12018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). 2Intelligent and connected vehicles: Current status and future perspectives显示文摘Intelligent connected vehicles(ICVs) are believed to change people's life in the near future by making the transportation safer,cleaner and more comfortable. Although many prototypes of ICVs have been developed to prove the concept of autonomous driving and the feasibility of improving traffic efficiency, there still exists a significant gap before achieving mass production of high-level ICVs. The objective of this study is to present an overview of both the state of the art and future perspectives of key technologies that are needed for future ICVs. It is a challenging task to review all related works and predict their future perspectives, especially for such a complex and interdisciplinary area of research. This article is organized to overview the ICV key technologies by answering three questions: what are the milestones in the history of ICVs; what are the electronic components needed for building an ICV platform; and what are the essential algorithms to enable intelligent driving? To answer the first question, the article has reviewed the history and the development milestones of ICVs. For the second question, the recent technology advances in electrical/electronic architecture, sensors, and actuators are presented. For the third question, the article focuses on the algorithms in decision making, as the perception and control algorithm are covered in the development of sensors and actuators. To achieve correct decision-making, there exist two different approaches: the principle-based approach and data-driven approach. The advantages and limitations of both approaches are explained and analyzed. Currently automotive engineers are concerned more with the vehicle platform technology, whereas the academic researchers prefer to focus on theoretical algorithms. 3Laparoscopic distal pancreatectomy is as safe and feasible as open procedure:A meta-analysis显示文摘AIM:To evaluate the feasibility and safety of laparoscopic distal pancreatectomy(LDP) compared with open distal pancreatectomy(ODP).METHODS:Meta-analysis was performed using the databases,including PubMed,the Cochrane Central Register of Controlled Trials,Web of Science and BIOSIS Previews.Articles should contain quantitative data of the comparison of LDP and ODP.Each article was reviewed by two authors.Indices of operative time,spleen-preserving rate,time to fluid intake,ratio of malignant tumors,postoperative hospital stay,incidence rate of pancreatic fistula and overall morbidity rate were analyzed.RESULTS:Nine articles with 1341 patients who underwent pancreatectomy met the inclusion criteria.LDP was performed in 501(37.4%) patients,while ODP was performed in 840(62.6%) patients.There were significant differences in the operative time,time to fluid intake,postoperative hospital stay and spleen-preserving rate between LDP and ODP.There was no difference between the two groups in pancreatic fistula rate [random effects model,risk ratio(RR) 0.996(0.663,1.494),P = 0.983,I2 = 28.4%] and overall morbidity rate [random effects model,RR 0.81(0.596,1.101),P = 0.178,I2 = 55.6%].CONCLUSION:LDP has the advantages of shorter hospital stay and operative time,more rapid recovery and higher spleen-preserving rate as compared with ODP.Kun Xie Yi-Ping Zhu Xiao-Wu Xu Ke Chen Jia-Fei Yan Yi-Ping Mou 2012World Journal of Gastroenterology2012,18,16:22
4Kinetics and equilibrium of adsorption of dissolved organic matter fractions from secondary effluent by fly ash显示文摘Fly ash was used as a low-cost adsorbent for removing dissolved organic matter (DOM) in secondary effluent. Batch experiments were conducted under various adsorbent dosages, pH, contact time, temperatures and DOM fractional characteristics. Under the optimum conditions of fly ash dosage of 15 g/L, temperature of 303 K and contact time of 180 min, a removal of 22.5% of the dissolved organic carbon (DOC), 23.7% of UV-254, 25.9% of the trihalomethanes precursors in secondary effluent was obtained. The adsorption of DOM fractions onto fly ash all followed the pseudo second-order kinetic model, and the hydrophilic fraction adsorption by fly ash also fitted the intraparticle diffusion model quite well. Freundlich and Langmuir models were applicable to the fly ash adsorption and their constants were evaluated. The maximum adsorption capacities of the adsorptions revealed that fly ash was more effective in adsorbing hydrophilic fraction than the acidic fractions. Structure changes of the DOM fractions after fly ash adsorption were also characterized via spectrum analyzing. 5Circular RNA ciRS-7 promotes the proliferation and metastasis of pancreatic cancer by regulating miR-7-mediated EGFR/STAT3 signaling pathway显示文摘Background: Pancreatic ductal adenocarcinoma(PDAC) is the most deadly type of tumor, and its pathogenesis remains unknown. Circular RNAs(circRNAs) may be functional and bind to micro RNAs and consequently, influence the activity of targeted mRNAs. Recent researches indicate that one circRNA, ciRS-7, acts as a sponge of miR-7 and thus, inhibits its activity. It is well known that miR-7 is a cancer suppressor in many cancers. However, the relationship between ciRS-7 and miR-7, and the role of ciRS-7 in PDAC, remains to be elucidated. Methods: miR-7 and ciRS-7 expression in 41 pairs of PDAC tumors and their paracancerous tissues were detected by quantitative reverse transcription polymerase chain reaction(qRT-PCR). The relationships between their expression levels and clinicopathological features in PDAC tissues were assessed. The relationship between miR-7 and ciRS-7 was also assessed by Spearman’s correlation. We also used cell lines to evaluate the role of ciRS-7 in cell line behavior. The ciRS-7 interfere RNA(si RNA) and its empty vector were transfected into PDAC cells. PDAC cells proliferation and invasion abilities were detected by MTT assay and invasion analysis. The expression of proteins was assessed by Western blotting. Results: ciRS-7 expression was significantly higher in PDAC tissues than paracancerous tissues( P = 0.002). However, miR-7 expression showed the opposite trend( P = 0.048). Moreover, ciRS-7 expression was inversely correlated with miR-7 in PDAC( r s =-0.353, P = 0.023). ciRS-7 expression was also significantly elevated in venous invasion(3.72 ± 2.93 vs. 2.14 ± 1.26;P = 0.028) and lymph node metastasis(4.19 ± 2.75 vs. 2.32 ± 1.90;P = 0.016) in PDAC patients. Furthermore, ciRS-7 knockdown suppressed cell proliferation and invasion of PDAC cells( P < 0.05), and the downregulation of ciRS-7 resulted in miR-7 overexpression and subsequent inhibition of epidermal growth factor receptor(EGFR) and signal transducer and activator of transcription 3(STAT3). 6Interfacial Microstructure and Properties of a Vacuum Hot Roll-bonded Titanium-Stainless Steel Clad Plate with a Niobium Interlayer显示文摘Vacuum hot roll bonding(VHRB)was used to bond pure titanium(Ti)plate to a 304 stainless steel(SS)plate with a niobium(Nb)interlayer,with the aim of producing a high-quality Ti-SS clad plate.The roll-bonding process was performed at diferent temperatures in the range of 850–1000 C,followed by characterization of microstructure and mechanical properties.The study demonstrates that the interfaces are free from cracks and discontinuities,and interdifusion between the stainless steel and the titanium is efectively prevented by inserting a layer of pure Nb foil.No intermetallic reaction layer occurred at the Nb-Ti interface at any of the investigated temperatures.An intermetallic FeNb phase was formed at the Nb-SS interface when bonding was performed at 950 C and above.The presence of the FeNb layer was confirmed by x-ray difraction.The maximum shear strength of^396 MPa was obtained when bonding is carried out at 900 C.However,the formation of the FeNb layer at roll bonding temperature greater than 900 C led to decrease in shear strength.Ductile fracture occurred through the Ti-Nb interface for roll-bonded temperatures of up to 900 C.On the other hand,at temperature of 950 C and above,failure occurred through the Nb-SS interface,with brittle fracture characteristics.Zongan LUO Guanglei WANG Guangming XIE Lipeng WANG Kun ZHAO 2013Acta Metallurgica Sinica(English Letters)2013,26,6:18
7Magnolol additive as a replacer of antibiotic enhances the growth performance of Linwu ducks显示文摘Magnolol rich in Magnolia officinalis is a bioactive polyphenolic compound. The aim of this study was to examine the effects of magnolol additive(MA) on growth performance, expression levels of antioxidantrelated genes, and intestinal mucosal morphology of Linwu ducks aged from 49 to 70 days, comparing with that of an antibiotic additive(colistin sulfate [CS]). A total of 275,49-day-old ducks were assigned to5 groups with 5 cages of 11 ducks each and fed diets supplemented with 0,100, 200 and 300 mg of MA/kg and 300 mg of CS/kg for 3 weeks, respectively. The results showed that the average daily body weight gain(ADG) was increased significantly in MA-fed groups(200 and 300 mg/kg), compared with the basal diet(BD) group(P < 0.05). The mRNA levels of superoxide dismutase-1(SOD1), manganese superoxide dismutase-2(MnSOD2) and catalase(CAT) were also increased significantly in MA groups(P < 0.05). In addition, hematoxylin and eosin staining revealed that Linwu ducks fed the diets with MA had more intact intestinal mucosa than those fed the BD and CS diets. In addition, ileal villus height, ileal villus height/crypt depth ratio(V/C) and duodenal V/C were also improved significantly(P < 0.05). 8Comparsion analysis of data mining models applied to clinical research in Traditional Chinese Medicine显示文摘OBJECTIVE: To help researchers selecting appropriate data mining models to provide better evidence for the clinical practice of Traditional Chinese Medicine(TCM) diagnosis and therapy.METHODS: Clinical issues based on data mining models were comprehensively summarized from four significant elements of the clinical studies:symptoms, symptom patterns, herbs, and efficacy.Existing problems were further generalized to determine the relevant factors of the performance of data mining models, e.g. data type, samples, parameters, variable labels. Combining these relevant factors, the TCM clinical data features were compared with regards to statistical characters and informatics properties. Data models were compared simultaneously from the view of applied conditions and suitable scopes.RESULTS: The main application problems were the inconsistent data type and the small samples for the used data mining models, which caused the inappropriate results, even the mistake results. 9Extracellular vesicles from human urine-derived stem cells prevent osteoporosis by transferring CTHRC1 and OPG显示文摘Osteoporosis is a debilitating bone disease affecting millions of people. Here, we used human urine-derived stem cells(USCs),which were noninvasively harvested from unlimited and easily available urine, as a 'factory' to obtain extracellular vesicles(USCEVs) and demonstrated that the systemic injection of USC-EVs effectively alleviates bone loss and maintains bone strength in osteoporotic mice by enhancing osteoblastic bone formation and suppressing osteoclastic bone resorption. More importantly, the anti-osteoporotic properties of USC-EVs are not notably disrupted by the age, gender, or health condition(with or without osteoporosis) of the USC donor. Mechanistic studies determined that collagen triple-helix repeat containing 1(CTHRC1) and osteoprotegerin(OPG) proteins are enriched in USC-EVs and required for USC-EV-induced pro-osteogenic and anti-osteoclastic effects. 12Computational Prediction and Identification of Epstein-Barr Virus Latent Membrane Protein 2A Antigen-Specific CD8^+ T-Cell Epitopes显示文摘Epstein-Barr virus (EBV) associated nasopharyngeal carcinoma (NPC) is a high incidence tumor in Southeast Asia. Among EBV encoded proteins, latent membrane protein 2A (LMP2A) is an important antigen for T cell therapy of EBV. In this study, we predicted six HLA-A2 restricted CTL candidate epitopes of LMP2A by SYFPEITHI, NetMHC and MHCPred methods combined with the polynomial method. Subsequently, biological functions of these peptides were tested by experiments in vitro. In ELISPOT assay, the positive response of the LMP2A specific CTL stimulated by three (LMP2A264-272, LMP2A426-434 and LMP2A356-364) of six peptides respectively showed that the numbers of spots forming cells (SFC) ranged from 55.7 to 80.6 SFC/5 × 104 CD8+ T cells and the responding index (RI) ranged from 5.4 to 7. These three epitope-specific CTLs could effectively kill specific HLA-A2-expressing target cells. As a result, LMP2A264-272 (QLSPLLGAV), LMP2A426-434 (CLGGLLTMV) and LMP2A356-364 (FLYALALLL) were identified as LMP2A-specific CD8+ T-cell epitopes. 13Long-term outcomes after hepatectomy of huge hepatocellular carcinoma: A single-center experience in China显示文摘Background: Currently, hepatectomy remains the first-line therapy for hepatocellular carcinoma(HCC). However, surgery for patients with huge( > 10 cm) HCCs is controversial. This retrospective study aimed to explore long-term survival after hepatectomy for patients with huge HCC. Methods: The records of 188 patients with pathologically confirmed HCC who underwent curative hepatectomy between 2007 and 2017 were reviewed;patients were divided into three groups according to tumor size: huge( > 10 cm;n = 84), large(5–10 cm;n = 51) and small( < 5 cm;n = 53) HCC. Kaplan-Meier analysis was used to assess overall survival(OS) and disease-free survival(DFS), and log-rank analysis was performed for pairwise comparisons among the three groups. Risk factors for survival and recurrence were analyzed using the Cox proportional hazard model. Results: The median follow-up period was 20 months. Although the prognosis of small HCC was better than that of huge and large HCC, OS and DFS were not significantly different between huge and large HCC( P = 0.099 and P = 0.831, respectively). A family history of HCC, poor Child-Pugh class, vascular invasion, diolame, pathologically positive margins, and operative time ≥240 min were identified as independent risk factors for OS and DFS in a multivariate model. Tumor size( > 10 cm) had significant effect on OS, and postoperative antiviral therapy and postoperative complications also had significant effects on DFS. Conclusions: Huge HCC is not a contraindication of hepatectomy. 14GIS-based analysis of population exposure to PM2.5 air pollution-A case study of Beijing显示文摘PM_(2.5), formally defined as particulate matter with diameter less than 2.5 μm, is one of most harmful air pollutants threatening human health. Numerous epidemiological studies have shown that both short-term and long-term exposures to PM_(2.5) are strongly linked with respiratory diseases. In this study, various types of spatio-temporal data were collected and used to estimate the spatio-temporal variation of PM_(2.5) exposure in Beijing in 2014. The seasonal and daily variation of the population-weighted exposure level(PWEL) in 2014 was estimated and compared. 16Preliminary study of a new pathological evolution-based clinical hepatolithiasis classification显示文摘AIM:To investigate clinical features,treatment strategies and outcomes of patients with hepatolithiasis(HL)undergoing surgical treatment,using a new clinicalclassification.METHODS:Sixty-eight HL patients were hospitalized and treated surgically from August 2011 to December2012 and they were classified into four HL types according to pathological evolution of the disease.These four HL types included typeⅠprimary type(defined as no previous biliary tract surgery),typeⅡinflammatory type(with previous biliary tract surgery and cholangitis),typeⅢmass-forming type(HL complicated by hepatic mass-forming lesion),and typeⅣterminal type(with secondary biliary cirrhosis and resultant portal hypertension).The perioperative data including general information,imaging data,postoperative complications,and immediate and final stone clearance rate were obtained and analyzed.RESULTS:In all 68 patients,the proportion of HL typeⅠ-Ⅳwas 50%(34/68),36.8%(25/68),10.3%(7/68)and 2.8%(2/68),respectively.Abdominal pain was the main clinical manifestation in typeⅠ(88.2%),fever was predominant in typeⅡ(52.0%),the malignancy rate in typeⅢwas high(71.4%),and portal hypertension and spleen enlargement were common in typeⅣ(2/2,100.0%).Liver resection rate for typesⅠ-Ⅲwas 79.4%,72.0%and 71.4%,respectively.The overall incidence of postoperative complications was23.5%(16/68).There were no perioperative deaths.The average length of hospital stay was 12.7±7.3 d.Immediate and final stone clearance rate was 73.5%(50/68)and 89.7%(61/68),respectively.Fifty-nine of68 patients(86.8%)were followed-up for>1 year after surgery,and 96.6%of these patients(57/59)had a good quality of life according to a criterion recommended for postoperative evaluation of quality of life.CONCLUSION:The pathological evolution-based clinical classification of HL has a role in optimizingtreatment strategy,and patients can benefit from thisclassification when it is used properly.Fu-Bao Liu Xiao-Jun Yu Guo-Bing Wang Yi-Jun Zhao Kun Xie Fan Huang Jiang-Ming Cheng Xin-Rao Wu Chao-Jie Liang Xiao-Ping Geng 2015World Journal of Gastroenterology2015,21,7:9
17Analysis of 1D consolidation with non-Darcian flow described by exponent and threshold gradient显示文摘Numerous experiments have shown that the water flow in fine-grained soils can obey an exponential relationship at small gradients and a linear relationship when the hydraulic gradient exceeds a certain limit. Based on the non-Darcian flow described by exponent and threshold gradient, the theory of 1D consolidation is modified in this paper to consider a linear variation in the vertical total stress with depth and the effect of ramp loading. The numerical solutions were derived in detail by the finite difference method for excess pore water pressure and the average degree of consolidation. Finally, the influence of various parameters on consolidation behavior was investigated. The results show that the rate of consolidation is reduced when non-Darcian flow described by exponent and threshold gradient is adopted in the theory of 1D consolidation. As well the distribution of vertical total stress has a great influence on the dissipation of excess pore water pressure, either for pervious top and pervious bottom (PTPB) or for pervious top and impervious bottom (PTIB). For the case of PTIB, the distribution of vertical total stress in a foundation has a great influence on the rate of consolidation; however, for the case of PTPB, the rate of consolidation is independent of the distribution of vertical total stress. The rate of consolidation is dependent on the ratio of the thickness of a soil layer to the equivalent head of the final average vertical total stress; the greater the value of this ratio, the slower the rate of consolidation. Finally, an increase in construction time reduces the consolidation rate of a foundation. 19Task-related functional magnetic resonance imaging-based neuronavigation for the treatment of depression by individualized repetitive transcranial magnetic stimulation of the visual cortex显示文摘To determine whether repetitive transcranial magnetic stimulation(rTMS)of the visual cortex(VC)provides effective and welltolerated treatment and whether magnetic resonance imaging(MRI)measures functional change of the VC as a biomarker of therapeutic effect in major depressive disorder(MDD),we performed a sham-controlled,double-blind,randomized,three-arm VC rTMS treatment study in 74 MDD patients.Neuronavigated rTMS(10 Hz,90%of resting motor threshold,1,600 pulses over20 min twice per day)was performed over the VC for five days.Clinical outcome was measured by Hamilton Depression Rating Scale(HAMD-24)at days 0,1,3,5 and after terminating rTMS,with follow-up at four weeks.MRI was measured at days 0 and5.The individualized group exhibited the greatest change in HAMD-24 scores after VC rTMS for 5 days(F=5.53,P=0.005),which were maintained during follow-up period(F=4.22,P=0.016).All patients reported good tolerance.Changes in VC taskrelated functional MRI correlated with symptomatic reduction in the individualized group.Treatment reduced the initially abnormal increase in resting state functional connectivity from the VC to the pre/subgenual anterior cingulate cortex at day 5,especially in the individualized group.We demonstrated therapeutic potential and good tolerance of VC rTMS in MDD patients,indicated by biomarkers of f MRI measurement.Zhijun Zhang Hongxing Zhang Chun-Ming Xie Meng Zhang Yachen Shi Ruize Song Xiang Lu Haisan Zhang Kun Li Bi Wang Yongfeng Yang Xianrui Li Jianli Zhu Yang Zhao Ti-Fei Yuan Georg Northoff 2021Science China(Life Sciences)2021,64,1:8
