2Multimodality treatment in hepatocellular carcinoma patients with tumor thrombi in portal vein显示文摘AIM To compare the therapeutic effect andsignificances of multimodality treatment forhepatocellular carcinoma (HCC) with tumorthrombi in portal vein (PVTT).METHODS HCC patients (n = 147) with tumortrombi in the main portal vein or the first branchof portal vein were divided into four groups bythe several therapeutic methods. There wereconservative treatment group in 18 out ofpatients (group A); and hepatic artery ligation(HAL) and/or hepatic artery infusion (HAl)group in 18 patients (group B), in whompostoberative chemoembolization was doneperiodically; group of removal of HCC with PVTTin 79 (group C) and group of transcatheterhepatic arterial chemoembolization (TACE) orHAl and/or portal vein infusion (PVI) afteroperation in 32 (group D).RESULTS The median survival period was 12months in our series and the 1-, 3-, and 5-yearsurvival rates were 44.3%, 24.5% and 15.2%,respectively. The median survival times were 2,5, 12 and 16 months in group A, B, C and D,respectively. The 1-, 3- and 5-year survival rateswere 5.6%, 0% and 0% in group A; 22.2%,5.6% and 0% in group B; 53.9%, 26.9% and16.6% in group C; 79.3%, 38.9% and 26.8% ingroup D, respectively. 3Mapping wetland changes in China between 1978 and 2008显示文摘Four wetland maps for all China have been produced,based on Landsat and CBERS-02B remote sensing data between 1978 and 2008 (1978,1990,2000 and 2008).These maps were mainly developed by manual interpretation and validated by substantial field investigation in 2009.Based on these maps,we analyzed the 2008 wetland distribution in China and discussed wetland changes and their drivers over the past 30 years.(i) There were about 324097 km 2 of wetlands in 2008,for which inland marshes or swamps were the most common wetland type (35%),with lakes (26%) second.Most of the wetlands were in Heilongjiang,Inner Mongolia,Qinghai and Tibet,occupying about 55% of the national wetland area.(ii) From 1978 to 2008,China's wetland area continually and significantly decreased,by about 33% based on changes in the wetland map.This was in sharp contrast to the increase in artificial wetlands,which increased by about 122%.Inland marshes accounted for the main loss of total wetlands from 1978 to 2000.From 2000 through 2008,riverine and lacustrine wetlands constituted the main wetland loss.Fortunately however,the rate of wetland loss decreased from 5523 to 831 km 2 /a.(iii) The change ratio of lost natural wetlands (including inland and coastal wetlands) to non-wetlands has decreased slightly over the past 30 years.From 1978 to 1990,nearly all natural wetlands (98%) lost were transformed into non-wetlands.However,the ratio declined to 86% from 1990 to 2000,and to 77% from 2000 to 2008.(iv) All Chinese provinces were divided into three groups according to patterns of wetland changes,which could relate to the driving forces of such changes.Tibet was completely different from other provinces,as it was one representative example in which there was a net wetland increase,because of global warming and decreased human activity since 1990.Increased economic development caused considerable wetland loss in most eastern provinces,and artificial wetlands increased.NIU ZhenGuo ZHANG HaiYing WANG XianWei YAO WenBo ZHOU DeMin ZHAO KuiYi ZHAO Hui LI NaNa HUANG HuaBing LI CongCong YANG Jun LIU CaiXia LIU Shuang WANG Lin LI Zhan YANG ZhenZhong QIAO Fei ZHENG YaoMin CHEN YanLei SHENG YongWei GAO XiaoHong ZHU WeiHong WANG WenQing WANG Hong WENG YongLing ZHUANG DaFang LIU JiYuan LUO ZhiCai CHENG Xiao GUO ZiQi GONG Peng 2012Chinese Science Bulletin2012,57,22:51
4Transplantation of UC-MSCs on collagen scaffold activates follicles in dormant ovaries of POF patients with long history of infertility显示文摘Premature ovarian failure(POF) is a refractory disease for clinical treatment with the goal of restoring fertility. In this study,umbilical cord mesenchymal stem cells on a collagen scaffold(collagen/UC-MSCs) can activate primordial follicles in vitro via phosphorylation of FOXO3 a and FOXO1. Transplantation of collagen/UC-MSCs to the ovaries of POF patients rescued overall ovarian function, evidenced by elevated estradiol concentrations, improved follicular development, and increased number of antral follicles. Successful clinical pregnancy was achieved in women with POF after transplantation of collagen/UC-MSCs or UC-MSCs. 5A life support-based comprehensive treatment regimen dramatically lowers the in-hospital mortality of patients with fulminant myocarditis: a multiple center study显示文摘Fulminant myocarditis(FM) has unacceptable high mortality. This study aimed to evaluate the therapeutic efficacy of a life support-based comprehensive treatment regimen(LSBCTR), a completely novel treatment regimen, for FM. A total of 169 FM patients recruited from January 2008 to December 2018 were divided into two groups: patients receiving LSBCTR(81 cases),which includes(i) mechanical life support(positive pressure respiration, intra-aortic balloon pump with or without extracorporeal membrane oxygenation),(ii) immunomodulation therapy using sufficient doses of glucocorticoids and immunoglobulins, and(iii) application of neuraminidase inhibitors, and those receiving conventional treatment(88 cases). The endpoints were in-hospital death and heart-transplantation. Of all the population, 44 patients(26.0%) died in hospitals. Inhospital mortality was 3.7%(3/81) for LSBCTR group and 46.6%(41/88) for traditional treatment(P<0.001). Early application of LSBCTR, mechanical life support, neuraminidase inhibitors, and immunomodulation therapy significantly contributed to reduction in in-hospital mortality. This study describes a novel treatment regimen for FM patients that dramatically reduces inhospital mortality. 6Indocyanine green clearance test and model for end-stage liver disease score of patients with liver cirrhosis显示文摘BACKGROUND:The indocyanine green(ICG)clearance test(clearance rate(K)and retention rate at 15 minutes (R15))is a sensitive indicator to evaluate liver function. The model for end-stage liver disease(MELD)score has emerged as a useful tool for estimating the mortality of patients awaiting liver transplantation and has recently been validated on patients with liver diseases of various etiologies and severity.In this study,we investigated the correlation between the ICG clearance test and MELD score of patients with liver cirrhosis. METHODS:From June 2007 to March 2008,52 patients with liver cirrhosis admitted to our center were classified into Child-Pugh class A(8 patients),B(14)and C(30).The ICG clearance test(K value and R15)was performed by ICG pulse spectrophotometry(DDG-3300K),and the MELD scores of patients were calculated. RESULTS:As the Child-Pugh classification of liver function gradually deteriorated,the K value decreased, while R15 and MELD score increased.There were significant statistical differences in K value,R15 and MELD score in patients with different Child-Pugh classifications. 12Anti-SARS-CoV-2 activities in vitro of Shuanghuanglian preparations and bioactive ingredients显示文摘Human infection with severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)causes coronavirus disease 2019(COVID-19)and there is no cure currently.The 3CL protease(3CLpro)is a highly conserved protease which is indispensable for CoVs replication,and is a promising target for development of broad-spectrum antiviral drugs.In this study we investigated the anti-SARS-CoV-2 potential of Shuanghuanglian preparation,a Chinese traditional patent medicine with a long history for treating respiratory tract infection in China.We showed that either the oral liquid of Shuanghuanglian,the lyophilized powder of Shuanghuanglian for injection or their bioactive components dose-dependently inhibited SARS-CoV-23CLpro as well as the replication of SARS-CoV-2 in Vero E6 cells.Baicalin and baicalein,two ingredients of Shuanghuanglian,were characterized as the first noncovalent,nonpeptidomimetic inhibitors of SARS-CoV-23CLpro and exhibited potent antiviral activities in a cell-based system.Remarkably,the binding mode of baicalein with SARS-CoV-23CLpro determined by X-ray protein crystallography was distinctly different from those of known 3CLpro inhibitors.Baicalein was productively ensconced in the core of the substrate-binding pocket by interacting with two catalytic residues,the crucial S1/S2 subsites and the oxyanion loop,acting as a“shield”in front of the catalytic dyad to effectively prevent substrate access to the catalytic dyad within the active site.Overall,this study provides an example for exploring the in vitro potency of Chinese traditional patent medicines and effectively identifying bioactive ingredients toward a specific target,and gains evidence supporting the in vivo studies of Shuanghuanglian oral liquid as well as two natural products for COVID-19 treatment.Hai-xia Su Sheng Yao Wen-feng Zhao Min-jun Li Jia Liu Wei-juan Shang Hang Xie Chang-qiang Ke Hang-chen Hu Mei-na Gao Kun-qian Yu Hong Liu Jing-shan Shen Wei Tang Lei-ke Zhang Geng-fu Xiao Li Ni Dao-wen Wang Jian-ping Zuo Hua-liang Jiang Fang Bai Yan Wu Yang Ye Ye-chun Xu 2020Acta Pharmacologica Sinica2020,41,9:32
13Small intestinal bacteria overgrowth decreases small intestinalmotility in the NASH rats显示文摘AIM: To explore the relationship between small intestinalmotility and small intestinal bacteria overgrowth(SIBO) in Nonalcoholic steatohepatitis (NASH), andto investigate the effect of SIBO on the pathogenesisof NASH in rats. The effect of cidomycin in alleviatingseverity of NASH is also studied. METHODS: Forty eight rats were randomly dividedinto NASH group (n = 16), cidomycin group (n = 16)and control group (n = 16). Then each group weresubdivided into small intestinal motility group (n = 8),bacteria group (n = 8) respectively. A semi-solid coloredmarker was used for monitoring small intestinal transit.The proximal small intestine was harvested under sterilecondition and processed for quantitation for aerobes(E. coli) and anaerobes (Lactobacilli). Liver pathologicscore was calculated to qualify the severity of hepatitis.Serum ALT, AST levels were detected to evaluate theseverity of hepatitis. RESULTS: Small intestinal transit was inhibited inNASH group (P < 0.01). Rats treated with cidomycinhad higher small intestine transit rate than rats in NASHgroup (P < 0.01). High fat diet resulted in quantitativealterations in the aerobes (E. coli ) but not in theanoerobics (Lactobacill). There was an increase in thenumber of E. coli in the proximal small intestinal florain NASH group than in control group (1.70 ± 0.12 log10(CFU/g) vs 1.28 ± 0.07 log10 (CFU/g), P < 0.01). TNF-αconcentration was significantly higher in NASH groupthan in control group (1.13 ± 0.15 mmol/L vs 0.57 ±0.09 mmol/L, P < 0.01). TNF-α concentration was lowerin cidomycin group than in NASH group (0.63 ± 0.09mmol/L vs 1.13 ± 0.15 mmol/L, P < 0.01). Treatmentwith cidomycin showed its effect by significantly loweringserum ALT, AST and TNF-α levels of NASH rats. CONCLUSION: SIBO may decrease small intestinalmovement in NASH rats. 14The Association of Maternal Body Composition and Dietary Intake with the Risk of Gestational Diabetes Mellitus during the Second Trimester in a Cohort of Chinese Pregnant Women显示文摘Objective To investigate the association of maternal body composition and dietary intake with the risk of gestational diabetes mellitus(GDM). Methods A total 154 GDM subjects and 981 controls were enrolled in a prospective cohort study in 11 hospitals from May 20, 2012 to December 31, 2013. Bioelectrical impedance analysis and dietary surveys were used to determine body composition and to evaluate the intake of nutrients in subjects at 21-24 weeks' gestation(WG). Logistic regression analysis was applied to explore the relationships of maternal body composition and dietary intake with the risk of GDM morbidity. Results Age, pre-pregnant body weight(BW), and body mass index(BMI) were associated with increased risk of GDM. Fat mass(FM), fat mass percentage(FMP), extracellular water(ECW), BMI, BW, energy, protein, fat, and carbohydrates at 21-24 WG were associated with an increased risk of GDM. In contrast, fat free mass(FFM), muscular mass(MM), and intracellular water(ICW) were associated with a decreased risk of GDM. 16Microvessel density is a prognostic marker of human gastric cancer显示文摘AIM: To investigate whether microvessel density (MVD) is related with prognosis in gastric cancer patients, and the expression of cyclooxygenase-2 (COX-2) and vessel endothelial growth factor (VEGF) so as to determine the possible role of COX-2 and VEGF in gastric cancer angiogenesis.METHODS: Forty-seven formalin-fixed paraffin-embedded tissue samples of gastric cancer were evaluated for COX-2, VEGF by immunohitochemical staining. To assess tumor angiogenesis, MVD was determined by immunohitochemical staining of endothelial protein factor Ⅷ-related antigen. The relationship among COX-2 and VEGF expression, MVD, and clinicopathologic parameters was analyzed. RESULTS: Among the 67 samples, high MVD was significantly associated with lymph node metastasis and poor survival. Multivariate survival analysis showed that MVD value and lymph node metastasis were independent prognostic factors. The expression rate of COX-2 and VEGF was significantly higher than that of the adjacent tissues. COX-2 and VEGF expression in gastric cancer was significantly correlated with tumor differentiation and depth of invasion, but not with survival. The mean MVD value of COX-2 or VEGF positive tumors was higher than that of COX-2 or VEGF negative tumors. A significant correlation was found between the expressions of COX-2and VEGF. CONCLUSION: MVD may be one of the important prognostic factors for gastric cancer patients. COX-2 and VEGF may play an important role in tumor progression by stimulating angiogenesis. VEGF might play a main role in the COX-2 angiogenic pathway. 20Sliding mode control based guidance law with impact angle constraint显示文摘The terminal guidance problem for an unpowered lifting reentry vehicle against a stationary target is considered. In addition to attacking the target with high accuracy,the vehicle is also expected to achieve a desired impact angle. In this paper,a sliding mode control(SMC)-based guidance law is developed to satisfy the terminal angle constraint. Firstly,a specific sliding mode function is designed,and the terminal requirements can be achieved by enforcing both the sliding mode function and its derivative to zero at the end of the flight. Then,a backstepping approach is used to ensure the finite-time reaching phase of the sliding mode and the analytic expression of the control effort can be obtained. The trajectories generated by this method only depend on the initial and terminal conditions of the terminal phase and the instantaneous states of the vehicle.In order to test the performance of the proposed guidance law in practical application,numerical simulations are carried out by taking all the aerodynamic parameters into consideration.
