| 1 | 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). | 8 | Immunopotentiator Thymosin Alpha-1 Promotes Neurogenesis and Cognition in the Developing Mouse via a Systemic Th1 Bias显示文摘In early life, the immune system plays an essential role in brain development. In our study, the immunopotentiator thymosin alpha-1(Ta1) was peripherally administered to neonatal mice to explore whether the peripheral immunopotentiator affects neurodevelopment and cognition, and to further investigate the relevant mechanism. Compared with the control group, the Ta1 mice displayed better cognitive abilities in early life. The numbers of 5-bromodeoxyuridine(Brd U)+, nestin+,T-box transcription factor 2(Tbr2)+, Brd U+/doublecortin(DCX)+, Brd U+/ionized calcium-binding adaptor molecule 1(Iba1)+, and Brd U+/neuronal nuclei(Neu N)+ cells in the hippocampus were increased in the Ta1 group,accompanied by increased interleukin-4(IL-4), interferon-gamma, brain-derived neurotrophic factor, nerve growth factor, and insulin-like growth factor-1 as well as decreased IL-6 and tumor necrosis factor-a. Furthermore, the Ta1-group showed a Th1-polarized immune response, and the neurotrophic factors were positively associated with the Th1/Th2 ratio. More importantly, administration of Ta1 blocked lipopolysaccharide-induced impairment of hippocampal neurogenesis in early life. | 9 | Association of Overlapped and Un-overlapped Comorbidities with COVID-19 Severity and Treatment Outcomes: A Retrospective Cohort Study from Nine Provinces in China显示文摘Objective Several COVID-19 patients have overlapping comorbidities. The independent role of each component contributing to the risk of COVID-19 is unknown, and how some non-cardiometabolic comorbidities affect the risk of COVID-19 remains unclear.Methods A retrospective follow-up design was adopted. A total of 1,160 laboratory-confirmed patients were enrolled from nine provinces in China. Data on comorbidities were obtained from the patients’ medical records. Multivariable logistic regression models were used to estimate the odds ratio(OR) and 95% confidence interval(95% CI) of the associations between comorbidities(cardiometabolic or non-cardiometabolic diseases), clinical severity, and treatment outcomes of COVID-19.Results Overall, 158(13.6%) patients were diagnosed with severe illness and 32(2.7%) had unfavorable outcomes. Hypertension(2.87, 1.30–6.32), type 2 diabetes(T2 DM)(3.57, 2.32–5.49),cardiovascular disease(CVD)(3.78, 1.81–7.89), fatty liver disease(7.53, 1.96–28.96), hyperlipidemia(2.15, 1.26–3.67), other lung diseases(6.00, 3.01–11.96), and electrolyte imbalance(10.40, 3.00–26.10)were independently linked to increased odds of being severely ill. T2 DM(6.07, 2.89–12.75), CVD(8.47,6.03–11.89), and electrolyte imbalance(19.44, 11.47–32.96) were also strong predictors of unfavorable outcomes. Women with comorbidities were more likely to have severe disease on admission(5.46,3.25–9.19), while men with comorbidities were more likely to have unfavorable treatment outcomes(6.58, 1.46–29.64) within two weeks.Conclusion Besides hypertension, diabetes, and CVD, fatty liver disease, hyperlipidemia, other lung diseases, and electrolyte imbalance were independent risk factors for COVID-19 severity and poor treatment outcome. | 11 | Central Obesity and Metabolic Risk Factors in Middle-aged Chinese显示文摘Objective Central obesity is considered to be a central component of metabolic syndrome. Waist circumference(WC) has been widely used as a simple indicator of central obesity. This study is aimed to evaluate the sensitivity of WC cut-off values for predicting metabolic risk factors in middle-aged Chinese. Methods The study involved 923 subjects aged 40-65 years. The metabolic risk factors were defined according to the Chinese Joint Committee for Developing Chinese Guidelines on Prevention and Treatment of Dyslipidemia in Adults. WC cut-off 85-90 cm and ≥90 cm were used as cut-off values of central pre-obesity and central obesity in males, respectively, while WC 80-85 cm and ≥85 cm were used as cut-off values of central pre-obesity and central obesity in females. Results First, WC values corresponding to body mass index(BMI) 24 kg/m2 and visceral fat area(VFA) 80 cm2 were 88.55 cm and 88.51 cm in males, and 81.46 cm and 82.51 cm in females respectively. Second, receiver operating characteristic curves showed that the optimal WC cut-off of value was 88.75 cm in males, higher than that in females(81.75 cm). Third, the subjects with higher WC values were more likely to have accumulating metabolic risk factors. The prevalence of metabolic risk factors increased linearly and significantly in relation to WC levels. | 12 | Prevalence and Risk Factors of Primary Drug-Resistant Tuberculosis in China显示文摘Objective To investigate the prevalence of primary drug-resistant tuberculosis(TB)and associated risk factors in China.We also explored factors contributing to the transmission of multidrug-resistant tuberculosis(MDR-TB).Methods A total of 2794 representative,Mycobacterium tuberculosis isolates from treatment-naive patients were subjected to drug susceptibility testing,and risk factors for drug-resistant TB were analyzed.We also analyzed MDR-TB strain sublineages,drug-resistance-conferring mutations,and risk factors associated with clustered primary MDR strains.Results Among 2794 Mycobacterium tuberculosis isolates from treatment-naive patients,the prevalence of any resistance to first-line drugs was 33.2%and the prevalence of MDR-TB was 5.7%.We did not find any risk factors significantly associated with resistance to first-line drugs.The 93 primary MDR-TB isolates were classified into six sublineages,of which,75(80.6%)isolates were the RD105-deleted Beijing lineage.The largest sublineage included 65(69.9%)isolates with concurrent deletions of RD105,RD207,and RD181.Twenty-nine(31.2%)primary MDR strains grouped in clusters;MDR isolates in clusters were more likely to have S531L rpoB mutation.Conclusion This study indicates that primary drug-resistant TB and MDR-TB strains are prevalent in China,and multiple measures should be taken to address drug-resistant TB. | WANG Sheng Fen ZHOU Yang PANG Yu ZHENG Hui Wen ZHAO Yan Lin | 2016 | Biomedical and Environmental Sciences2016,29,2: | 6 |
| 13 | From 30 million to zero malaria cases in China: lessons learned for China-Africa collaboration in malaria elimination显示文摘Malaria was once one of the most serious public health problems in China,with more than 30 million malaria cases annually before 1949.However,the disease burden has sharply declined and the epidemic areas has shrunken after the implementation of an integrated malaria control and elimination strategy,especially since 2000.Till now,China has successfully scaled up its efforts to become malaria-free and is currently being evaluated for malaria-free certification by the WHO.In the battle against malaria,China's efforts have spanned generations,reducing from an incidence high of 122.9/10000(6.97 million cases)in 1954 to 0.06/10000(7855 cases)in 2010.In 2017,for the first time,China reached zero indigenous case of malaria,putting the country on track to record three consecutive years of zero transmission by 2020,accoding to the National Malaria Elimination Action Plan(2010-2020).China's efforts to eliminate malaria is impressive,and the country is dedicated to sharing its lessons learned in malaria elimination-including,but not limited to,the application of novel genetics-based approaches-with other nations through new initiatives.China will promote international relationships and establish collaborative platforms on a wide range of topics in roughly 65 countries,including 20 African nations.China's experience in applying innovative genetics-based approaches and tools to characterize malaria parasite populations,including surveillance of markers related to drug resistance,categorization of cases as indigenous or imported,and objective identification of the likely sources of infections to inform efforts towards malaria control and elimination in Africa could offer game-changing results when applied to settings with ongoing transmission. | Jun-Hu Chen Jun Fen Xiao-Nong Zhou | 2021 | Infectious Diseases of Poverty2021,10,2: | 5 |

| 16 | Quality metrics and outcomes among critically ill patients in China:results of the national clinical quality control indicators for critical care medicine survey 2015-2019显示文摘Background:It is crucial to improve the quality of care provided to ICU patient,therefore a national survey of the medical quality of intensive care units(ICUs)was conducted to analyze adherence to quality metrics and outcomes among critically ill patients in China from 2015 to 2019.Methods:This was an ICU-level study based on a 15-indicator online survey conducted in China.Considering that ICU care quality may vary between secondary and tertiary hospitals,direct standardization was adopted to compare the rates of ICU quality indicators among provinces/regions.Multivariate analysis was performed to identify potential factors for in-hospital mortality and factors related to ventilator-associated pneumonia(VAP),catheter-related bloodstream infections(CRBSIs),and catheter-associated urinary tract infections(CAUTIs).Results:From the survey,the proportions of structural indicators were 1.83%for the number of ICU inpatients relative to the total number of inpatients,1.44%for ICU bed occupancy relative to the total inpatient bed occupancy,and 51.08%for inpatients with Acute Physiology and Chronic Health Evaluation II scores≥15.The proportions of procedural indicators were 74.37%and 76.60%for 3-hour and 6-hour surviving sepsis campaign bundle compliance,respectively,62.93%for microbiology detection,58.24%for deep vein thrombosis prophylaxis,1.49%for unplanned endotracheal extubations,1.99%for extubated inpatients reintubated within 48 hours,6.38%for unplanned transfer to the ICU,and 1.20%for 48-hour ICU readmission.The proportions of outcome indicators were 1.28‰for VAP,3.06‰for CRBSI,3.65‰for CAUTI,and 10.19%for in-hospital mortality.Although the indicators varied greatly across provinces and regions,the treatment level of ICUs in China has been stable and improved based on various quality control indicators in the past 5 years.The overall mortality rate has dropped from 10.19%to approximately 8%.Conclusions:The quality indicators of medical care in China’s ICUs are heterogeneous,which is reflected in geographic disparities and grades of hospitals.This study is of great significance for improving the homogeneity of ICUs in China. | Xi Rui Fen Dong Xudong Ma Longxiang Su Guangliang Shan Yanhong Guo Yun Long Dawei Liu Xiang Zhou | 2022 | Chinese Medical Journal2022,,9: | 4 |

However,PDLSCs residing in an inflammatory environment generally exhibit compromised functions, as demonstrated by an impaired ability to differentiate into cementoblasts, which are responsible for regrowing the cementum. This study investigated the role of mitochondrial function and downstream long noncoding RNAs(lnc RNAs) in regulating inflammation-induced changes in the cementogenesis of PDLSCs. We found that the inflammatory cytokine-induced impairment of the cementogenesis of PDLSCs was closely correlated with their mitochondrial function, and lnc RNA microarray analysis and gain/loss-of-function studies identified GACAT2 as a regulator of the cellular events involved in inflammation-mediated mitochondrial function and cementogenesis.Subsequently, a comprehensive identification of RNA-binding proteins by mass spectrometry(Ch IRP-MS) and parallel reaction monitoring(PRM) assays revealed that GACAT2 could directly bind to pyruvate kinase M1/2(PKM1/2), a protein correlated with mitochondrial function. Further functional studies demonstrated that GACAT2 overexpression increased the cellular protein expression of PKM1/2, the PKM2 tetramer and phosphorylated PKM2, which led to enhanced pyruvate kinase(PK) activity and increased translocation of PKM2 into mitochondria. We then found that GACAT2 overexpression could reverse the damage to mitochondrial function and cementoblastic differentiation of PDLSCs induced by inflammation and that this effect could be abolished by PKM1/2 knockdown. | 18 | Inhibition of miR-135a-5p attenuates vascular smooth muscle cell proliferation and vascular remodeling in hypertensive rats显示文摘Proliferation of vascular smooth muscle cells(VSMCs)greatly contributes to vascular remodeling in hypertension.This study is to determine the roles and mechanisms of miR-135a-5p intervention in attenuating VSMC proliferation and vascular remodeling in spontaneously hypertensive rats(SHRs).MiR-135a-5p level was raised,while fibronectin type III domain-containing 5(FNDC5)mRNA and protein expressions were reduced in VSMCs of SHRs compared with those of Wistar–Kyoto rats(WKYs).Enhanced VSMC proliferation in SHRs was inhibited by miR-135a-5p knockdown or miR-135a-5p inhibitor,but exacerbated by miR-135a-5p mimic.VSMCs of SHRs showed reduced myofilaments,increased or even damaged mitochondria,increased and dilated endoplasmic reticulum,which were attenuated by miR-135a-5p inhibitor.Dual-luciferase reporter assay shows that FNDC5 was a target gene of miR-135a-5p.Knockdown or inhibition of miR-135a-5p prevented the FNDC5 downregulation in VSMCs of SHRs,while miR-135a-5p mimic inhibited FNDC5 expressions in VSMCs of both WKYs and SHRs.FNDC5 knockdown had no significant effects on VSMC proliferation of WKYs,but aggravated VSMC proliferation of SHRs.Exogenous FNDC5 or FNDC5 overexpression attenuated VSMC proliferation of SHRs,and prevented miR-135a-5p mimic-induced enhancement of VSMC proliferation of SHR.MiR-135a-5p knockdown in SHRs attenuated hypertension,normalized FNDC5 expressions and inhibited vascular smooth muscle proliferation,and alleviated vascular remodeling.These results indicate that miR-135a-5p promotes while FNDC5 inhibits VSMC proliferation in SHRs.Silencing of miR-135a-5p attenuates VSMC proliferation and vascular remodeling in SHRs via disinhibition of FNDC5 transcription.Either inhibition of miR-135a-5p or upregulation of FNDC5 may be a therapeutically strategy in attenuating vascular remodeling and hypertension. | Chao Ye Ying Tong Nan Wu Guo-wei Wan Fen Zheng Jing-yu Chen Jian-zhen Lei Hong Zhou Ai-dong Chen Jue-jin Wang Qi Chen Yue-hua Li Yu-ming Kang Guo-qing Zhu | 2021 | Acta Pharmacologica Sinica2021,42,11: | 4 |
