2Isolation and characterization of H7N9 viruses from live poultry markets--Implication of the source of current H7N9 infection in humans显示文摘On March 31, 2013, the National Health and Family Planning Commission announced that human infections with a previously undescribed influenza A (H7N9) virus had occurred in Shanghai and Anhui Province, China. To investigate the possible origins of the H7N9 viruses causing these human infections, we collected 970 samples, including drinking water, soil, and cloacal and tracheal swabs of poultry from live poultry markets and poultry farms in Shanghai and Anhui Province. Twenty samples were positive for the H7N9 influenza virus. Notably, all 20 viruses were isolated from samples collected from live poultry markets in Shanghai. Phylogenetic analyses showed that the six internal genes of these novel human H7N9 viruses were derived from avian H9N2 viruses, but the ancestor of their HA and NA genes is uncertain. When we examined the phylogenetic relationship between the H7N9 isolates from live poultry markets and the viruses that caused the human infections, we found that they shared high homology across all eight gene segments. We thus identified the direct avian origin of the H7N9 influenza viruses that caused the human infections. Importantly, we observed that the H7N9 viruses isolated from humans had acquired critical mutations that made them more 'human-like'. 4North Qinling Paleozoic granite associations and their variation in space and time: Implications for orogenic processes in the orogens of central China显示文摘Integrated zircon U-Pb dating and whole rock geochemical analyses have been carried out for two typical S-and I-type granitoids in the north Qinling. Zircon dating by SIMS of the Piaochi S-type grani- toids yields an emplacement age of 495±6 Ma. The granitoids show whole-rock εNd(t)=-8.2--8.8, zircon εHf(t)=-6―-39. The Huichizi I-type granitoids have emplacement ages of 421±27 Ma and 434±7 Ma es- tablished by LA-ICP-MS and SIMS methods, respectively. Their whole-rock εNd(t)=-0.9-0.9 and zircon εHf(t)=-11-8.4. Combined with statistical analyses of 28 zircon ages of granitoid plutons collected from the literature, Paleozoic magmatism in the north Qinling can be divided into three stages. The first-stage magmatism (~505-470 Ma) mainly occurred in the east part of the north Qinling and has features of an I-type arc, associated with which are S-type granitoids such as Piaochi pluton. The early granitoids (~505-490 Ma) have close spatio-temporal relations to ultra-high-pressure (UHP) rocks, and thus are interpreted as an oceanic subduction system along a continental margin. The second-stage magmatism (~450-422 Ma) occured through the whole north Qinling, and is characterized by I-type granitoids represented by the Huichizi pluton. The magma is interpreted as partial melting of lower crust mixed by mantle-derived magma in a collisional setting with the uplift of terranes. The third-stage magmatism (~415-400 Ma) is dominated by I-type granitoids and only took place in the middle part of the north Qinling, and is regarded as a late-stage collision. The spatial and temporal variations of the Qinling Paleozoic magmatism reveal protracted subduction/collision. The subduction was initiated from the east part of the north Qinling, earlier than that in the Qilian-northern Qaidam, Kunlun, and northern Dabie regions. 5Prevalence of minimal hepatic encephalopathy and quality of life evaluations in hospitalized cirrhotic patients in China显示文摘AIM:To investigate the prevalence of minimal hepatic encephalopathy(MHE)and to assess corresponding health-related quality of life(HRQoL)in hospitalized cirrhotic patients in China.METHODS:This multi-center cross-sectional study included 16 teaching hospitals,which were members of 'Hepatobiliary Cooperation Group,Society of Gastroenterology,Chinese Medical Association',from different areas of China carried out between June and October in 2011.All the eligible hospitalized cirrhotic patients(n = 538)were required to complete triplicate number connection tests combined with one digit symbol test for diagnosing MHE.Patients' clinical examination data were complemented by a modified questionnaire assessing HRQoL.Written informed consent was obtained from each patient.RESULTS:Male was predominant(68.6%)in 519 patients who met the criteria of the study,with a mean age of 49.17 ± 11.02 years.The most common cause of liver cirrhosis was chronic hepatitis B(55.9%).The prevalence of MHE was 39.9% and varied by ChildPugh-Classification score(CPC-A:24.8%,CPC-B:39.4% and CPC-C:56.1%,P < 0.01).MHE(P < 0.01)and higher CPC scores(P < 0.01)were associated with a high HRQoL scores(reflecting poorer quality of life).The prevalence of MHE was proportionate to CPC(P = 0.01)and high quality of life scores(P = 0.01).CONCLUSION:Hospitalized cirrhotic patients have a high prevalence of MHE that is proportionate to the degree of liver function and HRQoL impairment.Ji-Yao Wang Ning-Ping Zhang Bao-Rong Chi Yu-Qing Mi Li-Na Meng Ying-Di Liu Jiang-Bin Wang Hai-Xing Jiang Jin-Hui Yang Yun Xu Xiao Li Jian-Ming Xu Guo Zhang Xin-Min Zhou Yu-Zheng Zhuge De-An Tian Jin Ye Yu-Lan Liu 2013World Journal of Gastroenterology2013,19,30:64

10Exercise as a prescription for patients with various diseases显示文摘A growing understanding of the benefits of exercise over the past few decades has prompted researchers to take an interest in the possibilities of exercise therapy.Because each sport has its own set of characteristics and physiological complications that tend to occur during exercise training,the effects and underlying mechanisms of exercise remain unclear.Thus,the first step in probing the effects of exercise on different diseases is the selection of an optimal exercise protocol.This review summarizes the latest exercise prescription treatments for 26 different diseases:musculoskeletal system diseases(low back pain,tendon injury,osteoporosis,osteoarthritis,and hip fracture),metabolic system diseases(obesity,type 2 diabetes,type 1 diabetes,and nonalcoholic fatty liver disease),cardio-cerebral vascular system diseases(coronary artery disease,stroke,and chronic heart failure),nervous system diseases(Parkinson’s disease,Huntington’s disease,Alzheimer’s disease,depression,and anxiety disorders),respiratory system diseases(chronic obstructive pulmonary disease,interstitial lung disease,and after lung transplantation),urinary system diseases(chronic kidney disease and after kidney transplantation),and cancers(breast cancer,colon cancer,prostate cancer,and lung cancer).Each exercise prescription is displayed in a corresponding table.The recommended type,intensity,and frequency of exercise prescriptions are summarized,and the effects of exercise therapy on the prevention and rehabilitation of different diseases are discussed.Xin Luan Xiangyang Tian Haixin Zhang Rui Huang Na Li Peijie Chen Ru Wang 2019Journal of Sport and Health Science2019,8,5:61
11Effects of different resuscitation fluid on severe acute pancreatitis显示文摘AIM: To compare effects of different resuscitation fluid on microcirculation, inflammation, intestinal barrier and clinical results in severe acute pancreatitis (SAP). METHODS: One hundred and twenty patients with SAP were enrolled at the Pancreatic Disease Institute between January 2007 and March 2010. The patients were randomly treated with normal saline (NS group), combination of normal saline and hydroxyethyl starch (HES) (SH group), combination of normal saline, hydroxyethyl starch and glutamine (SHG group) in resuscitation. The ratio of normal saline to HES in the SH and SHG groups was 3:1. The glutamine (20% glutamine dipeptide, 100 mL/d) was supplemented into the resuscitation liquid in the SHG group. Complications and outcomes including respiratory and abdominal infection, sepsis, abdominal hemorrhage, intra-abdominal hypertension, abdominal compartment syndrome (ACS), renal failure, acute respiratory distress syndrome (ARDS), multiple organ dysfunction syndrome (MODS), operation intervention, length of intensive care unit stay, length of hospital stay, and mortality at 60 d were compared. Moreover, blood oxygen saturation (SpO 2 ), gastric intramucosal pH value (pHi), intra-abdominal pressure (IAP), inflammation cytokines, urine lactulose/mannitol (L/M) ratio, and serum endotoxin were investigated to evaluate the inflammatory reaction and gut barrier. RESULTS: Compared to the NS group, patients in the SH and SHG groups accessed the endpoint more quickly (3.9 ± 0.23 d and 4.1 ± 0.21 d vs 5.8 ± 0.25 d, P < 0.05) with less fluid volume (67.26 ± 28.53 mL/kg/d, 61.79 ± 27.61 mL/kg per day vs 85.23 ± 21.27 mL/kg per day, P < 0.05). Compared to the NS group, incidence of renal dysfunction, ARDS, MODS and ACS in the SH and SHG groups was obviously lower. Furthermore, incidence of respiratory and abdominal infection was significantly decreased in the SH and SHG groups, while no significant difference in sepsis was seen. Moreover, less operation time was needed in the SH and SHG group than the NS group, but the difference was not significant. The mortality did not differ significantly among these groups. Blood SpO 2 and gastric mucosal pHi in the SH and SHG groups increased more quickly than in the NS group, while IAP was significantly decreased in the SH and SHG group. Moreover, the serum tumor necrosis factor-α, interleukin-8 and C-reactive protein levels in the SH and SHG groups were obviously lower than in the NS group at each time point. 122018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). 13Persistence and clearance of viral RNA in 2019 novel coronavirus disease rehabilitation patients显示文摘Background:A patient’s infectivity is determined by the presence of the virus in different body fluids,secretions,and excreta.The persistence and clearance of viral RNA from different specimens of patients with 2019 novel coronavirus disease(COVID-19)remain unclear.This study analyzed the clearance time and factors influencing 2019 novel coronavirus(2019-nCoV)RNA in different samples from patients with COVID-19,providing further evidence to improve the management of patients during convalescence.Methods:The clinical data and laboratory test results of convalescent patients with COVID-19 who were admitted to from January 20,2020 to February 10,2020 were collected retrospectively.The reverse transcription polymerase chain reaction(RT-PCR)results for patients’oropharyngeal swab,stool,urine,and serum samples were collected and analyzed.Convalescent patients refer to recovered non-febrile patients without respiratory symptoms who had two successive(minimum 24 h sampling interval)negative RT-PCR results for viral RNA from oropharyngeal swabs.The effects of cluster of differentiation 4(CD4)+T lymphocytes,inflammatory indicators,and glucocorticoid treatment on viral nucleic acid clearance were analyzed.Results:In the 292 confirmed cases,66 patients recovered after treatment and were included in our study.In total,28(42.4%)women and 38 men(57.6%)with a median age of 44.0(34.0-62.0)years were analyzed.After in-hospital treatment,patients’inflammatory indicators decreased with improved clinical condition.The median time from the onset of symptoms to first negative RT-PCR results for oropharyngeal swabs in convalescent patients was 9.5(6.0-11.0)days.By February 10,2020,11 convalescent patients(16.7%)still tested positive for viral RNA from stool specimens and the other 55 patients’stool specimens were negative for 2019-nCoV following a median duration of 11.0(9.0-16.0)days after symptom onset.Among these 55 patients,43 had a longer duration until stool specimens were negative for viral RNA than for throat swabs,with a median delay of 2.0(1.0-4.0)days.Results for only four(6.9%)urine samples were positive for viral nucleic acid out of 58 cases;viral RNA was still present in three patients’urine specimens after throat swabs were negative.Using a multiple linear regression model(F=2.669,P=0.044,and adjusted R2=0.122),the analysis showed that the CD4+T lymphocyte count may help predict the duration of viral RNA detection in patients’stools(t=-2.699,P=0.010).The duration of viral RNA detection from oropharyngeal swabs and fecal samples in the glucocorticoid treatment group was longer than that in the non-glucocorticoid treatment group(15 days vs.8.0 days,respectively;t=2.550,P=0.013)and the duration of viral RNA detection in fecal samples in the glucocorticoid treatment group was longer than that in the non-glucocorticoid treatment group(20 days vs.11 days,respectively;t=4.631,P<0.001).There was no statistically significant difference in inflammatory indicators between patients with positive fecal viral RNA test results and those with negative results(P>0.05).Conclusions:In brief,as the clearance of viral RNA in patients’stools was delayed compared to that in oropharyngeal swabs,it is important to identify viral RNA in feces during convalescence.Because of the delayed clearance of viral RNA in the glucocorticoid treatment group,glucocorticoids are not recommended in the treatment of COVID-19,especially for mild disease.The duration of RNA detection may relate to host cell immunity.Yun Ling Shui-Bao Xu Yi-Xiao Lin Di Tian Zhao-Qin Zhu Fa-Hui Dai Fan Wu Zhi-Gang Song Wei Huang Jun Chen Bi-Jie Hu Sheng Wang En-Qiang Mao Lei Zhu Wen-Hong Zhang Hong-Zhou Lu 2020Chinese Medical Journal2020,,9:58

15Contrasting trends of PM2.5 and surface-ozone concentrations in China from 2013 to 2017显示文摘Although much attention has been paid to investigating and controlling air pollution in China,the trends of air-pollutant concentrations on a national scale have remained unclear.Here,we quantitatively investigated the variation of air pollutants in China using long-term comprehensive data sets from 2013 to 2017,during which Chinese government made major efforts to reduce anthropogenic emission in polluted regions.Our results show a significant decreasing trend in the PM2.5 concentration in heavily polluted regions of eastern China,with an annual decrease of〜7%compared with measurements in 2013.The measured decreased concentrations of SO2>NO2 and CO(a proxy for anthropogenic volatile organic compounds)could explain a large fraction of the decreased PM2.5 concentrations in different regions.As a consequence;the heavily polluted days decreased significantly in corresponding regions.Concentrations of organic aerosol;nitrate;sulfate,ammonium and chloride measured in urban Beijing revealed a remarkable reduction from 2013 to 2017;connecting the decreases in aerosol precursors with corresponding chemical components closely.However;surface-ozone concentrations showed increasing trends in most urban stations from 2013 to 2017;which indicates stronger photochemical pollution.The boundary-layer height in capital c让ies of eastern China showed no significant trends over the Beijing-Tianjin-Hebei,Yangtze River Delta and Pearl River Delta regions from 2013 to 2017;which confirmed the reduction in anthropogenic emissions.Our results demonstrated that the Chinese government was successful in the reduction of particulate matter in urban areas from 2013 to 2017,although the ozone concentration has increased significantly;suggesting a more complex mechanism of improving Chinese air quality in the future.Yonghong Wang Wenkang Gao Shuai Wang Tao Song Zhengyu Gong Dongsheng Ji Lili Wang Zirui Liu Guiqian Tang Yanfeng Huo Shili Tian Jiayun Li Mingge Li Yuan Yang Biwu Chu Tuukka Petaja Veli-Matti Kerminen Hong He Jiming Hao Markku Kulmala Yuesi Wang Yuanhang Zhang 2020National Science Review2020,7,8:51

19L-forms of H. pylori显示文摘AIM: To study the occurrence of L-forms of H. pyloriinfection in patients with peptic ulcers and its association with possible changes of cellular immune function in the patients.METHODS: Endoscopic biopsy specimens of gastric antrum and gastric corpus were taken from 228 patients with peptic ulcers and inoculated into Skirrow selective medium for H.pylorivegetative forms and special medium for H. pylori Lforms, followed by bacterial isolation and identification. And peripheral venous blood of the patients was taken to detect the percentage of CD3+, CD4+ and CD8+ with biotin-streptavidin (BSA) and the level of IL-2, IL-6 and IL-8 with ElISA.RESULTS: (1) The detection rates of H. pyloriL-forms and vegetative forms in the patients were 50.88 % (116/228)and 64.91 % (148/228) respectively, and the co-infection rate of H. pyloriL-forms and vegetative forms was 78.38 % (116/148). To be more exact, the detection rates of H. pylori L-forms in male and female patients were 57.04 % (77/135) and 41.94 % (39/93) respectively, and statistics found significant difference between them (P<0.05). Furthermore, the detection rates of H. pyloriL-forms in patients aged 14 years-, 30 years-, 40 years- and 50 years- were 31.91%(15/47), 42.86 % (24/56), 56.94 % (41/72) and 67.92 %(36/53) respectively, and there was significant difference between them (P<0.011). (2) The percentages of CD3+, CD4+,CD8+, the ratio of CD4+/CD8+, and the level of IL-2, IL-6,IL-8 in H. pylori-positive patients were (52.59±5.44) %,(35.51±5.74) %, (27.77±8.64) %, (1.56±0.51), (2.66±0.47)mg/L, (108.62±5.85) ng/L and (115.79±7.18) ng/Lrespectively, compared with those in H. pylori-negative patients, the percentages of CD3+, CD4+ and the ratio of CD4+/CD8+ decreased, but the level of IL-2, IL-6 increased, and the difference was significant (P<0.001-P<0.011).Moreover, the percentages of CD3+, CD4+, CD8+, the ratio of CD4+/CD8+, and the level of IL-2, IL-6, IL-8 in the patients with mixed infection of bothH. pyloriL-forms and vegetative forms were (51.69±5.28) %, (34.75±5.89) %, (27.15±7.45) %, (1.48±0.47), (2.16±0.38) mg/L, (119.45±5.44) ng/L and (123.64±6.24) ng/L respectively, compared with those in patients with simple infection of H. pylorivegetative forms,the percentage of CD4+, the ratio of CD4+/CD8+ and the level of IL-2 increased, but the level of IL-6 and IL-8decreased, statistical difference was found between them (P<0.001-P<0.05).CONCLUSION: L-forms variation often occurs in patients with peptic ulcers who are infected byH. pylori, which is commonly found in male patients and related to ages. 20Mapping QTLs with epistatic effects and QTL×environment interactions for plant height using a doubled haploid population in cultivated wheat显示文摘Quantitative trait loci (QTLs) for plant height in wheat (Triticum aestivum L.) were studied using a set of 168 doubled haploid (DH) lines, which were derived from the cross Huapei 3/Yumai 57. A genetic linkage map was constructed using 283 SSR and 22 EST-SSR markers. The DH population and the parents were evaluated for wheat plant height in 2005 and 2006 in Tai’an and 2006 in Suzhou. QTL analyses were performed using the software of QTLNetwork version 2.0 based on the mixed linear model. Four additive QTLs and five pairs of epistatic effects were detected, which were distributed on chromosomes 3A, 4B, 4D, 5A, 6A, 7B, and 7D. Among them, three additive QTLs and three pairs of epistatic QTLs showed QTL×environment interactions (QEs). Two major QTLs, Qph4B and Qph4D, which accounted for 14.51% and 20.22% of the phenotypic variation, were located similar to the reported locations of the dwarfing genes Rht1 and Rht2, respectively. The Qph3A-2 with additive effect was not reported in previous linkage mapping studies. The total QTL effects detected for the plant height explained 85.04% of the phenotypic variation, with additive effects 46.07%, epistatic effects 19.89%, and QEs 19.09%. The results showed that both additive effects and epistatic effects were important genetic bases of wheat plant height, which were subjected to environmental modifications, and caused dramatic changes in phenotypic effects.
