| 1 | A rapid advice guideline for the diagnosis and treatment of 2019 novel coronavirus(2019-nCoV) infected pneumonia(standard version)显示文摘In December 2019, a new type viral pneumonia cases occurred in Wuhan, Hubei Province;and then named '2019 novel coronavirus(2019-nCoV)' by the World Health Organization(WHO) on 12 January 2020. For it is a never been experienced respiratory disease before and with infection ability widely and quickly, it attracted the world’s attention but without treatment and control manual. For the request from frontline clinicians and public health professionals of 2019-nCoV infected pneumonia management, an evidence-based guideline urgently needs to be developed. Therefore, we drafted this guideline according to the rapid advice guidelines methodology and general rules of WHO guideline development;we also added the first-hand management data of Zhongnan Hospital of Wuhan University. This guideline includes the guideline methodology, epidemiological characteristics, disease screening and population prevention, diagnosis, treatment and control(including traditional Chinese Medicine), nosocomial infection prevention and control, and disease nursing of the 2019-nCoV. Moreover, we also provide a whole process of a successful treatment case of the severe 2019-nCoV infected pneumonia and experience and lessons of hospital rescue for 2019-nCoV infections. | 3 | A complete sequence and comparative analysis of a SARS-associated virus(Isolate BJ01)显示文摘The genome sequence of the Severe Acute Respiratory Syndrome (SARS)-associated virus provides essential information for the identification of pathogen(s), exploration of etiology and evolution, interpretation of transmission and pathogenesis, development of diagnostics, prevention by future vaccination, and treatment by developing new drugs. We report the complete genome sequence and comparative analysis of an isolate (BJ01) of the coronavirus that has been recognized as a pathogen for SARS. The genome is 29725 nt in size and has 11 ORFs (Open Reading Frames). It is composed of a stable region encoding an RNA-dependent RNA polymerase (composed of 2 ORFs) and a variable region representing 4 CDSs (coding sequences) for viral structural genes (the S, E, M, N proteins) and 5 PUPs (putative uncharacterized proteins). Its gene order is identical to that of other known coronaviruses. The sequence alignment with all known RNA viruses places this virus as a member in the family of Coronaviridae. Thirty putative substitutions have been identified by comparative analysis of the 5 SARS- associated virus genome sequences in GenBank. Fifteen of them lead to possible amino acid changes (non-synonymous mutations) in the proteins. Three amino acid changes, with predicted alteration of physical and chemical features, have been detected in the S protein that is postulated to beinvolved in the immunoreactions between the virus and its host. Two amino acid changes have been detected in the Mprotein, which could be related to viral envelope formation. Phylogenetic analysis suggests the possibility of non-human origin of the SARS-associated viruses but provides noevidence that they are man-made. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. | 8 | Chinese expert consensus statement on the diagnosis and treatment of fulminant myocarditis显示文摘Fulminant myocarditis is primarily caused by infection with any number of a variety of viruses. It arises quickly, progresses rapidly, and may lead to severe heart failure or circulatory failure presenting as rapid-onset hypotension and cardiogenic shock,with mortality rates as high as 50%–70%. Most importantly, there are no treatment options, guidelines or an expert consensus statement. Here, we provide the first expert consensus, the Chinese Society of Cardiology Expert Consensus Statement on the Diagnosis and Treatment of Fulminant Myocarditis, based on data from our recent clinical trial(NCT03268642). In this statement, we describe the clinical features and diagnostic criteria of fulminant myocarditis, and importantly, for the first time,we describe a new treatment regimen termed life support-based comprehensive treatment regimen. The core content of this treatment regimen includes(i) mechanical life support(applications of mechanical respirators and circulatory support systems,including intraaortic balloon pump and extracorporeal membrane oxygenation),(ii) immunological modulation by using sufficient doses of glucocorticoid, immunoglobulin and(iii) antiviral reagents using neuraminidase inhibitor. | 9 | Serum Uric Acid is Associated with the Predicted Risk of Prevalent Cardiovascular Disease in a Community-dwelling Population without Diabetes显示文摘Objective To examine the association between serum uric acid levels and cardiovascular disease risk among individuals without diabetes. Methods We investigated the association between serum uric acid levels and the risk of prevalent cardiometabolic diseases, 10-year Framingham risk for coronary heart disease, and 10-year risk for atherosclerotic cardiovascular diseases(ASCVD) among 8,252 participants aged ≥ 40 years without diabetes from Jiading district, Shanghai, China. Results Body mass index, waist circumference, blood glucose, glycated hemoglobin, blood pressure, and serum lipids increased progressively across the sex-specific quartiles of uric acid(all P trend < 0.05). Compared with individuals in the lowest quartile, those in the higher quartiles had a significantly higher prevalence of obesity, hypertension, and dyslipidemia(all P trend < 0.05). A fully adjusted logistic regression analysis revealed that individuals in the highest quartile had an increased risk of predicted cardiovascular disease compared with those in the lowest quartile of uric acid. The multivariate adjusted odds ratios(ORs) [95% confidence intervals(CIs)] for the highest quartiles for high Framingham risk were 3.00(2.00-4.50) in men and 2.95(1.08-8.43) in women. The multivariate adjusted ORs(95% CIs) for the highest quartile for high ASCVD risk were 1.93(1.17-3.17) in men and 4.53(2.57-7.98) in women. | 10 | Activation of mTORC1 in subchondral bone preosteoblasts promotes osteoarthritis by stimulating bone sclerosis and secretion of CXCL12显示文摘Increasing evidences show that aberrant subchondral bone remodeling plays an important role in the development of osteoarthritis(OA).However,how subchondral bone formation is activated and the mechanism by which increased subchondral bone turnover promotes cartilage degeneration during OA remains unclear.Here,we show that the mechanistic target of rapamycin complex 1(mTORC1)pathway is activated in subchondral bone preosteoblasts(Osterix+)from OA patients and mice.Constitutive activation of mTORC1 in preosteoblasts by deletion of the mTORC1 upstream inhibitor,tuberous sclerosis 1,induced aberrant subchondral bone formation,and sclerosis with little-to-no effects on articular cartilage integrity,but accelerated posttraumatic OA development in mice.In contrast,inhibition of mTORC1 in preosteoblasts by disruption of Raptor(mTORC1-specific component)reduced subchondral bone formation and cartilage degeneration,and attenuated post-traumatic OA in mice.Mechanistically,mTORC1 activation promoted preosteoblast expansion and Cxcl12 secretion,which induced subchondral bone remodeling and cartilage degeneration during OA.A Cxcl12-neutralizing antibody reduced cartilage degeneration and alleviated OA in mice.Altogether,these findings demonstrate that mTORC1 activation in subchondral preosteoblasts is not sufficient to induce OA,but can induce aberrant subchondral bone formation and secrete of Cxcl12 to accelerate disease progression following surgical destabilization of the joint.Pharmaceutical inhibition of the pathway presents a promising therapeutic approach for OA treatment. | Chuangxin Lin Liangliang Liu Chun Zeng Zhong-Kai Cui Yuhui Chen Pinling Lai Hong Wang Yan Shao Haiyan Zhang Rongkai Zhang Chang Zhao Hang Fang Daozhang Cai Xiaochun Bai | 2019 | Bone Research2019,7,1: | 12 |

| 14 | Origin and evolution of waters in the Hancheng coal seams,the Ordos Basin,as revealed from water chemistry and isotope(H,O,^(129)I) analyses显示文摘The coal seams in the Permian Taiyuan Formation and the Carboniferous Shanxi Formation are the primary reservoirs for the coalbed methane(CBM)in the Hancheng region in the Ordos Basin.In this paper,the origin and evolution of waters associated with CBM production were studied on the basis of water chemistry and isotopes including the chloride and iodine compositions,oxygen and hydrogen stable isotopes,and radioactive isotope ratio of129I/127I.The ratio of129I/127I of water was determined by accelerator mass spectrometry(AMS).The result shows that the formation water is of NaHCO3and NaCl types with the total dissolved solids(TDS)varying from 1532.29 mg/L to 7061.12 mg/L.The values of129I and I/Cl ratio indicate that the formation waters were diluted by meteoric water.The129I/127I ratios range from 6.6×10 13to 1459.5×10 13.The129I/127I ratios for most of the samples are between the129I/127I initial value and that of recent anthropogenic water.This age of the formation water samples,obtained through the129I decay curve method,ranges from 0 Ma to 18.5 Ma,suggesting that the waters from the Taiyuan Formation and the Shanxi Formation are very young.Two different origins of water are identified in the Hancheng region.One group is dominated by pre-anthropogenic meteoric water,and is characterized by129I/127I ratios lower than the initial value of 15×10 13andδD,δ18O values of waters below the Global Meteoric Water Line.The other group is characterized by129I/127I ratios in excess of 15×10 13,which has undergone variable degrees of dilution by recent anthropogenic water. | MA XingZhi SONG Yan LIU ShaoBo JIANG Lin HONG Feng | 2013 | Science China Earth Sciences2013,56,11: | 8 |

| 16 | Effects of Soothing Liver and Invigorating Spleen Recipe on Lipid Metabolism Disorders in Kupffer Cells of NAFLD Rats by LXRα/SREBP-1c Signal Pathway显示文摘Objective To investigate the mechanism of lipid metabolism disorders in Kupffer cells(KCs) of non-alcoholic fatty liver disease(NAFLD) rats mediated by LXRα-SREBP-1c pathway and the interference of soothing liver and invigorating spleen recipe(SLISR) on it. Methods SD male rats were randomly divided into five groups: normal, model,soothing liver recipe(SLR), invigorating spleen recipe(ISR), and soothing liver and invigorating spleen recipe(SLISR) groups. The rats in treatment groups wereadministered for 8 weeks. The liver tissue was stained with H&E and oil red O. The levels of hepatic lipid and blood lipid were measured by biochemical analyzer. KCs were isolated from the livers of rats to evaluate the expression of LXRα, SREBP-1C, and FAS mRNA by real-time fluorescence quantitative PCR tests; LXRα, SREBP-1C, and FAS proteins were measured by Western blotting. Results The H&E and oil red O staining results showed that the model rats successfully reproduced typical pathogenetic and histopathological features of NAFLD. The levels of hepatic lipid and blood lipid in the model rats were dramatically increased. Compared with the model group, the values of hepatic lipid and blood lipid in the treatment groups were significantly ameliorated(P <0.05, 0.01). The yields of purified KCs from each rat were 2×107-3×107. The viability ofKCs was higher than 95%, with the purity over 90.18%. Compared with the model group, the expression of LXRα, SREBP-1C, and FAS mRNA and proteins was decreased in all treatment groups, especially in the SLR group(P < 0.05). | 18 | Identification of biomarkers for hepatocellular carcinoma by semiquantitative immunocytochemistry显示文摘AIM:To investigate the expression of key biomarkers in hepatoma cell lines,tumor cells from patients’blood samples,and tumor tissues.METHODS:We performed the biomarker tests in two steps.First,cells plated on coverslips were used to assess biomarkers,and fluorescence intensities were calculated using the NIH Image J software.The measured values were analyzed using the SPSS19.0 software to make comparisons among eight cell lines.Second,eighty-four individual samples were used to assess the biomarkers’expression.Negative enrichment of the blood samples was performed,and karyocytes were isolated and dropped onto pretreated glass slides for further analysis by immunofluorescence staining.Fluorescence intensities were compared among hepatocellular carcinoma(HCC)patients,chronic HBV-infected patients,and healthy controls following methods similar to those used for cell lines.The relationships between the expression of biomarkers and clinical pathological parameters were analyzed by Spearman rank correlation tests.In addition,we studied the distinct biomarkers’expression with three-dimensional laser confocal microscopy reconstructions,and Kaplan-Meier survival analysis was performed to understand the clinical significance of these biomarkers.RESULTS:Microscopic examination and fluorescence intensity calculations indicated that cytokeratin 8/18/19(CK)expression was significantly higher in six of the seven HCC cell lines examined than in the control cells,and the expression levels of asialoglycoprotein receptor(ASGPR)and glypican-3(GPC3)were higher in all seven HCC cell lines than in the control.Cells obtained from HCC patients’blood samples also displayed significantly higher expression levels of ASGPR,GPC3,and CK than cells from chronic HBV-infected patients or healthy controls;these proteins may be valuable surface biomarkers for identifying HCC circulating tumor cells isolated and enriched from the blood samples.The stem cell-like and epithelial-mesenchymal transition-related biomarkers could be detected on the karyocyte slides.ASGPR and GPC3 were expressed at high levels,and thus three-dimensional reconstructions were used to observe their expression in detail.This analysis indicated that GPC3 was localized in the cytoplasm and membrane,but that ASGPR had a polar localization.Survival analyses showed that expression of GPC3 and ASGPR is associated with a patient’s overall survival(OS).CONCLUSION:ASGPR,GPC3,and CK may be valuable HCC biomarkers for CTC detection;the expression of ASGPR and GPC3 might be helpful for understanding patients’OS. | Hong Mu Kai-Xuan Lin Hong Zhao Shu Xing Cong Li Fang Liu Hai-Zhen Lu Ze Zhang Yu-Lin Sun Xi-Yun Yan Jian-Qiang Cai Xiao-Hang Zhao | 2014 | World Journal of Gastroenterology2014,20,19: | 8 |
| 19 | Association of Overlapped and Un-overlapped Comorbidities with COVID-19 Severity and Treatment Outcomes: A Retrospective Cohort Study from Nine Provinces in China显示文摘Objective Several COVID-19 patients have overlapping comorbidities. The independent role of each component contributing to the risk of COVID-19 is unknown, and how some non-cardiometabolic comorbidities affect the risk of COVID-19 remains unclear.Methods A retrospective follow-up design was adopted. A total of 1,160 laboratory-confirmed patients were enrolled from nine provinces in China. Data on comorbidities were obtained from the patients’ medical records. Multivariable logistic regression models were used to estimate the odds ratio(OR) and 95% confidence interval(95% CI) of the associations between comorbidities(cardiometabolic or non-cardiometabolic diseases), clinical severity, and treatment outcomes of COVID-19.Results Overall, 158(13.6%) patients were diagnosed with severe illness and 32(2.7%) had unfavorable outcomes. Hypertension(2.87, 1.30–6.32), type 2 diabetes(T2 DM)(3.57, 2.32–5.49),cardiovascular disease(CVD)(3.78, 1.81–7.89), fatty liver disease(7.53, 1.96–28.96), hyperlipidemia(2.15, 1.26–3.67), other lung diseases(6.00, 3.01–11.96), and electrolyte imbalance(10.40, 3.00–26.10)were independently linked to increased odds of being severely ill. T2 DM(6.07, 2.89–12.75), CVD(8.47,6.03–11.89), and electrolyte imbalance(19.44, 11.47–32.96) were also strong predictors of unfavorable outcomes. Women with comorbidities were more likely to have severe disease on admission(5.46,3.25–9.19), while men with comorbidities were more likely to have unfavorable treatment outcomes(6.58, 1.46–29.64) within two weeks.Conclusion Besides hypertension, diabetes, and CVD, fatty liver disease, hyperlipidemia, other lung diseases, and electrolyte imbalance were independent risk factors for COVID-19 severity and poor treatment outcome.
