3Prevalence and evolution of drug resistance HIV-1 variants in Henan, China显示文摘To understand the prevalence and evolution of drug resistant HIV strains in Henan China after the implementation of free antiretroviral therapy for AIDS patients. 45 drug nave AIDS patients, 118 AIDS patients who received three months antiretroviral therapy and 124 AIDS patients who received six months antiretroviral treatment were recruited in the southern part of Henan province. Information on general condition, antiretroviral medicines, adherence and clinical syndromes were collected by face to face interview. Meanwhile, 14ml EDTA anticoagulant blood was drawn. CD4/CD8 T cell count, viral load and genotypic drug resistance were tested. The rates of clinical improvement were 55.1% and 50.8% respectively three months and six months after antiretroviral therapy. The mean CD4 cell count after antiretroviral therapy was significantly higher than in drug nave patients. The prevalence rate of drug resistant HIV strains were 13. 9%, 45.4% and 62.7% in drug nave patients, three month treatment patients and six month treatment patients, respectively. The number of resistance mutation codons and the frequency of mutations increased significantly with continued antiretroviral therapy. The mutation sites were primarily at the 103, 106 and 215 codons in the three-month treatment group and they increased to 15 codon mutations in the six-month treatment group. From this result, the evolution of drug resistant strains was inferred to begin with the high level NNRTI resistant strain, and then develop low level resistant strains to NRTIs. The HIV strains with high level resistance to NVP and low level resistance to AZT and DDI were highly prevalent because of the AZT+DDI+NVP combination therapy. These HIV strains were also cross resistant to DLV, EFV, DDC and D4T. Poor adherence to therapy was believed to be the main reason for the emergence and prevalence of drug resistant HIV strains. The prevalence of drug resistant HIV strains was increased with the continu- ation of antiretroviral therapy in the southern part of Henan province. 4A national report from China Liver Transplant Registry: steroid avoidance after liver transplantation for hepatocellular carcinoma显示文摘Objective:We aimed to evaluate the efficacy and safety of steroid-free immunosuppression after liver transplantation(LT)for hepatocellular carcinoma(HCC).Methods:We retrospectively analyzed HCC recipients without steroids after LT(SF group,n=368)based on the China Liver Transplant Registry(CLTR)database.These recipients were matched 1:2 with patients using steroids(S group,n=736)for the same period after LT for HCC,according to propensity scores.Results:Multivariate analysis indicates that recipients with younger age[odds ratio(OR),1.053;P=0.011],preoperative hepatitis B virus(HBV)DNA≥1,000 copies/mL(OR,2.597;P=0.004)and beyond Milan criteria(OR,4.255;P<0.001)were identified as the risk factors associated with tumor recurrence in steroid avoidance recipients after LT.The patients fulfilling the Milan criteria in the SF group presented higher overall and tumor-free survival rates than those in the S group(P<0.05).Multivariate analysis revealed that recipient beyond Milan criteria was an independent prognostic factor for overall survival(OR,1.690;P<0.001)and tumor-free survival(OR,2.066;P<0.001).The incidences of new-onset diabetes mellitus(21.20% vs.33.29%,P<0.001),new-onset hypertension(10.05%vs.18.61%,P<0.001)and hyperlipidemia(4.08% vs.7.20%,P=0.042)were significantly lower in the SF group.Conclusions:Steroid-free immunosuppression could be safe and feasible for HBV-related HCC patients in LT.Age,HBV DNA level and Milan criteria maybe risk factors associated with tumor recurrence in steroid avoidance recipients.Recipient beyond Milan criteria was an independent prognostic factor and recipient fulfilling Milan criteria can benefit the most from steroid-free immunosuppression.Qiang Wei Feng Gao Runzhou Zhuang Qi Ling Qinghong Ke Jian Wu Tian Shen Mangli Zhang Min Zhang Xiao Xu Shusen Zheng 2017Chinese Journal of Cancer Research2017,29,5:25

6Evaluating Beijing’s human carrying capacity from the perspective of water resource constraints显示文摘As the demands on limited water resources intensify, concerns are being raised about the human carrying capacity of these resources. However, few researchers have studied the carrying capacity of regional water resources. Beijing, the second-largest city in China, faces a critical water shortage that will limit the city’s future development. We developed a method to quantify the carrying capacity of Beijing’s water resources by considering water-use structures based on the proportions of water used for agricultural, industrial, and domestic purposes. We defined a reference structure as 45:22:33 (% of total, respectively), an optimized structure as 40:20:40, and an ideal structure as 50:15:35. We also considered four domestic water quotas: 55, 75, 95, and 115 m 3 /(person·yr). The urban carrying capacity of 10–12 million was closest to Beijing’s actual 2003 population for all three water-use structures with urban domestic water use of 75 m 3 /(person·yr). However, after accounting for our underlying assumptions, the adjusted carrying capacity is closer to 5–6 million. Thus, Beijing’s population in 2003 was almost twice the adjusted carrying capacity. 8Optimization of beamforming and path planning for UAV-assisted wireless relay networks显示文摘Recently, unmanned aerial vehicles(UAVs) acting as relay platforms have attracted considerable attention due to the advantages of extending coverage and improving connectivity for long-range communications. Specifically, in the scenario where the access point(AP) is mobile, a UAV needs to find an efficient path to guarantee the connectivity of the relay link. Motivated by this fact, this paper proposes an optimal design for beamforming(BF) and UAV path planning. First of all, we study a dual-hop amplify-and-forward(AF) wireless relay network, in which a UAV is used as relay between a mobile AP and a fixed base station(BS). In the network, both of the AP and the BS are equipped with multiple antennas, whereas the UAV has a single antenna. Then, we obtain the output signal-to-noise ratio(SNR) of the dual-hop relay network. Based on the criterion of maximizing the output SNR, we develop an optimal design to obtain the solution of the optimal BF weight vector and the UAV heading angle. Next, we derive the closed-form outage probability(OP) expression to investigate the performance of the dual-hop relay network conveniently. Finally, computer simulations show that the proposed approach can obtain nearly optimal flying path and OP performance, indicating the effectiveness of the proposed algorithm. 9Liposomes co-loaded with metformin and chlorin e6 modulate tumor hypoxia during enhanced photodynamic therapy显示文摘Though photodynamic therapy (PDT) has been widely used in the non-invasive destruction of solid tumors, the therapeutic efficacy of PDT is often limited by the hypoxic tumor environment. Herein, we report the innovative use of metformin, an oral hypoglycemic agent commonly used in the treatment of type II diabetes, to improve tumor oxygenation, and overcome tumor hypoxia-associated resistance to PDT. In our design, hydrophilic metformin and modified hydrophobic chlorin e6 (HCe6) are co-encapsulated within the inner cavity and outer membrane of liposomes, respectively. Due to the high uptake of liposome nanoparticles by tumors, and the sustained release of metformin, the intravenous administration of metformin (Met)-HCe6-Liposome nanoparticles greatly improves tumor oxygenation in several different tumor models, as revealed by in vivo photoacoustic imaging and ex vivo immunofluorescence staining. Systemic administration of Met-HCe6-Liposomes followed by in vivo PDT achieved significantly improved therapeutic effects compared to that of PDT without metformin. 12Combination of small interfering RNAs mediates greater suppression on hepatitis B virus cccDNA in HepG2.2.15 cells显示文摘AIM: To observe the inhibition of hepatitis B virus (HBV) replication and expression in HepG2.2.15 cells by combination of small interfering RNAs (siRNAs). METHODS: Recombinant plasmid psil-HBV was constructed and transfected into HepG2.2.15 cells. At 48 h,72 h and 96 h after transfection,culture media were collected and cells were harvested for HBV replication assay. HBsAg and HBeAg in the cell culture medium were detected by enzyme-linked immunoadsorbent assay (ELISA). Intracellular viral DNA and covalently closed circular DNA (cccDNA) were quantifi ed by real-time polymerase chain reaction (PCR). HBV viral mRNA was reverse transcribed and quantifi ed by reverse-transcript PCR (RT-PCR). RESULTS: siRNAs showed marked anti-HBV effects. siRNAs could specifically inhibit the expression of HBsAg and the replication of HBV DNA in a dose-dependent manner. Furthermore,combination of siRNAs,compared with individual use of each siRNA,exerted a stronger inhibition on antigen expression and viral replication. More importantly,combination of siRNAs significantly suppressed HBV cccDNA amplifi cation. 13Panaxdiol Saponins Component Promotes Hematopoiesis and Modulates T Lymphocyte Dysregulation in Aplastic Anemia Model Mice显示文摘Objective: To investigate the potential efficacy of panaxadiol saponins component(PDS-C) in the treatment of aplastic anemia(AA) model mice. Methods: Totally 70 mice were divided into 7 groups as follows: normal, model, low-, medium-, high-dose PDS-C(20, 40, 80 mg/kg, namely L-, M-, H-PDS-C), cyclosporine(40 mg/kg), and andriol(25 mg/kg) groups, respectively. An immune-mediated AA mouse model was established in BALB/c mice by exposing to 5.0 Gy total body irradiation at 1.0 Gy/min, and injecting with lymphocytes from DBA mice. On day 4 after establishment of AA model, all drugs were intragastrically administered daily for 15 days, respectively, while the mice in the normal and model groups were administered with saline solution. After treatment, the peripheral blood counts, bone marrow pathological examination, colony forming assay of bone marrow culture, T lymphocyte subpopulation analysis, as well as T-bet, GATA-3 and Fox P3 proteins were detected by flow cytometry and Western blot. Results: The peripheral blood of white blood cell(WBC), platelet, neutrophil counts and hemoglobin(Hb) concentration were significantly decreased in the model group compared with the normal group(all P<0.01). In response to 3 dose PDS-C treatment, the WBC, platelet, neutrophil counts were significantly increased at a dose-dependent manner compared with the model group(all P<0.01). The myelosuppression status of AA was significantly reduced in M-, H-PDS-C groups, and hematopoietic cell quantity of bone marrow was more abundant than the model group. The colony numbers of myeloid, erythroid and megakaryocytic progenitor cells in the model group were less than those of the normal mice in bone marrow culture, while, PDS-C therapy enhanced proliferation of hematopoietic progenitor cells by significantly increasing colony numbers(all P<0.01). Furthermore,PDS-C therapy increased peripheral blood CD3^+ and CD3^+CD4^+ cells and reduced CD3^+CD8^+ cells(P<0.05 or P<0.01). Meanwhile, PDS-C treatment at medium-and high doses groups also increased CD4^+CD25^+Fox P3^+ cells, downregulated T-bet protein expression, and upregulated GATA-3 and Fox P3 protein expressions in spleen cells(P<0.05). 15Decreased Average Albumin-to-Globulin Ratio During Radiotherapy Worsens the Prognosis of Esophageal Squamous Cell Carcinoma:Implications for Nutritional Management显示文摘Objective Malnutrition and cachexia are common in esophageal squamous cell carcinoma(ESCC)patients undergoing radiotherapy.This study evaluated how malnutrition-and cachexia-related indicators,including the albumin-to-globulin ratio(AGR),and their changes during radiotherapy predict the treatment outcomes.Methods We reviewed a total of 172 ESCC patients receiving radiotherapy(as a primary cohort)and performed a Cox regression analysis on potential prognostic factors,including the AGR,as well as the TNM stage and concomitant chemotherapy.A subsequent receiver operating characteristics(ROC)curve and Kaplan Meier survival analysis was performed for ESCC patients stratified by the average AGR cut-off point.Results In addition to the well-documented factors(i.e.TNM stage and concomitant chemotherapy),the average AGR was a significant prognostic factor for ESCC patients receiving radiotherapy.By plotting the ROC curve of the average AGR with regard to the ESCC prognosis,we obtained cut-off points for the overall patients(cut-off point:1.5,AUC:0.636),for patients with stageⅡ/Ⅲdisease(cut-off point:1.5,AUC:0.611),and for patients with stageⅣdisease(cut-off point:1.84,AUC:0.900).The ESCC patients with higher average AGR had a significantly more favorable OS compared with those with a lower average AGR.Notably,ESCC patients who had an increasingΔAGR during radiotherapy had a considerably more favorable OS compared with those with a decreasingΔAGR.All of these findings were reproducible in the validation cohort.Conclusion A lower average AGR is indicative of a poorer prognosis for ESCC patients following radiotherapy.Improving the nutritional status and preventing or ameliorating cachexia might contribute to improving the prognosis of ESCC patients.Fen Cai Xiu Lian Zhang Xiao Ling Wu Ting Ting Zhuang Lei Huang Yi Fei Tu Yuan Yuan Lu Min Xia Jiang Qing Ying Zhang Yan Lin Chang Chun Ma 2021Journal of Nutritional Oncology2021,6,2:8
16Evaluation of the safety and quality of day-case cataract surgery based on 4151 cases显示文摘AIM: To evaluate the safety, quality and prospects of day-case cataract surgery performed in a Jiangsu public tertiary hospital METHODS: The general and clinical data for patients who underwent day-case cataract surgery between August 1, 2014 and December 31, 2016 at this hospital were collected. The incidences of intraoperative and postoperative complications, preoperative and postoperative bestcorrected visual acuities(BCVAs), delayed discharge rate, rate of unplanned re-admission to hospital, and patient satisfaction were analyzed. RESULTS: A total of 4151 patients received cataract phacoemulsification surgery to correct age-related, congenital, traumatic, or complicated cataracts. Of these, age-related cataracts were the most frequently occurring. Patient age ranged from 18 to 101 y and the vast majority of patients were between 60 and 80 years old. Of the 4151 patients, 64.73%(2687/4151) had a systemic disease. The number of patients increased over the years, with the average number of patients per month being 90.4, 124.83, and 183.42 in 2014, 2015 and 2016, respectively. The average preoperative BCVA was 0.102±0.057 and average postoperative BCVAs at 1 d, 1 wk, and 1 mo post surgery were 0.453±0.264, 0.657±0.285, and 0.734±0.244, respectively. For intraoperative complications, 4.12%(171/4151) had posterior capsule rupture, 0.79%(33/4151) had iris or ciliary body injury, and 0.048%(2/4151) had suprachoroidal hemorrhage. For postoperative complications, 4.38%(182/4151) had cornea edema, 7.78%(323/4151) had intraocular hypertension, 0.096%(4/4151) had IOL toxicity syndrome, 0.28%(12/4151) had retained lens cortex, and 0.048%(2/4151) had hyphema. The delayed discharge rate was 0.82%(44/4151) and the unplanned re-admission to the hospital was 0(0/4151). The patient satisfaction rate was 91.42%(3795/4151). 19An AIEgens and exonuclease Ⅲ aided quadratic amplification assay for detecting and cellular imaging of telomerase activity显示文摘Monitoring telomerase activity with high sensitive and reliable is of great importance to cancer analysis.In this paper, we report a sensitive and facile method to detect telomerase activity using AIEgens modified probe(TPE-Py-DNA) as a fluorescence reporter and exonuclease Ⅲ(Exo Ⅲ) as a signal amplifier.With the aid of telomerase, repeat units(TTAGGG)n are extended from the end of template substrate oligonucleotides(TS primer) that form duplex DNAs with TPE-Py-DNA. Then, Exo Ⅲ catalyzes the digestion of duplex DNAs, liberating elongation product and releasing hydrophobic TPE-Py. The released hydrophobic TPE-Py aggregate together and produce a telomerase-activity-related fluorescence signal.The liberated product hybridizes with another TPE-Py-DNA probe, starting the second cycle. Finally,we obtain the target-to-signal amplification ratio of 1:N^2. 20Psychometrics of chronic liver disease questionnaire in Chinese chronic hepatitis B patients显示文摘AIM: To evaluate psychometrics of the Chinese (mainland) chronic liver disease questionnaire (CLDQ) in patients with chronic hepatitis B (CHB). METHODS: A cross-sectional sample of 460 Chinese patients with CHB was selected from the Outpatient Department of the Eighth Hospital of Xi'an, including CHB (CHB without cirrhosis) (n = 323) and CHB-related cirrhosis (n = 137). The psychometrics includes reliability, validity and sensitivity. Internal consistency reliability was measured using Cronbach's α. Convergent and discriminant validity was evaluated by item-scale correlation. Factorial validity was explored by principal component analysis with varimax rotation. Sensitivity was assessed using Cohen's effect size (ES), and independent sample t test between CHB and CHB-related cirrhosis groups and between alanine aminotransferase (ALT) normal and abnormal groups after stratifying the disease (CHB and CHB-related cirrhosis).RESULTS: Internal consistency reliability of the CLDQ was 0.83 (range: 0.65-0.90). Most of the hypothesized item-scale correlations were 0.40 or over, and all of such hypothesized correlations were higher than the alternative ones, indicating satisfactory convergent and discriminant validity. Six factors were extracted after varimax rotation from the 29 items of CLDQ. The eligible Cohen's ES with statistically significant independent sample t test was found in the overall CLDQ and abdominal, systematic, activity scales (CHB vs CHBrelated cirrhosis), and in the overall CLDQ and abdominal scale in the stratification of patients with CHB (ALT normal vs abnormal).
