1恶性肿瘤骨转移及骨相关疾病临床诊疗中国专家共识(英文)显示文摘Since 1980s, palliative medicine has been an independent educational project. The development of palliative care in China started in the early 1990s. During the last 18 years, we have seen an increasing allocation of resources to palliative care, the establishment of an academic chair in palliative medicine, and more focused interest and awareness of the need for palliative care in the society at large. So the doctors in China have established principles of treatment on clinical diagnosis and treatment of malignant tumor bone metastases and bone related complications. In order to share our experience with the world, after two years’ preparation, the top-level experts in palliative medicine have written the consensus statement on clinical diagnosis and treatment of malignant tumor bone metastases and bone related complications. The overview includes not only the achievements in the world, but the hands-on experience in clinic, especially the experience in combination of TCM and Western medicine. 5The Relationship Between Neuroimmunity and Bipolar Disorder:Mechanism and Translational Application显示文摘Neuroimmune system may be involved in the pathological process of bipolar disorder(BD),but the essential association is not fully understood.Accumulating evidence has shown that BD involves the activation of immune cells and the release of inflammatory substances in the central nerve system(CNS).Meanwhile,neuroimmune responses also interact with other hypothesis of the etiology of BD that are widely recognized,such as neurotransmitter systems,neuroendocrine systems,neurotrophic factors,and oxidative stress.Simultaneously,related genes and immune changes in peripheral blood vary with it.Overall,neuroimmunity may play an important role in the pathogenesis of BD,and the inflammatory cytokines,especially interleukin-6 and tumor necrosis factor-alpha,have potential value for the clinical diagnosis and prognosis of BD,as well as predicting the therapeutic effects of drugs.Large-scale studies are needed to extend the evidence on neuroimmunity in BD,and to examine its clinical value for applications such as early prediction and treatment.Zhiang Niu Lu Yang Xiaohui Wu Yuncheng Zhu Jun Chen Yiru Fang 2019Neuroscience Bulletin2019,35,4:8
6Combined prednisone and levothyroxine improve treatment of severe thrombocytopenia in hepatitis B with compensatory cirrhosis accompanied by subclinical and overt hypothyroidism显示文摘The aim of the present study was to investigate the relationship between hypothyroidism and thrombocytopenia in hepatitis Brelated compensatory liver cirrhosis and to determine whether treatment with levothyroxine and prednisone is superior in a multicenter, open-label, observational study in China. In total, 125 consecutive hepatitis B-related compensated liver cirrhosis patients with severe thrombocytopenia accompanied by hypothyroidism were included. The patients were divided into four groups according to treatment strategy: a control group(n=29), a prednisone group(n=25), a levothyroxine group(n=32) and a prednisone plus levothyroxine group(n=39). Severe thrombocytopenia was more prevalent in hepatitis B-associated compensatory liver cirrhosis patients with hypothyroidism than in euthyroid patients(29.6% vs. 14.7%, P<0.05). Combination treatment with prednisone and levothyroxine decreased the risk of bleeding and improved platelet recovery compared to control treatment and treatment with either prednisone or levothyroxine alone. The platelet count before therapy, serum thyroid stimulating hormone and combination treatment with prednisone and levothyroxine were associated with bleeding events. Therefore, the present study suggests that hypothyroidism is associated with severe thrombocytopenia in hepatitis B-associated compensatory liver cirrhosis. 7Perioperative rh-endostatin with chemotherapy improves the survival of conventional osteosarcoma patients: a prospective non-randomized controlled study显示文摘Objective: Anti-angiogenic drugs are an emerging treatment option against malignant tumors. The aim of this study was to determine whether the addition of perioperative rh-endostatin to chemotherapy could improve the probability of distant metastasis-free survival(DMFS) and overall survival(OS) in patients newly diagnosed with non-metastatic conventional osteosarcoma.Methods: This was a controlled non-randomized clinical study that included 388 patients without clinically detectable metastatic disease enrolled from January 2008 to April 2012. The control treatment group had 272 patients; 180 were male and 92, female,with a median age of 17 years. The treatment group had 58 patients; 36 were male and 22, female, with a median age of 16 years.The control group received preoperative chemotherapy followed by surgery and postoperative chemotherapy. The treatment group received 4 cycles of rh-endostatin perioperatively in addition to chemotherapy as per the control group. Patients were followed up from 6-101 months with a median follow-up period of 50.2 months.Results: The 5-year DMFS of the control group(61%) was significantly lower than that of the rh-endostatin group(79%)(P = 0.013). The 5-year OS of the control group(74%) was significantly lower than that of the rh-endostatin treatment group(87%)(P = 0.029). 10NiO nanoparticle-decorated SnO_(2)nanosheets for ethanol sensing with enhanced moisture resistance显示文摘In a high relative humidity(RH)environment,it is challenging for ethanol sensors to maintain a high response and excellent selectivity.Herein,tetragonal rutile SnO_(2)nanosheets decorated with NiO nanoparticles were synthesized by a two-step hydrothermal process.The NiO-decorated SnO_(2)nanosheet-based sensors displayed a significantly improved sensitivity and excellent selectivity to ethanol gas.For example,the 3 mol%NiO-decorated SnO_(2)(SnO_(2)-3Ni)sensor reached its highest response(153 at 100 ppm)at an operating temperature of 260℃.Moreover,the SnO_(2)-3Ni sensor had substantially improved moisture resistance.The excellent properties of the sensors can be attributed to the uniform dispersion of the NiO nanoparticles on the surface of the SnO_(2)nanosheets and the formation of NiO-SnO_(2)p–n heterojunctions.Considering the long-term stability and reproducibility of these sensors,our study suggests that the NiO nanoparticle-decorated SnO_(2)nanosheets are a promising material for highly efficient detection of ethanol.Gaoqiang Niu Changhui Zhao Huimin Gong Zhitao Yang Xiaohui Leng Fei Wang 2019Microsystems & Nanoengineering2019,5,1:3

13Massive allograft replacement in management of bone tumors显示文摘Objective:To evaluate the functional outcome and complications of allograft replacement in management of bone tumors. Methods:Between March 1992 and September 2002,164 patients underwent bone tumor resection and massive allograft reconstruction of bone defects. The length of the resected part ranged from 5-35 cm. The resections were classified as marginal or wide resections of the tumor on the basis of the Musculoskeletal Tumor Society staging system. Fresh-frozen allografts were employed as osteoarticular grafts(n = 95),hemi-condylar(n = 15),massive(n = 23),allograft-prosthesis composite(n = 12),intercalary grafts(n = 15) or hemi-pelvic grafts(n = 4). Most of the lesions were osteosarcoma and giant cell tumor of bone and located in proximal and distal femur,proximal tibia and humerus. Results:At a median follow-up of 47 months(range,12 to 168 months) after the operation,154 of the patients in the study were free of disease and 10 died of disease. Twenty-one(12.8%) patients had local recurrence and 38(23.2%) nonunion. Late complications included 11(6.7%) fractures of the allograft and 18(11.0%) infections of the graft. Instability of the joint in the form of subluxation was noted in 13(7.9%) patients. Ten extremities were amputated due to local recurrence or severe infection. Conclusion:Allografts can be used for reconstruction of bony defects after tumor resection. Allograft has nearly similar shape,strength,osteo-inductivity and osteo-conductivity with host bone. 18Offlattice Model in the Prediction of Protein 3D Structure显示文摘3-dimension HPNX offlattice model is developed from the 2-dimension HP offlattice model. In the HP model, 20 types of amino acid monomers are divided into two classes, H (non-polar monomer) and P (polar monomer). In the HPNX model, polar monomers are split into positively charged (P), negatively charged (N) and neutral (X) monomers. A new evolutionary algorithm is applied to study long chains of the HPNX offlattice protein model. 19Vojta and Bobath combined treatment for high risk infants with brain damage at early period显示文摘BACKGROUND: In the process of early screening and interventions to high risk infants with brain damage, the occasion and choosing methods of interventions and the combined application of different interventions are still at the exploratory phase. OBJECTIVE: To observe the efficacy of early intervention using Vojta and Bobath combined treatment in high risk infants with brain damage, and investigate the effect of early rehabilitation on the prognosis. DESIGN: A randomized controlled comparative observation. SETTING: Daqing Oil Field General Hospital of Heilongjiang Province. PARTICIPANTS: Eighty-four high risk infants younger than 1 year were selected from the Department of Pediatrics, Daqing Oil Field General Hospital of Heilongjiang Province from October 2005 to October 2006, including 52 boys (62%) and 32 girls (38%). The treatment started at the age of 0–3 months in 11 cases (13%), 4–6 months in 28 cases (33%), 7–9 months in 35 cases (42%), and 10–12 months in 10 cases (12%). Infants with at least two of the followings were enrolled, including 7 Vojta abnormal postural reflexes, slow or disorder of motor development, increase of muscular tension, postural abnormality, primary reflection residual and CT/MRI abnormalities. Informed consents were obtained from their guardians. The 84 infants were randomly divided into treatment group (n =42) and control group (n =42). METHODS: All the children were intravenously injected with cerebroprotein hydrolysate injection or cattle encephalon glycoside and ignotin injection, 10 times as a course for 2–5 courses; Besides, the infants in the treatment group also received early rehabilitative training of Vojta and Bobath combined treatment, once a day, 40 minutes per time, 5 times a week followed by a 2-day rest, 1 month as a course, and totally 2–5 courses. The Vojta method was to facilitate the automatic regulation by reflexlocomotion. Bobath method was to inhibit abnormal posture but facilitate the normal one, thus it is also called neurodevelopmental therapy realized by reflex inhibition and facilitation. The rehabilitation was evaluated clinically using Gesell Development Schedules (GDS) after the courses when the infants were 1 year old. GDS included five aspects of adaptive behavior, gross motor, refine motor, speech, individual-social behavior. The developmental quotient (DQ) could be calculated according to their development. DQ=developmental age/actual age×100, < 85 points was taken as abnormal; The higher the DQ value, the better the development. ② Evaluation of efficacy: Cured: Developmental order reached normal or generally normal, abnormal posture disappeared, muscular tension ameliorated, symmetric limbs' motor function, free movement, normal intelligence, powerful reaction, and clear speech. Significant effect: Developmental order tended to normal, abnormal posture disappeared incompletely, muscular tension ameliorated incompletely, transient pause in the flexion and extension of limbs, good reaction, and improved intelligence. Effective: Developmental order improved to a certain degree, muscular tension ameliorated, limbs' motor function and posture improved as compared with before treatment, but still under control, intelligence improved a little. Invalid: No obvious changes before and after treatment. Total significant effective rate=(cured cases+cases of significant effect)/total cases ×100%. ③ The enumeration data and measurement data were compared with chi-square test and t test respectively. MAIN OUTCOME MEASURES: Clinical efficacy and GDS results at 1 year old in both groups. RESUTLS: All the 84 infants were involved in the analysis of results. ① Efficacy: The total significant effective rate in the treatment group was higher than that in the control group (93%, 74%, χ2=4.2, P < 0.05). ② Results of GDS evaluation: The DQ values of individual intercourse and social adaptation in the treatment group (93.57±11.23, 95.13±9.32) were higher than those in the control group (88.27±10.18, 90.10±10.67, t =2.267, 2.301, P < 0.05); The DQ values of gross motor, fine motor and speech in the treatment group (90.78±10.20, 98.63±9.23, 93.69±11.31) were higher than those in the control group(84.32±11.33, 87.80±8.61, 85.52±10.61, t =2.746, 5.560, 2.746, P < 0.01). The interventional effects on each functional area were significant, especially the gross motor, fine motor and speech. 20Early Diagnosis of Bipolar Disorder Coming Soon:Application of an Oxidative Stress Injury Biomarker(BIOS)Model显示文摘Early distinction of bipolar disorder(BD)from major depressive disorder(MDD)is difficult since no tools are available to estimate the risk of BD.In this study,we aimed to develop and validate a model of oxidative stress injury for predicting BD.Data were collected from 1252 BD and 1359 MDD patients,including 64 MDD patients identified as converting to BD from 2009 through 2018.30 variables from a randomly-selected subsample of 1827(70%)patients were used to develop the model,including age,sex,oxidative stress markers(uric acid,bilirubin,albumin,and prealbumin),sex hormones,cytokines,thyroid and liver function,and glycolipid metabolism.Univariate analyses and the Least Absolute Shrinkage and Selection Operator were applied for data dimension reduction and variable selection.Multivariable logistic regression was used to construct a model for predicting bipolar disorder by oxidative stress biomarkers(BIOS)on a nomogram.Internal validation was assessed in the remaining 784 patients(30%),and independent external validation was done with data from 3797 matched patients from five other hospitals in China.10 predictors,mainly oxidative stress markers,were shown on the nomogram.The BIOS model showed good discrimination in the training sample,with an AUC of 75.1%(95%CI:72.9%–77.3%),sensitivity of 0.66,and specificity of 0.73.The discrimination was good both in internal validation(AUC 72.1%,68.6%–75.6%)and external validation(AUC 65.7%,63.9%–67.5%).In this study,we developed a nomogram centered on oxidative stress injury,which could help in the individualized prediction of BD.For better real-world practice,a set of measurements,especially on oxidative stress markers,should be emphasized using big data in psychiatry.Zhiang Niu Xiaohui Wu Yuncheng Zhu Lu Yang Yifan Shi Yun Wang Hong Qiu Wenjie Gu Yina Wu Xiangyun Long Zheng Lu Shaohua Hu Zhijian Yao Haichen Yang Tiebang Liu Yong Xia Zhiyu Chen Jun Chen Yiru Fang 2022Neuroscience Bulletin2022,38,9:1
