| 1 | Efficacy of rituximab in gastric diffuse large B cell lymphoma patients显示文摘AIM:To evaluate retrospectively the efficacy of rituximab plus chemotherapy in gastric diffuse large B cell lymphoma(DLBCL).METHODS:Sixty patients(median age:58 years)with histologically confirmed gastric DLBCL treated at four Italian institutions between 2000 and 2007,were included in this analysis.Patients were selected by stage (Ⅰ-Ⅳ,Lugano staging system),European Cooperative Oncology Group performance status(0-2)and treatment strategies.Treatment strategies were chemotherapy alone(group A,n=30)[scheduled as cyclophosphamide,doxorubicin,vincristine and prednisone (CHOP)and CHOP-like],and chemotherapy combined with rituximab(group B,n=30).The primary end point of the study was complete response(CR)rate;the secondary end points were disease-free survival (DFS)at 5 years and overall survival(OS).RESULTS:Median follow-up was 62 mo(range:31102 mo).We observed a significant difference between the two groups(A vs B)in terms of CR[76.6%(23/30) vs 100%,P=0.04)and DFS at 5 years[73.3%(22/30) vs 100%,P=0.03).To date,19 group A(63.3%) patients are alive and 11 have died,while all group B patients are alive.No significant differences in toxicity were observed between the two groups.CONCLUSION:Rituximab in combination with chemotherapy improves CR rate,DFS and OS.Further prospective trials are needed to confirm our results. | Davide Leopardo Giuseppe Di Lorenzo Amalia De Renzo Piera Federico Serena Luponio Carlo Buonerba Elide Matano Gerardina Merola Martina Imbimbo Enzo Montesarchio Antonio Rea Maria Carmela Merola Sabino De Placido Giovannella Palmieri | 2010 | World Journal of Gastroenterology2010,16,20: | 19 |
| 2 | Hepatic encephalopathy:An approach to its multiple pathophysiological features显示文摘Hepatic encephalopathy(HE)is a neuropsychiatric complex syndrome,ranging from subtle behavioral abnormalities to deep coma and death.Hepatic encephalopathy emerges as the major complication of acute or chronic liver failure.Multiplicity of factors are involved in its pathophysiology,such as central and neuromuscular neurotransmission disorder,alterations in sleep patterns and cognition,changes in energy metabolism leading to cell injury,an oxidative/nitrosative state and a neuroinflammatory condition.Moreover,in acute HE,a condition of imminent threat of death is present due to a deleterious astrocyte swelling.In chronic HE,changes in calcium signaling,mitochondrial membrane potential and long term potential expression,N-methyl-D-aspartate-cGMP and peripheral benzodiazepine receptors alterations,and changes in the mRNA and protein expression and redistribution in the cerebral blood flow can be observed.The main molecule indicated as responsible for all these changes in HE is ammonia.There is no doubt that ammonia,a neurotoxic molecule,triggers or at least facilitates most of these changes.Ammonia plasma levels are increased two-to three-fold in patients with mild to moderate cirrhotic HE and up to ten-fold in patients with acute liver failure. Hepatic and inter-organ trafficking of ammonia and its metabolite,glutamine(GLN),lead to hyperammonemic conditions.Removal of hepatic ammonia is a differentiated work that includes the hepatocyte,through the urea cycle,converting ammonia into GLN via glutamine synthetase.Under pathological conditions,such as liver damage or liver blood bypass,the ammonia plasma level starts to rise and the risk of HE developing is high. | 3 | Endoscopic features of gastro-intestinal lymphomas: From diagnosis to follow-up显示文摘Many progresses have been done in the management of gastrointestinal(GI) lymphomas during last decades, especially after the discovery of Helicobacter pylori-dependent lymphoma development. The stepwise implementation of new endoscopic techniques, by means of echoendoscopy or double-balloon enteroscopy, enabled us to more precisely describe the endoscopic features of GI lymphomas with substantial contribution in patient management and in tailoring the treatment strategy with organ preserving approaches. In this review, we describe the recent progresses in GI lymphoma management from disease diagnosis to follow-up with a specific focus on the endoscopic presentation according to the involved site and the lymphoma subtype. Additionally, new or emerging endoscopic technologies that have an impact on the management of gastrointestinal lymphomas are reported. We here discuss the two most common subtypes of GI lymphomas: the mucosaassociated lymphoid tissue and the diffuse large B cell lymphoma. | 4 | Effect of phosphate releasing in activated sludge on phosphorus removal from municipal wastewater显示文摘Aluminum and ferric salts are commonly used in municipal wastewater treatment plants(WWPTs) for phosphorus(P) removal. In this study, on-site jar tests were conducted to determine the removal of different P species from the fresh samples in the presence and absence of activated sludge(AS) with different doses of alum, poly-aluminum chloride,and ferric chloride at different p H. The soluble P(SP) concentration in the samples was about 0.63 mg/L. When the mixed liquor containing AS was treated with 8 mg/L of Al, SP could be reduced to 0.13 mg/L, while it was reduced to 0.16 mg/L with only 1 mg/L of Al after sedimentation removal of AS from sample. Chemical analysis determined that AS contained 59.8 mg-P/g-TSS and 43.8 mg-Al/g-TSS and most of the P was associated with the aluminum hydroxide. We discovered that the phosphate in the AS could readily be released from it, which was mainly responsible for ineffective removal of P to low levels in mixed liquor even with very high alum dose. | 5 | Total and not bevacizumab-bound vascular endothelial growth factor as potential predictive factors to bevacizumab-based chemotherapy in colorectal cancer显示文摘AIM: To identify suitable biomarkers of response to bevacizumab(BV)- it remains an open question. The measurement of serum vascular endothelial growth factor(VEGF) has been proposed as a predictive factor for this drug, even if literature data are contradictory. METHODS: We prospectively evaluated the role of BV, total and not BV-bound VEGF and angiopoietin-2(Ang-2) serum levels as potential predictive factors of response for BV in combination with an oxaliplatinbased chemotherapy. BV, Ang-2, total and not BVbound VEGF levels were measured at baseline, before 2^(nd) and 5^(th) cycle of oxaliplatin-based chemotherapy in 20 consecutive metastatic colorectal cancer patients. RESULTS: Results were correlated to response to treatment. Variability in BV levels have been found, with decreased level in less responding patients. In particular, the concentration of BV increased of 3.96 ± 0.69 folds in serum of responsive patients after 3 more cycles of therapy compared to those with stable or progressive disease with a 0.72 ± 0.25 and 2.10 ± 0.13 fold increase, respectively. The determination of free and total VEGF demonstrated that the ratio between the two values, evaluated immediately before the 2^(nd) and the 5^(th) cycle of therapy, decreased from 26.65% ± 1.33% to 15.50% ± 3.47% in responsive patients and from 53.41% ± 4.75 to 34.95% ± 2.88% in those with stable disease. Conversely, in those with progression of disease, the ratio showed the opposite behavior coming up from 25.99% ± 5.23% to 51.71% ± 5.28%. The Ang-2 levels did not show any relationship. | 6 | Oligometastatic disease, the curative challenge in radiation oncology显示文摘The concept of oligometastatic disease was first described by Hellman and Weichselbaum in 1995. The mere insight of this concept led to the hypothesis that this disease may be cured using local ablative weapons. Surgery has already demonstrated this hypothesis. Surgery limitations, either technical or due to refusalor associated comorbidity, have led to implement alternative ablative options such as stereotactic body radiation therapy(SBRT). SBRT evolved from(stereotactic radiosurgery) because of the need to irradiate extracranial lesions and has been shown to be safe and effective. SBRT achieves local control rates ranging from 70%-90%, but highly variable survival rates depending on the group analyzed. Series with heterogeneous metastatic sites and tumor origin have reported 20% survival rates at 2-3 years, similar to those achieved with surgery. Despite its excellent results, SBRT still faces significant clinical challenges. Its optimal integration with systemic treatment is unknown, and response assessment is very difficult. However, the greatest challenge lies in selection of patients most likely to remain oligometastatic, those who will most benefit from the technique. Biomarkers, molecular signatures, that accurately predict the biological behavior of malignancy are needed. | 8 | Antioxidant compounds produced by Pseudocercospora sp.ESL 02,an endophytic fungus isolated from Elaeocarpus sylvestris显示文摘Objective: To isolate endophytic fungi from Elaeocarpus sylvestris(E. sylvestris) and to isolate antioxidant compounds from a potential source fungus.Methods: Endophytic fungi were isolated from fresh leaves and stems of E. sylvestris and identified based on DNA analysis. 1,1-Diphenyl-2-picrylhydrazyl(DPPH) radical scavenging activity was used to evaluate the antioxidant activity of the fungi. The potential antioxidant fungus was further studied to isolate antioxidant compounds. The isolated compounds were identified by melting point analysis, optical rotation, spectral analysis using a UV spectrophotometer, high resolution fast atom bombardment mass spectrometry, X-ray crystallography analysis,~1H nuclear magnetic resonance analysis and ^(13)C nuclear magnetic resonance analysis. The isolated compounds were evaluated with DPPH radical scavenging, reducing power, and b-carotene bleaching assays.Results: Seven endophytic fungi were successfully isolated from E. sylvestris and identified as Pestalotiopsis sp. EST 01, Pestalotiopsis sp. EST 02, Diaporthales sp. EST03, Meyerozyma sp. EST 04, Diaporthales sp. EST 05, Pestalotiopsis sp. ESL 01, and Pseudocercospora sp. ESL 02. Of the seven fungi, Pseudocercospora sp. ESL 02 had the highest antioxidant activity [IC50=(30.54 ± 0.88) mg/mL]. From that fungus, two compounds identified as terreic acid(1) and 6-methylsalicylic acid(2) were isolated with an IC50 of DPPH radical scavenging activity of(0.22 ± 0.02) mmol/L and(3.87 ± 0.27)mmol/L, respectively. The compounds also had good activities from the reducing power and b-carotene bleaching assays.Conclusions: The Pseudocercospora sp. | 11 | Visual and ocular effects from the use of flat-panel displays显示文摘AIM: To evaluate the prevalence of eye symptoms in a non-presbyopic population of video display unit(VDU)users with flat-panel displays.METHODS: One hundred and sixteen VDU users with flat-panel display from an urban population participated in the study; their ages ranging from 20 to 34 y. There were 60 females and 56 males. An eye examination to rule out the presence of significant uncorrected refractive errors, general binocular dysfunctions and eye conditions was carried out. In order to determine and quantify the type and nature of eye symptoms,participants were asked to answer written questionnaire and the results were grouped by gender, age and number of hours a day spent using a VDU. RESULTS: Seventy-two percent of participants reported eye symptoms related to VDU use. Eye symptoms from moderate-to-severe were found in 23%of participants. The main symptom was moderate-to-severe tired eyes(14%); followed by sensitivity to bright lights(12%), blurred vision at far distances(10%),eyestrain or dry eye or irritated or burning eyes(9%),difficulty in refocusing from one distance to another or headache(8%) and blurred vision at near or intermediate distances(<4%). Eye symptoms were greater among females(P =0.005) and increased with VDU use, markedly above 6h spent using a VDU in a typical day(P =0.01).CONCLUSION: Significant eye symptoms relate to VDU use often occur and should not be underestimated. | 15 | GOECP/SEOR radiotheraphy guidelines for non-small-cell lung cancer显示文摘Non-small cell lung cancer(NSCLC) is a heterogeneous disease accounting for approximately 85% of all lung cancers. Only 17% of patients are diagnosed at an early stage. Treatment is multidisciplinary and radiotherapy plays a key role in all stages of the disease. More than 50% of patients with NSCLC are treated with radiotherapy(curative-intent or palliative). Technological advancesincluding highly conformal radiotherapy techniques, new immobilization and respiratory control systems, and precision image verification systems-allow clinicians to individualize treatment to maximize tumor control while minimizing treatment-related toxicity. Novel therapeutic regimens such as moderate hypofractionation and advanced techniques such as stereotactic body radiotherapy(SBRT) have reduced the number of radiotherapy sessions. The integration of SBRT into routine clinical practice has radically altered treatment of early-stage disease. SBRT also plays an increasingly important role in oligometastatic disease. The aim of the present guidelines is to review the role of radiotherapy in the treatment of localized, locally-advanced, and metastatic NSCLC. We review the main radiotherapy techniques and clarify the role of radiotherapy in routine clinical practice. These guidelines are based on the best available evidence.
