1Prognostic nutritional index predicts postoperative complications and long-term outcomes of gastric cancer显示文摘AIM:To investigate the impact of prognostic nutritional index(PNI)on the postoperative complications and long-term outcomes in gastric cancer patients undergoing total gastrectomy.METHODS:The data for 386 patients with gastric cancer were extracted and analyzed between January2003 and December 2008 in our center.The patients were divided into two groups according to the cutoff value of the PNI:those with a PNI≥46 and those with a PNI<46.Clinicopathological features were compared between the two groups and potential prognostic factors were analyzed.The relationship between postoperative complications and PNI was analyzed by logistic regression.The univariate and multivariate hazard ratios were calculated using the Cox proportional hazard model.RESULTS:The optimal cutoff value of the PNI was set at 46,and patients with a PNI≥46 and those with a PNI<46 were classified into PNI-high and PNI-low groups,respectively.Patients in the PNI-low group were more likely to have advanced tumor(T),node(N),and TNM stages than patients in the PNI-high group.The low PNI is an independent risk factor for the incidence of postoperative complications(OR=2.223).The 5-year overall survival(OS)rates were 54.1%and21.1%for patients with a PNI≥46 and those with a PNI<46,respectively.The OS rates were significantly lower in the PNI-low group than in the PNI-high group among patients with stagesⅡ(P=0.001)andⅢ(P<0.001)disease.CONCLUSION:The PNI is a simple and useful marker not only to identify patients at increased risk for postoperative complications,but also to predict long-term survival after total gastrectomy.The PNI should be included in the routine assessment of advanced gastric cancer patients.Nan Jiang Jing-Yu Deng Xue-Wei Ding Bin Ke Ning Liu Ru-Peng Zhang Han Liang 2014World Journal of Gastroenterology2014,20,30:41
2Strategic Interaction in Spending on Environmental Protection:Spatial Evidence from Chinese Cities显示文摘In China,the responsibility of protecting the environment lies largely with local governments. Within the framework of spatial econometrics,we investigate empirically the consequence of such an institutional setting.Using city-level data for China,the present study finds that city governments behave strategically in making spending decisions regarding environmental protection.This paper finds that a city government appears to cut its own spending as a response to the rise in environmental protection spending by its neighbors.Hence, environmental protection tends to be underprovided.As a result,we suggest that centralizing the environmental protection responsibility to a higher level of government would be beneficial in terms of controlling pollution in China.Huihui Deng Xinye Zheng Nan Huang Fanghua Li 2012China & World Economy2012,20,5:33
3World-class Xincheng gold deposit: An example from the giant Jiaodong gold province显示文摘The Jiaodong gold deposits are currently the most important gold resources(with Au reserves of>4000 t) in China,and the leading gold-producing country globally(with Au production of ~428 t in2013).Jiaodong is also considered as perhaps the only world-class to giant gold accumulation on the planet where relatively young gold ores(ca.130-120 Ma) were deposited in rocks that are 2 Ga older.The Xincheng world-class high-grade gold deposit,with a proven reserve of >200 t gold,is one of the largest deposits in the giant gold province of the Jiaodong Peninsula.It is located in the northwestern part of the jiaobei Uplift,and hosted by ca.132-123 Ma Xincheng quartz monzonites and monzogranites.Ore zones are structurally controlled by the NE-trending and NW-dipping Jiaojia Fault and subsidiary faults,and are mainly restricted to the footwall of the fault.The dominant disseminated- and stockworkstyle ores are associated with strong sericitization,silicification,sulfidation and K-feldspathization,and minor carbonate wallrock alteration halos.The four mineralization stages are pyrite-quartz-sericite(stage 1),quartz-pyrite(stage 2),quartz-polysulfide(stage 3) and quartz-carbonate(stage 4).Gold occurs dominantly as electrum,with lesser amounts of sulfide-hosted native gold and rare native silver and argentite,normally associated with pyrite,chalcopyrite,galena and sphalerite:the latter with proven resources of about 105 t Ag,713 t Cu,and 5100 t S.There are three types of ore-related fluid inclusions:type 1 aqueous-carbonate(H_2O-CO_2),type 2aqueous(liquid H_2O+vapor H_2O),and type 3 CO_2(liquid CO_2 and vapor CO_2) inclusions.Homogenization temperatures range from 221 to 304℃ for type 1 inclusions,with salinities of 2.4-13.3 wt.%NaCl eq.,and bulk densities of 0.858-1.022 g/cm^3.The δ^(34)S_(CDT) values of hydrothermal sulfides are 4.3-10.6‰and δ^(18)O values of hydrothermal quartz have a median value of 13.0‰.δD values of fluid inclusions in hydrothermal quartz have a median value of-75‰.Calculated δ^(18)Owater has a median value of 5.2‰.The timing of gold mineralization at the Xincheng gold deposit is younger than 123±1 Ma,and likely between 120.9 and 119.9 Ma.A minerals system genetic model for the probable epizonal orogenic Xincheng deposit suggests an initial medium temperature,CO_2-rich,and low salinity H_2O-CO_2 deeply sourced metamorphic ore fluid associated with dehydration and decarbonization of subducting Paleo-Pacific lithosphere.The Jiaojia Fault constrained the migration of ore-forming fluids and metals at the brittle-ductile transition.Fluid immiscibility,caused by episodic pressure drops,led to significant high-grade gold deposition in the giant Xincheng gold deposit.Liqiang Yang Jun Deng Ruipeng Guo Lin'nan Guo Zhongliang Wang Binghan Chen XudongWang 2016Geoscience Frontiers2016,7,3:14

5Direct technetium-99m labeling of anti-hepatoma monoclonal antibody fragment:a radioimmunoconjugate for hepatocellular carcinoma imaging显示文摘AIM To directly radiolabel an anti-hepatomamAb fragment HAb18 F(ab’)2 with 99mTc bystannous-reduced method,and assess thestability,biodistribution and radioimmun-oimaging(RⅡ).METHODS Immunoreactive fraction wasdetermined according to Lindmo’s method.Ellman’s reagent was used to determine thenumber of thiols in the reduced F(ab’)2.Labelingefficiency and homogeneity were measured bypaper chromatography,sodium dodecylsulphatepolyacrylamide gel electrophoresis(SDS-PAGE)and autoradiography.Challenge assay involvedthe incubation of aliquots of labeled antibody inethylenediaminetetraacetate( EDTA )and L-cysteine(L-cys)solutions with different molarratio at 37℃ for 1h,respectively.Investigationsin vivo utilized nude mice bearing humanhepatocellular carcinoma(HHCC)xenograftswith gamma camera imaging and tissuebiodistribution studies at regular intervals.RESULTS The labeling procedure was finishedwithin 1.5 h compared with the'pretinning'method which would take at least 21h.In vitrostudies demonstrated that the radiolabeled mAbfragment was homogeneous and retained itsimmunoreactivity.Challenge studies indicatedthat 99mTc-labeled HAb18 F(ab’)2 in EDTA is morestable than in L-cys.Imaging and biodistribution showed a significant tumor uptake at 24 h post-injection of 99mTc-labeled HAb18 F(ab’)2.Theblood,kidney,liver and tumor uptakes at 24hwere 0.56±0.09,56.45±11.36,1.43±0.27 and6.57±3.01(%ID/g),respectively.CONCLUSION 99mTc-HAb18 F(ab’)2 conjugateprepared by this direct method appears to be aneffective way to detect hepatoma in nude micemodel.Hui Jie Bian Zhi Nan Chen Jing Lan Deng 2000World Journal of Gastroenterology2000,6,3:13
6Effect of complication grade on survival following curative gastrectomy for carcinoma显示文摘AIM: To elucidate the potential impact of the grade of complications on long-term survival of gastric cancer patients after curative surgery.METHODS: A total of 751 gastric cancer patients who underwent curative gastrectomy between January 2002 and December 2006 in our center were enrolled in this study. Patients were divided into four groups: no complications, Grade Ⅰ, Grade Ⅱ and Grade Ⅲ complications, according to the following classification systems: T92(Toronto 1992 or Clavien), Accordion Classification, and Revised Accordion Classification. Clinicopathological features were compared amongthe four groups and potential prognostic factors were analyzed. The Log-rank test was used to assess statistical differences between the groups. Independent prognostic factors were identified using the Cox proportional hazards regression model. Stratified analysis was used to investigate the impact of complications of each grade on survival.RESULTS: Significant differences were found among the four groups in age, sex, other diseases(including hypertension, diabetes and chronic obstructive pulmonary disease), body mass index(BMI), intraoperative blood loss, tumor location, extranodal metastasis, lymph node metastasis, tumor-node-metastasis(TNM) stage, and chemotherapy. Overall survival(OS) was significantly influenced by the complication grade. The 5-year OS rates were 43.0%, 42.5%, 25.5% and 9.6% for no complications, and GradeⅠ, Grade Ⅱ and Grade Ⅲ complications, respectively(P < 0.001). Age, tumor size, intraoperative blood loss, lymph node metastasis, TNM stage and complication grade were independent prognostic factors in multivariate analysis. With stratified analysis, lymph node metastasis, tumor size, and intraoperative blood loss were independent prognostic factors for Grade Ⅰ complications(P < 0.001, P = 0.031, P = 0.030). Age and lymph node metastasis were found to be independent prognostic factors for OS of gastric cancer patients with Grade Ⅱ complications(P = 0.034, P = 0.001). Intraoperative blood loss, TNM stage, and chemotherapy were independent prognostic factors for OS of gastric cancer patients with Grade Ⅲ complications(P = 0.003, P = 0.005, P < 0.001). There were significant differences among patients with GradeⅠ, Grade Ⅱ and Grade Ⅲ complications in TNM stage Ⅱ and Ⅲ cancer(P < 0.001, P = 0.001).CONCLUSION: Complication grade may be an independent prognostic factor for gastric cancer following curative resection. 7Anti-inflammatory and Analgesic Effects of Extract from Roots and Leaves of Citrullus lanatus显示文摘Objective To study anti-inflammatory and analgesic effects of extract from the roots and leaves of Citrullus lanatus and assess their acute toxicity in animals. Methods The mouse model with ear edema induced by xylene and the rat model with paw edema or granuloma by carrageenin or cotton pellet were used for anti-inflammatory effects of the extract. Effects of the extract on analgesia was tested respectively by measuring the latency of mice licking hind foot from hot plates and by counting the times of body twisting in response to acetic acid. The acute toxicity of the extract was determined with the method of Bliss. Results The extract significantly inhibited the ear edema, granuloma hyperplasia, and paw edema. It significantly lifted the pain threshold on mouse hot-plate responses and reduced their writhing times. During the 7 d observation period in its acute toxicity assay, no apparent toxic reaction was shown and all mice survived at a dose of 87 g extract per kg body weight. 9Monoclonal Antibody-Based Enzyme Linked Immunosorbent Assay for the Analysis of Jasmonates in Plants显示文摘Methyl jasmonate (MeJA) and its free-acid form, jasmonic acid (JA) are naturally occurring plant growth regulators widely distributed in higher plants. In order to improve the sensitivity for the analysis of MeJA at low levels in small amounts of plant samples, a monoclonal antibody (MAb) (designated as MAb 3E5D7C4B6) against MeJA was derived from a JA-bovine serum albumin (BSA) conjugate as an immunogen. The antibody belongs to the IgG1 subclass with a κ type light chain and has a dissociation constant of approximately 6.07 × 10?9 M. MAb3E5D7C4B6 is very specific to MeJA. It was used to develop a direct competitive enzyme-linked immunosorbent assay (dcELISA), conventional and simplified indirect competitive ELISAs (icELISA). JA was derivatized into MeJA for the ELISA analysis. The IC50 value and detection range for MeJA were, respectively, 34 and 4-257 ng/mL by the conventional icELISA, 21 and 3-226 ng/mL by the simplified icELISA and 5.0 and 0.7-97.0 ng/mL by the dcELISA. The dcELISA was more sensitive than either the conventional or simplified icELISA. The assays were used to measure the content of jasmonates as MeJA in tobacco leaves under drought stress or inoculated with tobacco mosaic virus and tomato leaves inoculated with tomato mosaic virus or Lirioinyza sativae Blanchard as compared with the corresponding healthy leaves. 11The Use of Lipoprotein-Associated Phospholipase A2 in a Chinese Population to Predict Cardiovascular Events显示文摘Objective To explore associations between lipoprotein-associated phospholipase A2(Lp-PLA2)and the risk of cardiovascular events in a Chinese population,with a long-term follow-up.Methods A random sample of 2,031 participants(73.6%males,mean age=60.4 years)was derived from the Asymptomatic Polyvascular Abnormalities Community study(APAC)from 2010 to 2011.Serum Lp-PLA2 levels were determined by enzyme-linked immunosorbent assay(ELISA).The composite endpoint was a combination of first-ever stroke,myocardial infarction(MI)or all-cause death.Lp-PLA2 associations with outcomes were assessed using Cox models.Results The median Lp-PLA2 level was 141.0 ng/m L.Over a median follow-up of 9.1 years,we identified 389 events(19.2%),including 137 stroke incidents,43 MIs,and 244 all-cause deaths.Using multivariate Cox regression,when compared with the lowest Lp-PLA2 quartile,the hazard ratios with95%confidence intervals for developing composite endpoints,stroke,major adverse cardiovascular events,and all-cause death were 1.77(1.24–2.54),1.92(1.03–3.60),1.69(1.003–2.84),and 1.94(1.18–3.18)in the highest quartile,respectively.Composite endpoints in 145(28.6%)patients occurred in the highest quartile where Lp-PLA2(159.0 ng/m L)was much lower than the American Association of Clinical Endocrinologists recommended cut-off point,200 ng/m L.Conclusion Higher Lp-PLA2 levels were associated with an increased risk of cardiovascular event/death in a middle-aged Chinese population.The Lp-PLA2 cut-off point may be lower in the Chinese population when predicting cardiovascular events.XI Hui CHENG Guan Liang HU Fei Fei LI Song Nan DENG Xuan ZHOU Yong 2022Biomedical and Environmental Sciences2022,35,3:8
12Evaluation of the effects of combined endoscopic variceal ligation and splenectomy with pericardial devascularization on esophageal varices显示文摘AIM: To detect the hemodynamic alterations in collateral circulation before and after combined endoscopic variceal ligation (EVL) and splenectomy with pericardial devascularization by ultrasonography, and to evaluate their effect using hemodynamic parameters. METHODS: Forty-three patients with esophageal varices received combined EVL and splenectomy with pericardial devascularization for variceal eradication. The esophageal vein structures and azygos blood flow (AZBF) were detected by endoscopic ultrasonography and color Doppler ultrasound. The recurrence and rebleeding of esophageal varices were followed up. RESULTS: Patients with moderate or severe varices in the esophageal wall and those with severe peri- esophageal collateral vein varices had improvements after treatment, while the percentage of patients with severe para-esophageal collateral vein varices decreased from 54.49% to 2.33%, and the percentage of patients with detectable perforating veins decreased from 79.07% to 4.65% (P < 0.01). Color Doppler flowmetry showed a significant decrease both in AZBF (43.00%, P < 0.05) and in diameter of the azygos vein (28.85%, P < 0.05), while the blood flow rate was unchanged. The recurrence rate of esophageal varices was 2.5% (1/40, mild), while no re-bleeding cases were recorded. 13The Association of Socioeconomic Status with the Burden of Cataract-related Blindness and the Effect of Ultraviolet Radiation Exposure: An Ecological Study显示文摘Objective To assess the association of socioeconomic status with the burden of cataract blindness in terms of year lived with disability(YLD) rates and to determine whether ultraviolet radiation(UVR) levels modify the effect of socioeconomic status on this health burden.Methods National and subnational age-standardized YLD rates associated with cataract-related blindness were derived from the Global Burden of Disease(GBD) study 2017. The human development index(HDI) from the Human Development Report was used as a measure of socioeconomic status.Estimated ground-level UVR exposure was obtained from the Ozone Monitoring Instrument(OMI)dataset of the National Aeronautics and Space Administration(NASA).Results Across 185 countries, socioeconomic status was inversely associated with the burden of cataract blindness. Countries with a very high HDI had an 84% lower age-standardized YLD rate [95%confidence interval(CI): 60%–93%, P < 0.001] than countries with a low HDI;for high-HDI countries, the proportion was 76%(95% CI: 53%–88%, P < 0.001), and for medium-HDI countries, the proportion was48%(95% CI: 15%–68%, P = 0.010;P for trend < 0.001). The interaction analysis showed that UVR exposure played an interactive role in the association between socioeconomic status and cataract blindness burden(P value for interaction = 0.047).Conclusion Long-term high-UVR exposure amplifies the association of poor socioeconomic status with the burden of cataract-related blindness. 14Chemotherapy response evaluation in a mouse model of gastric cancer using intravoxel incoherent motion diffusionweighted MRI and histopathology显示文摘AIM To determine the role of intravoxel incoherent motion(IVIM) diffusion-weighted(DW) magnetic resonance imaging(MRI) using a bi-exponential model in chemotherapy response evaluation in a gastric cancer mouse model.METHODS Mice bearing MKN-45 human gastric adenocarcinoma xenografts were divided into four treated groups(TG1, 2, 3 and 4, n = 5 in each group) which received Fluorouracil and Calcium Folinate and a control group(CG, n = 7). DW-MRI scans with 14 b-values(0-1500 s/mm2) were performed before and after treatment on days 3, 7, 14 and 21. Fast diffusion component(presumably pseudo-perfusion) parameters including the fast diffusion coefficient(D*) and fraction volume(f p), slow diffusion coefficient(D) and the conventional apparent diffusion coefficients(ADC) were calculated by fitting the IVIM model to the measured DW signals. The median changes from the baseline to each posttreatment time point for each measurement(ΔADC, ΔD* and Δf p) were calculated. The differences in the median changes between the two groups were compared using the mixed linear regression model by the restricted maximum likelihood method shown as z values. Histopathological analyses including Ki-67, CD31, TUNEL and H&E were conducted in conjunction with the MRI scans. The median percentage changes were compared with the histopathological analyses between the pre-and post-treatment for each measurement.RESULTS Compared with the control group, D* in the treated group decreased significantly(ΔD*treated% =-30%,-34% and-20%, with z =-5.40,-4.18 and-1.95. P = 0.0001, 0.0001 and 0.0244) and f p increased significantly(Δfptreated% = 93%, 113% and 181%, with z = 4.63, 5.52, and 2.12, P = 0.001, 0.0001 and 0.0336) on day 3, 7 and 14, respectively. Increases in ADC in the treated group were higher than those in the control group on days 3 and 14(z = 2.44 and 2.40, P = 0.0147 and P = 0.0164). 15IL-17A-dependent gut microbiota is essential for regulating diet-induced disorders in mice显示文摘The gut microbiota plays a key role in obesity and related metabolic disorders, and multiple factors including diet, host genotype, and age regulate it. Many studies have examined the contribution of extrinsic factors to the regulation of the gut microbiota, but the importance of the host genetic constitution cannot be ignored. Interleukin 17A(IL-17A), a pro-inflammatory cytokine, is important in the defense against infection and diseases. Here, we investigated the association among IL-17, a high-fat diet(HFD), and the gut microbiota. Mice deficient in IL-17A were resistant to diet-induced obesity and related diseases. Compared with the Ⅱ-17^(-/-)mice, wild-type(WT) mice challenged with HFD showed obvious weight fluctuations, such as those seen in type 2 diabetes, and hematological changes similar to those associated with metabolic syndrome. However, housing WT mice and Ⅱ-17a^(-/-)mice together significantly alleviated these symptoms in the WT mice. A metagenomic analysis of the mouse feces indicated that the microbial community compositions of these two groups differed before HFD feeding. The HFD mediated shifts in the gut microbial compositions, which were associated with the mouse phenotypes.We also identified potentially beneficial and harmful species present during this period, and drew networks of the most abundant species. A functional analysis indicated pathway changes in the WT and Ⅱ-17a^(-/-)mice when fed the HFD. 16Source Contributions to PM2.5 under Unfavorable Weather Conditions in Guangzhou City,China显示文摘Historical haze episodes(2013–16) in Guangzhou were examined and classified according to synoptic weather systems.Four types of weather systems were found to be unfavorable, among which 'foreside of a cold front'(FC) and 'sea high pressure'(SP) were the most frequent(> 75% of the total). Targeted case studies were conducted based on an FC-affected event and an SP-affected event with the aim of understanding the characteristics of the contributions of source regions to fine particulate matter(PM_(2.5)) in Guangzhou. Four kinds of contributions—namely, emissions outside Guangdong Province(super-region), emissions from the Pearl River Delta region(PRD region), emissions from Guangzhou–Foshan–Shenzhen(GFS region), and emissions from Guangzhou(local)—were investigated using the Weather Research and Forecasting–Community Multiscale Air Quality model. The results showed that the source region contribution differed with different weather systems. SP was a stagnant weather condition, and the source region contribution ratio showed that the local region was a major contributor(37%), while the PRD region, GFS region and the super-region only contributed 8%, 2.8% and 7%, respectively, to PM_(2.5) concentrations. By contrast, FC favored regional transport. The super-region became noticeable,contributing 34.8%, while the local region decreased to 12%. A simple method was proposed to quantify the relative impact of meteorology and emissions. Meteorology had a 35% impact, compared with an impact of-18% for emissions, when comparing the FC-affected event with that of the SP. 19Splenectomy with endoscopic variceal ligation is superior to splenectomy with pericardial devascularization in treatment of portal hypertension显示文摘AIM: To investigate the therapeutic efficacy and complications of splenectomy with endoscopic variceal ligation (EVL) and splenectomy with pericardial devascularization (i.e. Hassab’s operation) in patients with portal hypertension. METHODS: A total of 103 patients with liver cirrhosis and portal hypertension were randomly selected to receive either splenectomy with EVL (n = 53, group A) or Hassab’s operation (n = 50, group B). RESULTS: The portal blood flow volume, the presence of portal vein thrombosis, gastric emptying time and free portal venous pressure (FPP) before and after the operation were determined. Patients were followed up for up to 64 mo with an average of 45 mo, and the Dagradi classification of variceal veins and the grading of portal hypertension gastropathy (PHG) were evaluated. It was found that all esophageal varices were occluded or decreased to grade Ⅱ or less in both groups. There was little difference in the recurrence rate of esophageal varices (11.9% vs 13.2%) and the re-bleeding rate (7.1% vs 5.3%) between groups A and B. The incidence of complications and the percentage of patients with severe PHG after the operation were significantly higher in group B (60.0% and 52.0%) than in group A (32.1% and 20.8%, P < 0.05). No patients died of operation-related complications. There was no significant difference in gastric emptying time, FPP and portal blood flow volume between the two groups.
