| 1 | Downstaging and resection of hepatocellular carcinoma in patients with extrahepatic metastases after stereotactic therapy显示文摘Background:A combination of tyrosine kinase inhibitors(TKIs)and anti-PD-1 antibodies with local regional therapy has elicited yield substantial clinical benefits in patients who have hepatocellular carcinoma(HCC)with extrahepatic metastases.Using this treatment strategy to convert HCC patients with extrahepatic metastases from unresectable to resectable has not yet been reported.Methods:Consecutive hepatocellular carcinoma patients with extrahepatic metastases who received first-line therapy with a combination of TKIs and anti-PD-1 antibodies and at least one local regional therapy were analysed.Results:Nine patients with localized disease who received first-line systemic therapy were enrolled.At baseline,all of them had oligometastatic disease,namely,Barcelona Clinic Liver Cancer stage C(or Chinese Liver Cancer stage IIIB).The most common treatment administered was lenvatinib plus anti-PD-1 antibody and transarterial chemoembolization,and the median time span from systemic therapy to surgery was 3.2(IQR,2.8-6.2)months.Three patients achieved a pathological complete response.Six patients underwent laparoscopic surgery,and the other 3 patients underwent open surgery.After a median follow-up of 10.2(IQR,8.6-20.0)months,7 patients survived without disease recurrence,and 2 experienced tumour recurrence.All patients had any-grade AEs,and 55.6%of the patients experienced grade 3 AEs.Fatigue was the most common AE,followed by elevated aminotransferase levels and hypertension.Conclusions:Stereotactic therapy is a feasible conversion therapy for HCC patients with extrahepatic metastases to become resectable.This is the first study to analyse therapeutic outcomes of patients receiving these therapies for HCC with extrahepatic metastases. | Xiaobo Yang Haifeng Xu Bangyou Zuo Xu Yang Jin Bian Junyu Long Dongxu Wang Junwei Zhang Cong Ning Yanyu Wang Ziyu Xun Yunchao Wang Xin Lu Yilei Mao Xinting Sang Haitao Zhao | 2021 | Hepatobiliary Surgery and Nutrition2021,10,4: | 17 |
| 2 | Anatomical variation of infra-pyloric artery origination: A prospective multicenter observational study (IPA-Origin)显示文摘Objective: Infra-pyloric artery(IPA) is an important anatomical landmark in treatment of gastric cancer and is the key vessel for pylorus-preserving gastrectomy and subgroup of infra-pyloric lymph nodes. However, its anatomical variation is not thoroughly understood. Our study aimed to clarify the origination of the IPA.Methods: We did this prospective, multicenter, open-label, observational study at gastric surgery departments of34 hospitals in China. Gastric cancer patients aged 18 years or older and scheduled to undergo elective total or distal gastrectomy were assigned. During the surgery, IPA dissecting and exposing the origination point with photographs or video clips were required. The primary outcome was the origination of the IPA. Analysis of variance, χ~2 tests and Fisher's tests were used to analyze the differences between groups. The study is registered at Clinicaltrials.gov(No. NCT03071237).Results: Between May 8 and July 31, 2017, 429 patients were assigned for the study, and 419(97.7%) patients had the IPA dissected and recorded through photograph or video and were included in the primary outcome analysis. The median age was 62 years old, and 73.7% were male. Among the patients, 78.5% received laparoscopic surgery. Single IPA origination was identified in 398(95.0%) patients, including gastroduodenal artery(GDA) in154(36.8%) patients, anterior superior pancreaticoduodenal artery(ASPDA) in 130(31.0%) patients, and right gastroepiploic artery(RGEA) in 114(27.2%) patients. Fifteen(3.6%) patients were identified with multiple IPA and 6(1.4%) patients were identified as IPA absence. The differences in the distribution of surgical approach(P=0.003) and geographic area(P=0.030) were statistically significant. | 4 | The five major autoimmune diseases increase the risk of cancer:epidemiological data froma large-scale cohort study in China显示文摘Background:Cancer incidence and mortality have received critical attention during the long-term management of morbidities in patients with autoimmune diseases(AIDs).This study aimed to investigate and compare the risk of cancer associated with five major AIDs in a large-scale Chinese cohort.Methods:A total of 8,120 AID patients consecutively admitted to a national tertiary referral center in China were included and followed-up for 38,726.55 patient-years,including those with systemic lupus erythematosus(SLE),rheumatoid arthritis(RA),Sjoren’s syndrome(SS),systemic scleroderma(SSc),and idiopathic inflammatory myositis(IIM).Demographic data,cancer incidence,predilecting sites and cancer onset time were recorded and compared among the five AIDs.Results:Four hundred and thirty(5.3%)patients developed cancer.Their median agewas 57.5 years and AID durationwas 79.8 months.The estimated total standardized incidence ratio(SIR)of cancer in AIDs patients was 3.37,with the highest SIR observed in IIM(4.31),followed by RA(3.99),SSc(3.77),SS(2.88)and SLE(2.58).The increased SIR of cancers in AID patients showed a female predominance(female vs.male:3.59 vs.2.77)and younger patient involvement(age<50 vs.≥50 years:4.88 vs.3.04).Patientswith SLE had increased SIRs for developing hematologic malignancies and solid tumors located in the urinary bladder,corpus uteri and cervix uteri.Patients with SS had a significantly high SIR for developing non-Hodgkin’s lymphoma.Within 3 years of IIM diagnosis,74.6%of the patients developed cancer and they had a high risk of ovarian cancer.RA was associated with a wide distribution of scancers,including non-Hodgkin’s lymphoma,gynecologic,urinary tract,thyroid gland and lung cancers.SSc patients had increased SIRs for developing cervical uterine,lung,and breast cancers.Conclusion:Patients with five major AIDs in China had an increased risk of developing cancer,with a predominance in women and younger patients,although cancer incidence,predilection sites and cancer onset time may vary greatly in each AID entity. | Ziyue Zhou Huazhen Liu Yiying Yang Jingya Zhou Lidan Zhao Hua Chen Yunyun Fei Wen Zhang Mengtao Li Yan Zhao Xiaofeng Zeng Fengchun Zhang Huaxia Yang Xuan Zhang | 2022 | Cancer Communications2022,42,5: | 4 |
| 5 | Formulation of Determining the Gravity Potential Difference Using Ultra-High Precise Clocks via Optical Fiber Frequency Transfer Technique显示文摘Based on gravity frequency shift effect predicted by general relativity theory, this study discusses an approach for determining the gravity potential(geopotential) difference between arbitrary two points P and Q by remote comparison of two precise optical clocks via optical fiber frequency transfer. After synchronization, by measuring the signal's frequency shift based upon the comparison of bidirectional frequency signals from P and Q oscillators connected with two optical atomic clocks via remote optical fiber frequency transfer technique, the geopotential difference between the two points could be determined, and its accuracy depends on the stabilities of the optical clocks and the frequency transfer comparison technique. Due to the fact that the present stability of optical clocks achieves 1.6×10-18 and the present frequency transfer comparison via optical fiber provides stabilities as high as 10-19 level, this approach is prospective to determine geopotential difference with an equivalent accuracy of 1.5 cm. | 7 | Doped graphene/carbon black hybrid catalyst giving enhanced oxygen reduction reaction activity with high resistance to corrosion in proton exchange membrane fuel cells显示文摘Nitrogen doping of the carbon is an important method to improve the performance and durability of catalysts for proton exchange membrane fuel cells by platinum–nitrogen and carbon–nitrogen bonds. This study shows that p-phenyl groups and graphitic N acting bridges linking platinum and the graphene/carbon black(the ratio graphene/carbon black = 2/3) hybrid support materials achieved the average size of platinum nanoparticles with(4.88 ± 1.79) nm. It improved the performance of the lower-temperature hydrogen fuel cell up to 0.934 W cm^(-2) at 0.60 V, which is 1.55 times greater than that of commercial Pt/C. Doping also enhanced the interaction between Pt and the support materials, and the resistance to corrosion, thus improving the durability of the low-temperature hydrogen fuel cell with a much lower decay of 10 mV at 0.80 A cm^(-2) after 30 k cycles of an in-situ accelerated stress test of catalyst degradation than that of 92 mV in Pt/C, which achieves the target of Department of Energy(<30 mV). | 10 | A real-time and modular approach for quick detection and mechanism exploration of DPIs with different carrier particle sizes显示文摘Dry powder inhalers(DPIs) had been widely used in lung diseases on account of direct pulmonary delivery, good drug stability and satisfactory patient compliance. However, an indistinct understanding of pulmonary delivery processes(PDPs) hindered the development of DPIs. Most current evaluation methods explored the PDPs with over-simplified models, leading to uncompleted investigations of the whole or partial PDPs. In the present research, an innovative modular process analysis platform(MPAP) was applied to investigate the detailed mechanisms of each PDP of DPIs with different carrier particle sizes(CPS). The MPAP was composed of a laser particle size analyzer, an inhaler device,an artificial throat and a pre-separator, to investigate the fluidization and dispersion, transportation,detachment and deposition process of DPIs. The release profiles of drug, drug aggregation and carrier were monitored in real-time. The influence of CPS on PDPs and corresponding mechanisms were explored. The powder properties of the carriers were investigated by the optical profiler and Freeman Technology four powder rheometer. The next generation impactor was employed to explore the aerosolization performance of DPIs. | 11 | Test on dynamic characteristics of subgrade of heavy-haul railway in cold regions显示文摘Dynamic characteristics of heavy-haul railway subgrade under vibratory loading in cold regions are investigated via low-temperature dynamic triaxial tests with multi-stage cyclic loading process. The relationship between dynamic shear stress and dynamic shear strain of frozen soil of subgrade under train loading and the influence of freezing temperatures on dynamic constitutive relation, dynamic shear modulus and damping ratio are observed in this study. Test results show that the dynamic constitutive relations of the frozen soils with different freezing temperatures comply with the hyperbolic model, in which model parameters a and b decrease with increasing freezing temperature. The dynamic shear modulus of the frozen soils decreases with increasing dynamic shear strains initially, followed by a relatively smooth attenuation tendency, whereas increases with decreasing freezing temperatures. The damping ratios decrease with decreasing freezing temperatures. | 18 | Effect of layer thickness on the flexural property and microstructure of 3D-printed rhomboid polymer-reinforced cemented tailing composites显示文摘For mines with poor ore bodies and surrounding rocks,the general mining method does not allow the ore to be extracted from underground safely and efficiently.For these mines,the downward layered filling mining technique is undoubtedly the most suitable mining method.The downward filling mining technique may eliminate the troubles relating to poor ore deposit conditions,such as production safety,ore loss rate,and depletion rate.However,in this technique,the safety of the artificial roof of the next stratum is of paramount importance.Cementitious tailings backfilling(CTB)that is not sufficiently cemented and causes collapses could threaten ore production.This paper explores a diamond-shaped composite structure to mimic the stability of a glued false roof in an actual infill mine based on the recently emerged three-dimensional(3D)printing technology.Experimental means such as three-point bending and digital image correlation(DIC)techniques were used to explore the flexural characteristics of 3D construction specimens and CTB combinations with different cement/tailings weight ratios at diverse layer heights.The results show that the 3D structure with a 14-mm ply height and CTB has strong flexural characteristics,with a maximum deflection value of 30.1 mm,while the 3D-printed rhomboid polymer(3D-PRP)structure with a 26-mm ply height is slightly worse in terms of flexural strength characteristics,but it has a higher maximum flexural strength of 2.83 MPa.A combination of 3D structure and CTB has more unique mechanical properties than CTB itself.This research work offers practical knowledge on the artificial roof performance of the downward layered filling mining technique and builds a scientific knowledge base regarding the successful application of CTB material in mines. | Ziyue Zhao Shuai Cao Erol Yilmaz | 2023 | International Journal of Minerals,Metallurgy and Materials2023,30,2: | 1 |
| 19 | Enhanced ethanol sensing performance of N-doped ZnO derived from ZIF-8显示文摘Metal oxides derived from metal-organic framework(MOF) have attracted considerable attention due to its excellent performance and unique structure. Doping is considered as an effective method to improve gas-sensing performance. However, nonmetal doped metal oxides derived from MOF as gas-sensing materials have not been reported. Within this work, N atoms were successfully doped into the lattice of ZnO nanoparticles using ZIF-8 as a self-sacrificial template through a thermal treatment process with the assistant of urea. The obtained N-ZnO exhibited competitive ethanol-sensing performance, in which the response value of N-ZnO-5 to 100 ppm ethanol reached 115 at 190 ℃ with a satisfactory selectivity. It was found that the N-doping in ZnO facilitated the formation of oxygen vacancy that promoted the generation of adsorbed oxygen species to achieve the enhanced gas-sensing performance. | 20 | Treatment and survival status of patients with EGFR mutation-positive stage Ⅳ lung adenocarcinoma: five-year follow-up results in the Ordos Area of Inner Mongolia, China显示文摘Objective We aimed to determine the epidermal growth factor receptor(EGFR) mutation status and treatment survival of patients with stage Ⅳ lung adenocarcinoma living in the Ordos area of Inner Mongolia, China.Methods EGFR testing and first-line tyrosine kinase inhibitor(TKI) treatment rates of patients with stage Ⅳ lung adenocarcinoma were analyzed from June 2012 to June 2016. Kaplan-Meier survival curves were constructed to compare patients who received different treatment strategies and those harboring different EGFR mutation statuses.Results EGFR testing and mutation rates were 65.60% and 52.90%, respectively, and improved continuously from June 2012 to June 2016. Among patients with EGFR mutations, 38.9% had EGFR 19 del, 48.2% had L858 R, 4.2% had co-existing mutations in exons 19 and 21, and 8.4% had uncommon mutations. The median overall survival(OS) was 29.5, 26.5, and 16.0 months for patients receiving both TKI and chemotherapy, TKI alone, and chemotherapy alone, respectively(P = 0.047). The OS was 26.5 and 30.0 months for patients harboring EGFR 19 del and L858 R mutations, respectively(P = 0.096).Conclusion The high OS rates of stage Ⅳ lung adenocarcinoma patients living in the Ordos area may be attributed to continuous improvements in EGFR testing and first-line TKI treatment rates.
