| 2 | Factors associated with early recurrence after curative surgery for gastric cancer显示文摘AIM: To characterize patterns of gastric cancer recurrence and patient survival and to identify predictors of early recurrence after surgery.METHODS: Clinicopathological data for 417 consecutive patients who underwent curative resection for gastric cancer were retrospectively analyzed. Tumor and node status was reclassified according to the 7th edition of the American Joint Committee on Cancer tumor-node-metastasis classification for carcinoma of the stomach. Survival data came from both the patients' follow-up records and telephone followups.Recurrent gastric cancer was diagnosed based on clinical imaging, gastroscopy with biopsy, and/or cytological examination of ascites, or intraoperative findings in patients who underwent reoperation.Predictors of early recurrence were compared in patients with pT1 and pT2-4a stage tumors. Pearson's χ 2 test and Fisher's exact test were used to compare differences between categorical variables. Survival curves were constructed using the Kaplan-Meier method and compared via the log-rank test. Variables identified as potentially important for early recurrence using univariate analysis were determined by multivariate logistic regression analysis.RESULTS: Of 417 gastric cancer patients, 80(19.2%)were diagnosed with early gastric cancer and the remaining 337(80.8%) were diagnosed with locally advanced gastric cancer. After a median follow-up period of 56 mo, 194 patients(46.5%) experiencedrecurrence. The mean time from curative surgery to recurrence in these 194 patients was 24 ± 18 mo(range, 1-84 mo). Additionally, of these 194 patients,129(66.5%) experienced recurrence within 2 years after surgery. There was no significant difference in recurrence patterns between early and late recurrence(P < 0.05 each). For pT1 stage gastric cancer, tumor size(P = 0.011) and pN stage(P = 0.048) were associated with early recurrence of gastric tumors.Patient age, pT stage, pN stage, Lauren histotype,lymphovascular invasion, intraoperative chemotherapy,and postoperative chemotherapy were independent predictors of early recurrence in patients with pT2-4a stage gastric cancer(P < 0.05 each).CONCLUSION: Age, pT stage, pN stage, Lauren histotype, lymphovascular invasion, intraoperative chemotherapy, and postoperative chemotherapy are independent factors influencing early recurrence of pT2-4a stage gastric cancer. | Wei-Ming Kang Qing-Bin Meng Jian-Chun Yu Zhi-Qiang Ma Zhi-Tian Li | 2015 | World Journal of Gastroenterology2015,21,19: | 37 |

| 4 | Laparoscopic-endoscopic cooperative surgery for gastric submucosal tumors显示文摘AIM:To assess the feasibility,safety,and advantages of minimally invasive laparoscopic-endoscopic cooperative surgery(LECS)for gastric submucosal tumors(SMT).METHODS:We retrospectively analyzed 101 consecutive patients,who had undergone partial,proximal,or distal gastrectomy using LECS for gastric SMT at Peking Union Medical College Hospital from June 2006to April 2013.All patients were followed up by visit or telephone.Clinical data,surgical approach,pathological features such as the size,location,and pathological type of each tumor;and follow-up results were analyzed.The feasibility,safety and effectiveness of LECS for gastric SMT were evaluated,especially for patients with tumors located near the cardia or pylorus.RESULTS:The 101 patients included 43(42.6%)menand 58(57.4%)women,with mean age of 51.2±13.1 years(range,14-76 years).The most common symptom was belching.Almost all(n=97)patients underwent surgery with preservation of the cardia and pylorus,with the other four patients undergoing proximal or distal gastrectomy.The mean distance from the lesion to the cardia or pylorus was 3.4±1.3 cm,and the minimum distance from the tumor edge to the cardia was 1.5 cm.Tumor pathology included gastrointestinal stromal tumor in 78 patients,leiomyoma in 13,carcinoid tumors in three,ectopic pancreas in three,lipoma in two,glomus tumor in one,and inflammatory pseudotumor in one.Tumor size ranged from 1 to8.2 cm,with 65(64.4%)lesions<2 cm,32(31.7%)>2 cm,and four>5 cm.Sixty-six lesions(65.3%)were located in the fundus,21(20.8%)in the body,10(9.9%)in the antrum,three(3.0%)in the cardia,and one(1.0%)in the pylorus.During a median follow-up of 28 mo(range,1-69 mo),none of these patients experienced recurrence or metastasis.The three patients who underwent proximal gastrectomy experienced symptoms of regurgitation and belching.CONCLUSION:Laparoscopic-endoscopic cooperative surgery is feasible and safe for patients with gastric submucosal tumor.Endoscopic intraoperative localization and support can help preserve the cardia and pylorus during surgery. | Wei-Ming Kang Jian-Chun Yu Zhi-Qiang Ma Zi-Ran Zhao Qing-Bin Meng Xin Ye | 2013 | World Journal of Gastroenterology2013,19,34: | 17 |
| 5 | Esophagojejunostomy after laparoscopic total gastrectomy by Or VilTM or hemi-double stapling technique显示文摘AIM: To investigate the feasibility, advantages and disadvantages of two types of anvil insertion techniques for esophagojejunostomy after laparoscopic total gastrectomy.METHODS: This was an open-label prospective cohort study. Laparoscopy-assisted radical total gastrectomy with D2 lymph node dissection was performed in 84 patients with primary non-metastatic gastric cancer confirmed by pre-operative histological examination. Overweight patients were excluded, as well as patients with peritoneal dissemination and invasion of adjacent organs. After total gastrectomy, all patients were randomized into two groups. Patients in Group Ⅰ underwent esophagojejunostomy using a transorally-inserted anvil(Or VilTM), while patients in Group Ⅱ underwent esophagojejunostomy using the hemi-double stapling technique(HDST). Both types of esophagojejunostomy were performed under laparoscopy. Patients' baseline characteristics, preoperative characteristics, perioperative characteristics, short-term postoperative outcomes and operation cost were comparedbetween the two groups. The primary endpoint was evaluation of the surgical outcome(operating time, time of digestive tract reconstruction and time of anvil insertion) and the medical cost of each operation(operation cost and total cost of hospitalization). The secondary endpoints were time to solid diet, post-surgical hospitalization time, time to defecation, time to ambulation and intra-operative blood loss. In addition, complications were assessed and compared. RESULTS: Laparoscopic total gastrectomy and esophagojejunostomy were successfully performed in all 84 patients, without conversion to laparotomy. There were no significant differences in the operative time and time for total gastrectomy between the two groups(287.8 ± 38.4 min vs 271.8 ± 46.1 min, P = 0.09, and 147.7 ± 31.6 min vs 159.8 ± 33.8 min, P = 0.09, respectively). The time for digestive tract reconstruction and for anvil insertion were significantly decreased in Group Ⅱ compared with Group I(47.8 ± 12.1 min vs 55.4 ± 15.7 min, P = 0.01, and 12.6 ± 4.7 min vs 18.7 ± 7.5 min, P = 0.001, respectively). Intraoperative blood loss(96.4 ± 32.7 m L vs 88.2 ± 36.9 m L, P = 0.28), time to defecation(3.5 ± 0.9 d vs 3.2 ± 1.1 d, P = 0.12), time to ambulation(3.9 ± 0.7 d vs 3.6 ± 1.1 d, P = 0.12), time to solid diet(7.6 ± 1.4 d vs 8.0 ± 2.7 d, P = 0.31) and total hospitalization(10.6 ± 2.6 d vs 10.8 ± 3.5 d, P = 0.80) were similar between the two groups. In addition, the total costs of hospitalization were similar between the two groups(73848.7 ± 11781.0 RMB vs 70870.3 ± 14003.5 RMB, P = 0.296), but operation cost was significantly higher in Group I compared with Group Ⅱ(32401.9 ± 1981.6 RMB vs 26961.9 ± 2293.8 RMB, P < 0.001).CONCLUSION: Anvil insertion was faster and easier using the HDST technique compared with Or VilTM, and was more cost-effective. Although metal-organic framework (MOF) scaffolds are promisingstrategies for bone tissue regeneration, their potential use for exosome loading remains unexplored. In this study,motivated by the potential advantages of hADSCs-Exos and Mg-GA MOF, we designed and synthesized anexosome-functionalized cell-free PLGA/Mg-GA MOF (PLGA/Exo-Mg-GA MOF) scaffold, taking using of thebenefits of hADSCs-Exos, Mg2+, and gallic acid (GA) to construct unique nanostructural interfaces to enhanceosteogenic, angiogenic and anti-inflammatory capabilities simultaneously. Our in vitro work demonstrated thebeneficial effects of PLGA/Exo-Mg-GA MOF composite scaffolds on the osteogenic effects in human bonemarrow-derived mesenchymal stem cells (hBMSCs) and angiogenic effects in human umbilical endothelial cells(HUVECs). Slowly released hADSCs-Exos from composite scaffolds were phagocytosed by co-cultured cells,stabilized the bone graft environment, ensured blood supply, promoted osteogenic differentiation, and acceleratedbone reconstruction. Furthermore, our in vivo experiments with rat calvarial defect model showed thatPLGA/Exo-Mg-GA MOF scaffolds promoted new bone formation and satisfactory osseointegration. | 12 | Distribution Patterns and Associations of Dominant Tree Species in a Mixed Coniferous-Broadleaf Forest in the Changbai Mountains显示文摘In 2012 a plot was established with 1-ha area in a mixed coniferous-broadleaf forest in the Changbai Mountains, northeastern China for examining local forest processes, structure and succession. A method of O-ring statistics(paircorrelation function) was applied to analyze the spatial patterns and associations of the dominant species within different vertical layers. After the evaluation by their importance values, six tree species(or group)(i.e. Abies nephrolepis, Picea jezoensis, Pinus koraiensis, Tilia amurensis, and species group of Betula ssp. and species group of Acer ssp.) were determined as dominant trees species. It was found that some of these species exhibited closely clustered distributions at fine distances. As spatial distance increased, a random or even regular distribution gradually appeared with the exception of the upper layers of A. nephrolepis and P. koraiensis, and the lower layers of P. jezoensis, P. koraiensis and Betula ssp., which were substantially randomly distributed. Intra- and inter-species spatial associations varied in accordance with species, tree height and reciprocal distances. Positive associations were observedbetween the lower and upper height classes of trees of the same species(except for that of P. jezoensis) at fine distances. This may be owing to limited seed dispersal and geological heterogeneity. The aggregation intensity declines with increasing distances and this consistent with the predictions of self-thinning. Some coniferous trees(e.g. Pinus koraiensis) in the lower height class were positively associated with T. amurensis and group of Betula ssp. of the upper height class at some distances, suggesting that saplings of coniferous trees occupy a broader niche and can grow well under the canopy of the adult of broad-leaved trees. | 14 | Tests and analysis of mechanical behaviours of rock bolt componentsfor China's coal mine roadways显示文摘A series of laboratory tests were performed to study the mechanical behaviours of newly developed high strength rock bolt components, including rebar, thread, plate, and domed washer. The characteristics of deformation and damage of each component were presented. The stress distribution of plate and domed washer was investigated through fi nite element modelling. The numerical results show that the yield and tensile strengths of the developed high strength rebar are 33.6%e58.3% and 17.2%e28.7% greater than those of the conventional rebar, respectively. The increase in yield strength was higher than that in tensile strength, suggesting an increase in yield to tensile strength ratio and a decrease in elongation. It is well-known that the thread processing may not be of high precision and accuracy as expected, which is characterised as rough thread surface, non-identical tooth height, toe stripping, and cracks in the surface.Hardening during thread processing tends to increase the thread yield and tensile strengths. In this paper, the typical deformation process of arch-shaped plate is classi fi ed into fi ve stages. The tested plates exhibited distinct deformation characteristics and bearing capacities due to variations in shape, size,material and presence of washer. It was observed that uneven bottom surface, low bearing arch and large radius of the transitional arc connecting bearing arch and bottom surface were the major reasons accounting for low load-bearing capacity of plates. The performance of domed washer has a close relation with the shape, size, strength, and deformation compatibility with plate. | 18 | Structure and regeneration dynamics of three forest types at different succession stages of spruce – fir mixed forest in Changbai Mountain, northeastern China显示文摘Analyzing and understanding the structure and growth dynamics of forests at different stages is helpful to promote forest succession, restoration and management. Three spots representing three succession stages of spruce-fir mixed forest(SF: polar-birch secondary forest, MF: spruce-fir mixed forest and PF: spruce-fir near primary forest) were established. Structure, growth dynamics during two growth seasons for dominant tree species, regeneration were examined, and a univariate O-ring function statistic was used to analyze the spatial patterns of main regeneration tree species. Results showed that,(1) composition of tree species, periodic annual increment(PAI) of the diameter at breast height(DBH), basal area for overstory trees and of ground diameter(DGH) for saplings, were significantly different with the succession;(2) the current species composition and regeneration dynamics of SF suggested a development towards spruce-fir mixed forests. Pioneer species like Betula platyphyllaa will gradually disappear while climax species, such as Abies nephrolepis, Pinus koraiensis, Picea koraiensis and Tilia amurensis will dominate forest stands;(3) Despite the highest volume occurring in PF, and saplings in it grew better than in the others, this forest type is unstable because of its unsustainable structure of DBH class and insufficient regeneration; and(4) MF had the most reasonable distribution of DBH class for adult trees(DBH > 5.0 cm) and DGH class for saplings(H ≥30 cm and DBH ≤5 cm), as well as an optimal volume increment. Limiting canopy opening size can lessen the physiological stress and promote the growth and competitive status of regeneration.
