2A rapid advice guideline for the diagnosis and treatment of 2019 novel coronavirus(2019-nCoV) infected pneumonia(standard version)显示文摘In December 2019, a new type viral pneumonia cases occurred in Wuhan, Hubei Province;and then named '2019 novel coronavirus(2019-nCoV)' by the World Health Organization(WHO) on 12 January 2020. For it is a never been experienced respiratory disease before and with infection ability widely and quickly, it attracted the world’s attention but without treatment and control manual. For the request from frontline clinicians and public health professionals of 2019-nCoV infected pneumonia management, an evidence-based guideline urgently needs to be developed. Therefore, we drafted this guideline according to the rapid advice guidelines methodology and general rules of WHO guideline development;we also added the first-hand management data of Zhongnan Hospital of Wuhan University. This guideline includes the guideline methodology, epidemiological characteristics, disease screening and population prevention, diagnosis, treatment and control(including traditional Chinese Medicine), nosocomial infection prevention and control, and disease nursing of the 2019-nCoV. Moreover, we also provide a whole process of a successful treatment case of the severe 2019-nCoV infected pneumonia and experience and lessons of hospital rescue for 2019-nCoV infections. 8Colorectal cancer lymph node staining by activated carbon nanoparticles suspension in vivo or methylene blue in vitro显示文摘AIM:To investigate whether activated carbon nanoparticles suspension(ACNS) or methylene blue(MB) can increase the detected number of lymph nodes in colorectal cancer.METHODS:Sixty-seven of 72 colorectal cancer patients treated at our hospital fulfilled the inclusion criteria of the study which was conducted from December 2010 to February 2012.Seven patients refused to participate.Eventually,60 patients were included,and randomly assigned to three groups(20 in each group):ACNS group(group A),MB group(group B) and non-stained conventional surgical group(group C).In group A,patients received subserosal injection of 1 mL ACNS in a 4-quadrant region around the mass.In group B,the main artery of specimen was identified and isolated after the specimen was removed,and 2 mL MB was slowly injected into the isolated,stretched and fixed vessel.In group C,no ACNS and MB were injected.All the mesentery lymph nodes were isolated and removed systematically by visually inspecting and palpating the adipose tissue.RESULTS:No difference was observed among the three groups in age,gender,tumor location,tumor diameter,T-stage,degree of differentiation,postoperative complications and peritoneal drainage retention time.The total number of detected lymph nodes was 535,476 and 223 in the three groups,respectively.The mean number of detected lymph nodes per patient was significantly higher in group A than in group C(26.8 ± 8.4 vs 12.2 ± 3.2,P < 0.001).Similarly,there were significantly more lymph nodes detected in group B than in group C(23.8 ± 6.9 vs 12.2 ± 3.2,P < 0.001).However,there was no significant difference between group A and group B.There were 50,46 and 32 metastatic lymph nodes dissected in 13 patients of group A,10 patients of group B and 11 patients of group C,without significant differences among the three groups.Eleven of the 60 patients had insufficient number of detected lymph nodes(< 12).Only one patient with T 4a rectal cancer had 10 lymph nodes detected in group B,the other 10 patients were all from group C.Based on the different diameter categories,the number of detected lymph nodes in groups A and B was significantly higher than in group C.However,there was no statistically significant difference between group A and group B.The metastatic lymph nodes were not significant different among the three groups.Similarly,tumor location,T stage and tumor differentiation did not affect the staining results.Body mass index was a minor influencing factor in the two different staining methods.The stained lymph nodes can easily be identified from the mesenteric adipose tissues,and the staining time for lymph nodes was not significantly different compared with unstained group.None of the patients in groups A and B had drug-related complications.CONCLUSION:Both activated carbon nanoparticles suspension in vivo and methylene blue in vitro can be used as tracers to increase the detected number of lymph nodes in colorectal cancer.Hong-Ke Cai Hai-Fei He Wei Tian Mei-Qi Zhou Yue Hu Yong-Chuan Deng 2012World Journal of Gastroenterology2012,18,42:39
9Role of Epstein-Barr Virus Encoded Latent Membrane Protein 1 in the Carcinogenesis of Nasopharyngeal Carcinoma显示文摘Epstein-Barr virus (EBV)-encoded latent membrane protein 1 (LMP1) has been known to have oncogenic properties during latent infection in nasopharyngeal carcinoma (NPC). Our studies focused on the role of LMP1 in NPC, and showed that LMP1 triggers the NF-κB, AP-1 and STAT signaling pathways. Strikingly, LMP1 was found to mediate the formation of a new heterodimer between c-Jun and JunB. Also, we have identified JAK/STAT and PI-PLC-PKC activation triggered by LMP1 through upregulating the expression of JAK3 and enhancing the phosphorylation of STAT. The constitutive activation of these signaling cascades explains LMP1’s ability to induce such a diverse array of morphological and phenotypic effects in cells and provides insight into how LMP1 may induce cell transformation, in which multihit targeted genes in the downstream play an essential role. All signaling cascades triggered by LMP1 ultimately lead to the disruption of the cell cycle: the acceleration of G1/S phase and the arrest of G2/M phase. We also found that LMP1 induced the expression of hTERT and promoted cell immortalization. Importantly, by intervening physical intracellular signal transduction pathways and disturbing the progression of the cell cycle, LMP1, an important oncoprotein encoded by EBV, is thought to be a key modulator in the pathogenesis of NPC. 10Analysis of factors associated with disease outcomes in hospitalized patients with 2019 novel coronavirus disease显示文摘Background Since early December 2019,the 2019 novel coronavirus disease(COVID-19)has caused pneumonia epidemic in Wuhan,Hubei province of China.This study aimed to investigate the factors affecting the progression of pneumonia in COVID-19 patients.Associated results will be used to evaluate the prognosis and to find the optimal treatment regimens for COVID-19 pneumonia.Methods Patients tested positive for the COVID-19 based on nucleic acid detection were included in this study.Patients were admitted to 3 tertiary hospitals in Wuhan between December 30,2019,and January 15,2020.Individual data,laboratory indices,imaging characteristics,and clinical data were collected,and statistical analysis was performed.Based on clinical typing results,the patients were divided into a progression group or an improvement/stabilization group.Continuous variables were analyzed using independent samples t-test or Mann-Whitney U test.Categorical variables were analyzed using Chi-squared test or Fisher’s exact test.Logistic regression analysis was performed to explore the risk factors for disease progression.Results Seventy-eight patients with COVID-19-induced pneumonia met the inclusion criteria and were included in this study.Efficacy evaluation at 2 weeks after hospitalization indicated that 11 patients(14.1%)had deteriorated,and 67 patients(85.9%)had improved/stabilized.The patients in the progression group were significantly older than those in the disease improvement/stabilization group(66[51,70]vs.37[32,41]years,U=4.932,P=0.001).The progression group had a significantly higher proportion of patients with a history of smoking than the improvement/stabilization group(27.3%vs.3.0%,χ^2=9.291,P=0.018).For all the 78 patients,fever was the most common initial symptom,and the maximum body temperature at admission was significantly higher in the progression group than in the improvement/stabilization group(38.2[37.8,38.6]vs.37.5[37.0,38.4]°C,U=2.057,P=0.027).Moreover,the proportion of patients with respiratory failure(54.5%vs.20.9%,χ^2=5.611,P=0.028)and respiratory rate(34[18,48]vs.24[16,60]breaths/min,U=4.030,P=0.004)were significantly higher in the progression group than in the improvement/stabilization group.C-reactive protein was significantly elevated in the progression group compared to the improvement/stabilization group(38.9[14.3,64.8]vs.10.6[1.9,33.1]mg/L,U=1.315,P=0.024).Albumin was significantly lower in the progression group than in the improvement/stabilization group(36.62±6.60 vs.41.27±4.55 g/L,U=2.843,P=0.006).Patients in the progression group were more likely to receive high-level respiratory support than in the improvement/stabilization group(χ^2=16.01,P=0.001).Multivariate logistic analysis indicated that age(odds ratio[OR],8.546;95%confidence interval[CI]:1.628-44.864;P=0.011),history of smoking(OR,14.285;95%CI:1.577-25.000;P=0.018),maximum body temperature at admission(OR,8.999;95%CI:1.036-78.147,P=0.046),respiratory failure(OR,8.772,95%CI:1.942-40.000;P=0.016),albumin(OR,7.353,95%CI:1.098-50.000;P=0.003),and C-reactive protein(OR,10.530;95%CI:1.224-34.701,P=0.028)were risk factors for disease progression.Conclusions Several factors that led to the progression of COVID-19 pneumonia were identified,including age,history of smoking,maximum body temperature at admission,respiratory failure,albumin,and C-reactive protein.These results can be used to further enhance the ability of management of COVID-19 pneumonia.Wei Liu Zhao-Wu Tao Lei Wang Ming-Li Yuan Kui Liu Ling Zhou Shuang Wei Yan Deng Jing Liu Hui-Guo Liu Ming Yang Yi Hu 2020Chinese Medical Journal2020,,9:31

12Matrine attenuates oxidative stress and cardiomyocyte apoptosis in doxorubicin-induced cardiotoxicity via maintaining AMPKα/UCP2 pathway显示文摘Oxidative stress and cardiomyocyte apoptosis are involved in the pathogenesis of doxorubicin(DOX)-induced cardiotoxicity. Matrine is well-known for its powerful anti-oxidant and anti-apoptotic capacities. Our present study aimed to investigate the effect of matrine on DOX-induced cardiotoxicity and try to unearth the underlying mechanisms. Mice were exposed with DOX to generate DOX-induced cardiotoxicity or normal saline as control. H9 C2 cells were used to verify the effect of matrine in vitro.DOX injection triggered increased generation of reactive oxygen species(ROS) and excessive cardiomyocyte apoptosis, which were significantly mitigated by matrine. Mechanistically, we found that matrine ameliorated DOX-induced uncoupling protein 2(UCP2) downregulation, and UCP2 inhibition bygenipin could blunt the protective effect of matrine on DOX-induced oxidative stress and cardiomyocyte apoptosis. Besides, 50-AMP-activated protein kinase α2(Ampkα2) deficiency inhibited matrine-mediated UCP2 preservation and abolished the beneficial effect of matrine in mice. Besides, we observed that matrine incubation alleviated DOX-induced H9 C2 cells apoptosis and oxidative stress level via activating AMPKα/UCP2, which were blunted by either AMPKα or UCP2 inhibition with genetic or pharmacological methods. 16Isolation,characterization,and phylogenic analysis of three new severe fever with thrombocytopenia syndrome bunyavirus strains derived from Hubei Province,China显示文摘Hubei Province is a major epidemic area of severe fever with thrombocytopenia syndrome bunyavirus(SFTSV) in China. However, to date, a few SFTSV strains have been isolated from Hubei Province, preventing effective studies of epidemic outbreaks. Here, we report three confirmed patients(2015–2016) with typical symptoms of severe fever with thrombocytopenia syndrome disease(SFTS) who were farmers resident in different regions in Hubei Province. Three new SFTSV strains were isolated from the serum samples of each patient. Characterization of viral growth properties showed that there were no significant differences in virus production. All strains were completely sequenced, and phylogenetic analysis showed that unlike the other strains from Hubei province, which belonged to the SFTSV C3 genotype, one of the three strains belonged to the SFTSV C2 genotype. For an environmental magnetic study, it is important to understand the magnetic properties of various magnetic minerals in lacustrine sediments and the processes involved in the formation, transport, and preservation of these magnetic minerals. The magnetic response of lacustrine sediments to climatic change may vary widely from lake to lake. 19A 5-DOF Model for Aeroengine Spindle Dual-rotor System Analysis显示文摘This paper develops a five degrees of freedom(5-DOF) model for aeroengine spindle dual-rotor system dynamic analysis.In this system,the dual rotors are supported on two angular contact ball bearings and two deep groove ball bearings,one of the latter-mentioned bearings works as the inter-shaft bearing.Driven by respective motors,the dual rotors have different co-rotating speeds.The proposed model mathematically formulates the nonlinear displacements,elastic deflections and contact forces of bearings with consideration of 5-DOF and coupling of dual rotors.The nonlinear equations of motions of dual rotors with 5-DOF are solved using Runge-Kutta-Fehlberg algorithm.In order to investigate the effect of the introduced 5-DOF and nonlinear dy-namic bearing model,we compare the proposed model with two models:the 3-DOF model of this system only considering three translational degrees of freedom(Gupta,1993,rotational freedom is neglected);the 5-DOF model where the deep groove ball bearings are simplified as linear elastic spring(Guskov,2007).The simulation results verify Gupta's prediction(1993) and show that the rotational freedom of rotors and nonlinear dynamic model of bearings have great effect on the system dynamic simula-tion.The quantitative results are given as well.HU Qinghua DENG Sier TENG Hongfei 2011Chinese Journal of Aeronautics2011,24,2:18
20Staging laparoscopy improves treatment decision-makingfor advanced gastric cancer显示文摘AIM To evaluate the clinical value of staging laparoscopyin treatment decision-making for advancedgastric cancer (GC).METHODS: Clinical data of 582 patients with advancedGC were retrospectively analyzed. All patientsunderwent staging laparoscopy. The strength ofagreement between computed tomography (CT) stage,endoscopic ultrasound (EUS) stage, laparoscopic stage,and final stage were determined by weighted Kappastatistic (Kw). The number of patients with treatmentdecision-changes was counted. A χ 2 test was used toanalyze the correlation between peritoneal metastasisor positive cytology and clinical characteristics.RESULTS: Among the 582 patients, the distributions ofpathological T classifications were T2/3 (153, 26.3%),T4a (262, 45.0%), and T4b (167, 28.7%). Treatmentplans for 211 (36.3%) patients were changed after staging laparoscopy was performed. Two (10.5%) of19 patients in M1 regained the opportunity for potentialradical resection by staging laparoscopy. Unnecessarylaparotomy was avoided in 71 (12.2%) patients. Thestrength of agreement between preoperative T stageand final T stage was in almost perfect agreement (Kw= 0.838; 95% confidence interval (CI): 0.803-0.872;P 〈 0.05) for staging laparoscopy; compared with CTand EUS, which was in fair agreement. The strengthof agreement between preoperative M stage andfinal M stage was in almost perfect agreement (Kw= 0.990; 95% CI: 0.977-1.000; P 〈 0.05) for staginglaparoscopy; compared with CT, which was in slightagreement. Multivariate analysis revealed that tumorsize (≥ 40 mm), depth of tumor invasion (T4b), andBorrmann type (Ⅲ or Ⅳ) were significantly correlatedwith either peritoneal metastasis or positive cytology.The best performance in diagnosing P-positive wasobtained when two or three risk factors existed.CONCLUSION: Staging laparoscopy can improvetreatment decision-making for advanced GC anddecrease unnecessary exploratory laparotomy.Yan-Feng Hu Zhen-Wei Deng Hao Liu Ting-Yu Mou Tao Chen Xin Lu Da Wang Jiang Yu Guo-Xin Li 2016World Journal of Gastroenterology2016,22,5:17
