| 1 | Expenditure of hospital care on cancer in China, from 2011 to 2015显示文摘Objective: A solid understanding of levels and trends of spending on cancer is important to evaluate whether our healthcare resources were wisely spent and to prioritize future resources for cancer treatment and prevention.However, studies on economic burden of cancers in China are scant and the results are inconsistent.Methods: The Chinese hospital information database and nearly 350 million inpatient medical record data were used. As the ratios of cancer inpatient payments to total inpatient payments were mainly influenced by the grades and sites of hospitals, the estimates of payments of cancer inpatients in this study were stratified by hospital grades and provinces. Only the payments of cancer inpatients happened in grade 2, grade 3 and specialized cancer hospitals were included in the analyses. The total medical payments of cancers in China were estimated based on the ratios of outpatient payments to inpatient payments in specialized cancer hospitals.Results: From 2011 to 2015, the payments of cancer inpatients in China have increased by 84.1% and the total inpatient payments reached 177.1 billion RMB in 2015, accounting for 4.3% of the total health expenditure in China. Based on the ratio of outpatient payments to inpatient payments, the total payments on cancer treatments in China were estimated to be 221.4 billion RMB in 2015. Among different cancer types, the highest payments were the treatment of trachea, bronchus and lung cancer. The major cancer inpatient payments(67.1% in 2015) spent in grade 3 general hospitals and this ratio increased continually from 2011 to 2015. | 4 | In-Season Root-Zone Nitrogen Management Strategies for Improving Nitrogen Use Efficiency in High-Yielding Maize Production in China显示文摘Many recently developed N management strategies have been extremely successful in improving N use efficiency.How-ever,attempts to further increase grain yields have had limited success.Field experiments were conducted in 2007 and 2008 at four sites to evaluate the effect of an in-season root-zone N management strategy on maize (Zea mays L.).According to the in-season root-zone N management,the optimal N rate (ONR) was determined by subtracting measured soil mineral N (NH + 4--N and NO 3--N) in the root zone from N target values.Other treatments included a control without N fertilization,70% of ONR,130% of ONR,and recommended N rate (RNR) by agronomists in China that have been shown to approach maize yield potentials.Although apparent N recovery for the ONR treatment was significantly higher than that under RNR in 2007,grain yield declined from 13.3 to 11.0 Mg ha 1 because of an underestimation of N uptake.In 2008,N target values were adjusted to match crop uptake,and N fertilization rates were reduced from 450 kg N ha-1 for RNR to 225 to 265 kg N ha-1 for ONR.High maize yields were maintained at 12.6 to 13.5 Mg ha 1,which were twice the yield from typical farmers' practice.As a result,apparent N recovery increased from 29% to 66%,and estimated N losses decreased significantly for the ONR treatment compared to the RNR treatment.In conclusion,the in-season root-zone N management approach was able to achieve high yields,high NUE and low N losses. | MENG Qing-Feng CHEN Xin-Ping ZHANG Fu-Suo CAO Ming-Hui CUI Zhen-Ling BAI Jin-Shun YUE Shan-Chao CHEN Su-Yi T.MULLER | 2012 | Pedosphere2012,22,3: | 22 |

| 7 | Incidence of infectious complications is associated with a high mortality in patients with hepatitis B virus-related acute-on-chronic liver failure显示文摘BACKGROUND In China,hepatitis B virus-related acute-on-chronic liver failure (HBV-ACLF) is the most common liver failure characterized by serious clinical syndromes of liver decompensation with a very high mortality.Bacterial and/or fungal infections are the most common complications that are associated with high short-term mortality.Bacterial translocation from the intestine,impaired hepatic clearance,and immune paralysis of circulating immune cells are thought to contribute to infectious complications in liver failure.The control of bacterial and fungal infections is the key to improving HBV-ACLF outcomes.Active prevention,early diagnosis,and timely treatment of bacterial and fungal infections are essential for treating HBV-ACLF.AIM To investigate the frequency and role of bacterial and fungal infections in patients with HBV-ACLF.METHODS Patients with HBV-ACLF hospitalized at Taihe Hospital,Hubei University of Medicine from January 2014 to December 2017 were retrospectively enrolled.Patient-related information was retrieved from the hospital case database,including general information,blood biochemistry,complications,etc.According to the occurrence of secondary infection or not,the patients were divided into an infection group and a non-infection group.The sites,types,and incidences of bacterial and fungal infections and the influence of infections on the prognosis of HBV-ACLF were statistically analyzed.The risk factors for infections were assessed by unconditional logistic regression.RESULTS There were 174 cases of HBV-ACLF that met the enrollment criteria,of which 114 (65.52%) were diagnosed with infectious complications.Infections occurred in the abdominal cavity (87 cases),respiratory tract (51 cases),urinary tract (18 cases),and biliary tract (10 cases).Patients with infectious complications had a significantly higher 28-d mortality (70.18%,80/114) than those without (40.00%,24/60)(70.18% vs 40.00%,P < 0.05).And patients with infectious complications had a much higher incidence of non-infectious complications (54.39%,62/114)(54.39% vs 15.00%,P < 0.05),leading to an extremely high 28-d mortality of 88.71%(55/62)(P < 0.05).The grade of liver failure,period of hospital stay ≥ 30 d,age ≥ 45 years,and percentage of neutrophils > 70% were identified as risk factors for infectious complications.CONCLUSION The high incidence of infectious complications in patients with HBV-ACLF is associated with severity and deterioration of the disease and may contribute to the extremely high mortality of these patients. | Chen Wang De-Qiang Ma Sen Luo Chuan-Min Wang De-Ping Ding You-You Tian Kang-Jian Ao Yin-Hua Zhang Yue Chen Zhong-Ji Meng | 2019 | World Journal of Clinical Cases2019,7,16: | 18 |

The fractional Fourier transform (FRFT) is a powerful tool for the analysis of nonstationary signals, especially, chirp-like signal. Thus, it has become an active area in the signal processing community, with many applications of radar, communication, electronic warfare, and information security. Therefore, it is necessary for us to generalize the theorem for Fourier domain analysis of decimation and interpolation. Firstly, this paper defines the digital fre- quency in the fractional Fourier domain (FRFD) through the sampling theorems with FRFT. Secondly, FRFD analysis of decimation and interpolation is proposed in this paper with digital frequency in FRFD followed by the studies of interpolation filter and decimation filter in FRFD. Using these results, FRFD analysis of the sam- pling rate conversion by a rational factor is illustrated. The noble identities of decimation and interpolation in FRFD are then deduced using previous results and the fractional convolution theorem. The proposed theorems in this study are the bases for the generalizations of the multirate signal processing in FRFD, which can advance the filter banks theorems in FRFD. | 12 | Sirtuin 3 deficiency exacerbates diabetic cardiomyopathy via necroptosis enhancement and NLRP3 activation显示文摘Sirtuin 3(SIRT3)is a potential therapeutic target for cardiovascular,metabolic,and other aging-related diseases.In this study,we investigated the role of SIRT3 in diabetic cardiomyopathy(DCM).Mice were injected with streptozotocin(STZ,60 mg/kg,ip)to induce diabetes mellitus.Our proteomics analysis revealed that SIRT3 expression in the myocardium of diabetic mice was lower than that of control mice,as subsequently confirmed by real-time PCR and Western blotting.To explore the role of SIRT3 in DCM,SIRT3-knockout mice and 129S1/SvImJ wild-type mice were injected with STZ.We found that diabetic mice with SIRT3 deficiency exhibited aggravated cardiac dysfunction,increased lactate dehydrogenase(LDH)level in the serum,decreased adenosine triphosphate(ATP)level in the myocardium,exacerbated myocardial injury,and promoted myocardial reactive oxygen species(ROS)accumulation.Neonatal rat cardiomyocytes were transfected with SIRT3 siRNA,then exposed to high glucose(HG,25.5 mM).We found that downregulation of SIRT3 further increased LDH release,decreased ATP level,suppressed the mitochondrial membrane potential,and elevated oxidative stress in HG-treated cardiomyocytes.SIRT3 deficiency further raised expression of necroptosis-related proteins including receptor-interacting protein kinase 1(RIPK1),RIPK3,and cleaved caspase 3,and upregulated the expression of inflammation-related proteins including NLR family pyrin domain-containing protein 3(NLRP3),caspase 1 p20,and interleukin-1βboth in vitro and in vivo.Collectively,SIRT3 deficiency aggravated hyperglycemia-induced mitochondrial damage,increased ROS accumulation,promoted necroptosis,possibly activated the NLRP3 inflammasome,and ultimately exacerbated DCM in the mice.These results suggest that SIRT3 can be a molecular intervention target for the prevention and treatment of DCM. | Shu Song Yue Ding Guo-liang Dai Yue Zhang Meng-ting Xu Jie-ru Shen Ting-ting Chen Yun Chen Guo-liang Meng | 2021 | Acta Pharmacologica Sinica2021,42,2: | 14 |
| 13 | When water meets iron at Earth's core-mantle boundary显示文摘Hydrous minerals in subducted crust can transport large amounts of water into Earth's deep mantle. Our laboratory experiments revealed the surprising pressure-induced chemistry that, when water meets iron at the core-mantle boundary, they react to form an interlayer with an extremely oxygen-rich form of iron, iron dioxide, together with iron hydride. Hydrogen in the layer will escape upon further heating and rise to the crust, sustaining the water cycle. With water supplied by the subducting slabs meeting the nearly inexhaustible iron source in the core, an oxygen-rich layer would cumulate and thicken, leading to major global consequences in our planet. The seismic signature of the D″ layer may echo the chemical complexity of this layer. Over the course of geological time, the enormous oxygen reservoir accumulating between the mantle and core may have eventually reached a critical eruption point. | 16 | Insight-HXMT observations of the first binary neutron star merger GW170817显示文摘Finding the electromagnetic(EM) counterpart of binary compact star merger, especially the binary neutron star(BNS) merger,is critically important for gravitational wave(GW) astronomy, cosmology and fundamental physics. On Aug. 17, 2017,Advanced LIGO and Fermi/GBM independently triggered the first BNS merger, GW170817, and its high energy EM counterpart,GRB 170817 A, respectively, resulting in a global observation campaign covering gamma-ray, X-ray, UV, optical, IR, radio as well as neutrinos. The High Energy X-ray telescope(HE) onboard Insight-HXMT(Hard X-ray Modulation Telescope) is the unique high-energy gamma-ray telescope that monitored the entire GW localization area and especially the optical counterpart(SSS17 a/AT2017 gfo) with very large collection area(~1000 cm^2) and microsecond time resolution in 0.2-5 MeV. In addition,Insight-HXMT quickly implemented a Target of Opportunity(ToO) observation to scan the GW localization area for potential X-ray emission from the GW source. Although Insight-HXMT did not detect any significant high energy(0.2-5 MeV) radiation from GW170817, its observation helped to confirm the unexpected weak and soft nature of GRB 170817 A. Meanwhile,Insight-HXMT/HE provides one of the most stringent constraints(~10^(-7) to 10^(-6) erg/cm^2/s) for both GRB170817 A and any other possible precursor or extended emissions in 0.2-5 MeV, which help us to better understand the properties of EM radiation from this BNS merger. In this study,motivated by the potential advantages of hADSCs-Exos and Mg-GA MOF, we designed and synthesized anexosome-functionalized cell-free PLGA/Mg-GA MOF (PLGA/Exo-Mg-GA MOF) scaffold, taking using of thebenefits of hADSCs-Exos, Mg2+, and gallic acid (GA) to construct unique nanostructural interfaces to enhanceosteogenic, angiogenic and anti-inflammatory capabilities simultaneously. Our in vitro work demonstrated thebeneficial effects of PLGA/Exo-Mg-GA MOF composite scaffolds on the osteogenic effects in human bonemarrow-derived mesenchymal stem cells (hBMSCs) and angiogenic effects in human umbilical endothelial cells(HUVECs). Slowly released hADSCs-Exos from composite scaffolds were phagocytosed by co-cultured cells,stabilized the bone graft environment, ensured blood supply, promoted osteogenic differentiation, and acceleratedbone reconstruction. Furthermore, our in vivo experiments with rat calvarial defect model showed thatPLGA/Exo-Mg-GA MOF scaffolds promoted new bone formation and satisfactory osseointegration. | 18 | Assimilating Doppler radar observations with an ensemble Kalman filter for convection-permitting prediction of convective development in a heavy rainfall event during the pre-summer rainy season of South China显示文摘This study examines the effectiveness of an ensemble Kalman filter based on the weather research and forecasting model to assimilate Doppler-radar radial-velocity observations for convection-permitting prediction of convection evolution in a high-impact heavy-rainfall event over coastal areas of South China during the pre-summer rainy season. An ensemble of 40 deterministic forecast experiments(40 DADF) with data assimilation(DA) is conducted, in which the DA starts at the same time but lasts for different time spans(up to 2 h) and with different time intervals of 6, 12, 24, and 30 min. The reference experiment is conducted without DA(NODA).To show more clearly the impact of radar DA on mesoscale convective system(MCS)forecasts, two sets of 60-member ensemble experiments(NODA EF and exp37 EF) are performed using the same 60-member perturbed-ensemble initial fields but with the radar DA being conducted every 6 min in the exp37 EF experiments from 0200 to0400 BST. It is found that the DA experiments generally improve the convection prediction. The 40 DADF experiments can forecast a heavy-rain-producing MCS over land and an MCS over the ocean with high probability, despite slight displacement errors. The exp37 EF improves the probability forecast of inland and offshore MCSs more than does NODA EF. Compared with the experiments using the longer DA time intervals, assimilating the radial-velocity observations at 6-min intervals tends to produce better forecasts. The experiment with the longest DA time span and shortest time interval shows the best performance.However, a shorter DA time interval(e.g., 12 min) or a longer DA time span does not always help. The experiment with the shortest DA time interval and maximum DA window shows the best performance, as it corrects errors in the simulated convection evolution over both the inland and offshore areas. | 19 | Hyperbaric oxygen improves functional recovery of rats after spinal cord injury via activating stromal cell-derived factor-1/CXC chemokine receptor 4 axis and promoting brain-derived neurothrophic factor expression显示文摘Background: Spinal cord injury (SCI) is a worldwide medical concern. This study aimed to elucidate the mechanism underlying protective effect of hyperbaric oxygen (HBO) against SCI-induced neurologic defects in rats via exploring the stromal cell-derived factor-1 (SDF-1)/CXC chemokine receptor 4 (CXCR4) axis and expression of brain-derived neurotrophic factor (BDNF). Methods: An acute SCI rat model was established in Sprague-Dawley rats using the Allen method. Sixty rats were divided into four groups (w = 15 in each group): sham-operated, SCI, SCI treated with HBO (SCI + HBO), and SCI treated with both HBO and AMD3100 (an antagonist of CXCR4;SCI + HBO + AMD) groups. The rats were treated with HBO twice a day for 3 days and thereafter once a day after the surgery for up to 28 days. Following the surgery, neurologic assessments were performed with the Basso-Bettie-Bresnahan (BBB) scoring system on postoperative day (POD) 7, 14, 21, and 28. Spinal cord tissues were harvested to assess the expression of SDF-1, CXCR4, and BDNF at mRNA and protein levels, using quantitative real-time polymerase chain reaction, Western blot analysis, and histopathologic analysis. Results: HBO treatment recovered SCI-induced descent of BBB scores on POD 14,(1.25±0.75 vs. 1.03 ±0.66, P< 0.05), 21 (5.27± 0.89 vs. 2.56± 1.24, P< 0.05), and 28 (11.35±0.56 vs. 4.23± 1.20, P<0.05) compared with the SCI group. Significant differences were found in the mRNA levels of SDF-1 (mRNA: day 21, SCI + HBO vs. SCI + HBO + AMD, 2.89± 1.60 vs. 1.56±0.98, P<0.05), CXCR4 (mRNA: day 7, SCI + HBO vs. SCI, 2.99± 1.60 vs. 1.31 ±0.98, P<0.05;day 14, SCI + HBO vs. SCI + HBO + AMD, 4.18± 1.60 vs. 0.80±0.34, P<0.05;day 21, SCI + HBO vs. SCI, 2.10±1.01 vs.1.15±0.03, P<0.05), and BDNF (mRNA: day 7, SCI + HBO vs. SCI, 3.04±0.41 vs. 2.75±0.31, P<0.05;day 14, SCI + HBO vs. SCI, 3.88± 1.59 vs. 1.11 ±0.40, P<0.05), indicating the involvement of SDF-1/CXCR4 axis in the protective effect of HBO. | 20 | Concurrent intracranial and extracranial artery stenosis and the prognosis of transient ischaemic symptoms or imaging-negative ischaemic stroke显示文摘Background and purpose Transient ischaemic attack(TIA),transient symptoms with infarction(TSI)and diffusion-weighted imaging(DWI)-negative acute ischaemic stroke(AIS)share similar aetiologies but are considered to have a rather benign prognosis.We intended to investigate the association between intracranial atherosclerotic stenosis(ICAS),extracranial atherosclerotic stenosis(ECAS)and the prognosis of patients with TIA,TSI and DWI-negative AIS.Methods Clinical and imaging data of eligible participants were derived from the Chinese Intracranial Atherosclerosis study,according to symptom duration,acute infarction on DWI and discharge diagnosis.Based on the severity and location of arterial atherosclerosis,we categorised the study population into four groups:no or<50% ICAS and no ECAS;≥50% ICAS but no ECAS;no or<50%ICAS with ECAS;and concurrent≥50% ICAS and ECAS.Using multivariable Cox regression models,we analysed the relationship between the severity and distribution of large artery atherosclerosis and the prognosis of TIA,TSI and DWI-negative AIS.Results A total of 806 patients were included,67.3% of whom were male.The median age of the study participants was 63 years.Patients in the concurrent≥50% ICAS and ECAS subgroup had both a significantly higher 1-year recurrence rate(adjusted HR 3.4(95%CI 1.15 to 10.04),p=0.027)and a higher risk of composite vascular events(adjusted HR 3.82(95%CI 1.50 to 9.72),p=0.005).Conclusions Concurrent ICAS and ECAS is associated with a higher possibility of 1-year recurrent stroke or composite vascular events.Large artery evaluation is necessary to assess patients with transient ischaemic symptoms or DWI-negative AIS.Progress in shortening the time interval between symptom onset and large vessel evaluation is needed. | Yue Suo Jing Jing Yuesong Pan Weiqi Chen Hongyu Zhou Hao Li Yuehua Pu Liping Liu Xingquan Zhao Yilong Wang Xia Meng Yongjun Wang | 2021 | Stroke & Vascular Neurology2021,6,1: | 10 |
