12018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). 4Radiofrequency ablation in the treatment of small hepatocellular carcinoma:A meta analysis显示文摘AIM:To evaluate survival and recurrence after radiofrequency ablation(RFA) for the treatment of small hepatocellular carcinoma(HCC) using a meta-analysis.METHODS:Literature on RFA vs surgical resection for the treatment of small HCC published between January 1990 and December 2008 was retrieved.A metaanalysis was conducted to estimate pooled survival and recurrence ratios.A fixed or random effect model was established to collect the data.RESULTS:The differences in overall survival at 1-year,3-years and at end of follow-up were not statistically significant between the RFA and surgery groups(P > 0.05).There were no differences in 1-year and 3-year recurrences between the RFA and surgery groups(P > 0.05).However,recurrence in the RFA group was lower than that in the surgery group up to the end of follow-up(P = 0.03).Survival was not significantly different.There was a significant difference in recurrences at the end of follow-up after RFA compared with surgical resection.CONCLUSION:RFA did not decrease the number of overall recurrences,and had no effect on survival when compared with surgical resection in a selected group of patients.Jun-Guo Liu Yi-Jun Wang Zhi Du 2010World Journal of Gastroenterology2010,16,27:19
5Methylation of Dickkopf-3 as a prognostic factor in cirrhosis-related hepatocellular carcinoma显示文摘AIM:To investigate the prevalence and time of Dickkopf(DKK) family methylation and its clinical significance in hepatocarcinogenesis. METHODS:Methylation of DKK family genes was quantitatively analyzed in 115 liver tissue samples,including 50 pairs of primary hepatocellular carcinoma(HCC) and matched noncancerous cirrhotic tissue samples,as well as 15 liver cirrhosis biopsy samples. RESULTS:The methylation level of DKK3 was significantly higher in HCC tissue samples than in matched noncancerous cirrhotic tissue samples(P<0.0001) orin liver cirrhosis biopsy samples(P=0.0139) .Receiver operator characteristic curve analysis confirmed that the percent of methylated reference(PMR) values of DKK3 could effectively discriminate HCC tissue samples from noncancerous tissue samples(AUC=0.8146) or liver cirrhosis biopsy samples(AUC=0.7093) .Kaplan-Meier survival curves revealed that the progression-free survival time of patients with a higher DKK3 methylation level(PMR >1%) was significantly shorter than that of those with a lower DKK3 methylation level(PMR≤1%)(P=0.0255) . Multivariate Cox analysis indicated that methylated DKK3 was significantly and independently related with a shorter survival time(relative risk=2.527,95%CI:1.063-6.008,P=0.036) of HCC patients. 6Role of RECK methylation in gastric cancer and its clinical significance显示文摘AIM:To investigate the relation between RECK methylation and clinicopathological characteristics of gastric cancer patients and evaluate the role of RECK methylation in peritoneal metastasis of gastric cancer.METHODS:Methylation of RECK gene in 40 paired samples of gastric cancer and its corresponding adjacent normal mucosa,lymph nodes and peritoneal irrigation fluid was detected by methylation-specific polymerase chain reaction.RESULTS:Aberrant methylation of RECK gene was detected in 27.5%(11/40)of the adjacent normal mucosa samples,in 47.5%(19/40)of gastric cancer samples,in 57.1%(12/21)of the lymph node samples,and in 35%(14/40)of peritoneal irrigation fluid samples,respectively,with a significant difference between the adjacent normal mucosa and lymph node samples(P=0.023).Presence of RECK methylation in the primary tumor samples was significantly correlated with tumor invasion(P=0.023).The accuracy of RECK methylation in peritoneal lavage fluid samples for the diagnosis of peritoneal metastasis of gastric cancer was 72.5%(26/40),with a sensitivity of 66.7%(6/9) and a specificity of 74.2%(23/31).CONCLUSION:Aberrant methylation of RECK gene may provide useful information for the early diagnosis and treatment of peritoneal metastasis of gastric cancer.Du, Yun-Yi Dai, Dong-Qiu Yang, Zhi 2010World Journal of Gastroenterology2010,16,7:16
7A Prospective Randomized Multicenter Controlled Trial on Salvianolate for Treatment of Unstable Angina Pectoris in A Chinese Elderly Population显示文摘Objective: To evaluate the efficacy and safety of salvianolate in elderly patients with unstable angina pectoris(UAP). Methods: A prospective double-blind randomized placebo-controlled multicenter trial in elderly patients with UAP from 13 third-grade class-A hospitals in China was performed. A total of 318 patients were randomly allocated in a 1:1 ratio to an experimental group(160 patients) and a control group(158 patients). The experimental group was treated with salvianolate for 14 days on the basis of conventional medicine, and the control group was given a placebo for 14 days with the same criteria. Follow-up was lasted 28 days in both groups. The primary endpoint was biweekly frequency of angina pectoris attacks. The secondary endpoints included biweekly dosage of nitroglycerin, the Seattle Angina Questionnaire, angina pectoris severity and duration, myocardial injury markers, high-sensitivity C-reactive protein(hs-CRP) and N-terminal pro-B-type natriuretic peptide(NT-proBNP), as well as major adverse cardiovascular events(MACEs). Safety was assessed according to adverse events and serious adverse events. Results: Baseline characteristics were similar between treatment groups. Compared with those in the control group, the frequency of biweekly angi na attacks(2.92 vs. 4.08, P =0.025), th e biweekly dosage of nitroglycerin, as well as the severity and duration of angina attacks(P <0.01) were reduced by salvianolate. The Seattle Angina Questionnaire score was also significantly improved in the experimental group than in the control group(P <0.05). No significant differences were observed between the two groups with respect to the incidence of MACEs. Salvianolate was well tolerated. 8Dental caries and first permanent molar pit and fissure morphology in 7-to 8-year-old children in Wuhan,China显示文摘To obtain the caries experience and,plaque accumulation severity and pit and fissure morphology in first permanent molars in 7-8 children in Wuhan,as a reasonable prediction of caries risk and preventive attention in the future,a convenient sample of five primary schools in the vicinity of the Wuhan University School and Hospital of Stomatology was drawn.Two calibrated examiners orally examined all present grade 2 children in the classroom,using standard caries plaque and tooth morphology criteria.Dental caries was scored at enamel(D 2) and dentine(D 3) for tooth and surface level.Independent variables were age,gender and school.Data analysis used analysis of variance and t-test.The sample comprised 1 043 7-and 8-year-olds.The prevalence of dental caries in permanent dentition was 8.7% and in primary dentition,68.7%.Mean Decayed,Missing,Filled Teeth/S(DMFT/S) scores were 0.11 and 0.14,respectively.Mean dmft/s scores were 2.8 and 5.0.The d-component constituted 75% of the d 3 mft index,while enamel carious lesions constituted 36% of the total number of carious lesions(d 2,3-component).Prevalence of medium and deep pits and fissures was 84.6%.Prevalence of medium and severe plaque accumulation was 67.4%.Prevalence of dental caries in the deciduous and permanent dentitions of 7-to 8-year-old children was high.Deep pits and fissures in high caries risk children should be sealed.Jin-Dong Wang Xi Chen Jo Frencken Min-Quan Du Zhi Chen 2012International Journal of Oral Science2012,4,3:13
9Insight-HXMT observations of the first binary neutron star merger GW170817显示文摘Finding the electromagnetic(EM) counterpart of binary compact star merger, especially the binary neutron star(BNS) merger,is critically important for gravitational wave(GW) astronomy, cosmology and fundamental physics. On Aug. 17, 2017,Advanced LIGO and Fermi/GBM independently triggered the first BNS merger, GW170817, and its high energy EM counterpart,GRB 170817 A, respectively, resulting in a global observation campaign covering gamma-ray, X-ray, UV, optical, IR, radio as well as neutrinos. The High Energy X-ray telescope(HE) onboard Insight-HXMT(Hard X-ray Modulation Telescope) is the unique high-energy gamma-ray telescope that monitored the entire GW localization area and especially the optical counterpart(SSS17 a/AT2017 gfo) with very large collection area(~1000 cm^2) and microsecond time resolution in 0.2-5 MeV. In addition,Insight-HXMT quickly implemented a Target of Opportunity(ToO) observation to scan the GW localization area for potential X-ray emission from the GW source. Although Insight-HXMT did not detect any significant high energy(0.2-5 MeV) radiation from GW170817, its observation helped to confirm the unexpected weak and soft nature of GRB 170817 A. Meanwhile,Insight-HXMT/HE provides one of the most stringent constraints(~10^(-7) to 10^(-6) erg/cm^2/s) for both GRB170817 A and any other possible precursor or extended emissions in 0.2-5 MeV, which help us to better understand the properties of EM radiation from this BNS merger. 12Inconsistent seasonal variation of antibiotics between surface water and groundwater in the Jianghan Plain:Risks and linkage to land uses显示文摘Antibiotics are widely used in humans and animals,but their transformation from surface water to groundwater and the impact of land uses on them remain unclear.In this study,14 antibioticswere systematically surveyed in a complex agricultural area in Central China.Results indicated that the selected antibiotic concentrations in surface waters were higher in winter(average:32.7 ng/L)than in summer(average:17.9 ng/L),while the seasonal variation in groundwaters showed an opposite trend(2.2 ng/L in dry winter vs.8.0 ng/L in summer).Macrolides were the predominant antibiotics in this area,with a detected frequency of over 90%.A significant correlation between surface water and groundwater antibiotics was only observed in winter(R^(2)=0.58).This study further confirmed the impact of land uses on these contaminants,with optimal buffer radii of 2500 m in winter and 500 m in summer.Risk assessment indicated that clarithromycin posed high risks in this area.Overall,this study identified the spatiotemporal variability of antibiotics in a typical agricultural area in Central China and revealed the impact of land uses on antibiotic pollution in aquatic environments.Xi Liu Zhi Wang Lu Zhang Weiying Fan Chao Yang Enhua Li Yun Du Xuelei Wang 2021Journal of Environmental Sciences2021,33,11:9
13Regulatory Effect of E2,IL-6 and IL-8 on the Growth of Epithelial Ovarian Cancer Cells显示文摘To determine the regulatory effects of estrogen and cytokine IL-6 and IL-8 on the growth of epithelial ovarian cancer (OVCA), we first examined the status of estrogen receptors (ERα and ERβ), IL-6 receptor (IL-6Rα and gp130), and IL-8 receptor (IL-8RA and IL-8RB) on five epithelial OVCA cell lines by semiquantitative RT-PCR and Western blot analysis. Results showed that the expressions of these receptors were variable on the five cells.Those OVCA cells expressing the receptors were selected to study related molecular mechanism. MTT assay was performed to observe the effects of 17β-estradiol (E2), IL-6 and IL-8 on cell proliferation. We discovered that E2 markedly promoted the proliferation of CAOV-3 and OVCAR-3 cell in a time- and dose-dependent manner.Tamoxifen (Txf), an ER inhibitor, completely blocked the proliferation of the E2-induced cells, and IL-6- or/and IL-8-neutralizing antibody only showed partially blocking activity. IL-6 and IL-8 were able to significantly stimulate CAOV-3 and OVCAR-3 cell proliferation in a time- and dose-dependent manner, which had a potential synergistic effect on CAOV-3 cells but not on OVCAR-3 cells. The cell proliferation induced by these two cytokines was abolished completely by their specific neutralizing antibodies, partially by Txf, but not by unrelated goat IgG.Taken together, our results suggested that estrogen, IL-6 and IL-8 could modulate OVCA growth by forming a reciprocal cascade with amplifying effect. 14Internal compared with external drainage of pancreatic duct during pancreaticoduodenectomy: a retrospective study显示文摘Objective: To compare internal with external drainage of the pancreatic duct during pancreaticoduodenectomy with regard to the incidence of postoperative pancreatic fistula(POPF) and other complications.Methods: We retrospectively analyzed 316 patients who underwent pancreaticoduodenectomy with a placed drainage tube(external, n=128; internal, n=188) in the pancreatic duct from 1 January 1999 to 31 December 2011 in Tianjin Third Central Hospital of China. The incidence of POPF and some other complications were compared.Results: There was no difference in the incidence rates of POPF between those given an internal or external drainage tube(P=0.788), but POPF was more severe in the former(P=0.014). Intraperitoneal bleeding rate was also higher in the patients with internal drainage(P=0.040), but operative time and postoperative hospitalization were longer in those with external drainage(P=0.002 and P=0.007, respectively). 15Extractive Distillation of Methyl Acetate-Methanol Azeotrope Using [DMIM]DMP as Solvent显示文摘In order to separate methyl acetate and methanol azeotrope, the ionic liquid(IL) 1,3-dimethylimidazolium dimethylphosphate([DMIM]DMP) was used as the solvent. The Aspen Plus software was used to design and optimize the extractive distillation process. Under the optimized conditions, the mass fractions of methyl acetate and methanol were both above 99.5%. Compared with the dimethyl sulfoxide(DMSO) process, the [DMIM]DMP process has the advantages of saving energy and lower equipment investment cost. 16Human liver tissue metabolic profiling research on hepatitis B virus-related hepatocellular carcinoma显示文摘AIM: To select characteristic endogenous metabolites in hepatitis B virus (HBV)-related hepatocellular carcinoma (HCC) patients and to identify their molecular mechanism and potential clinical value. METHODS: An ultra performance liquid chromatography and linear trap quadrupole-Orbitrap XL-mass spectrometry platform was used to analyze endogenous metabolites in the homogenate of central tumor tissue, adjacent tissue and distant tissue obtained from 10 HBV-related HCC patients. After pretreatment with Mzmine software, including peak detection, alignment and normalization, the acquired data were treated with Simca-P+software to establish multivariate statistical analysis based on a pattern recognition technique and characteristic metabolites highly correlated with changing trends in metabolic profiling were selected and further identified. RESULTS: Based on data acquired using Mzmine software, a principal component analysis model (R2X = 66.9%, Q2 = 21.7%) with 6 principal components and an orthogonal partial least squares discriminant analy-sis model (R2X = 76.5%, R2Y = 93.7%, Q2 = 68.7%) with 2 predicted principal components and 5 orthogonal principal components were established in the three tissue groups. Forty-nine ions were selected, 33 ions passed the 2 related samples nonparametric test (P < 0.05) and 14 of these were further identified as characteristic metabolites that showed significant differences in levels between the central tumor tissue group and distant tumor tissue group, including 9 metabolites (L -phenylalanine, glycerophosphocholine, lysophosphatidylcholines, lysophosphatidylethanolamines and chenodeoxycholic acid glycine conjugate) which had been reported as serum metabolite biomarkers for HCC diagnosis in previous research, and 5 metabolites (betasitosterol, quinaldic acid, arachidyl carnitine, tetradecanal, and oleamide) which had not been reported before. 17A Scan of Obesogenic Environments and a Spatial Inference of Obesity Prevalence in Chinese Children and Adolescents: Based on the Chinese Health and Nutrition Survey 2011 Data显示文摘Objective To identify the characteristics of Chinese obesogenic environments at a provincial level, infer a spatial distribution map of obesity prevalence in 31 provinces, and provide a foundation for development of policy to reduce obesity in children and adolescents. Methods After scanning obesity data on subjects aged 7-17 years from 12 provinces in the China Health and Nutrition Survey 2011 and environmental data on 31 provinces from the China Statistical Yearbook 2011 and other sources, we selected 12 predictors. We used the 12 surveyed provinces as a training sample to fit an analytical model with partial least squares regression and prioritized the 12 predictors using variable importance in projection. We also fitted a predictive model with Bayesian analysis. Results We identified characteristics of obesogenic environments. We fitted the predictive model with a deviance information criterion of 61.96 and with statistically significant(P < 0.05) parameter estimates of intercept [95% confidence interval(CI): 329.10, 963.11], log(oil)(CI: 13.11, 20.30), log(GDP)(CI: 3.05, 6.93), log(media)(CI:-234.95,-89.61), and log(washing-machine)(CI: 0.92, 5.07). The total inferred average obesity prevalence among those aged 7-17 was 9.69% in 31 Chinese provinces in 2011. We also found obvious clustering in occurrences of obesity in northern and eastern provinces in the predicted map. Conclusion Given complexity of obesity in children and adolescents, concerted efforts are needed to reduce consumption of edible oils, increase consumption of vegetables, and strengthen nutrition, health, and physical activity education in Chinese schools. 18IL-17A-dependent gut microbiota is essential for regulating diet-induced disorders in mice显示文摘The gut microbiota plays a key role in obesity and related metabolic disorders, and multiple factors including diet, host genotype, and age regulate it. Many studies have examined the contribution of extrinsic factors to the regulation of the gut microbiota, but the importance of the host genetic constitution cannot be ignored. Interleukin 17A(IL-17A), a pro-inflammatory cytokine, is important in the defense against infection and diseases. Here, we investigated the association among IL-17, a high-fat diet(HFD), and the gut microbiota. Mice deficient in IL-17A were resistant to diet-induced obesity and related diseases. Compared with the Ⅱ-17^(-/-)mice, wild-type(WT) mice challenged with HFD showed obvious weight fluctuations, such as those seen in type 2 diabetes, and hematological changes similar to those associated with metabolic syndrome. However, housing WT mice and Ⅱ-17a^(-/-)mice together significantly alleviated these symptoms in the WT mice. A metagenomic analysis of the mouse feces indicated that the microbial community compositions of these two groups differed before HFD feeding. The HFD mediated shifts in the gut microbial compositions, which were associated with the mouse phenotypes.We also identified potentially beneficial and harmful species present during this period, and drew networks of the most abundant species. A functional analysis indicated pathway changes in the WT and Ⅱ-17a^(-/-)mice when fed the HFD. 20Digital high-efficiency print forming method and device for multi-material casting molds显示文摘Sand mold 3D printing technology based on the principle of droplet ejection has undergone rapid development in recent years and has elicited increasing attention from engineers and technicians.However,current sand mold 3D printing technology exhibits several problems,such as single-material printing molds,low manufacturing efficiency,and necessary post-process drying and heating for the manufacture of sand molds.This study proposes a novel high-efficiency print forming method and device for multi-material casting molds.The proposed method is specifically related to the integrated forming of two-way coating and printing and the shortflow manufacture of roller compaction and layered heating.These processes can realize the high-efficiency print forming of high-performance sand molds.Experimental results demonstrate that the efficiency of sand mold fabrication can be increased by 200%using the proposed two-way coating and printing method.The integrated forming method for layered heating and roller compaction presented in this study effectively shortens the manufacturing process for 3D-printed sand molds,increases sand mold strength by 63.8%,and reduces resin usage by approximately 30%.The manufacture of multi-material casting molds is demonstrated on typical wheeled cast-iron parts.This research provides theoretical guidance for the engineering application of sand mold 3D printing.Zhongde SHAN Zhi GUO Dong DU Feng LIU Wenjiang LI 2020Frontiers of Mechanical Engineering2020,15,2:5
