2Cluster analysis for syndromes of real-world coronary heartdisease with angina pectoris显示文摘Syndromes of coronary heart disease with angina pectoris were analyzed to provide guidance forclinical practice and to improve accuracy of traditional Chinese medicine (TCM) diagnoses and efficacy of TCMtreatment. A total of 860 cases with coronary heart disease with angina pectoris were selected from TCM ClinicalResearch Information Sharing System for TCM clinics and research. Syndromes were automatically extractedwith the cluster method and were analyzed to provide objective evidence for clinical studies. Final syndromeclassifications were recognized and confirmed by clinical experts. Popular syndromes included Qi and blooddeficiency, blood stasis and obstruction collaterals, liver depression and spleen deficiency, and Qi stagnation andblood stasis. Syndromes Qi and blood deficiency and blood stasis and obstruction collaterals accounted for 28.61%of total syndromes, whereas liver depression and spleen deficiency and Qi stagnation and blood stasis accountedfor 26.44%. 4Mechanism of acupuncture regulating visceral sensation and mobility显示文摘Chinese ancient medical scientists have long focused on the internal and external contacts between acupoints on the surface of the body and the viscera.The Miraculous Pivot(it is one of the earliest medical classics in China)stated,“Twelve regular channels belong to the zang-fu organs internally,and connect to the extremities and joints externally.”Traditional Chinese medicine considers acupoints as defined areas where the Qi of viscera and meridians are transfused.These include the reaction points of visceral diseases on the body surface as well as the acupuncture trigger points that promote the flow of Qi and blood,and regulate visceral function.Chinese ancient medical scientists classified the specificity of the main acupoints in the body based on the meridian doctrine,which has been instructing clinical application for about 2000 years.Laws on the domino effect of acupoints have mainly focused on conclusions to clinical experiences.Indications of some acupoints exceed the practical paradigm since the excessive extension occurred during theory derivation.The current research direction on acupuncture focuses on three aspects:the effectiveness of acupuncture and moxibustion;the relevances and associations between meridians and viscera;and the physical and chemical properties and relevant physical basis of acupoints.The relevance between meridians and viscera is the central theory in the meridian doctrine,and acupoints are regarded as an important link in the relationship between meridians and viscera.Specific relationships between acupoints and target organs exist.Stimulating different acupoints on the body surface can help deal with different diseases,especially visceral diseases.In addition,acupoints have a dual function of reflecting and treating visceral diseases.There is no systemic research available on acupoint specificity,despite current knowledge and clinical experiences,which results in a weak foundation for acupuncture theory.This study focuses on the relevance and associations between meridians and viscera.A summary of the mechanisms of acupuncture regulating visceral sensation and mobility and the specific relationships between acupoints and their target organs are presented in this review.Peijing Rong Bing Zhu Yuqing Li Xinyan Gao Hui Ben Yanhua Li Liang Li Wei He Rupeng Liu Lingling Yu 2011Frontiers of Medicine2011,5,2:8

6PK-tailored tertiary prophylaxis in patients with severe hemophilia A at beijing Children’s Hospital显示文摘Importance:Tertiary prophylaxis using a low-dose regimen is usually insufficient to prevent recurrent joint bleeding and deterioration in joint diseases in children with severe hemophilia A.Pharmacokinetic(PK)dosing is a useful approach to increase the precision and efficiency of prophylaxis.Objective:To explore the efficacy of PK-tailored tertiary prophylaxis in children with severe hemophilia A.Methods:We implemented a PK-tailored tertiary prophylaxis program for 15 boys with severe hemophilia A aged 5-16 years at Beijing Children’s Hospital.Following PK testing and a 6-month evaluation period(phase I),15 patients were divided in two groups according to individual PK data and actual bleeding:(1)a PK-tailored group[modified prophylaxis regimen according to PK data for the next 6 months(phase II);n=8]and(2)a maintenance group(continued the original regimen for the next 6 months;n=7).We compared the bleeding rate,infusion frequency,and factor VIII(FVIII)consumption between the two groups.results:In the PK-tailored group,the median annual joint bleeding rate was reduced from 7.8 in phase I to 1.4 in phase II,mean annual total factor consumption increased from 1619.0 IU/kg in phase I to 2401.9 IU/kg in phase II,and median infusion frequency for prophylaxis increased from 104 times/year in phase I to 156 times/year in phase II(P<0.05).Although the FVIII consumption increased,it remained at approximately half of the standard method.Interpretation:PK-tailored prophylaxis may represent a more efficient approach to individual prophylaxis in China,but further studies are required to verify this.Peijing Li Zhenping Chen Xiaoling Cheng Yan Wang Ningning Zhang Yingzi Zhen Xinyi Wu Runhui Wu 2019Pediatric Investigation2019,3,1:3
7Electroacupuncture of Neiguan(PC 6) inhibits enhanced voltage-gated sodium currents in ischemic ventricular myocytes显示文摘OBJECTIVE: To examine the effect of electroacupuncture(EA) at bilateral Neiguan(PC 6) on voltage-gated Na+currents(INa) and channels(Nav) in ischemic ventricular myocytes.METHODS: EA serum was prepared from six male adult Sprague-Dawley rats that had received EA at bilateral Neiguan(PC 6). Eighteen ventricular myocytes were prepared from six SD rats using an enzymolysis approach. Myocardial ischemia was mimicked by perfusion of ischemic solution. Whole-cell patch-clamping was used to record three currents evoked from isolated cells. The first current was the control, and recorded in absence of ischemic solution current. The second was the ischemic current,and recorded after perfusion of ischemic solution for 5 min, while the EA current was last, and recorded after perfusion of EA serum for 5 min. Navkinetic curves were fitted using related formulas.RESULTS: Compared with those in controls, in the presence of ischemic solution, peak amplitudes of INasignificantly increased from ﹣40 m V to +30 m V,and half-maximal inactivation potentials of Navincreased significantly, while half-maximal activation potentials, slope factors and the recovery time from inactivation to activation of Navwere unchanged. 9Review of controlled clinical trials on acupuncture versus sham acupuncture in Germany显示文摘OBJECTIVE: To examine German controlled clinical trials on the therapeutic effects of acupuncture vs sham acupuncture, and to find whether there are problems with the conclusion that sham acupuncture has no significant deviation from acupuncture. METHODS: We focused on literature from the last ten years (2002-2011) included in PubMed about controlled clinical trials on acupuncture vs sham acupuncture carried out in Germany. The methods applied in sham acupuncture are summarized, and the difference between the acupuncture and sham groups were analyzed. We measured effects based on the following criteria: acupuncture is effective and superior to sham, acupuncture is effective but similar to sham, both of them have uncertainty regarding treatment effect, or no significant effect. Finally, we reviewed the hypotheses of different scholars on sham acupuncture and analyzed their results.RESULTS: Four types of controlled clinical trials including sham acupuncture on non-Traditional Chinese Medicine acupoints, minimal acupuncture on non-acupoints, placebo needle and sham laser acupuncture had varying results in the 57 articles analyzed. Some showed that acupuncture had a better effect than sham, while some suggest acupuncture and sham had similar effects. In all studies using sham acupuncture on non-therapeutic points, sham electrodes, and sham electro-acupuncture, the therapeutic effect was better than sham. Of the trials, 37 demonstrated that acupuncture had a better effect than sham acupuncture. Only nine trials found no significant difference between acupuncture and sham. Two controlled trials for the same condition (neck pain) conducted by two different German research institutes used the same control method, but reached contradictory conclusions. CONCLUSION: We found problems in conclusions based on results of controlled clinical trials of sham acupuncture in Germany. Therefore, there is still not enough evidence to support the statements that 'acupuncture and sham acupuncture have no difference in treatment effect' and 'acupuncture is just a placebo effect.' The control methods of sham acupuncture used in Germany may not be standardized and may not be suitable for acupuncture clinical trial research. 13Auricular vagus nerve acupressure for patients with emotional distress under the COVID-19 pandemic:A smartphone-based,randomized controlled trial显示文摘Objective: To confirm whether self-administered AVNA treatment is effective in improving emotional distressunder the COVID-19 pandemic.Methods: A smartphone-based online, randomized, controlled trial was designed from 26 February 2020 to 28April 2020 in four study sites, including Wuhan, Beijing, Shenyang, and Guangzhou of China. Local residentswho had considerable emotional distress with a score of the Hospital Anxiety and Depression Scale (HADS) ≥9 were recruited. Participants were randomly assigned to three times of AVNA (n = 191) per day, in morning,around noon, and in evening or usual care (UC, n = 215) once daily for 14 days. The primary outcome was theresponse rate, which was the proportion of participants whose Hospital Anxiety and Depression Scale (HADS)score reduced from baseline by ≥ 50%. The assessment was conducted at baseline, 3 days, and 14 days.Results: The AVNA group had a markedly higher response rate than the UC group at 3 days (35.6% vs. 24.9%,P = 0.02) and at 14 days (70.7% vs. 60.6%, P = 0.02). The AVNA group showed significantly greater reductionin scores of HADS at the two measurement points and BAI at 3 days (P ≤ 0.03), with average respective effectsize of 0.217 and 0.195. Participants with AVNA spent less time falling asleep and rated their sleep qualitybeing remarkably higher than those with UC at endpoint.Conclusion: During COVID-19 pandemic period, treatment with self-administrated AVNA was more effectivethan UC in reducing emotional distress of isolated populations. 14Design and performance investigation of modified dual reflector parabolic trough collector with double planar mirrors显示文摘In a typical parabolic trough collector(PTC), sunlight is concentrated at the bottom of the absorber tube. This concentrated solar flux leads to uneven heat distribution, resulting in high local temperatures and significant thermal stress on the absorber tube.These limitations have restricted the application of PTCs in solar thermochemistry and other fields and have impacted their safe operation. In this study, a new PTC with dual planar mirrors(DPMS) is proposed to homogenize the circumferential solar flux distribution of the absorber tube. A design method and single-objective optimization of the new PTC with a DPMS are proposed,and an uncertainty analysis of the operational and structural parameters is performed. A coupled light-heat-structure numerical model was developed to study the heat transfer performance and structural mechanical properties. The thermodynamic properties of the PTC with DPMS under different boundary conditions were analyzed. The results show that the circumferential temperature difference of the new PTC is within 2.6 K, and the circumferential thermal deformation is within 0.9 mm under typical working conditions(the inlet velocity of the heat transfer fluid is 3 m/s, inlet temperature is 573.15 K, and the direct normal irradiance is 1000 W/m^(2)). Compared with conventional PTCs, the circumferential temperature difference is reduced by 74%–90%, and the maximum thermal deformation along the y-axis is reduced by more than 95% under all working conditions(1–5 m/s, 373.5–675.15 K, 200–1000 W/m^(2)). 16Performance Enhancement of Parabolic Trough Collector by Using Homogenizer and Spiral显示文摘In conventional parabolic trough collectors(PTCs),sunlight is concentrated at the bottom of the absorber tube,resulting in a significant circumferential temperature gradient across the absorber tube,heat loss and thermal deformation,which affects the safety and thermal performance of PTCs.In this study,a new receiver with homogenizer and spiral(RHS) is proposed,achieving the optical and thermal synergy to ameliorate the thermal deformation of the absorber tube and enhance thermal efficiency.A plane structure homogenizer is designed to improve uniformity of the concentrated solar flux of absorber tube through second reflection.In combination with the spiral,it improves the optical-thermal efficiency of the PTC by enhancing heat exchange between the fluid and the backlight side of the absorber tube.The performance of the collector is numerically studied by building a three-dimensional coupled light-thermal-structure model.The results show that the thermal deformation of the RHS is reduced by more than 96% and the optical-thermal efficiency is improved by 1.2%-0.63% compared with conventional receivers(CRs) under the same inlet temperature conditions.The proposed receiver is validated to be effective in reducing thermal deformation and improving optical-thermal efficiency.LI Peijing LIU Taixiu QIN Yuanlong ZHENG Zhimei ZHAO Kai LIU Qibin 2024Journal of Thermal Science2024,33,2:0

18PI3K/AKT/mTOR pathway,hypoxia,and glucose metabolism:Potential targets to overcome radioresistance in small cell lung cancer显示文摘Small cell lung cancer(SCLC)is a highly aggressive tumor type for which limited therapeutic progress has been made.Platinum-based chemotherapy with or without thoracic radiotherapy remains the backbone of treatment,but most patients with SCLC acquire therapeutic resistance.Given the need for more effective therapies,better elucidation of the molecular pathogenesis of SCLC is imperative.The phosphoinositide 3-kinase(PI3K)/protein kinase B(AKT)/mammalian target of rapamycin(mTOR)pathway is frequently activated in SCLC and strongly associated with resistance to ionizing radiation in many solid tumors.This pathway is an important regulator of cancer cell glucose metabolism,and its activation probably effects radioresistance by influencing bioenergetic processes in SCLC.Glucose metabolism has three main branches-aerobic glycolysis,oxidative phosphorylation,and the pentose phosphate pathway-involved in radioresistance.The interaction between the PI3K/AKT/mTOR pathway and glucose metabolism is largely mediated by hypoxia-inducible factor 1(HIF-1)signaling.The PI3K/AKT/mTOR pathway also influences glucose metabolism through other mechanisms to participate in radioresistance,including inhibiting the ubiquitination of rate-limiting enzymes of the pentose phosphate pathway.This review summarizes our understanding of links among the PI3K/AKT/mTOR pathway,hypoxia,and glucose metabolism in SCLC radioresistance and highlights promising research directions to promote cancer cell death and improve the clinical outcome of patients with this devastating disease.Huan Deng Yamei Chen Peijing Li Qingqing Hang Peng Zhang Ying Jin Ming Chen 2023Cancer Pathogenesis and Therapy2023,1,1:0
19Optimization of horizontal well staggered patterns显示文摘Staggered line-drive patterns are widely used in oilfields.In this paper,to optimize a staggered pattern of horizontal wells,a 3D problem was divided into two 2D(x-y plane and y-z plane)problems with the pseudo-3D method,conformal transformation and superposition principle.A productivity equation for a horizontal well was deduced,which can be used to optimize the well pattern.A relationship between the length of horizontal wells and the shape factor of well patterns was established.The result shows that optimized well patterns can improve oil production from horizontal wells.This provides a theoretical basis for horizontal well applications to the development of oilfields,especially for overall development of oilfields by horizontal wells.Zhao Chunsen Li Peijing Guan Dan Liu Qingjuan 2008Petroleum Science2008,5,4:0
