| 2 | Association of Overlapped and Un-overlapped Comorbidities with COVID-19 Severity and Treatment Outcomes: A Retrospective Cohort Study from Nine Provinces in China显示文摘Objective Several COVID-19 patients have overlapping comorbidities. The independent role of each component contributing to the risk of COVID-19 is unknown, and how some non-cardiometabolic comorbidities affect the risk of COVID-19 remains unclear.Methods A retrospective follow-up design was adopted. A total of 1,160 laboratory-confirmed patients were enrolled from nine provinces in China. Data on comorbidities were obtained from the patients’ medical records. Multivariable logistic regression models were used to estimate the odds ratio(OR) and 95% confidence interval(95% CI) of the associations between comorbidities(cardiometabolic or non-cardiometabolic diseases), clinical severity, and treatment outcomes of COVID-19.Results Overall, 158(13.6%) patients were diagnosed with severe illness and 32(2.7%) had unfavorable outcomes. Hypertension(2.87, 1.30–6.32), type 2 diabetes(T2 DM)(3.57, 2.32–5.49),cardiovascular disease(CVD)(3.78, 1.81–7.89), fatty liver disease(7.53, 1.96–28.96), hyperlipidemia(2.15, 1.26–3.67), other lung diseases(6.00, 3.01–11.96), and electrolyte imbalance(10.40, 3.00–26.10)were independently linked to increased odds of being severely ill. T2 DM(6.07, 2.89–12.75), CVD(8.47,6.03–11.89), and electrolyte imbalance(19.44, 11.47–32.96) were also strong predictors of unfavorable outcomes. Women with comorbidities were more likely to have severe disease on admission(5.46,3.25–9.19), while men with comorbidities were more likely to have unfavorable treatment outcomes(6.58, 1.46–29.64) within two weeks.Conclusion Besides hypertension, diabetes, and CVD, fatty liver disease, hyperlipidemia, other lung diseases, and electrolyte imbalance were independent risk factors for COVID-19 severity and poor treatment outcome. | 4 | High Physical Activity is Associated with an Improved Lipid Profile and Resting Heart Rate among Healthy Middle-aged Chinese People显示文摘Objective To investigate the effects of physical activity(PA) on dyslipidemia and elevated resting heart rate(RHR) in a large-scale cross-sectional study in China. Methods We recruited community-based individuals who were 40-60 years old using a cluster sampling method. The PA levels of the participants were classified as low, moderate, or high, using the International Physical Activity Questionnaire. Dyslipidemia was defined as the detection of abnormalities in lipid indicators, and 4 lipid parameters were evaluated using fasting blood samples. Multivariate logistic regression analyses were used to evaluate the associations of PA with dyslipidemia and RHR. Results A total of 10,321 participants(38.88% men) were included in this study. The percentages of individuals with high, moderate, and low PA levels were 46.5%, 43.9%, and 9.6%, respectively. In both men and women, high PA provided odds ratios of 0.88 [95% confidence interval(CI): 0.83, 0.94] for dyslipidemia and 0.82(95% CI: 0.73, 0.92) for elevated RHR, compared to participants with low PA. | 11 | Porcupine inhibitor CGX1321 alleviates heart failure with preserved ejection fraction in mice by blocking WNT signaling显示文摘Heart failure with preserved ejection fraction(HFpEF)is highly prevalent,and lacks effective treatment.The aberration of WNT pathway underlies many pathological processes including cardiac fibrosis and hypertrophy,while porcupine is an acyltransferase essential for the secretion of WNT ligands.In this study we investigated the role of WNT signaling pathway in HFpEF as well as whether blocking WNT signaling by a novel porcupine inhibitor CGX1321 alleviated HFpEF.We established two experimental HFpEF mouse models,namely the UNX/DOCA model and high fat diet/L-NAME(“two-hit”)model.The UNX/DOCA and“two-hit”mice were treated with CGX1321(3 mg kg^(−1)·d^(−1))for 4 and 10 weeks,respectively.We showed that CGX1321 treatment significantly alleviated cardiac hypertrophy and fibrosis,thereby improving cardiac diastolic function and exercise performance in both models.Furthermore,both canonical and non-canonical WNT signaling pathways were activated,and most WNT proteins,especially WNT3a and WNT5a,were upregulated during the development of HEpEF in mice.CGX1321 treatment inhibited the secretion of WNT ligands and repressed both canonical and non-canonical WNT pathways,evidenced by the reduced phosphorylation of c-Jun and the nuclear translocation ofβ-catenin and NFATc3.In an in vitro HFpEF model,MCM and ISO-treated cardiomyocytes,knockdown of porcupine by siRNA leads to a similar inhibitory effect on WNT pathways,cardiomyocyte hypertrophy and cardiac fibroblast activation as CGX1321 did,whereas supplementation of WNT3a and WNT5a reversed the anti-hypertrophy and anti-fibrosis effect of CGX1321.We conclude that WNT signaling activation plays an essential role in the pathogenesis of HFpEF,and porcupine inhibitor CGX1321 exerts a therapeutic effect on HFpEF in mice by attenuating cardiac hypertrophy,alleviating cardiac fibrosis and improving cardiac diastolic function. | Hao Wu Lu-xun Tang Xue-mei Wang Liang-peng Li Xiao-kang Chen Yan-ji He De-zhong Yang Yu Shi Jia-ling Shou Zong-shu Zhang Liang Wang Bing-jun Lu Songzhu Michael An Chun-yu Zeng Wei Eric Wang | 2023 | Acta Pharmacologica Sinica2023,44,6: | 1 |
| 12 | Envonalkib versus crizotinib for treatment-naive ALK-positive non-small cell lung cancer: a randomized, multicenter, open-label, phase III trial显示文摘Anaplastic lymphoma kinase(ALK)rearrangements are present in about 5–6%of non-small cell lung cancer(NSCLC)cases and associated with increased risks of central nervous system(CNS)involvement.Envonalkib,a novel ALK inhibitor,demonstrated promising anti-tumor activity and safety in advanced ALK-positive NSCLC in the first-in-human phase I study.This phase III trial(ClinicalTrials.gov NCT04009317)investigated the efficacy and safety of first-line envonalkib in advanced ALK-positive NSCLC cases.Totally 264 participants were randomized 1:1 to receive envonalkib(n=131)or crizotinib(n=133).Median independent review committee(IRC)-assessed progression-free survival(PFS)times were 24.87(95%confidence interval[CI]:15.64–30.36)and 11.60(95%CI:8.28–13.73)months in the envonalkib and crizotinib groups,respectively(hazard ratio[HR]=0.47,95%CI:0.34–0.64,p<0.0001).IRC-assessed confirmed objective response rate(ORR)was higher(81.68%vs.70.68%,p=0.056)and duration of response was longer(median,25.79[95%CI,16.53–29.47]vs.11.14[95%CI,9.23–16.59]months,p=0.0003)in the envonalkib group compared with the crizotinib group.In participants with baseline brain target lesions,IRC-assessed CNS-ORR was improved with envonalkib compared with crizotinib(78.95%vs.23.81%).Overall survival(OS)data were immature,and median OS was not reached in either group(HR=0.84,95%CI:0.48–1.47,p=0.5741).The 12-month OS rates were 90.6%(95%CI,84.0%–94.5%)and 89.4%(95%CI,82.8%–93.6%)in the envonalkib and crizotinib groups,respectively.Grade≥3 treatment-related adverse events were observed in 55.73%and 42.86%of participants in the envonalkib and crizotinib groups,respectively.Envonalkib significantly improved PFS and delayed brain metastasis progression in advanced ALK-positive NSCLC. | Yunpeng Yang Jie Min Nong Yang Qitao Yu Ying Cheng Yanqiu Zhao Manxiang Li Hong Chen Shou’an Ren Jianying Zhou Wu Zhuang Xintian Qin Lejie Cao Yan Yu Jian Zhang Jianxing He Jifeng Feng Hao Yu Li Zhang Wenfeng Fang | 2023 | Signal Transduction and Targeted Therapy2023,8,9: | 0 |
| 13 | 3D magnetic field guided sunflower-like nanocatalytic active swarm targeting patients-derived organoids显示文摘Nanocatalytic medicine triggering in situ catalytic reactions has been considered as a promising strategy for tumor-selective therapeutics.However,the targeted distribution of nanocatalysts was still low,considering the absence of targeting propulsion capability.Here,encouraged by the fast-developing controllable microrobotics for targeting delivery,a sunflower-like nanocatalytic active swarm(SNCAS)controlled by a three-dimensional(3D)magnetic field was proposed for synergistic tumorselective and magnetic-actively tumor-targeting therapeutics.Furthermore,a patient-derived renal cancer cell 3D organoid was utilized for the verification of the effective tumor therapeutic outcomes.Under the targeted control of 3D magnetic field,the multiple cascade catalytic efficiency of SNCAS based on Fenton reaction was evaluated,resulting in efficient tumor cell apoptosis and death.For the patient-derived organoid treatment,the SNCAS presented significant lethality toward 3D organoid structure to induce cell apoptosis with the collapse of organoid morphology.The targeting efficiency was further enhanced under the magnetic-controllable of SNCAS.Overall,empowered by the magnetic control technology,the synergistic therapeutic strategy based on controllable swarm combined active targeting and tumor-specific catalytic nanomedicine has provided a novel way for advanced cancer therapy.Meanwhile,3D patient-derived organoids were proved as a powerful tool for the effectiveness verification of nanocatalytic medicine. | Dong Liu Ruirui Guo Shuangshuang Mao Yanjie Huang Bin Wang Zijian Wu Xuanjie Xia Jian Dong Yu Xin Ruiyang Xie Jianzhong Shou Wei Sun Yuan Pang Yuan Lu | 2023 | Nano Research2023,16,1: | 0 |

One of the significant characteristics of CM is to treat diseases based on syndrome differentiation. For Western medicine, it is of important clinical significance to formulate guidelines for the diagnosis and treatment of cancer patients based on the characteristics of disease differentiation. In Chinese clinical practice, the combination of disease differentiation and syndrome differentiation is an important feature for cancer treatment in the past. Currently, molecular profiling and genomic analysis-based precision medicine optimizes the anticancer drug design and holds the greatest success in treating cancer patients. Therefore, we want to know which populations of cancer patients can benefit more from CM treatment if the theory of precision medicine is applied to CM clinical practice. So, we developed a novel diagnostic and therapeutic strategy 'disease-syndrome differentiation-genomic profilingprescriptions' for cancer patients by CM syndrome differentiation and precision medicine. As a result, this strategy has greatly enhanced the anti-tumor efficacy of CM and improved clinical outcomes for cancer patients with some gene mutations. | 18 | 3-to 24-month Follow-up on COVID-19 with Pulmonary Tuberculosis Survivors after Discharge:Results from a Prospective,Multicenter Study显示文摘Objective Coronavirus disease 2019(COVID-19)and tuberculosis(TB)are major public health and social issues worldwide.The long-term follow-up of COVID-19 with pulmonary TB(PTB)survivors after discharge is unclear.This study aimed to comprehensively describe clinical outcomes,including sequela and recurrence at 3,12,and 24 months after discharge,among COVID-19 with PTB survivors.Methods From January 22,2020 to May 6,2022,with a follow-up by August 26,2022,a prospective,multicenter follow-up study was conducted on COVID-19 with PTB survivors after discharge in 13 hospitals from four provinces in China.Clinical outcomes,including sequela,recurrence of COVID-19,and PTB survivors,were collected via telephone and face-to-face interviews at 3,12,and 24 months after discharge.Results Thirty-two COVID-19 with PTB survivors were included.The median age was 52(45,59)years,and 23(71.9%)were men.Among them,nearly two-thirds(62.5%)of the survivors were moderate,three(9.4%)were severe,and more than half(59.4%)had at least one comorbidity(PTB excluded).The proportion of COVID-19 survivors with at least one sequela symptom decreased from 40.6%at 3 months to 15.8%at 24 months,with anxiety having a higher proportion over a follow-up.Cough and amnesia recovered at the 12-month follow-up,while anxiety,fatigue,and trouble sleeping remained after 24months.Additionally,one(3.1%)case presented two recurrences of PTB and no re-positive COVID-19during the follow-up period.Conclusion The proportion of long symptoms in COVID-19 with PTB survivors decreased over time,while nearly one in six still experience persistent symptoms with a higher proportion of anxiety.The recurrence of PTB and the psychological support of COVID-19 with PTB after discharge require more attention. | WANG Jing Ya ZONG Xing Yu WU Gui Hui QI Yuan Lin LI Hui Zhen JI Xin Yu TONG Lin ZHANG Lei YANG Ming Bo YANG Pu Ye LI Ji Ke XIAO Fu Rong ZHANG Lin Song HU Yun Hong LIU Hong De XU Shou Fang SUN Sheng WU Wei MAO Ya LI Min Qing HOU Hao Hua GONG Zhao Yuan GUO Yang JIAO Li Wen QIN Jin WANG Ding Yi WANG Fang GUAN Li LIN Gang MA Yan WANG Yan Ping SHI Nan Nan | 2022 | Biomedical and Environmental Sciences2022,35,12: | 0 |
