is usually dependent on molecular markers,with the epithelial cell adhesion molecule being the most widely used,although molecular markers vary between different types of cancer.Properties associated with epithelial-tomesenchymal transition and stemness have been identified in CTCs,indicating their increased metastatic capacity.Only a small proportion of CTCs can survive and eventually initiate metastases,suggesting that an interaction and modulation between CTCs and the hostile blood microenvironment is essential for CTC metastasis.Single-cell sequencing of CTCs has been extensively investigated,and has enabled researchers to reveal the genome and transcriptome of CTCs.Herein,we also review the clinical applications of CTCs,especially for monitoring response to cancer treatment and in evaluating prognosis.Hence,CTCs have and will continue to contribute to providing significant insights into metastatic processes and will open new avenues for useful clinical applications. | Danfeng Lin Lesang Shen Meng Luo Kun Zhang Jinfan Li Qi Yang Fangfang Zhu Dan Zhou Shu Zheng Yiding Chen Jiaojiao Zhou | 2021 | Signal Transduction and Targeted Therapy2021,6,12: | 8 |

| 13 | Validity of the Chinese Version of the Patient-Generated Subjective Global Assessment (PG-SGA) in Lung Cancer Patients显示文摘The prevalence of malnutrition is high among oncology patients in China. Although the Patient- Generated Subjective Global Assessment (PG-SGA) is widely recognized as an important nutritional assessment tool, it has not been validated for Chinese cancer patients. The purpose of this study was to conduct an analysis of nutrition screening and assessment data to validate the Chinese version of the PG-SGA in Chinese patients with lung cancer. This was an observational, cross-sectional study of 2,000 consecutive adult patients with lung cancer treated at several tertiary hospitals in China. Anthropometric and patient descriptive data were collected. The PG-SGA generated a score for the nutritional risk and a global rating for the nutritional status. The internal consistency, external consistency, and construct validity were evaluated, and a known-groups comparison and principal component analyses were conducted to evaluate the reliability and validity of the PG-SGA. The known-groups comparison demonstrated the ability of the PG-SGA to differentiate lung cancer patients of different nutritional and body mass index (BMI) groups. A confirmatory factor analysis supported the original four-factor structure of the PG-SGA. In terms of the internal and external consistency, the PG-SGA demonstrated intra-class correlation coefficients (ICC) up to 0.987. Alternative form validity was also demonstrated between Box 4 of the PG-SGA and the Karnofsky Performance Status score, with excellent external consistency (r = 0.972, P < 0.001). The construct validity was supported, and selected questionnaire dimensions were evident in the principal component analysis. Significant Spearman correlations (P < 0.001) were demonstrated. The Chinese version of the PG-SGA demonstrated significant reliability and sufficient exploratory properties to support its validity. It seems to be a valid tool that can be used to access the nutritional status of Chinese lung cancer patients. | 16 | Assessment of prophylactic antibiotics administration for acute pancreatitis:a meta-analysis of randomized controlled trials显示文摘Background:Recent evidence has shown that prophylactic antibiotic treatment in patients with acute pancreatitis is not associated with a significant decrease in mortality or morbidity.The use and efficacy of prophylactic antibiotic treatment in acute pancreatitis remain controversial.This meta-analysis was conducted to assess whether antibiotic prophylaxis is beneficial in patients with acute pancreatitis.Methods:We searched randomized controlled trials(RCTs)of prophylactic use of antibiotics using Medline(PubMed),Embase,the Cochrane Library,and Web of Science.The data were analyzed using Review Manager 5.3 software.We performed pooled analyses for infected pancreatic necrosis,mortality,surgical intervention,and non-pancreatic infection.Odds ratios(ORs)from each trial were pooled using a random or fixed effects model,depending on the heterogeneity of the included studies.Sub-group analysis or sensitivity analysis was conducted to explore potential sources of heterogeneity,when necessary.Results:Totally,11 RCTs involving 747 participants were included,with an intervention group(prophylactic use of antibiotics,n=376)and control group(n=371).No significant differences were found regarding antibiotic prophylaxis with respect to incidence of infected pancreatic necrosis(OR,0.74;95%confidence interval[CI],0.50–1.09;P=0.13),surgical intervention(OR,0.92;95%CI,0.62–1.38;P=0.70),and morality(OR,0.71;95%CI,0.44–1.15;P=0.16).However,antibiotic prophylaxis was associated with a statistically significant reduction in the incidence of non-pancreatic infection(OR,0.59;95%CI,0.42–0.84;P=0.004).Conclusions:Prophylactic antibiotics can reduce the incidence of non-pancreatic infection in patients with AP. | Nan Ding Yong-Hui Sun Li-Mei Wen Jian-Hua Wang Jian-Hua Yang Kun Cheng Hai Lin Qi-Long Chen | 2020 | Chinese Medical Journal2020,,2: | 6 |
