1Impact of an innovative financing and payment model on tuberculosis patients'financial burden:is tuberculosis care more affordable for the poor?显示文摘Background:In response to the high financial burden of health services facing tuberculosis(TB)patients in China,the China-Gates TB project,PhaseⅡ,has implemented a new financing and payment model as an important component of the overall project in three cities in eastern,central and western China.The model focuses on increasing the reimbursement rate for TB patients and reforming provider payment methods by replacing fee-for-service with a case-based payment approach.This study investigated changes in out-of-pocket(OOP)health expenditure and the financial burden on TB patients before and after the interventions,with a focus on potential differential impacts on patients from different income groups.Methods:Three sample counties in each of the three prefectures:Zhenjiang,Yichang and Hanzhong were chosen as study sites.TB patients who started and completed treatment before,and during the intervention period,were randomly sampled and surveyed at the baseline in 2013 and final evaluation in 2015 respectively.OOP health expenditure and percentage of patients incurring catastrophic health expenditure(CHE)were calculated for different income groups.OLS regression and Iogit regression were conducted to explore the intervention's impacts on patient OOP health expenditure and financial burden after adjusting for other covariates.Key-informant interviews and focus group discussions were conducted to understand the reasons for any observed changes.Results:Data from 738(baseline)and 735(evaluation)patients were available for analysis.Patient mean OOP health expenditure increased from RMB 3576 to RMB 5791,and the percentage of patients incurring CHE also increased after intervention.The percentage increase in OOP health expenditure and the likelihood of incurring CHE were significantly lower for patients from the highest income group as compared to the lowest.Qualitative findings indicated that increased use of health services not covered by the standard package of the model was likely to have caused the increase in financial burden.Conclusions:The implementation of the new financing and payment model did not protect patients,especially those from the lowest income group,from financial difficulty,due partly to their increased use of health service.More financial resources should be mobilized to increase financial protection,particularly for poor patients,while cost containment strategies need to be developed and effectively implemented to improve the effective coverage of essential healthcare in China.Wei-Xi Jiang Qian Long Henry Lucas Di Dong Jia-Ying Chen Li Xiang Qiang Li Fei Huang Hong Wang Chris Elbers Frank Cobelens Sheng-Lan Tang 2019Infectious Diseases of Poverty2019,8,2:4
2Can intravesical prostatic protrusion predict bladder outlet obstruction even in men with good flow?显示文摘Objective:Men with benign prostate hyperplasia(BPH)with good urinary flow may still have bladder outlet obstruction(BOO).Intravesical prostatic protrusion(IPP)has been shown to be able to predict BOO.We aim to investigate the use of IPP to predict BOO in men with good urinary flow.Methods:One hundred and fourteen consecutive men(>50 years old)presenting with lower urinary tract symptoms suggestive of BPH were recruited in 2001 and 2002.They were evaluated with serum prostate specific antigen(PSA),uroflowmetry and transabdominal ultrasound measurement of IPP and prostate volume(PV).Pressure-flow urodynamic studies were performed on all men and BOO was defined by BOO index>40.Men with Qmax12.0 mL/s were considered to have good flow.Results:Among the 114 men,61 patients had good urinary flow.Their median age,PV and Qmax were 66 years,32.9 mm3 and 14.5 mL/s respectively.14/61(23.0%)patients had BOO and their distribution of IPP were as follows:Grade 1 e 0/20(0%)obstructed,Grade 2 e 6/22(27.3%)and Grade 3 e 8/19(42.1%).Sensitivity of Grade 2/3 IPP for BOO was 100% while specificity of Grade 3 IPP was 76.6%.The area-under-curve(AUC)for IPP was greater than that for PV(0.757 vs.0.696).Conclusion:Even in men with good flow,high grades of IPP were more likely to have BOO and hence,may be a useful adjunct to predict BOO.Alvin Lee Han Jie Lee Kok Bin Lim Hong Hong Huang Henry Ho Keong Tatt Foo 2016Asian Journal of Urology2016,3,1:4
3First results on ^(76)Ge neutrinoless double beta decay from CDEX-1 experiment显示文摘We report the first results on ^(76)Ge neutrinoless double beta decay from stage one of the China dark-matter experiment(CDEX).A p-type point-contact high-purity germanium detector with a mass of 994 g has been installed to detect neutrinoless double beta decay events, as well as to directly detect dark matter particles. An exposure of 304 kg d has been analyzed over a wide spectral band from 500 keV to 3 MeV. The average event rate obtained was about 0.012 counts per keV per kg per day over the 2.039 MeV energy range. The half-life of ^(76)Ge neutrinoless double beta decay derived based on this result is T^(0ν)1/2>6.4×10^(22) yr(90% C.L.). 18Performances of a prototype point-contact germanium detector immersed in liquid nitrogen for light dark matter search显示文摘The CDEX-10 experiment searches for light weakly interacting massive particles, a form of dark matter, at the China Jinping Underground Laboratory, where approximately 10 kg of germanium detectors are arranged in an array and immersed in liquid nitrogen. Herein, we report on the experimental apparatus, detector characterization, and spectrum analysis of one prototype detector. Owing to the higher rise-time resolution of the CDEX-10 prototype detector as compared with CDEX-1 B, we identified the origin of an observed category of extremely fast events. For data analysis of the CDEX-10 prototype detector, we introduced and applied an improved bulk/surface event discrimination method. The results of the new method were compared to those of the CDEX-1 B spectrum.
